Since 1989 JDM Medical has benefited from the input of hundreds
of therapists and other health care professionals. This accumulated
knowledge, which we refer to as “resident-specific” positioning, is the
driving force behind JDM’s approach to manufacturing for long-term care.

How Do You Code A Cushion?
JDM uses a code system to specify our products. This system is very easy and useful to
both the party ordering and the party taking the order. The system attempts to ensure that no
mistakes are made during the ordering process.
The Code system uses the product code, which defines the type of product (please see page
6), followed by measurements, such as the thickness, the width, and/or the depth of the item,
followed by the feature codes (please see page 6).
The following is an example of how JDM codes its products:
Example : Item # 031816-AH-AS-V
Your resident needs a flat cushion that is 3-inches thick. The wheelchair measures 18”
wide by 16” deep. You would like the cushion to be Anti-Hammocking with a layer of
Visco on top and an Anti-Skid Bottom. The order of JDM’s coding system is as follows:

For the example listed above, the cushion is designated as “0” for the product code
(see page 6 for list of product codes). The product code would be followed by the measurements of the cushion: 3 is the thickness of the wedge. Next, the “18” is the width
of the cushion and of the wheelchair and the “16” is the depth of the cushion and of the
wheelchair. The -AH-AS-V are the added feature codes ( -AH = Anti-Hammocking;
-AS = Anti-Skid Bottom; and -V = Visco (see page 6 for feature codes).

Statement of Materials
I. SUBSTRATES:
A. Polyurethane Foams
All polyurethanes used by JDM meet or exceed California 117B Fire retardancy testing. JDM uses
two types of polyurethanes:
1) HR (High Resiliency) has excellent longevity and buoyancy properties with minimal pressure
redistribution capability. This type of polyurethane is most often found in high-dollar furniture for
its “cushioning” capabilities.
2) G.I.V.E. (Gel-Infused Visco Elastic) has exceptional pressure redistribution properties.
This material was introduced to the medical industry in 2011 by JDM. This Visco Elastic
Polyurethane has 28% of its content composed of Gel “Micro Beads,” which are infused right into
the Visco. This combination Gel and Visco is considered by many to be perfect for LTC.
Why use Visco?
Hard, flat surfaces often cause high local pressure and instability.
Stability
Visco foam molds to the shape of the resident’s body. This ability, coupled with its pressure
and shock-reducing properties, makes it an excellent material for seating devices, back supports, bed positioning devices and contracture relief devices. Visco is recommended anytime there is a skin integrity problem.
B. Gel
JDM will add an additional thin layer of Gel to Bariatric cushions. This layer of gel is in addition to
the HR base and layer of G.I.V.E. (for pressure redistribution).

II. COVERING:
Devices manufactured by JDM Medical are covered in a material that is incontinent proof,
antibacterial, anti-fungal, self-deodorizing, stain-resistant and flame retardant.
Antibacterial/Antifungal
Vital contribution to infection control. The patented dual-agent, antimicrobial system
has been proven effective in decreasing the potential problems of cross infection by
reducing the growth of both Gram positive and Gram negative organisms, such as
Staphylococcus aureus, Klebsiella pneumoniae, and Legionella pneumophilia on the
fabrics’ non-allergenic surfaces.
Flame Retardant
Resists combustion and will self-extinguish when the source of flame is removed. This
provides an extra measure of protection against the ever present dangers of accidental
fires. Approved or registered by every major fire regulatory agency.
Antistatic
Anstat, an enduring antistatic property that prevents the buildup or retention of electrostatic charges that could interfere with the functioning of sensitive life support
equipment. Meets NFPA standards.
Non-Absorbent
Fluid proof and serve as an impermeable barrier that guards against “hidden” contamination caused by the seep-through of liquids and patient discharges. Virtually unaffected by exposure to excreta, urine, blood, and commonly used medications.
The fabrics will not crack or become brittle.
Self-Deodorizing
Self-sanitizing and eliminate undesirable organic odors by inhibiting the growth of
odor-forming surface bacteria. This is another key contribution to both the comfort
of the patient and the attending personnel.
Simplified Maintenance
Resists unsightly stains. Most medications, oils, greases and contaminants wipe
clean with water, mild detergents and/or properly diluted disinfectants and germicides. Soil and stain resistant properties minimize labor, work time, and
maintenance costs for housekeeping.
Longer Lasting
Made to withstand severe physical and environmental abuse and are resistant to
wear, tear, and excessive punishment. This results in extended product life with
fewer replacements, reduced inventories, and greater overall economy.
Comfortable
Designed for maximum comfort. Specially textured diamond patterned surfaces
reduce heat buildup and alleviate skin irritation by channeling off perspiration
vapors and permitting air to circulate beneath the patient.

Policies and Procedures
Warranty:
Stock items may be returned for credit within the first 30 days following
the invoice date. Custom and Specially-Designed devices which have
been manufactured in accordance with your requests are not restockable and may not be returned for credit. You may return custom
devices for modification. Each modification is unique, so please
call for a modification fee (normally 25% of initial cost).
Return Policy:
If there is a need to return a restockable device, please call JDM within
30 days of the purchase date. You will be assigned a Return Authorization (R.A.) Number and will receive instructions for returning the merchandise. Returns without the R.A. number are not accepted and will
automatically be returned at freight costs to you. Returns are subject
to a 20 percent restocking fee.
We are unable to accept the return of any device that has been
marked upon or damaged by the customer.
Credit Memos are issued when the devices are returned to and
received by JDM.

ANTI-HAMMOCKING CUSHIONS AND WEDGES
Considerations when utilizing Anti-Hammocking Cushions or
Wedges:
1. The Anti-Hammocking bottom is convex to eliminate the hammocking
effect of a sling-bottom wheelchair.
2. The convex bottom allows the patient to sit lower than if they were seated
on a solid-insert equivalent.
3. The removal of the solid insert in conjunction with adding Visco layering
increases comfort without substantially increasing height. Example: A 4-inch
cushion with 2 inches of Visco and 2 inches of HR Foam will sit like a
2 1/2-inch cushion.
4. All of JDM’s Anti-Hammocking Cushions have Velcro strapping to prevent
the cushions from twisting and sliding.
5. All of JDM’s Anti-Hammocking Cushions have a rubberized anti-skid
bottom to further prevent the cushion from sliding, as well as to prevent the
wheelchair’s upholstery tubing from “rubbing” into the fabric.
6. By utilizing JDM’s feature code system, the cushion or wedge may be
modified to accommodate many of the problems found in long-term care.

Anti-Hammocking Cushion )
The Anti-Hammocking Cushion is particularly
effective when used with residents who are currently
able to maneuver their wheelchairs by foot movement.
Many wedges and cushions have solid inserts, thereby
elevating the resident up too high, thus causing the
resident to lose mobility. Because the Anti-Hammocking bottom is convex, the health care professional
may cushion and position without taking away the
resident’s ability to foot propel. All Anti-Hammocking
Cushions come with an Anti-Skid bottom and Velcro
straps to prevent the cushion from sliding in the
wheelchair.

Anti-Hammocking Cushion
With Ischial Step and Anti-Skid Bottom)
The Anti-Hammocking Ischial-Step Wedge is
designed to inhibit sliding by utilizing your resident’s
“natural curvature.” The Ischial Step utilizes this
natural curvature to give an anchored support to the
posterior tilt. In order to properly utilize this anchor,
the step should be located at the resident’s gluteal
fold. Please refer to the diagram on the right for
proper measuring instructions. Because the posterior tilt increases the pressure on the Ischials, the
back half of all JDM’s Ischial Steps have one-inch
layer or more of Visco. The anti-hammocking bottom allows residents to sit lower in the wheelchair
than they would in a solid-insert type of cushion. All
Anti-Hammocking Ischial-Step Cushions come with
an Anti-Skid bottom and Velcro straps to prevent the
cushion from sliding in the wheelchair.

The Coccyx Cutout Option for a cushion or
wedge is designed for those residents with existing difficulties at the coccyx area, such as decubitus
ulcers. As the name implies, the coccyx portion
of the cushion is “cut out.” The entire cushion or

to Elevate Left Hip
The Pelvic-Obliquity Cushion is used for a flexible
obliquity and is designed to build up the low side of
the pelvis. The picture on the left is an example of a
Pelvic Obliquity Cushion designed for a patient with
a left flexible pelvic obliquity.
To address the pressures associated with this
positioning correction, JDM’s Pelvic Obliquity
Cushions are layered with Visco elastic pressureredistributing polyurethane.
To measure for correction of a flexible pelvic
obliquity, seat the patient on a PT mat. With an
assistant’s aid, place a yardstick level across the
patient’s PSIS. At each end of the yardstick, measure
the distance from the yardstick to the mat and advise
JDM Medical. (Example Left = 3” Right =2”).
If the patient has a fixed obliquity (often caused
by muscle or joint contractures), you will need to
select a cushion that builds up the high side of the
pelvis. This will allow for a more stable base of
support. JDM recommends you use an AntiHammocking Fixed Obliquity cushion. (See next
page.)

Fixed Pelvic-Obliquity Wedges are designed
for those residents with a “fixed” pelvic obliquity. Fixed tilt/obliquity patients should have
a cushion that accommodates the individual’s
tilt without trying to correct the imbalance. If a
patient’s pelvic obliquity is fixed, you build up
under the high side. In the picture to the left, the
patient’s fixed obliquity is on the patient’s right
side. Building up the high side allows the pelvis
to be fully supported. All Pelvic Obliquity Cushions are layered with Visco for proper pressure
redistribution.
Fixed Pelvic-Obliquity Wedges can be right or
left quadrant fixed. Fixed Pelvic-Obliquity
Wedges are always “Resident Specific.”The

SOLID-INSERT CUSHIONS AND WEDGES
Considerations when utilizing a Solid-Insert Cushion or
Wedge:
1. The Solid-Insert in these cushions eliminate the “hammocking effect” of
a sling-bottom wheelchair by elevating the individual above the upholstery
tubing.
2. All of JDM’s Solid Inserts must have a minimum of three inches of
our High Resiliency Polyurethane (HR) as a cushioning damper between
the insert and the patient, or the cushions/wedge may have a combination of at least two inches of HR along with an additional inch of “G.I.V.E.”
(Gel-Infused Visco Elastic) for pressure redistribution. In other words, all
standard solid inserts must be a minimum of three-inches thick. Please note:
due to G.I.V.E.’s compression capabilities, the additional one inch added to the
cushion or wedges’s height is minimized. Example: 3” x 18” 16” with Visco
(031816-AS-V) will sit like a 2” thick cushion with a solid insert.
3. All of JDM’s Solid-Insert Cushions have Velcro strapping to attach under
the vinyl seat to prevent the cushion from twisting and sliding.
4. All of JDM’s Solid-Insert Cushions have a rubberized anti-skid bottom to
further prevent the cushion from sliding, as well as to prevent the wheelchair’s
upholstery tubing from “rubbing” into the fabric.
5. By utilizing JDM’s feature code system, the cushion or wedge may be
modified to accommodate many of the problems found in long-term care.

Ischial-Step Cushions differ from “Anti-Thrust”
Cushions in the fact that the “Step” is correctly placed
directly in front of the I.T.’s. The Ischial Step utilizes
the natural curvature of the body to give an “anchored
support” and uses the posterior tilt of a wedge to
prevent sliding. This combination moves the weightbearing pressure to the I.T.’s. Because this pressure is being placed onto the solid-insert bottom, all
solid-bottom Ischial Steps will have a “minimum” of 2
inches of HR Polyurethane foam, as well as one inch
of Visco at the rear of the cushion to accommodate
this I.T. pressure. Due to Visco’s compression capabilities, the additional one inch of Visco adds very little

The Coccyx Cutout Option for a cushion or
wedge is designed for those residents with existing
difficulties at the coccyx area, such as decubitus
ulcers. As the name implies, the coccyxBportion of
the cushion is “cut out.” The idea is to eliminate any
pressure exerted on the wound and to redistribute the

pressure surrounding the wound, promoting faster
healing. The entire cushion or wedge is layered
with Visco elastic polyurethane to redistribute
pressure over the entire surface area; but, in particular, the Visco foam moulds to the shape of the
body surrounding the cutout.
The “Cut-Out” may be positioned in different areas
on the cushion or wedge as shown in the example to
the right. This allows for pressure removal at the groin,
coccyx, ischials, etc. Please use the adjacent diagram
to draw desired cutouts at the appropriate area. If a
Cutout Cushion is required for a 3-inch to 2-inch
wedge, please refer to the anti-hammocking
section.

This Coccyx Scoop comes with an anti-skid
bottom, a coccyx scoop, a contoured two-leg
channeled medial thigh separator and a one-inch
layer of Visco. The contoured leg channels added
to the wedge shape provide a slight abduction and
improved alignment of the lower extremities. The
coccyx scoop and Visco layering help redistribute
pressure on the Coccyx, the I.T.’s and the entire
seating area. The wedge shape helps to inhibit sliding. All coccyx scoop cushions are layered with
Visco elastic polyurethane to redistribute pressure at key pressure points. These cushions are
an excellent “preventative” device for those residents
with slight lower extremity adduction problems, and
with recently “healed” decubs.

Amputee Cushions have an extended side to support above-knee or below-knee amputations. Amputee
Cushions come in two types: 1) Rigid Extension Cushions (see examples above) and 2) Flip-Down Cushions
(see next page). Amputee Cushions and Wedges come with a solid insert to eliminate hammocking and to
support the lower extremity. They are manufactured with a minimum of two inches of HR Polyurethane foam

(Drop Seat for 18” x 16” Wheelchair )
Drop Seats are used to lower the height of the
resident’s seat so that the health care professional
can properly seat the resident without elevating
the feet beyond his or her functional capability.
This often allows the resident to continue to be
mobile. Because JDM’s Drop Seats are adjustable at all four corners, you may tilt the resident, if
so needed. This sometimes inhibits the hips from
sliding forward. This device is especially useful in
conjunction with a Kyphotic-Back Device for those
residents with a “C-shaping” forward.
JDM manufactures the Drop Seat utilizing a
composite material which is strong enough to
support the patient, but doesn’t add a lot of weight
to the wheelchair. The hooks are made with a
high-grade aluminum to prevent rusting and other
decomposition. The Drop Seat can be modified
to accommodate many different wheelchair sizes.

Drop-Seat Coccyx Cutouts
As the name implies, the coccyx portion of the
cushion is “cut out.” The idea is to eliminate any
pressure exerted on the wound and to redistribute the pressure surrounding the wound, promoting faster healing. The entire cushion or wedge
is layered with Visco elastic polyurethane to
redistribute pressure over the entire surface
area; but, in particular, the Visco foam molds
to the shape of the body surrounding the
cutout.

The Pelvic-Obliquity Cushion is used for a flexible
obliquity and is designed to build up the low side of
the pelvis. The picture on the left is an example of a
Pelvic Obliquity Cushion designed for a patient with
a left flexible pelvic obliquity.
To address the pressures associated with
this positioning correction, JDM’s Pelvic Obliquity Cushions are layered with Visco elastic
pressure-redistributing polyurethane.
To measure for correction of a flexible pelvic
obliquity, seat the patient on a PT mat. With an
assistant’s aid, place a yardstick level across the
patient’s PSIS. At each end of the yardstick, measure the distance from the yardstick to the mat and
advise JDM Medical. (Example Left = 3” Right =2”).
If the patient has a fixed obliquity (often caused
by muscle or joint contractures), you will need to
select a cushion that builds up the high side of the
pelvis. This will allow for a more stable base of
support. JDM recommends you use a Fixed Obliquity cushion (see picture on page 30). Call JDM for
instructions.

The Wedged Back Pad is designed to cushion

strapping is secured to the rear of the wheelchair

The “Cozie Back” is designed to give the

The Cozie Back with Swing-Away Lateral Supports (SALS), have the same
benefits as the “Cozie Back” plus the Swing Away Laterals.
• They have Pressure-Redistributing G.I.V.E (Gel Infused Visco Elastic), replacing
the standard wheelchair back sling
• They take up limited seat area- allowing the patient to more comfortably lean into
the soft, body-molding back.
• The SALS attach to the wheelchair “canes” after the sling back is removed and
replaced with the Cozie Back.
These Swing Away Lateral Supports are curved (as shown on the picture) to “cup and
support” the upper torso. By simply pressing a button, they swing out of the way allowing the patient better access to the wheelchair.

Tubing Circumference: The distance around the tubing on most wheelchair backs is either 7/8
inch or 1 inch. To determine this distance, wrap a soft measuring tape around the tubing. If the

The Bariatric Back Support with a “Gluteus Contour” is designed and manufactured for the resident
who when seated in a wheelchair doesn’t have
enough seating-surface depth (front to back). JDM
hollows out the polyurethane at the lower half of
the back. This increases the depth available to
those individuals for proper weight bearing and
positioning when they are seated in a wheelchair.
This increased depth allows the residents to more
comfortably “slide back” in the wheelchair, thereby
reducing the constant “repositioning” due to being
pushed forward because of insufficient wheelchair
depth. All “Gluteus Contoured” Back Supports have
a minimum of one inch of pressure-redistributing
Visco foam. Seating cushions are sold separately (see next page).
NOTE: The Gluteus area has been enhanced in
order to better illustrate how to measure properly.

Extended Back Support
The Extended Back Support is manufactured so that
the resident may be reclined slightly, or not. The
standard height is 33 inches, but it can be made taller
in order to be more resident specific. As with all of
JDM’s Back Supports, you may add lateral wedges,
lumbar rolls, various head supports and occipital
supports as needed.
Some common reasons for ordering an Extended Back
Support are:
1) The resident has head or neck flexion problems.
Slightly reclining him or her brings the head back.
2) The resident‘s height dictates the need for more
support along the spinal and torso areas.
3) The resident has a “restraining belt” which might
not be necessary if he or she were slightly reclined.

Solid Bolsters are best used under the knees
for support when the heels are elevated.

Hip Bolsters are designed to support the
torso. The torso portion should be approximately
two inches shorter than the distance from the
wheelchair seat to the axilla. This positioning
prevents undue pressure being placed under the
axilla. The device also extends front to back in
the wheelchair to prevent the leg from “sweeping.”
Hip Bolsters are covered with a medical grade
fabric, which is an incontinent proof, antibacterial,
anti-fungal, self-deodorizing, and stain-resistant
material. They are manufactured with a wood
insert that is 1/2-inch thick and the foam is
normally two to three inches thick. The depth is
determined by the size that is needed to support

Geri Chair Torso Supports
The Geri Chair Torso Support is designed
to support and align the resident in the
middle of the Geri Chair. This device
works particularly well for slightly Kyphotic
individuals due to the “open area” created
by the Lateral Support “Legs” of the Torso

Generally, the reason to use a Geri-Chair
Wedge is to keep the resident’s hips at the
back of the Geri Chair. The Geri-Chair Ischial
Step utilizes the natural curvature of the body
to give an “Anchor Support.” Ischial Steps
inhibit the hips from sliding forward without
using any type of “groin” abductor; therefore,
they are not considered a restraint by most
survey teams. Ischial Steps are layered with
Visco on the back portion for pressure redistribution. These devices have an Anti-Skid

extremity devices. They are all resident specific. Since they are
resident specific, we ask you, the health care professional, to aid
us by accurately measuring each resident. This is very important:
No contracture device may be returned for a refund; however,
we realize mistakes are made. We will modify a device if, for some
reason, the measurements are slightly off. The cost of modifying the
device will be priced accordingly.

dealing with a multitude of lower and upper extremity contracture

All Knee Wedges are laminated with soft Visco
elastic for pressure redistribution. The wedges are
fully covered with medical-grade fabric and are

Contrac-Tree
A Contrac-Tree consists of three separate pieces
attached by Velcro and nylon felt. The ContracTree is extremely helpful at inhibiting lower extremity contractures, particularly if each lower
extremity has more than plus or minus 5 degrees
difference. Examples of appropriate situations for
a Contrac-Tree are:
1. The left lower extremity will range to almost full
extension, but has heel decub and is adducting
hard towards the right lower extremity.
2. The right lower extremity has a range of 90
degrees flexion and you want to prevent further
contracture.
3. The resident can comfortably be abducted the
desired inches (2, 3 or 4 Inches) between knees.

For those residents with lateral motion problems,
minor leg contractures, sliding problems, etc., JDM
recommends the Geri Chair System. The System
aligns the resident in the center of the chair,
supports the head, prevents climbing out over
the sides, keeps the hips to the rear, and extends
slightly contracted legs.
The Geri Chair System (pictured on the left) is

Reclining Back Wheelchair Kit
The Reclining Back Wheelchair System
takes a Reclining Back Wheelchair and converts it into one of the most comfortable seating/positioning systems around. This is an excellent replacement for Geri chairs. It supports
laterally, cushions and protects. The foot boxes
swing away for easier patient transfer. Also the
patient may hand propel if capable.

Hemi Height Kit
The Hemi Height Kit is designed to take a
Hemi or Super Hemi height wheelchair and
convert it into an excellent positioning and
seating device that allows “foot propellers” to
retain their mobility. It supports laterally with
adjustable supports. Cushions and bolsters
protect the individual from skin tears. The
head support allows the resident to rest comfortably without neck strain. The back may be
reclined and “fixed” up to 30°.
