The recall information included in this newsletter is for May 2012 through August 2012. TOY PRODUCT RECALLS PRODUCT/FIRM: Imaginarium 5-sided Activity Center by Toys “R” Us, Inc. BRAND/MODEL: This recall involves the Imaginarium 5-Sided Activity Center. The multi-sided center has two triangle-shaped ends with a wooden multi-colored xylophone on one end and a mirror on the other end. The three sides are square-shaped and include moveable block letters, rotating gears and sliding shapes. The name “Imaginarium” appears in a blue, oval-shaped logo underneath the mirror end. The second “I” in the logo is dotted with a shooting star. This recalled product has the model number 46284; Toys “R” Us item number 295909; and has the barcode number 000799985462841. The model number is printed on the back of the box, which is directly above the bar code. These numerical markings do not appear on the actual product. This product was sold exclusively at Toys “R” Us stores nationwide and online via www.toysrus.com from August 2009 through September 2010 for about $25. PROBLEM: The small wooden knobs attaching the xylophone keys to the end can detach, causing a choking hazard to young children. REMEDY: Consumers should immediately stop using the product, move it out of the reach of young children and return it to a Toys “R” Us store for a refund or store credit. PRODUCT/FIRM: First Fitness® Trampolines with Handlebars BRAND/MODEL: This recall involves First Fitness® kid’s first trampolines with handlebars. The child-size toy trampolines have a red and blue metal handlebar, a blue nylon deck guard and a black jumping deck. “First Fitness” is embossed on the jumping deck in white letters. The trampolines can be identified by model number FF-6902TR and Toys “R” Us SKU number 491463. The model and store numbers can be found on the lower right corners of the back of the packaging. A sewn-in tag on the bottom of the deck lists the factory date code of five numbers followed by “GLTX.” This product was sold exclusively at Toys “R” Us stores nationwide from September 2010 through April 2012 for between $45 and $70. PROBLEM: Metal fatigue can cause the handlebar to break away during use, posing a risk of laceration from exposed metal surfaces or other injuries from a fall. REMEDY: Consumer should immediately take the recalled trampolines away from children and contact Aqua-Leisure’s at (888) 912-7087 or visit www.aqualeisure.com recall hotline for a full refund. PRODUCT/FIRM: Sportspower BouncePro 14’ Trampolines by Wal-Mart Stores, Inc. BRAND/MODEL: This recall involves the Sportspower BouncePro 14’ Trampolines with brown mesh netting. UPC codes 68706404210 and 68706404244 are printed on the trampoline box. The words “Sportspower BouncePro 14” and “TR-netting” measure about 6 feet high on the perimeter of the trampoline. The netting is designed to contain individuals bouncing on the trampoline. This product was sold exclusively at Wal-Mart stores nationwide from February 2009 through February 2012 for about $275. PROBLEM: The netting surrounding the trampoline can break, allowing children to fall through the netting. REMEDY: Consumers should stop using the trampolines immediately and contact Sportspower at (888) 965-0565 or visit www.sportspowerltd.net/recallbouncepro-14fthtml to receive replacement black netting for the trampoline. PRODUCT/FIRM: In-Ground Pool Slides by Banzai BRAND/MODEL: This recall involves Banzai in-ground pools. The vinyl slides have a blue base, yellow sliding mat and an arch going over the top of the slide. By connecting a hose to the top of the slide, water can be sprayed on its downward slop. The words “Banzai Splash” are printed in a circular blue, orange and white logo, shaped like a wave on either side of the slide. The recalled slides have the barcode number 2675315734 and model number 15734. Both the barcode and model number appear on the original packaging but are not on the actual slide. This product was sold at Walmart and Toys “R” Us stores nationwide from January 2005 through June 2009 for about $250. PROBLEM: During use, the slide can deflate, allowing the user to hit the ground underneath the slide and become injured. The slide is also unstable and can topple over in both still and windy conditions. This product carries inadequate warnings and instructions. REMEDY: Consumers should immediately stop using the product and return it to the nearest Walmart or Toys R Us for a full refund. Consumer can also cut the two safety warning notices out of the slide and just return that portion. For additional information, contact Walmart at (8000)925-6278 or Toys R Us at (800)869-7787. PRODUCT/FIRM: Discovery Kids™ Animated Marine and Safari Lamps by Innovage, L.L.C. BRAND/MODEL: This recall involves Discovery Kids Animated Marine and Safari lamps that feature rotating films with marine or safari scenes. The words “Discovery Kids” are printed on the front top-left corner of the product. The recalled products have both an 11-digit batch number that begins with either 584894 or 10125 and a model number of 1628626, 1642433, 1641522, 1641523, 1645729, or 1645853. Batch numbers can be found imprinted in the plastic underneath the lamps and on the bottom of the packaging near the barcode. This product was sold at Bed Bath & Beyond, Bonton, JCPenney, Kohls, and Toys “R” Us stores nationwide as well as the online retailers Amazon, Ideeli, Macy’s and Overstock from June 2010 through March 2010 for between $10 and $20. PROBLEM: The replacement of internal wires near the circuit board can cause electrical short-circuiting and sparking, posing a fire and a burn hazard to consumers. REMEDY: Consumers should immediately stop using the lamps and contact Innovage at (888) 232-1535 or visit www.innovage.net for instructions on how to obtain a full refund. PRODUCT/FIRM: Flexible Flyer Swing Sets by the Troxel Company BRAND/MODEL: This recall involves Flexible Flyer swing sets. The Flexible Flyer swing sets come in 11 different models each with a see saw attachment along with swings, bars or a slide. The model number can be found on a sticker located underneath the center of the top bar of each swing set unit. Model numbers and names include: # 41575 Big Adventure, # 41577 Fun Fantastic, # 41578 Fun Fantastic II, # 42003 Backyard Flyer, # 42013 Backyard Fun, # 42023 Backyard Swingin’Fun, # 42030 Windsor II, # 42124 Fun Time, # 42126 Fantastic Playground, #42544 Triple Fun, and #42560 Triple Fun II. This product was sold by WalMart, Toys “R” Us, Academy and other specialty stores and online retailers from December 2011 through May 2012 for between $130 and $280. PROBLEM: The see saw seats can break away from the bolt fasteners during use, posing a fall hazard. REMEDY: Consumers should stop using the see saws immediately and contact Troxel at (888) 770-7060 or visit http://www.regcen.com/flexibleflyer.com to receive a free repair kit. PRODUCT/FIRM: Alex® Model 786X Little Jumpers Trampoline by Panline USA, Inc. BRAND/MODEL: This recall involves Little Jumper trampolines. This product is a small, toddler-sized trampoline with a yellow and blue colored handlebar over the top of the trampoline for toddlers to hold on to while jumping. The trampoline has a blue mat and orange pads with different colored circles printed onto the pads, yellow legs and blue feet. A white label is sewn into the underside of the orange pads which has “786X Little Jumpers Tramopline” printed above the bar code. This recall involves trampolines with the codes 21011-P0003070, 21011P0003246, 25511-P0003071, 27811-P0003372, 29811P0003373 and 34211-P0003375. The code number is printed underneath the barcode. This product was sold at independent specialty toy and retail stores from January through March 2012 for about $100. PRODUCT: The handlebar can break, causing a fall hazard. REMEDY: Consumers should stop using the product immediately and contact the firm for instructions on receiving a replacement trampoline. BRAND/MODEL: This recall involves Green Toys ™ Mini Vehicles. These products are made of plastic and available in yellow, green, red, white and blue. The following model numbers and names are included in this recall: #MVHA-1014 Mini Vehicle Set, #MAMW1015 Mini Ambulance, #MVSA-1016 Mini Vehicle Assortment, #MFBR-1018 Mini Red Fastback, #MPCB-1020 Mini Police Car, #MTXY-1021 Mini Taxi, and #MVFA-1022 Mini Fastback Set. For Mini Vehicle (MVHA-1014) and Fastback (MVFA-1022) sets, the model numbers can be found on the bottom of the toys’ packaging. For all other cars, a label with the model number can be found on the bottom of the car itself. The recalled Mini Vehicles were manufactured from March 2012 through June 2012. The manufacturing date is represented by a circle with an arrow in the middle that can be found imprinted on the underside of the front of the car. To the left and right of the arrow are numbers that represent the year and the arrow itself points to the number that represents the month. Cars with an “I” etched into the underside of the car next to the date stamp are not part of the recall. This product was sold by mass merchandisers, grocery stores and independent toy and specialty stores nationwide and by online retailers from April 2012 to June 2012. The cars cost about $5 each or $18 for the set. PROBLEM: The wheels and hubcaps on the toy cars can detach, posing a choking hazard to young children. REMEDY: Consumers should take the recalled product away from children immediately and contact Green Toys, Inc. at (888) 973-3421 or visit www.greentoys.com/recall to receive a full refund. PRODUCT/FIRM: Animal Snap Bracelet by Toysmith BRAND/MODEL: This recall involves Animal Snap Bracelets in assorted metallic fabrics with animal figures, including: Seashore, Frog, Snake, Starfish, Crab, Lizard, Dolphin and Fish. Manufacturing date codes between 07/11 and 01/12 are printed on a sewn-in label found on the inside of the bracelet. The label includes the Toysmith name and address. A round paper tag is attached to the product with the product name, Toysmith information, the words “Item #2360 Animal Snap Bracelets,” SKU number and bar code. This product was sold at Cost Plus World Market, Cracker Barrel Old Country Store and other retailers from September 2011 to February 2012 for about $3. PROBLEM: The metal snap band can wear through the fabric covering, exposing sharp edges and posing a laceration hazard. PRODUCT/FIRM: Scooter by Kickboard USA and Micro-Mobility Ltd. BRAND/MODEL: This recall involves the three-wheeled Mini Micro scooter for children ages 3-5. The scooter was sold in the following colors: blue, orange, pink and aqua. “Micro” Is written in black and blue letters on the metal handlebar and is imprinted on the black hand grips. “Mini Micro” is written on the front plastic covering. The following recalled model numbers are on a sticker on the bottom of the scooter: #MM0079, #MM0080, #MM0109, and #MM0113. This product was sold at independent toy stores, Amazon.com and Kickboardusa.com for about $85 from May 2012 to July 2012. PROBLEM: The one-piece plastic platform that covers the front wheel base can break, posing a laceration hazard to children. REMEDY: Consumers should take this product away from children and contact Kickboard USA (888) 2365657 or visit www.kickboardusa.com for a free replacement product. CHILD PRODUCT RECALLS PRODUCT/FIRM: “Goddess” Hooded Sweatshirts with Drawstrings by LANY Group, LLC BRAND/MODEL: This recall involves “Goddess” hooded sweatshirts. The girl’s sweatshirts zip up the front and have a hood with a drawstring. The sweatshirts are red with gold hearts, stars and crowns. The sweatshirts were sold in children’s sizes 4T through 12. “Goddess” is printed on the label at the neck. This product was sold at dd’s Discount stores nationwide from October 2011 through January 2012 for about $6. PROBLEM: The sweatshirts have a drawstring through the hood which poses a strangulation hazard to children. REMEDY: Consumer should immediately remove the drawstring to eliminate the hazard or return the sweatshirts to a dd’s Discounts store for a full refund. PRODUCT/FIRM: Girl’s Winter Jackets with drawstrings by LA Fashion Hub BRAND/MODEL: This recall involves girl’s winter jackets with drawstrings through the hoods. The jackets were sold in girls’ sizes 4T, 5, 6 and 6X and in the following colors: pink, turquoise, red and yellow. The jackets are polyester with polyester fill, have a front zipper, snap or Velcro closures. Different designs stamped on the collar, down the front on the pockets, and a flower or heart design is embroidered on the front of the jackets. This product was sold exclusively at dd’s Discount stores nationwide from January 2011 through January 2012 for about $10. PROBLEM: The jackets have a drawstring through the hood which poses a strangulation hazard to children. REMEDY: Consumers should immediately remove the drawstring to eliminate the hazard, or return the jacket to a dd’s Discounts store for a full refund. PRODUCT/FIRM: “YMI” Girls’ Hooded Sweatshirts with Drawstrings by YMI Jeanswear BRAND/MODEL: This recall involves girls’ sweatshirts sold under the brand name “YMI Jeanswear.” The sweatshirts have a front zipper and a fur-lined hood with a drawstring stitched to each side. “YMI” is printed on the label at the neck. The sweatshirts were sold in gray, purple and black, with either a snowflake or a flower design on the front and back. A heart charm with “YMI” engraved on it is attached to the zipper. The recalled sweatshirts were sold in children’s sizes 6 through 16. This product was sold exclusively at dd’s Discount stores nationwide from December 2011 through January 2012 for about $13. PROBLEM: The sweatshirts have drawstrings at the hood that poses a strangulation hazard to children. REMEDY: Consumers should immediately take the recalled sweatshirts from children and remove the drawstrings or return the sweatshirts to a dd’s Discounts store for a full refund. PRODUCT/FIRM: Pluto Hooded Flannel Shirts by La Jolla Sports USA, Inc. dba O’Neil Clothing BRAND/MODEL: This recall involves O’Neill Pluto Brand boy’s flannel long sleeve shirts with button front, with a hood and two chest pockets. Style name “Pluto Flannel” and Style No. 41204106 is printed on a label sewn inside the wearer’s left side seam. “O’NEILLL” is printed on a label sewn inside the back neck. The shirts were sold in dark grey with horizontal black stripes and in child sizes small, medium, large and extra large. This product was sold at East Coast Surf Products, Hansens Surfboards, Huntington Surf & Sport, Jacks Surf and Sport, Main Beach Surf & Sport, O’Neill, Riders Outlet, Sports Chalet, Valley Skate N Surf and other surfing specialty stores nationwide between September 2011 and December 2011 for about $60. PROBLEM: The hooded sweatshirts have drawstrings in the neck that can pose a strangulation hazard to children. REMEDY: Consumers should immediately take the recalled sweatshirts from children and remove the drawstring from the hooded flannel shirt, to eliminate the hazard, and return the garment to the place of purchase or to the company for a full refund. BRAND/MODEL: This recall involves M.M.M. 100% polyester boys’ two-color jogging suits. “M.M.M.” appears on the label stitched into the back of the neck. The pants are a solid color matching the jacket, which has a second color on the top of both sleeves and top portion of the jacket. The sets’ color combinations include blue and white, gray and blue, and gray and red. The jackets were sold in sizes 4-16. This product was sold exclusively at dd’s Discount stores nationwide from December 2011 through January 2012 for about $10. PROBLEM: The jogging suit’s jacket has a drawstring at the waist that could become snagged or caught in small spaces or vehicle doors and it poses an entanglement hazard. REMEDY: Consumers should immediately take the recalled jackets from children and remove the waist drawstrings or return the product to a dd’s Discount store for a full refund. For additional information, contact Hot Chocolate at (877) 841-0646. PRODUCT/FIRM: Tender Vibes & Light Vibes Bassinets by Kolcraft Enterprises, Inc. BRAND/MODEL: This recall includes Kolcraft Tender Vibers bassinets with model numbers KB021-ARC, KB022-VER, KB039-CMR1 and Light Vibes Bassinet with model number KB043-BNT1. A label with the model/item number of the bassinet is located on one of the legs of the metal frame. The recalled bassinets were manufactured from July 2008 through May 2010. The words “Kolcraft Tender Vibes” or “Kolcraft Light Vibers” is located on the removable music box attached to the side of the bassinet. This product was sold at mass market and independent juvenile specialty stores nationwide and online from July 2008 through May 2012 for between about $50 and $100. PROBLEM: The latches that may attaches the bassinet base onto the metal frame can appear to be locked in place but still remain unlocked. This allows the bassinet to become detached from the metal frame, causing the bassinet to fall and the infant to be injured. REMEDY: Consumer should immediately stop using the recalled bassinets and contact Kolcraft at (888) 6241908 or visit www.kolcraft.com to receive a repair kit and instructions for securing the latches to the metal frame. In the meantime, parents are urged to find an alternate, safe sleeping environment for the child, such as a crib that meets current safety standards or a play yard depending on the child’s age. PRODUCT/FIRM: Class Club Children’s Letterman Jackets by Supreme Trading Limited BRAND/MODEL: This recall involves Class Club brand children’s letterman jackets. The 100% polyester jackets were sold in navy with white sleeves in sizes 2 to 3. A patch on the upper, right chest of the jacket reads “28F Football Athlete State Champs.” Style number “F14CL112L” is printed on the back of the sewn-in size label in the back neck. This product was sold exclusively at Dillard’s stores nationwide and online at www.dillards.com between September 2011 and November 2011 for about $25. PROBLEM: Snaps on the jacket can detach, posing a choking hazard to young children. REMEDY: Consumers should immediately take the jackets from children and return them to Dillard’s for a full refund. BRAND/MODEL: This recall involves boys’ long sleeved zip-front hooded jackets. The jackets have a fake fur lining and were sold in the following three colors: army green, aqua green and black. The jackets have one of four designs: the number “2” or the letter “B’ embroidered on the left front chest; or the work “ROCK” or “AB Sportwear” embroidered across the chest. The jackets were sold in sizes 4-7 or S, M, and L. Either “SX” or “QH” appears on the label stitched at the neck. The jackets have model numbers: 1006, 1029, 1058 and 1061, however the model number only appears on the original hang tag and not on the product. This product was sold exclusively at dd’s Discount stores nationwide from August 2011 through December 2011 for about $8. PROBLEM: The jackets have a drawstring through the hood which poses a strangulation hazard to children. REMEDY: Consumers should immediately take the recalled jackets from children and remove the drawstring or contact Bonded Apparel at (888) 974-1555 for instructions on returning the product for a full refund. PRODUCT/FIRM: Toilet and Cabinet Locks by Dorel Juvenile Group (DJG), Inc. st BRAND/MODEL: This recall involves Safety 1 fit toilet locks with model numbers 48003 and 48103. The toilet lock is attached to the tank behind the lid and is intended to prevent a child’s access to the toilet bowl. This recall st also involves Safety 1 cabinet slide locks with model numbers 12013 and 12014. The cabinet slide lock is attached to cabinet knobs or handles to prevent access to the contents of the cabinet. Model numbers can be found on the back of the locks. This product was sold at Bed, Bath and Beyond; Burlington Coat Factory; Great Beginnings; Home Depot; Target and Walmart from January 2005 through April 2010 for between $8 and $20 for the toilet locks, and from January 2000 through March 2009 for between $2 and $11 for the cabinet locks. Amazon.com sold both locks through April 2012. PROBLEM: Young children can unexpectedly disengage the toilet locks and gain access to water in the toilet, posing the risk of drowning. The cabinet locks are being recalled, because young children can disengage the lock which poses a risk of injury from dangerous or unsafe items stored in the cabinet. REMEDY: Consumer should immediately remove the recalled locks and contact DJG at (877) 416-8105 or visit www.djgusa.com for a free replacement lock of a different model. When removing the recalled locks, consumers are urged to immediately store dangerous items out of reach of children and to prevent unsupervised access to bathroom. PRODUCT/FIRM: Convertible High Chairs by Evenflo, Inc. BRAND/MODEL: This recall involves Evenflo high chairs that convert from a high chair to a toddler-size table and chair. The convertible high chair can be identified by the model names and numbers listed below. Model numbers are located on a label on the lower portion of one of the high chair’s legs. The following model name and model numbers are included in this recall: Dottie Lime #29111259, Dottie Rose #29111271 and Marianna #29111234. This product was sold at Toys “R” Us and Walmart stores nationwide and online at Walmart.com and Wayfair.com between December 2011 and June 2012 for about $40. PROBLEM: The activity tray on the high chair can unexpectedly detach and allow an unrestrained child to fall, posing a risk of injury to the child. REMEDY: Consumers with the recalled highchairs should immediately contact Evenflo at (800) 233-5921 or visit http://safety.evenflo.com for a replacement tray with installation and use instructions. PRODUCT/FIRM: Contours Options three and fourwheeled strollers by Kolcraft Enterprises, Inc. BRAND/MODEL: This recall involves Kolcraft Contours Option three and four-wheeled strollers. Strollers included in the recall have model numbers starting with ZL002, ZL005, ZL015 and ZL018. On the ZL002 model, the model number and date of manufacture is printed on a sticker above the left wheel. On the ZL005, ZL008, ZL015 and ZL018 models, the model number and date of manufacture is printed on a label sewn into the back of the stroller seat pad. The strollers were manufactured from January 2006 through November 2009 and sold in various color schemes. This product was sold at product specialty stores nationwide and online at Amazon.com, Target.com and Toysrus.com from January 2006 and June 2012 for between $150 and $160. PROBLEM: A child or consumer’s finger can become caught in the opening formed when locking and unlocking the hinge mechanism used to adjust the handlebars on the strollers. This presents an amputation and laceration hazard to children and adults handling the stroller. REMEDY: Consumers should immediately stop using the product and contact Kolcraft at (800) 453-7673 or visit www.kolcraft.com to receive a free repair kit. PRODUCT/FIRM: Gabiano Collection Boys and Girls Pajamas, Sets and Gowns by Ishtex Textile Product, Inc. BRAND/MODEL: This recall involves all styles of boys and girls 100 percent cotton pajamas, including sets (tops and bottoms), onepiece suits and gowns in a variety of colors and designs in sizes 2 through 14. “Gabino” is printed on a tag sewn into the center back neckline of the tops and gown and at the center back of the bottoms. This recall includes style numbers GB201, GB204, GB205, GB207, GB208, GB213, GB215, GB220, GB225, GB230, GB245, GB250, GB260, GB275, GB2001, GB2002, GB2011, GB2012, GB2021, GB2022, GB2031, GB2032, GB2041, GB2042, GB3001 and GB3011. The style number is only printed on the sales tag and does not appear on the garments. This product was sold at children’s clothing stores and specialty retailers nationwide and online including at The Pajama Princess in Austin, TX. Anklebiter’s in Roswell, GA., Cute As A Button in Tarboro, N.C., Bhumbles in Winder, Ga., and www.bhumbles.com, from February 2010 to December 2011 for between about $20 and $50. PROBLEM: The pajamas fail to meet the federal flammability standards for children’s sleepwear posing a risk of burn injury to children. The garments were advertised and sold as children’s sleepwear. REMEDY: Children should stop wearing the recalled sleepwear immediately and consumers should return it for a refund, exchange or store credit. PRODUCT/FIRM: Children’s Pajamas by PajamaGram BRAND/MODEL: This recall involves children’s pajamas which are 100 percent polyester fleece sold in sizes 12 months to 4T in boys and girls sizes 6 through 14. “PajamaGram” is printed on the neck tag. The pajamas come in two styles: one-piece hood and feet pajamas, and two-piece sets. The style/GPU number is printed on a second tag sewn into either the neck or side of the pajamas and the pajama pants. The following pajamas are included in the recall: Hoodie Footie for Girls in pink with zip-off feet and hood #JSHFP1, Hoodie Footie for Boys and Girls in blue with zip-off feet and hood #JSLHFB1, Sweetheart Hoodie Footie for Girls in pink with a red heart print and zip-off feet and hood #JSLHFV1, Winter Whimsy Hoodie Footie for Boys and Girls in red with a snowman/penguin print and zipoff feet and hood #JSLHFW1, Lodge Fleece Set for Toddlers for Boys and Girls in a two-piece set in a blue shirt with moose appliqué and white moose/snowflake print pant #JSLLFB1, Snuggle Fleece Argyle Set for Girls two-piece set in light blue shirt with pink and blue argyle print pant #JSLAFG1, and Infant Lodge Fleece One-Piece in white with moose/snowflake print #JSLLFB2. This product was sold at the PajamaGram website, telephone and catalogue from September 2011 through March 2012 for between $26 and $40. PROBLEM: The pajamas fail to meet the federal flammability standards for children’s sleepwear, posing a risk of burn injury to children. REMEDY: Children should stop wearing the pajamas immediately and consumers should contact PajamaGram at (800) 262-1162 for a replacement, exchange or full refund. PRODUCT/FIRM: Boys and Girls Pull-On Lounge Pants and Girls Boxers by Rigo International, Inc. BRAND/MODEL: This recall involves Academy pull-on lounge pants for boys and girls, and boxers for girls. The garments are made of 100 percent cotton and have elastic or fabric waistbands. The pants and boxers have a variety of colors and print designs, including guitars, bikes, skates, footballs, baseballs, softballs, soccer balls, volleyballs, deer, zebra, horses, penguins, and leopards. Some clothing may have the words “cheer,” “champs,” “soccer stars,” “peaceful,” “dance” or “perfect 10” printed on them. The garments are sold in sizes XS through L for boys or 6 through 14 for girls. The garments have identification numbers on a tag directly under the size tag on the rear section of the waistband. Garments with the following identification numbers are included in this recall: 377676, 377677, 377678, 377679, 454818, 454819, 454820, 454821, 454822, 454823, 454824, 454825, 454826, 454827, 454828, 454829, 466629, 466630, 555571, 555572, 564822, 564904 and 564939. This product was sold exclusively at Academy Sports and Outdoor stores in Texas from January 2010 to March 2011 for about $8. PROBLEM: The pants and boxers fail to meet the federal flammability standards for children’s sleepwear, posing a risk of burn injury to children. REMEDY: Children should stop wearing the recalled sleepwear immediately. Consumers should return the sleepwear to Academy for a refund, exchange or store credit. The slip-on shoes have pull-on tabs on both the front and back of the foot opening the shoe. These Aqua Socks were sold in pink and purple in Toddler sizes 511. The recall includes Old Navy Toddler Girl Aqua Socks with style number 896452. A small tag inside the foot opening contains the style number (896452), size information and country of origin (Canada). This product was sold at Old Navy stores in the U.S. and Canada and at oldnavy.com in the U.S. and at oldnavy.ca in Canada from February 2012 through June 2012. The shoes sold for between $12 and $15. PROBLEM: This style of Aqua Socks have less traction when worn on wet or smooth surfaces such as hardwood or tile, creating a slip and fall hazard. REMEDY: Consumers should immediately stop using these shoes and return them to any Old Navy store for a full refund. Consumers who return the shoes before November 9, 2012 will also receive a coupon for $10 off their next purchase at Old Navy. PRODUCT/FIRM: Folding Beach Chairs by Downeast Concepts, Inc. dba Backyard and Beyond BRAND/MODEL: The recalled children’s folding beach chairs have white aluminum tube frames and pink, yellow, blue or purple fabric seats and chair backs with fish, palm trees or mermaid decorations. The chairs measure 13 inches wide by 18 inches high by 20 inches deep. This product was sold at Home Goods and other stores nationwide from June 2011 through June 2012 for between $13 and $25. PROBLEM: The recalled children’s beach chairs have exposed, sharp metal rivets, posing a laceration hazard. REMEDY: Consumers should stop using the recalled beach chairs immediately and return them to Downeast Concepts for a full refund. BRAND/MODEL: This recall involves two different older versions of the Peg Prego strollers, Venezia and PlikoP3, manufactured between January 2004 and September 2007 in a variety of colors. Only strollers that have a child tray with one cup holder are part of this recall. Strollers with a bumper bar in front of the child tray or a tray with two cup holders are not included in this recall. The strollers were sold at various retailers nationwide, including Babies “R” Us and Buy Buy Baby from January 2004 through September 2010 for between $270 and $330 for the Pliko P-3 stroller and between $350 and $450 for the Venezia stroller. The strollers were manufactured in Italy. PROBLEM: Entrapment and strangulation can occur, especially to infants younger than 12 months of age, when a child is not harnessed. An infant can pass through the opening between the stroller tray and seat bottom, and his/her head and neck can become entrapped by the tray. Infants who become entrapped at the neck are at risk of strangulation. REMEDY: Consumers should immediately stop using the recalled strollers and contact Peg Prego USA at (888) 734-6020 or visit www.PegPregoUSA.com for a free repair kit. Do not return the stroller to the retailers as they will not be able to provide the repair kit. PRODUCT/FIRM: Contours Options LT Tandem Strollers by Kolcraft Enterprises, Inc. BRAND/MODEL: This recall involves all Contours Option LT tandem strollers with model numbers ZT012. The model number and date of manufacturer are printed on a label on the rear leg of the stroller. The dual-seat strollers have one mesh basket beneath both seats and were sold in two colors schemes; black with red canopies and accents, and gray with yellow canopies and accents. “Options LT” is printed on a bar along the side of the stroller. This product was sold at Burlington Coat Factory and juvenile product specialty stores nationwide and online at Amazon.com, Target.com and other online retailers between February 2012 and July 2012 for about $250. PROBLEM: The front wheel assembly can break, posing a fall hazard to the child in the stroller. In addition, for strollers manufactured in January and February 2012, the nuts that hold the stroller’s basket support screws in place can detach. Detached nuts can pose a choking hazard to young children. REMEDY: Consumers should immediately stop using the product and contact Kolcraft Enterprises at (800) 453-7673 or visit www.kolcraft.com to receive free replacement wheels. Consumers with strollers manufactured in January and February 2012 will also receive replacement nuts for the basket support screws. PRODUCT/FIRM: Chicco Polly Hair Chair by Artsana USA, Inc. BRAND/MODEL: This recall involves a range of Chicco Polly high chairs with pegs on the back legs intended for tray storage. The high chairs have a folding metal frame for storage and a reclining seat. The recalled high chairs can be identified by the model number and date code printed on a label on the underside of the seat, close to the footrest. The date code is in the format DDMMYYYY or YYYY-MMDD. The high chairs included in this recall were manufactured prior to October 13, 2010. This product was sold at retail stores including Babies “R” Us, Burlington Coat Factory, Buy Buy Baby, Shopko and Toys “R” Us, as well as online outlets including Albeebaby.com, Amazon.com, Babyage.com, Babysupermart.com, Diapers.com and Target.com from January 2005 through July 2012 for between $100 and $150. PROBLEM: Children can fall on or against the pegs on the rear legs of the highchair, resulting in a bruising or laceration injury. REMEDY: Consumers should contact Chicco at (800) 807-8817 or visit www.chiccousa.com/pollykit for a free peg cover kit which will be mailed to them. To help prevent injuries before repair, consumers should store the tray on the pegs when the high chair is not in use. PRODUCT/FIRM: Kenta and Kenta Plus Child Carriers by Liberty Mountain BRAND/MODEL: This recall involves backpack-style child carriers sold under the Kenta and Kenta Plus model name. “VAUDE” is printed on the back and between the shoulder straps of the carrier. “Kenta” or “Kenta Plus” is printed on the lower back or hip belt of the carrier. The carriers were sold in black, red and brown and were designated for children up to 31 pounds. This product was sold at outdoor camping and climbing stores nationwide from January 2010 through May 2012 for about $150 for the Kenta and $170 for the Kenta Plus models. PROBLEM: The side strap’s seam can unravel and cause the strap to separate, posing a fall hazard to the child in the carrier. REMEDY: Consumers should immediately stop using the recalled child carriers and return the product to an authorized VAUDE dealer for a replacement. BRAND/MODEL: This recall involves My Clothes’ children’s long-sleeved nightgown and set consisting of a long-sleeves and long pants is being recalled. The long-sleeved nightgown, Style 14, is cotton knit with multi-colored polka dots. The sleeves and hem are ruffled. The shirt and pants sets, Style 15 or Style 314R, is cotton with a red and white gingham print pattern and white piping on the collar, sleeve and pant legs. The shirt has four buttons. The garments were sold in sizes 12 months through size 14. The style number and the words “PJs by My Clothes” are printed on tags sewn into the center back of the gown, the neck of the shirt and the rear waistband of the pants. This product was sold at children’s boutiques and specialty retailers and online nationwide from August 2011 through December 2011 for between $35 and $45. PROBLEM: The pajamas fail to meet the federal flammability standards for children’s sleepwear, posing a risk of burn injury to children. REMEDY: Children should stop wearing the recalled sleepwear immediately and consumers should return it to the retailer where purchased for a full refund, exchange or store credit. PRODUCT/FIRM: Baby Seats by Bumbo International Trust BRAND/MODEL: This recall involves baby seats by Bumbo. The bottom of the Bumbo seat is round and flat with a diameter of about 15 inches. It is constructed out of a single piece of molded foam and comes in various colors. The seat has leg holes and the seat back wraps completely around the child. On the front of the seat in raised lettering is the word “Bumbo” with the image of an elephant on top. The bottom of the seat has the following words “Manufactured by Bumbo South Africa Material: Polyurethane World Patent No. PCT: ZA/1999/00030.” The back of the seat has several warnings and seats manufactured since 2008 have an additional label on the front of the seat warning against use on raised surfaces. This product was sold by Sears, Target, Toys “R” Us, Babies “R” Us, USA Babies, Walmart, and various other toy and children’s stores nationwide, and various online sellers, from August 2003 through August 2012 for between $30 and $50. PROBLEM: Babies can maneuver out of or fall from the Bumbo seat, posing a risk of serious injuries. REMEDY: Consumers should immediately stop using the product until they order and install a free repair kit, which includes: a restraint belt with a warning label, installation instructions, safe use instructions and a new warning sticker. The belt should always be used when a child is placed in the seat. Even with the belt, the seat should never be used on any raised surface. Consumers should also immediately stop using Bumbo seat covers that interfere with the installation and use of the belt. A video demonstrating proper installation of the restraint belt and proper use of the Bumbo seat are available at www.BumboUSA.com. PRODUCT/FIRM: Crib Fringe by Babylicious Products, Inc. BRAND/MODEL: This recall involves crib fringe. This recalled product is a narrow fabric strip connecting several individual fabric triangles. It is designated to be attached to the side rail of a crib or along window valences or curtain rods. The product was sold in a variety of colors and patterns and with two fringes in each package. This product was sold online and at specialty children’s product retailers from January 2006 through May 2012 for about $24. PROBLEM: The narrow fabric strip connecting individual fabric triangles presents a strangulation hazard to young children. REMEDY: Consumers should immediately remove the recalled product from cribs, window valences or curtain rods and contact Babylicious at (855) 684-8399 or visit www.babylicious.com to receive a refund. Consumers will be refunded $12 for each recalled fringe returned. PRODUCT/FIRM: Beadboard Bunk Beds by PBteen, a division of Williams-Sonoma, Inc. BRAND/MODEL: This recall involves wooden bunk beds with beadboard paneling. The upper bunk, accessible by a four-step ladder, is designed for a twin mattress and the lower bunk for a full-size mattress. The bunk beds were sold in white, chestnut, dark espresso and honey, and have storage cubbies below the lower bunk. The product number is listed on the purchase receipt and a label on each of the bunk bed’s parts lists that part’s SKU number. This product was sold at PBteen.com and through the PBteen catalog nationwide from January 2008 to April 2009 for the Sun Valley white and Sun Valley honey models, and October 2011 to February 2012 for the dark espresso, chestnut and white models. The bunk beds were sold for between $1,700 and $2,000. PROBLEM: The front upper horizontal panel on the bunk beds can crack or break, posing a risk of injury to the consumer. REMEDY: Consumers should immediately stop using the upper bunk and contact PBteen at (855) 217-5223.
