HOW TO SIGN UP FOR ACTIVITIES:
• Find the activities that you want to participate in.
• The case manager will contact you the day of the activity to ask
whether you are still going and let you know about what time
they will pick you up.
• If the case manager is unable to speak with you the day of the
activity, they will have to move on down the list and you may
not be able to attend.
• If you can not attend for whatever reason, you need to call
your case manager to cancel at least 1 day in advance.
• Understand that spaces for activities are limited and you
may not get to attend the activity if it is full.
