GOLF CART LIABILITY WAIVER
Participants of the 5th Annual Spearmint Rhino Charity Golf Tournament who will be driving a golf cart are asked to
read, understand and complete the following golf cart liability form:
• Driver (listed below) agrees that they are 100% responsible for any and all damage done to the golf cart they are
assigned to.
• Driver agrees that they will be the only person operating the golf cart at all times.
• Driver agrees that they will NOT allow “caddies” to operate the golf cart at any time.
• Driver agrees that they will inspect the golf cart for damage before the start of the 5th Annual Spearmint Rhino
Charity Golf Tournament. If there is pre-existing damage, they will bring it to the attention of one of the following;
Erick Sikkenga, Tim McAnlis, Dena Hernandez or Renetia McDaniel before the start of the tournament so that the
damage may be documented at the bottom of this waiver.
