September - November 2014 FAMOUS CHINESE PHYSICIAN, SUN SI MIAO ON “WAYS TO MAINTAINING & KEEPING GOODby Life!Clinic’s HEALTH” PART I Physician Jia Hong Lu There are many remarkable and famous physicians throughout the Chinese civilisation of more than 5000 years. They include Hua Duo from the Three Kingdom, Li Shi Zhen from the Ming Dynasty. The Tang Dynasty is the most prosperous / flourishing period of the Chinese feudal society . Born in the Tang Dynasty, the famous physician, Sun Si Miao, has a famous saying: “Human lives are very precious, more precious than thousands of gold (⼈人命⾄至重,贵于千⾦金). Hence, if a physician could save a human life, it is worth more than thousands of gold”. As such, Sun Si Miao, who devoted his life to bringing benefits human lives, entitled his 2 books “Essential Formulas for Emergencies (Worth) a Thousand Pieces of Gold - (千⾦金要⽅方 Qian Jin Yao Fang)” and "Supplement to the Formulas of a Thousand Gold Worth - (千⾦金翼⽅方 Qian Jin Yi Fang)”. Not only is Sun Si Miao a remarkable physician, he is also a Taoist Priest who contributes significantly to Taoism. Sun Si Miao has been proclaimed as Taoist immortal and King of Medicine. There are also Hall of King of Medicine at many Taoist temples to commemorate Sun Si Miao. INTRODUCTION TO SUN SI MIAO - THE WAY TO MAINTAINING AND KEEPING GOOD HEALTH According to historical records, Sun Si Miao was born in 581(541) AD and died in 682 AD (a ripe old age of between 101 - 141 years). It is only natural that people are curious of Sun Si Miao’s longevity. Sun Si Miao came from a poor family. His parents farmed and weaved for a living. Sun Si Miao was a sickly child; his parents spent almost all their resources on treating his illness. Suffering from poor health as a child had made Sun Si Miao determined to be a physician when he grows up. Sun Si Miao’s longevity is due to his advocation of healthy techniques / methods. Sun Si Miao combined the good health maintenance path from Confucianism, Taoism and Buddhism with Traditional Chinese Medicine (TCM) theory of health preservation. There are still many feasible methods and techniques listed in his book, “Supplement to the Formulas of a Thousand Gold Worth (千⾦金翼⽅方 Qian Jin Yi Fang)” that are still applicable to our modern society today. Such as : 1. Maintain a balance healthy attitude, avoid aimless pursue of fame and wealth; 2. Refrain from excessive drinking and eating. Less meat more vegetable. More small meals. 3. Exercise to ensure good blood circulation. Eg. Flowing water does not rot; Constant activity prevents decay. 4. Maintain a healthy and regular lifestyle. Avoid extravagant and sumptuous lifestyle. ! 5. Take good care of personal hygiene. Sun Si Miao advocates that by following the above mentioned simple rules, longevity will follow suit. September - November 2014 TIPS ON HOW & WHAT TO EAT FOR A BEAUTIFUL YOU by Life!Clinic’s Physician Jean Ang Unless you were born with perfect skin, tender loving care is required to upkeep its condition and prevent it from premature aging. Slapping and piling on various expensive super-rich creams and serums with “results-promising” claims will not miraculously redeem the harm done to your skin if no effort was made to maintain its well-being. I strongly believe that beauty is from the inside out. Did you know you could actually eat your way to a more lustrous and healthier complexion? What, and more importantly, how to eat then? 1. 2. ! 3. ! 4. 5. 7 to 9 am: Breakfast According to TCM, this period belongs to our Stomach Meridian. It is the best time for breakfast. Unfortunately, it does not mean that you can consume whatever you relish. Breakfast is so termed because it is the first thing we take in after at least six hours of abstaining from food, therefore whatever goes in your mouth would be most absorbed by the stomach. Choose oily stuff for the first meal of the day and you will soak up the maximum oil imaginable, and it will most likely be reflected outwardly on your skin. Oil ignites fire and might cause you to suffer from bad breath, ulcers, cracked lips, oily T-zone and scalp and even chronic acne problem. Cereal or whole grains are preferable, as they are easily digestible and kinder to your digestive system. ! 9 to 11 am: No food. Just sips of lukewarm water The Spleen Meridian governs this couple of hours. While your Spleen zang organ does its work of converting breakfast into nutrient and sustenance for your body, it is not recommended that you put a strain on it by wolfing down what was meant for 7 to 9 am. TCM theory also infers that the condition of your skin reflects the state of your Spleen, so give it (and your Pancreas) a brief respite. Let your complexion be rejuvenated and allow your cells to be hydrated by drinking sips of lukewarm water. 11 am to 1 pm: No hot and spicy It is not recommended to “fire up” your Heart zang organ with hot and spicy food that “burns”. In addition, it is not advisable to exercise during this period as qì and blood “fills” the Heart Meridian at this time, risking the predicament of “blood not returning to the channels” when carrying out vigorous activities. Lunch from 1 to 3pm The small intestine is a long and convoluted tube, part of the digestive system and it absorbs about 90% of the nutrient from the food you eat. In TCM, 1 to 3 pm is the Small intestines Meridian’s rule. In general, lunch should be relatively more substantial and wholesome than dinner, but 70% full should suffice. Less meat, more grains and vegetables. ! If you are prone to water retention, refrain from gulping water from 9 to 11 pm YOU ARE WHAT YOU EAT, AND IN THIS CASE, HOW YOU EAT AS WELL. September - November 2014 SLIP DISC BY DR HEE HWAN TAK Slip or herniated disc is a condition whereby a major portion of the disc extrudes beyond its normal boundary, leading to physical compression and chemical irritation of the nearby nerves. The main complaints will be buttock pain “shooting” down the affected leg, perhaps accompanied by other symptoms e.g. numbness, “pins and needles”, or rarely weakness. ! In general, if a patient's leg pain due to herniated disc is going to get better, it will do so in about six weeks. As long as the pain is tolerable and the patient can function adequately in his or her daily chores, it is usually advisable to continue with nonsurgical treatment alone. Nonsurgical treatment will consist of physiotherapy, acupuncture, medications to relieve the inflammation, and perhaps a brace. ! If the leg pain does not get better with nonsurgical treatment, then microdiscectomy (type of minimally invasive spine surgery) is a reasonable option to relieve pressure on the nerve root and speed the healing process of the nerve. Immediate spine surgery is only necessary in cases of bowel/ bladder incontinence or progressive nerve damage, e.g. worsening and persistent numbness and/or obvious weakness of the leg. ! In most instances, a microdiscectomy is typically recommended for patients who have: • Experienced leg pain with or without other leg symptoms for at least six to eight weeks • Not found sufficient pain or symptom relief with conservative treatment After six to nine months of symptoms from the herniated disc, the results of the spine surgery are not quite as favorable. Therefore, it is not generally advisable to postpone microdiscectomy surgery for a prolonged period of time (more than six to nine months), especially if the symptoms do not improve by then. In microdiscectomy, a small portion of the bone over the nerve root and/or disc material from under the nerve root is removed (laminotomy) to relieve nerve compression and provide more room for the nerve to heal. Microdiscectomy is per formed with the aid of an operating microscope, which enables illumination and magnification of the structures being operated on, 290 Orchard Road #07-11/12 hence greatly increasing the safety of the surgery. An improvement of this technique is microendoscopic discectomy, where we use both the microscope and the endoscope. The incision using the microendoscopic technique is smaller (approximately 2 cm), causing less trauma to the surrounding tissue. A smaller incision allows for reduced post-operative pain and a faster recovery. ! At the end of the surgery, we may apply adhesion barrier to the exposed nerves. Adhesion barriers coat surfaces that are exposed during the decompression surgery, providing a temporary, protective physical barrier that isolates the exposed nerves from the surrounding tissues. These barriers function by various mechanisms: • They can prevent adhesions from forming, so that the nerve is not “stuck” to spinal structures or compressed by scar tissues. They can limit the exposure of nerves to chemical irritants that are produced during surgery. Usually, this surgical procedure is performed with one overnight stay in the hospital. Post-operatively, patients may return to a normal level of daily activity quickly. The patient is restricted from bending, lifting, or twisting for the first eight weeks following surgery. It is normal for a patient to experience postoperative pain, such as back pain and various leg symptoms. These symptoms will usually improve as the nerve heals and inflammation of the nerve decreases. Patients are given pain medications, antibiotics, and vitamin B during the healing process. • The success rate for microendoscopic discectomy spine surgery is approximately 90% to 95%, although 3% to 7% of patients may develop a recurrent disc herniation at some point in the future. This risk is usually higher in the first few months, and hence the rationale for restricted activity in the first 2 months, and gradual return to sporting activities thereafter.
