Callen-Lorde welcomes all who seek our services, regardless of any patient’s ability to pay. As you look through these pages, you’ll notice how we’ve been growing and changing. You’ll read about our facility renovation that will increase our medical exam space by almost 40% and add an on-site pharmacy. We are also happy to report that we have expanded our care coordination services to help our patients overcome any barriers to getting the care they need and deserve. In these difficult times, when more New Yorkers are in need of affordable, quality healthcare, Callen-Lorde is more vital than ever, and we remain committed to providing the world-class model of quality care that our patients have come to expect. When this visionary organization first opened its doors, it was a tiny volunteer-driven clinic serving mostly gay men who were experiencing discrimination and hostility in mainstream healthcare settings. Today, Callen-Lorde expands on this tradition and serves as a model for other healthcare providers across the country and around the world. As co-chairs of Callen-Lorde’s Board of Directors, we have helped guide this organization—but we cannot do this without you. Callen-Lorde is New York City’s only community health center dedicated to meeting the healthcare needs of lesbian, gay, bisexual, and transgender (LGBT) people and those living with HIV/AIDS, regardless of any patient’s ability to pay. For over 40 years, we have welcomed all, regardless of sexual orientation or gender identity, who otherwise lack access to respectful, empowering, and informed primary healthcare services. We continuously strive to create an environment that affirms humanity and individuality and inspires people to realize their own health and wellness. Our Mission Callen-Lorde Community Health Center provides sensitive, quality health care and related services primarily to New York's lesbian, gay, bisexual, and transgender communities—in all their diversity—regardless of ability to pay. To further this mission, Callen-Lorde promotes health education and wellness and advocates for gay, lesbian, bisexual, and transgender health issues. Our Licensure Callen-Lorde is licensed by the New York State Department of Health as a Diagnostic and Treatment Center to provide comprehensive outpatient medical services. Callen-Lorde is designated by the federal Health Resources and Services Administration as a Federally Qualified Health Center (FQHC). A Federally Qualified Health Center is a licensed health facility designated to improve the specific health needs of a medically underserved area and/ or population though the delivery of comprehensive medical services. 4 Our History Callen-Lorde's grassroots heritage dates back over 40 years to the volunteer-based St. Mark's Community Clinic, founded in 1969 to provide screening and treatment for sexually-transmitted infections. In 1983, St. Mark’s merged with the Gay Men's Health Project to form Community Health Project, a mostly volunteerstaffed, episodic care program housing the nation's first community-based HIV clinic. In 1998, Community Health Project changed its name to Callen-Lorde Community Health Center and moved into a modern, state-of-the-art facility in Manhattan’s Chelsea district. In 2002, Callen-Lorde became a Federally Qualified Health Center, a historic recognition by federal health agencies of the importance of improving health disparities faced by LGBT persons. Over the last decade Callen-Lorde has continued to grow and add new programs, including a dental clinic, mental health services, and an on-site pharmacy—and it remains the New York metropolitan area’s only healthcare facility targeting the needs of the LGBT communities and people living with HIV/AIDS. Michael Callen (1955-1993) Audre Lorde (1934-1992) Michael Callen was born in Rising Sun, Indiana, and began his career as a successful composer and singer. In 1982, he was diagnosed with AIDS (then known as Gay-Related Immune Deficiency) and quickly became a leader in the response to the growing epidemic. In 1985, he became a founding member of the People with AIDS Coalition, the first organization to spring from the AIDS Self-Empowerment movement. As an leading authority in the community, Michael was called to testify before the President’s Commission on AIDS and both houses of the United States Congress. In 1990, Michael chronicled the stories of long-term survivors in his book Surviving AIDS. Michael continued his work in the community and in music until his death at age 38. He is survived by his partner, Richard Dworkin. The daughter of Caribbean immigrants, Audre Lorde was born in New York City and wrote her first poem at age twelve. In the 1950s while at Hunter College, Audre worked as a librarian and educator and became a leader in the early lesbian activist community in Greenwich Village. Her poetry was published regularly throughout the 1960s and 70s and gained wide recognition with the publication of her book The Cancer Journals in 1980, which chronicled her experience with breast cancer. In this groundbreaking work, she examined her own fears, anger, and ultimately, her strength and hope. Audre was named New York State’s Poet Laureate in 1991, a post she retained until her death at age 58, after a 14-year struggle with breast cancer. She is survived by her children, Elizabeth and Jonathan, and her partner, Dr. Gloria Joseph. 5 PATIENT CARE SERVICES Primary Care Comprehensive primary care services delivered in a skilled and culturally sensitive multidisciplinary team model, stressing prevention and wellness. Our providers also offer management of chronic health problems ranging from asthma to heart disease, as well as easily accessible appointments for urgent care. 2009-2010 Update: Callen-Lorde provided over $4.45 million in unreimbursed services in the last year alone, and provided 14,000 New Yorkers—50% of whom had no health insurance—with critical healthcare services. Electronic medical records software was upgraded to enhance the quality and reliability of care. Facility Renovation & Expansion of Services In 2009, we initiated a multi-phased expansion project that will result in a 38% capacity increase – with ten new exam rooms, two new blood drawing suites, our first on-site pharmacy, and over 40 new staff members – enabling us to accommodate an additional 20,000 patient visits per year. Health Outreach To Teens (HOTT) Adolescent and young adult medical and mental health program targeting homeless youth, including New York State’s largest youth-focused HIV care program. Services are provided in a dedicated clinical suite on site and through a licensed mobile medical van. All services are free of charge or low-cost to young people ages 13-24. HIV/AIDS Care Comprehensive HIV-related primary care services with experienced providers meeting New York State Department of Health standards for HIV specialists. 2009-2010 Update: Care Coordination services for HIV positive patients were greatly expanded in 2010, adding 12 new Patient Navigators and an Intake Coordinator to our existing services. This new effort focuses on working with patients in the field, in their homes and at other agencies to remove barriers to needed care. Women’s Health Gynecological services and comprehensive primary care focused on the unique needs of lesbians and bisexual women, as well as women of transgender experience. In addition to routine care, experienced medical staff provide colposcopy, cryotherapy, and LEEP—procedures designed to prevent cervical cancer. Case management services are also provided to uninsured patients to obtain entitlements and access followup diagnostic and specialty care. Our Women’s Health programs place special emphasis on breast/chest and cervical cancer screening. To assist uninsured patients, we participate with Columbia Cancer Prevention Services, which provides coverage for all screening and diagnostic testing. For patients with abnormal screening tests, our case manager ensures that patients gain access to specialty tests to determine if they have cancer and facilitates their access to treatment. Mental Health Our services include individual and group psychotherapy, psychiatric assessment, psychotropic medication management, crisis intervention, substance abuse assessment, and stabilization services. 6 2009-2010 Update: Our current licensure allows us to provide short-term services, but we are exploring obtaining additional NYS licensure that would allow us to provide a greater scope and larger volume of needed services. Care Coordination One-on-one dialogues focused on increasing access to care to our most vulnerable patients. Care Coordinators help patients enroll in entitlement programs and provide supportive counseling, referrals, and advocacy. 2009-2010 Update: As part of our expanding efforts to reach out to all members of our communities, Care Coordination services are now specialized for patients 50 and older and for transgender and gender non-conforming patients—adding to our existing programs for adolescents, women, and HIV positive patients. Oral Health Comprehensive dental and oral health care including preventive, restorative, rehabilitative, cosmetic, endodontic, periodontal, and emergency services, as well as a dental hygiene program. Transgender Health Comprehensive, trans-sensitive primary care, GYN, and sexual health services, including counseling and education, hormone therapy, and referrals to social support services. 2009-2010 Update: Callen-Lorde’s Sexual Health Clinic was expanded to include comprehensive services for transgender and gender non-conforming patients, including chest/breast exams, pelvic exams and PAP tests, prostate exams, STI screening and treatment, HIV testing, and harm reduction counseling. Sexual Health Comprehensive screening, diagnosis and treatment of sexually transmitted infections and individual counseling, focused on the specific needs of the LGBT communities. The Sexual Health Clinic aims to assist patients stay safe and healthy while maintaining a satisfying sex life. In partnership with the New York City Department of Health, Sexual Health Clinic was opened on Saturdays to meet increasing demand. Alternative Insemination Uniquely tailored care encompassing family-building options, fertility awareness, assistance with donor selection, and sperm storage. Both office-based insemination services and instruction in home-based, nontechnical insemination are offered. This program provides sensitive care to anyone who wants to start a family but feels marginalized by other insemination clinics. Health Education, Training, and Outreach Health information, training, and education to groups and institutions outside of Callen-Lorde to ensure that LGBT people receive safe and supportive health care services beyond our doors. In addition, outreach activities recruit new audiences and new patients. 2009-2010 Update: Callen-Lorde has expanded its Health Education program with a variety of trainings and workshops designed to help educators, medical and social service professionals, and students better understand the needs of LGBT individuals. In 2009-2010, Callen-Lorde completed a visioning phase of a comprehensive strategic planning process that will guide the growth of the health center over the next five years. In the visioning phase we gathered information about our community's needs. In the upcoming implementation phase we will define how we will grow and change to meet those needs. Please visit our website to see the findings of our visioning phase. 7 PATIENT VOICES Community Advisory Boards Callen-Lorde’s Community Advisory Boards (CABs) provide patients with a forum where they can impact the services and programs offered at Callen-Lorde. Currently, CABs are available for HIV positive patients, adolescents, and transgender and gender non-conforming patients. The valuable experiences and input of the CAB members help Callen-Lorde provide the best possible services to our patients. In addition, the CABs provide patients with a meaningful outlet to discuss concerns about their care and to affect changes in policies and procedures—a process that can have positive clinical applications to patients who might otherwise feel isolated or powerless. Callen-Lorde is not only a facility for medical services, it is a family that breeds hope, compassion and love within each person who walks through the door. I am so blessed to have found such an amazing place that I can truly call my own and recommend to all of my friends knowing that they will receive the same stellar service that I have for the past four years. -Lanita Patient Satisfaction Survey To make sure that Callen-Lorde is providing the level of quality care that our patients deserve and have come to expect, we periodically conduct patient satisfaction surveys—and the results of the latest one revealed that most patients are very happy with the care they receive at Callen-Lorde, with patient satisfaction improving significantly across every measured category as compared to prior years’ results. This input helps us continually improve the quality of our services. For example, as a result of patient feedback we created a new phone system that was rolled out in the fall of 2008, which included a call center where live staff members now answer all incoming calls during our hours of operation. I am writing to thank you for providing such wonderful services. I am without health insurance and am so happy to have found you — affordable, clean, pleasant, professional, and mostly, sincerely caring. I was informed of many helpful opportunities, programs, free mammograms, without even inquiring. It is so important to be able to take care of myself with integrity without health insurance—and your facility allows me to do just that. -Joanne Callen-Lorde has made a profound difference in my life. Of that number, 20% are HIV-positive. Over 50% of our patients have no health insurance. 34% of our patients identify as women and 66% identify as men. 7% are seniors. Brown, M.D., former New York City Public Health Commissioner, helped change the image of gay men and lesbians by publicly affirming his homosexuality in 1973. In the spirit of Dr. Brown and his unwavering courage and dedication to the health of our communities, we have established the Howard J. Brown Society at Callen-Lorde Community Health Center. The mission of the society is to encourage major financial support for Callen-Lorde to offset the over $4 million in unreimbursed services we provide every year. Members make annual contributions of $1,000 or more to help Callen-Lorde continue its vital work offering the highest quality healthcare to the LGBT communities. Members of the Howard J. Brown Society are among Callen-Lorde’s most valued benefactors. Donors at this level provide the essential funding that translates into pioneering medical advances and unrivaled excellence in patient care. Without their generosity, our life-saving work could not go on and many of the most vulnerable in our communities would have nowhere else to turn. 19 ANNUAL EVENTS Community Health Awards Each fall for the last 10 years, Callen-Lorde has hosted the Community Health Awards to honor the leaders in the field of LGBT health. The event consistently draws around 400 of Callen-Lorde’s strongest friends and supporters. The evening includes a cocktail reception, banquet, presentation of the awards, entertainment, and inspirational speeches from community leaders. Past honorees include former Surgeon General Dr. Jocelyn Elders, tennis star and health advocate Martina Navratilova, and Dr. Kevin Fenton, Director of the CDC’s Center for HIV/AIDS Prevention. Honors also go to local physicians and institutions that are passionate about providing the highest quality care to the LGBT communities and those with HIV/AIDS. In 2009, Callen-Lorde honored Dr. Tony Urbina, Medical Director of HIV/AIDS Education and Training at St. Vincent’s Catholic Medical Center, the LGBT Equality Caucus of the U.S. House of Representatives, and the New York State Health Foundation. Mad Men’s Bryan Batt hosts the 2009 Community Health Awards Walk on the Wild Side Held at the beautiful Central Park Zoo, Callen-Lorde’s Walk on the Wild Side has been the unofficial kick-off of the summer Pride season for the last 10 years. The event draws around 1,000 supporters and their kids—being one of the only events in New York to target LGBT families. The festivities have traditionally been emceed by a celebrity host, and guests are treated to food and drink, arts and crafts, face painting, musical numbers, and a performance by the resident seals. Special guests have included Sex and the City’s Cynthia Nixon and cast members from the Broadway production of The Lion King. In 2010, the cast of Stomp performed a rousing musical number. Spring Fever In April 2008, Callen-Lorde hosted its first event exclusively for women, Spring Fever, at the beautiful Cheim & Read gallery in Chelsea. Over 100 women gathered for cocktails and hors d’oeuvres in support of Callen-Lorde’s vital Women’s Health services. The event was such a success that it was revived for 2009 and again in 2010. Special guests have included author and blogger Kim Severson, and musician Laura Coyle.
