On behalf of the Fire Chiefs’ Association of Broward County; welcome to the Eighteenth
Annual "First There, First Care" EMS Conference and Chief Michael Haygood Advanced
Life Support Competition. I commend every one of you for attending and taking the
opportunity to advance your training and education. Each year, our emergency medical
services face new and more challenging situations which remind us that your skills must
remain sharp, focused and innovative.
As we are all aware the first and for many, the only contact your department may ever
have with a citizen or family member is you. It is imperative you continue to deliver the
highest level of competent and compassionate customer service. Your presence here today is
indicative of your commitment to that goal.
I want to thank the EMS conference committee for their many hours of work organizing this
conference. The committee has done another outstanding job putting together an innovative
program for all to enjoy.
Please accept my thanks on behalf of all the Fire Chiefs in Broward County. Remember

May 22, 2014 Dear EMS Participants, On behalf of the entire Broward Regional Emergency Medical (EMS) Council and community, I would like to extend an earnest welcome to all of you who are attending the Eighteenth Annual "ʺFirst There -­‐‑ First Care"ʺ Emergency Medical Services Conference in Broward County. Every day in Broward County, you are often "ʺFirst There"ʺ to provide "ʺFirst Care"ʺ to citizens and visitors who are sick or injured. For that, you deserve the sincere praises of the Broward County Community. My complements go to each of you for increasing your knowledge and training at conferences such as this. The program for this year’s conference is again outstanding from the caliber of renowned speakers. The theme of the conference this year is "ʺBridging the Gap: From the Streets to the ED."ʺ Four leading (physician and paramedic) professionals will address how the care provided to our patients has transcended from both the ED and the Field, providing a valuable learning experience. The Achievement Awards presented during the conference, is a wonderful way to recognize those who have provided exemplary service in delivering pre-­‐‑
hospital emergency medical care. This year'ʹs conference planning committee has done an outstanding job in developing the program for the conference. STEVE BERRY, BA, NREMT-P; a former teacher and interpreter
for the hearing impaired, Steve has published 11 EMS related
cartoon books since his escapades as a paramedic/EMS
instructor began over 29 years ago. Steve has written over 120
monthly humor columns for Jems magazine along with cartoon
illustrations for Jems magazine. A paramedic for Southwest
Teller County EMS in Colorado, Steve seeks world peace along
with frequent flyer mileage.
EDWARD M. RACHT, M.D. is the Chief Medical Officer of
American Medical Response, the largest EMS provider in the
nation. He has been involved in Emergency Medical Services
and healthcare systems for over 25 years. Most recently, he
served as the Chief Medical Officer and Vice President of Medical
Affairs for Piedmont Newnan Hospital in Metro Atlanta. Prior to
this, Racht was the first full-time Medical Director for the Austin/
Travis County Emergency Medical Services System, where he
spent 13 years. The System was nationally recognized in the
Institute of Medicine’s Report on the state of emergency care for
its collaborative approach to challenging healthcare integration
issues. Racht helped develop a community-based approach
to dramatically decreasing time intervals to STEMI and Stroke
treatment, integrate a separate pediatric trauma facility into the community (the first such
effort to delineate care in Texas), eliminate Emergency Department diversion in collaboration
with the leadership of all hospitals in the region.
PAUL E. PEPE, M.D., M.P.H. (FACEP, MACP, FCCM, FCCP) Professor
of Medicine, Surgery, Pediatrics, Public Health and Riggs Family Chair in
Emergency Medicine, University of Texas Southwestern Medical Center
and the Parkland Health & Hospital System; Director, City of Dallas Medical
Emergency Services for Public Safety, Public Health & Homeland Security
and Medical Director, Dallas Metropolitan Medical Response System (MMRS)
for Counter-Terrorism and Disaster Mitigation and the Dallas Metropolitan
Area BioTel (EMS) System, Dallas, Texas, USA Emergency Medicine and
Trauma Consultant, White House Medical Unit, Washington, DC
DR. PETER ANTEVY is a Pediatric Emergency Medicine Physician at Joe
DiMaggio Children’s Hospital in South Florida and the founder of Pediatric
Emergency Standards Inc. He serves as the Medical Director for Davie FireRescue, Southwest Ranches Fire Rescue and American Ambulance, and is
the Associate Medical Director for 7 other agencies. He is also the Medical
Director for one of the largest paramedic training programs in the country. Dr.
Antevy is highly trained, holding degrees from the University of Miami School
of Medicine, Los Angeles Children’s Hospital and The Children’s Hospital of
Pittsburgh. While his expertise is in pediatric emergencies, Dr. Antevy is well
versed in adult emergency medicine and now spends a significant amount of
time in the pre-hospital arena.
DR. WEISMAN is Associate Dean for Admissions and Professor of Pediatrics
at the University of Miami Miller School of Medicine. He is the Director of
the Florida Poison Information Center – Miami at UM/Jackson Memorial
Hospital. Dr. Weisman received his undergraduate training in Pharmacy at
Temple University and his Doctorate in Clinical Pharmacy from Duquesne
University. Dr. Weisman is Board Certified in Toxicology and a Fellow of the
American Academy of Clinical Toxicology.
Dr. Weisman has over 18 years experience with medical school admissions
and served as Chairman of the Miller School of Medicine Admissions
Committee for 4 years before his appointment as Dean. Dr. Weisman
has been an editor and author of Goldfrank’s Toxicologic Emergencies and
has over 75 publications in the field of poison control and toxicology. Dr.
Weisman is a Past President of the American Association of Poison Control
Centers (AAPCC).
AMY M. MAY, RN [Associate of Science in Nursing, Florida Community
College at Jacksonville, Associate of Arts, Florida Community College at
Jacksonville, University of North Florida, Belmont Abbey College, North
Carolina] Amy represents the Thoratec Corporation as a Clinical Consultant
and Clinical Specialist (Florida)supporting Clinical and technical support of
Thoratec’s Ventricular Assist Device Systems. Providing training, in-service
support and clinical assistance to VAD centers maintaining data of ongoing
patients in territory. Assists TM (sales) and MDM (marketing) to promote
new center development/education plan. Amy has also been the Ventricular
Assist Device Coordinator and Heart Transplant/ Ventricular Assist Device
Coordinator at the Mayo Clinic in Jacksonville and the Team Leader for
Heart Lung Transplant Unit at St. Luke’s Hospital.

Sepsis. The new emergency we can’t even see!
Dr. Edward Racht, MD (09:00 - 10:00)
In this session and introduction to the problem and the challenges in recognition and
diagnosis of sepsis and associated syndromes will be discussed. Along with the pathophysiology of infection and sepsis and the spectrum of disease states caused by infectious agents. Helpful diagnostic tools for EMS providers in sepsis recognition including
the impact of EMS on morbidity and mortality will be identified and a suggested approach for implementing a sepsis identification and management program in EMS.
Prehospital Endotracheal Intubation for Trauma:
Elemental or Detrimental?
Dr. Paul Pepe (10:20 – 11:20)
While endotracheal intubation (ETI) remains the gold standard for airway management
in cardiac and trauma resuscitation, evolving evidence has demonstrated concerns
about the use of ETI in the prehospital setting. Studies, including clinical trials in children and case controlled studies in adult head injury situations, have shown the potential deleterious effects of prehospital ETI, including worsened outcomes. However, the
worsened outcomes may not be the result of the placement of an ETI, but rather subtle
and under-appreciated system factors, including the traditional training provided to
EMS personnel, EMS system configurations and deployment strategies, and also uncontrolled ventilatory techniques.
Pediatric Stroke: Can EMS Bring it Out of its Infancy?

In this presentation, discussions on exciting new keys elements regarding the transport
of pediatric stroke patients, recognizing the key pitfalls to avoid with pediatric stroke as
they relate to transport. A discussion on critical cases that paramedics may be confronted with which relate to pediatric stroke and understanding the basic tenets of history and physical exam that may help in the decision making process.

The Evolving Role of Hormones in Emergency Care
Dr. Paul Pepe (1:30 – 2:45)
The concept of sex-related differences in resuscitation will be explored in
terms of pharmaco-biology, physiological responses to injury, and the very
interesting mechanisms of action of sex hormone interventions in critical illness and trauma. Also presented will be the results in his team's most recent experimental work as well as preliminary human data involving sex
hormone infusions. The implications for future research and resuscitation
protocols in EMS resuscitation and trauma care will also be addressed.
The Dirty Little Secrets of EMS

In this session, a discussion of the very unique and unpredictable medical
environment of EMS. What motivates behavior? How to navigate the art &
science of scene care and an overview of “lessons learned” and nontraditional approaches to improve care.
Finding Humor in the Crazy World of EMS: Welcome to the Dark Side

Those working in the field of emergency medicine often times have to respond to scenes involving horrific trauma and death, and yet they must not
only keep working the remainder of their shift, but they must do it again the
next day and the next. How do they do this? For many, gallows humor offers
a momentary respite from their sense of loss and pain. Most people outside
our profession view this as going beyond the bounds of good taste. However, we know in spite of such tragedies, amusing incidents happen anyway,
and that sometimes humor is the only weapon we have. Sometimes it is just
an inner chuckle that tells us “we’ll get through this.” We all need to sharpen
our senses on calls. Join Steve as he encourages us to enhance our sense
of healthy humor while touching your heart with hope and understanding.

2014 Ways to Practice Evidence-Based and Experience-based
Medicine - Dr. Paul Pepe (9:00 – 10:30)
Over the past several decades, many of the early standard practices in trauma
resuscitation that seemed empirically logical have come under scrutiny. Standards of care for preoperative trauma resuscitative practices such as prehospital
endotracheal intubation, use of pneumatic anti-shock garments (PASG / MAST),
intravascular fluid infusions, and even cervical collar application, have been
questioned or even entirely discounted altogether. For example, using evidence
based approaches, both experimental data and formal clinical trials have indicated that early, rapid fluid resuscitation in patients with uncontrolled hemorrhage
may be harmful and not therapeutic.
LVAD, a Complete Review

In this session, an overview of VAD (Ventricular Assist Device) devices in which
a description of the path of blood follows in a patient with LVAD. Also identifying
components and function of the LVAS including the functions of buttons and symbols of the system controller. The exciting training collaboration efforts in Florida
for this unique patient population will be discussed. Potential complications and
appropriate interventions in the event of an emergency will be reviewed.
Toxicology Now: The top twenty Pediatric Poisonings and Drugs of
Abuse and Treatment; Dr. Richard Weisman (1:30 – 3:00)
In this two topic session, “What’s Toxic and What’s not in common Pediatric Exposures” and “Drugs of Abuse” will be discussed. The pediatric discussion will include poisons and non-toxic plants, caustic exposures, alkalis, oxidizers, cleaning
products, common household medications, alcohol containing products and miscellaneous products. Drugs of Abuse will include the history of drugs in the United
States, mechanisms of toxicity, multiple drug overviews including designer amphetamines.
