CBCHS Feedback Form Details of your Compliment /Complaint CBCHS recognises that you, as a client, have the right to compliment our staff or services, as well as have any complaints resolved. Your comments will in no way disadvantage you or jeopardise the care you receive by CBCHS. Complaints Procedure A complaint can be made regarding any aspect of CBCHS. This includes administrative practices, the physical environment and equipment, as well as your concerns about the care or treatment you received, which may include the staff person’s behaviour. If you don’t wish to raise a complaint directly with a staff person involved, then you may wish to write to the CEO, who will instruct the relevant manager to investigate and resolve your complaint. All complaints are dealt with confidentially. Compliments Procedure If you wish to acknowledge a particular person or service received, we encourage you to express your thoughts in writing. Your compliment should be addressed to the CEO, who will inform the particular staff person. Your compliment may be tabled at the next Board of Directors meeting.
