What are You Looking for in a Cushion?
For anyone who has to spend any extended time on a cushion, the first concern will always be tissue integrity.
We all know the discomfort of sitting for more than 20 minutes on a theatre or restaurant seat where the foam has
seen better days, or sitting on a plastic or vinyl-covered seat on a warm day with nowhere for the perspiration to go.
Tissue Integrity
Friction
One of the most frequent causes of tissue damage occurs during transfers
on and off the cushion. Use of a smoother water-resistant cover rather than
a mesh cover, reduces the risks from friction. On the other hand, people do
not like very slippery surfaces, since they find it difficult to keep their posture.

Shear
Shear is a much more complicated matter, and reflects on what is happening
between the layers of the skin, and to the individual cells in the tissues of
the backside. Current thinking is that differential pressures across a skin
cell can distort it to the extent that the cell membrane is compromised,
and cell contents thereby become unbalanced, leading eventually to cell
death. This has a more immediate damaging effect than occlusion of blood
vessels affecting nutrient perfusion of the cells. To reduce the potential cell
damage we want a cushion that will spread the forces on the skin as broadly
and evenly as possible.
Fail Safe
When selecting a cushion consider the consequences for the individual,
and the risk of tissue damage, in the event of failure. Pure air cushions, if
punctured, can leave the user without any protection. Fillings that move
out of place can leave the ischials exposed. Foam that develops a set will no
longer provide immersion. Etc.
Microclimate - Moisture
Of the factors that can lead to pressure ulcers developing, the most common
ones are those that affect the stratus corneum and epidermis, the outer
layers of the skin. Apart from the effects of friction mentioned above, the
microclimate is the next most important. It is important that the cover of a
cushion is breathable so that water vapour from perspiration can wick away,
and for air to circulate. Moist skin will undergo greater frictional forces than
dry skin, and therefore it is important to maintain a microclimate around
the skin that reduces this risk.
Microclimate - Temperature
Sitting on a cold material that takes time to reach body temperature
will cause blood capillaries to close down, and affect the nutrient supply
to the cells of the skin. On the other hand, one degree Celsius increase in
temperature lifts the metabolic rate by 13%, and thus if the skin tissues
heat up, there is higher demand for oxygen and nutrients, and a greater
need to remove metabolites. A cushion cover, and cushion materials that
allow for temperature regulation within a limited range will obviously be
beneficial to the user: not cold to sit on, and no tendency to heat up with
time. Good airflow around the skin is the sign of a good cushion design.

Pressure – Immersion and dispersion
Where the factors listed above affect the outer layers of the skin and can
lead to Stage 1 and 2 ulcers, pressure differences have a greater impact
on deeper tissues especially around bony prominences such as the ischial
tuberosities. Since altering an individual’s mass is not a ready option, the
best way to reduce the forces around the deep tissues is to spread the
forces over a wider area underneath the buttocks, displace more under the
thighs than under the pelvis, and take forces where possible under the feet,
forearms, and back. To achieve the spread of forces over a wider area we
look for greater dispersion, and this can be achieved by increased immersion
into a cushion, and this is best achieved by air dispersion in an air-filled or
air-foam cushion.
Be careful when considering using a cushion that has been contoured to an
individual’s backside. For example, following a Spinal Cord Injury (SCI) the
buttock tissues will atrophy over the first few months after injury, so what
was a good fit for optimal immersion, after a while will cease to be a good
fit. What happens next is that fatty deposits build up in the skin tissues,
and over the next few months the tissues will grow to a greater volume
than immediately after the injury, and the ‘overflow’ around the contoured
shaping puts damaging stresses on the buttock tissues in these areas.
The thigh tends to be much larger at the hip end (proximally) than at the
knee end (distally). Thus the natural biomechanics of the positioning of the
femur mean that to get pressure redistributed evenly along the thigh, the
feet need to be dropped close to the floor, or else you need a wedge under
the thighs (see Fig. 1 3 ).

Avoid the temptation to achieve this by putting ‘dump’ into the seat, since
this will put the individual into a posterior tilt, lead to a kyphotic spine, and
impair physiological function due to restriction of the abdominal space.

Likewise, it has been found that adding gluteal support at the rear of the
cushion into this softer tissue can take pressure from the bony parts of the
pelvis – this also has postural benefits, which brings us to:

Posture
To discourage the development of a kyphotic or scoliotic spine, we try to have the pelvis in a neutral
position, and minimise any obliquity.
Thigh positioning
The natural position for the femurs to rest in the hip joint is five degrees of
abduction.

It is important that sling seats are not allowed to bow since this tends to bring
the knees together. A crescent infill, or a firm base under the cushion help to
overcome this tendency in the seat.
Pelvic positioning
The design of a cushion to envelop the pelvis will help to stop the pelvis sliding
out of a neutral position. Anteriorly, a pre-ischial ridge, (see Fig 1 2 ) which
should be no more than 0.5 cm in height to avoid undue pressures to the anterior
of the ischials, helps to stop the pelvis slipping forward as the individual relaxes
into a posterior tilt. The benefit is lost without a PSIS block to stop the pelvis
rotating posteriorly (see Fig. 1 4 ).
Laterally and posteriorly there should be support to help control lateral or
rotational tendencies. The lateral supports can also provide a trochanteral ledge
to help offload forces from the ischials.
To deal with obliquities, ideally the cushion can be built up more on one side or
reduced in height on the other. In some cases this will be to accommodate the
deformity, and in others, where there is flexibility, it can help to decrease the
degree of asymmetry over time.
Stability
To maintain a good posture, and to be able to achieve day-to-day activities, it is
important that the cushion is stable and supportive. This has to be balanced with
any affects that components of the cushion may have on transfers.

Functionality
A cushion needs primarily to give protection to the tissues and to provide for good long term posture, but
has also to be functional. The following section looks at what most consumers feel are important aspects.
Cushion Weight
The weight of the cushion is important – if you obtain a lightweight chair for
ease of propulsion, then there is no point in negating this with a heavy cushion.
In addition, when moving the cushion in and out of the chair, the user can do
without strain on the shoulders caused by moving a heavy weight. Before the
final selection of the cushion the user should test out this aspect of functionality.
User weight limit
Some cushions are designed to work within only a narrow range of user weights,
while others have no upper weight limit.
Comfort
Crane (2004) found that the most important criterion for users was comfort. The
challenge with this is that comfort is so subjective, and will be built up from a
combination of feelings e.g. softness, stability, support, security, etc.
Cleanability
Are the covers washable at temperatures high enough to kill microbes (>60°C)?
Note: it is recommended that users obtain two covers as a minimum so that
there is one for use while the other is in the wash. Medical CE marking regulations
include the requirement that the Instructions for Use for the cushion include
washing instructions, and this includes the cushion components as well as the
cover.
Low maintenance
The user or carer has enough to handle in the day, and so looks for easy and/
or low maintenance. People do not want the hassles of resetting components,
finding adjustment tools, etc. They just want the cushion.
Accessibility
The materials used to construct a cushion need to assist the user in his/her
daily activities (access, transfers, etc). We need the right balance of contouring,
stability, and cover smoothness for that individual.
Vibration dampening
One of the phenomena that users find fatiguing is the vibrations transmitted
through their seating system, and thus it is better to select a cushion with higher
vibration dampening (one of the ISO 16840-2 tests). (DiGiovine et al, 2000)
Durability
Many seating materials perform very well on the day of purchase, but lose
their performance over time. For example, new foam is a very good material
for immersion, but over a relatively short time can develop a ‘set’ which stops
dispersion of the sitting pressures. The new ISO standard, ISO 16840-6 addresses
the changes of properties with use, and takes an important new line on classifying
cushions, namely by type of use (e.g. for sports, in a humid climate, in a hot
climate, etc) and not by their materials, ‘high’ risk, postural etc, the latter are
criteria used by commodity oriented purchasing bodies.

1. Air-Foam Floatation
The functional areas of the cushion are created by three types of foam;
soft foam under the ulcer-sensitive area of the ITs; medium foam for
the thigh trough, pelvic bucketing and pre-ischial bar; and firm foam to
provide support around the perimeter and at the medial thigh separator.
2. Positioning Support
Positioning supports improve stability, which is important for users with
compromised sitting balance or who transfer frequently. Multi-stiffness
foam provides support as the user is immersed in the cushion. Medial
and lateral supports minimise internal and external lower extremity
rotation. Pelvic bucketing and a pre-ischial ridge help maintain the pelvic
position, and thigh troughs promote a natural femoral position.
3. Valves for Adjusting Air Volume
The Evolution PSV uses Varilite’s patented Pressure Setting Valve. The
PSV is marked with three pre-set positions, which provide the best range
of immersion for most users. The PSV works by sensing the amount
of internal cushion pressure and automatically shutting off the flow
when it senses enough pressure has been released. The Evolution is also
available with a traditional two-way Varilite valve.
4. Cover
The Evolution comes complete with either a mesh or water-resistant cover
made from breathable two-way stretch fabric. Reticulated foam inside
promotes air exchange and improves pressure redistribution. The
Evolution cover is machine washable, tumble dryable and meet the
ISO 7176-16 fire ignition resistance standard.
5. Wedge
A modifiable closed-cell foam wedge is available with each Evolution, to
assist pressure redistribution from the sensitive ITs onto the thighs.

1. Air-Foam Floatation
The functional areas of the cushion are created by three types of foam: soft foam under the pressure risk area of the ITs;
medium foam for the thigh trough, pelvic bucketing and pre-ischial ridge; and firm foam to provide lateral support and a
medial thigh abductor.
2. Dual Chambers
The dual-chamber design of the Meridian allows for independent positioning of the thighs and pelvis. Adjustments
are quick and easy, and can be performed with the client seated. The front and back air chambers of the Meridian
limit sacral sitting and promote good posture. The pre-ischial shelf created by the dual-chambers stops the ITs
from sliding forward, thus limiting a posterior pelvic tilt and sacral sitting. The ‘squeeze’ created by the separate air
chambers provides postural support by ‘holding’ the client in place.
3. Valves for Adjusting Air Volume
Easy to use, two-way air valves allow simple and efficient adjustment. Once the client is seated on the fully inflated
cushion, the right valve can be released allowing air to escape from the pelvic chamber. Once proper immersion of the
pelvis has been achieved the user releases air from the front/thigh chamber by opening the left valve. Once optimum
load on the thighs has been achieved the valve is closed.
4. Cover
The Meridian is available with a mesh or water-resistant cover with breathable two-way stretch material. Reticulated
foam inside promotes air exchange and improves pressure distribution. The underside is a rugged nonslip material

Benefits of the Wave Bases:
The standard Evolution and Meridian cushions provide:
A flat seat base under the ischials, which encourages a neutral pelvis and a lordotic spine
(compared with an angled base which encourages posterior tilt and a kyphotic spine); a
pre-ischial ridge, which reduces the risk of the pelvis sliding forward and rotating into posterior
tilt; a ramp under the thighs that takes pressure away from the bony prominences onto safer
weight bearing tissues. A PSIS block behind the pelvis will work with the pre-ischial ridge and
an appropriately positioned postural support device (belt) across the thighs, to control pelvic
posterior tilt.
The Meridian and Evolution Wave Bases provide:
A family of contoured bases for positioning support. Clinicians can combine Meridian and
Evolution adjustable Air-Foam Floatation skin protection cushions with a solid ‘enhanced
positioning’ base.
Three different Wave designs are available:
• CPW (Contoured Positioning Wedge)
• LPB (Lateral Positioning Base)
• CPB (Contoured Positioning Base)
These bases enhance the Meridian’s and Evolution’s natural positioning benefits in the
sagittal (anterior - posterior) plane and provide further lateral and medial positioning support
(as viewed in the frontal plane) for both the pelvis and the legs, at the same time permitting
greater envelopment of soft tissues, and thereby providing wider pressure redistribution away
from the bony prominences, of the ischial tuberosities (ITs). Increased trochanteral and gluteal
off-loading of forces on the ITs is a further benefit.

Wave Contoured Positioning Wedge
Lateral thigh supports position the lower extremities and control abduction of
the thighs. The medial thigh support provides the correct amount of abduction
and controls adduction of the thighs. The Wave CPW increases the weight
bearing load on the lower surface of the thighs and decreases the load on the
buttocks, without changing the seat-to-back angle. The closed-cell foam will not
absorb fluid and is resistant to bacteria.
Meridian and Evolution Wave LPB (Lateral Positioning Base)
The Meridian and Evolution Wave LPB provide a solid base of support and a high
degree of pelvic and lower extremity lateral and medial positioning.

1. Contoured Base
Closed-cell foam base will not absorb fluid and is resistant to bacteria. The base can be cut with a knife or blade to
accommodate the individual needs of a client. Laterals and deep ischial pan provide maximum immersion and pressure
redistribution. Moulded thigh troughs improve lower-extremity positioning. Flex Hinge allows the base to flex when a
Modifiable Wedge is placed underneath.
2. Air Foam Floatation Cushion
Designed for addressing pelvic obliquities and other asymmetrical conditions, our exclusive Air-Foam Floatation cushion
contains two self-inflating, independently-adjustable chambers. Independent research has shown Varilite’s Air-Foam Floatation
to be the #1 in vibration dampening (DiGiovine et al 2000)
3. Modifiable Thigh Cushion
The closed-cell foam Thigh Cushion contours over the medial thigh separator and anterior laterals for added comfort
and stability. The modifiable Thigh Cushion is easy to modify with a blade or knife to match changes made to the
Contoured Base. Non absorbent and washable.
4. Valves for Adjusting Air Volume
Easy to use two way air valves allow simple and efficient adjustment. With a simple turn of the valves, adjustments can
be made to the independent right and left pelvic chambers, releasing air to immerse the user in the foam. The foam
contours to the user and the user’s weight is distributed over the surface of the cushion.
5. Cover
The ProForm NX is available with a mesh or water-resistant cover with breathable two-way stretch material.
Reticulated foam inside promotes air exchange and improves pressure redistribution. The underside is a rugged
nonslip material with hook and loop for added security. The ProForm NX cover meets the ISO 7176-16 ignition
standard, and is machine washable.
6. Modifiable Wedge
A modifiable closed-cell foam wedge is standard with each ProForm NX. The modifiable wedge increases the weight-bearing
load on the back of the thighs and decreases the load on the buttocks, without changing the seat-to-back angle. The 5cm (2”)
tapered wedge trims easily for custom fitting.

Cheat Sheets (Order Code - 02102)
Varilite Cheat Sheets are adhesive-backed sheets of closed-cell foam. Each
package contains four pieces of foam measuring 25cmx25cm (10”x10”). Cheat
Sheets can be cut with a blade or scissors and applied to a variety of surfaces.
Simply cut the desired shape, remove adhesive backing and apply the Cheat
Sheet to a clean, dry surface. Layer as needed and round off corners and edges.
Once the desired shape has been achieved, apply a final, smooth layer.

Solid Inserts
The Varilite Solid Insert counters the effects of sling upholstery. For some wheelchairs users, sling upholstery
promotes internal rotation of the lower extremities, sacral seating, posterior pelvic tilt, loss of natural lumbar lordosis
and obliquity of the pelvis. Such poor positioning can result in discomfort and long-term problems. A solid insert offers
an easy and cost effective solution to the problems of sling seating. The Varilite Solid Insert is made from durable
6mm (1/4”) plywood and is available in paediatric through adult sizes. It can be placed between the cushion and
upholstery, or, preferably, inside the cushion cover. With rounded edges and a smooth, sanded surface, there is no risk
of splintering or puncturing the cushion. All Varilite cushion covers are sewn with room for a Solid Insert.

The Varilite Original
Allowing Maximum Postural Adjustment
Varilite’s original cushion, over 20 years ago, was the Modular Seating
System, where on a sculpted base, you have a choice of 2, 3, or 4 air
filled compartments from which air can be released differentially to
compensate for, or make adjustments to, various postural discrepancies.
Thus accommodation can be made for pelvic obliquities, above knee
amputations, etc. Adjustments are quick and positive for reshaping or
repositioning as often as necessary. Transfers are facilitated in that air
can be released from front cushion(s) to decrease any ramping effect, and
then reintroduced by the cushion’s self-inflating capacity once the user is
reseated. No fuss. No pumps. Fail-safe.

1. Air-Foam Floatation
Functional areas of the cushion are created by three types of foam; soft foam for the decubitus-sensitive area of the
ITs; medium foam for main seat support area; and firm foam for the perimeter.
2. Positioning Supports
Extra-firm, bevelled foam increases lateral stability and sitting balance, while a large ischial pan protects the pelvis in both
anterior and posterior pelvic tilt positions. There is no medial thigh separator, therefore lower extremities can come into
midline for better fit in a tapered chair.
3. Valve for Adjusting Air Volume
The Zoid is supplied with Varilite’s revolutionary Pressure Setting Valve. The PSV is marked with three pre-set
positions, which provide the best range of immersion for most users. The PSV works by sensing the amount of
internal cushion pressure and automatically shutting off the flow when it senses enough pressure has been released.

1. Air-Foam Floatation
The Reflex foam is perforated in the sacral-ischial area for maximum protection of the pressure sensitive area of
the ITs. The Reflex provides medium density foam for the thigh trough, pelvic bucketing and pre-ischial bar, and
firm foam to provide lateral support and a medial thigh abductor.
2. Fabric
Reflex foam is bonded to puncture and water resistant polyester fabric for strength and durability.
3. Self-Setting Valve
A built-in air-release device automatically releases a portion of air when the user sits on the cushion. The air-release device
allows a fixed amount of immersion and limits bottoming out. When weight is taken off the cushion, it automatically
reinflates.
4. Cover
The Reflex features a tailored, removable, water-resistant cover. The underside is a rugged nonslip material with
hook and loop for added security. The cover is machine washable.

The Raft Wheelchair Seat
Sitting in a wheelchair for a prolonged period of time can become uncomfortable,
so comfort is an important factor for wheelchair users when choosing a seating base.
The Raft Wheelchair Seat’s contoured design, with the innovative port and pillars
technology, makes it particularly comfortable to sit on over extended periods.
The innovative ports and pillars technology is not only comfortable - it promotes continuous
air flow and creates a seating base that is breathable, giving cool comfort and helping to
protect the skin against two of the important factors that contribute to skin breakdown –
heat and moisture.
High peak pressure, particularly under bony
prominences, such as at the base of the pelvis, is a
major cause of discomfort and is another important
factor contributing to skin breakdown.
When pressure-mapped, the Wheelchair Seat was shown
to reduce peak pressures by 70%. This remarkable degree
of pressure reduction is achieved by the way the posts and
pillars of this innovative design yield, without collapsing,
to redistribute weight and therefore pressure.
Good posture is vitally important to seating comfort
and the Wheelchair Seat offers extra medial and lateral
contouring to support and maintain the position of the
thighs, as well as differential immersion into the base.
The immersion pillars of the Wheelchair Seat under
the tail bone and pelvis are structured to help position
and hold the pelvis correctly, and maintain the natural
curvature of the spine in sitting. The effect of these is
to improve posture and align the spine. Correct spinal
alignment and improved posture reduce muscle tension,
which in turn reduces back pain.
The Wheelchair Seat is made of waterproof, closed cell,
EVA foam, and can be machine washed in a dishwasher,
withstanding temperatures of up to 80°C.

The Raft Posture Seat and Posture Seat Plus
The Raft Posture Seat and Posture Seat Plus bases are designed using innovative ports
and pillars technology. The design promotes continuous air flow and creates a seating
base that is breathable, giving cool comfort. The pillars distribute weight and reduce
pressure on the spine that otherwise can lead to back pain. The pillars also contribute
towards maintaining good posture, helping to eliminate the poor posture and associated
pain that can result from long hours in front of a computer or behind a wheel.
Air flow
Air flow helps to keep the skin cool and lets it breathe. Heat and perspiration
are dispersed to give cool comfort and help protect skin. Hot, sticky seats
can be a thing of the past.
Weight Distribution
The Posture Seats have 49 pillars. Each pillar yields, but does not
collapse so that weight is more evenly distributed across the seating
surface. The base of the spine comfortably accommodated by
immersion pillars that contour and cradle, spreading the load and
relieving pressure on the hips and tail bone.
Good Posture: Reduced Back Pain
The immersion pillars under the base of the spine within the Posture
Seats allow the spine to settle in a natural position, while the immersion
pillars under the buttocks allow the pelvis to tilt forward. The result is
a well-balanced spine with natural curves and relaxed muscles, and a
reduction in hip and back pain arising as a result of poor posture.
Washable and Heat Resistant
The Posture Seats have a non-porous surface which makes them
waterproof. They can be washed in a washing machine – or even a
dishwasher! They can withstand water temperatures up to 80 degrees
Celsius. The Raft covers are also machine washable.

Head Supports
If you are looking for head supports to cater for clients with different degrees of head

Upper Body Positioning
Being supported by a comfortable back is normally a user’s first priority. It is also important
to provide posterior and lateral support to clients with poor trunk control. Weak trunk
muscles could mean that an individual trying to reach, for example to fulfil normal daily
activities, might not be able to return to a neutral balanced position. Dynamic secondary
supports such as harnesses are valuable aids to assist this movement, providing a certain
degree of elasticity that helps the torso to return to a neutral position. For clients that
require upper body support and positioning, BES offers ranges of back supports, laterals,
shoulder harnesses, and chest straps that cater to most needs.
Pelvic Positioning
Sitting is an activity which should ideally allow for dynamic and smooth movement of
the pelvis, excellent forward and lateral reach, and pelvic stability in order to maximise
functionality. It is very important to position and support the pelvic region correctly, as a
badly positioned pelvis can lead to the development of spinal deformities, and vice versa.
BES offers a range of secondary support devices for better pelvic positioning, including

Warranty
All VARILITE products have been carefully inspected prior to
shipping. All belt and harness packaging bags and cushion
tags have been marked with serial numbers that correlate to a
specific history of materials and workmanship. All products are
warranted against manufacturer’s defects for two years from
the original date of purchase with the exclusion of covers which
are warranted for one year. This warranty is limited to the repair
or replacement of components and does not cover incidental
or consequential damages. This warranty is only valid for the
original purchaser of the product.
Should a product be found defective under this warranty, we will
repair it or replace it, at our option, free of charge. Alteration
of the components, abuse (punctures, cuts, burns or abrasions),
misapplication, normal wear, or failure to close the valve are not
included. If a returned component is considered damaged due to
alteration, abuse, or normal wear, you will be contacted.
If a problem arises with your VARILITE product, please call us,
for a return under warranty, you will need a Return Authorization
number.

Past due invoices will incur finance charges of 10% late
payment surcharge and interest of 3% per month or part of
