PRIMARY IN-HOME SERVICES Skilled Nursing Comprehensive nursing assessment, treatment, teaching and counseling Pre- and Post-Surgical Care Identify Long Term Care needs PRI & Screen Restorative Care, Case Management Rehab Therapies/Care Multi-Disciplinary Rehab Care • Physical Therapy • Occupational Therapy • Speech/Language Pathology Rehab-trained home health aides Case Management Falls Prevention/Safety Assessment Medical Social Work Financial Counseling and Advocacy Emotional Support Long Term Care needs Linkage with community services Infusion Therapy Infusion Nurse Educators PICC/Midline insertion Maintenance of Central, Peripheral, PICC lines and Mediports Infusion of Antibiotics, TPN, Chemo, Cardiac meds, Hydration, other IV meds Lab draw via PICC/Central lines and Mediports Home Care Plus Home Health Aides to assist with activities of daily living, as required Lifetime Pharmacy - Infusion Services in the Home Working together to provide IV services benefits both patients and providers. For patients, in most cases there is no additional infusion pharmacy co-pay; 24/7 delivery; and less phone calls, scheduling and clinicians in the home. Providers have reduced phone calls and paperwork. SPECIALTY SERVICES Cardiopulmonary Care Nurse Specialists and specially-trained RNs who assess, treat and teach Common diagnoses: CHF, CAD, MI, COPD, dysrhythmias, peripheral vascular disease, post-cardiac surgery and pacemaker implantation Case management, interdisciplinary coordination, timely MD notification Remote monitoring, other technologies per orders Wound Care Certified wound, ostomy and continence nurse consults on cases by visit and/or digital photo review Chronic Wound Management using evidence-based guidelines Evaluation and referral for specialty beds and compression therapy Vacuum Assisted Wound Closure (VAC) Ostomy, fistula care and drainage management Diabetes Education/Care Certified Diabetes Nurse Educators and specially-trained team nurses instruct newly diagnosed patients in blood glucose monitoring, meal planning, exercise and medications Personalized care planning for chronic diabetic self-management including glycemic control, adaptive devices and coping strategies Joint Replacement Care Orthopedic Pre-surgery Review Multi-Disciplinary Rehab Care to speed resumption of normal activity • Physical Therapy • Occupational Therapy Cancer Care Medical and Post-Surgical Care Assessment, Treatment & Teaching Post Chemotherapy and Radiation At-home Chemo Infusion Identify Long Term Care needs Case Management Parkinson’s Disease Care Interdisciplinary team training through National Parkinson Foundation (NPF) with treatments including: • Evidence-based, targeted therapy interventions • Parkinson’s Disease education and connection to community resources • On-site Parkinson’s Disease exercise program led by a physical therapist Women’s and Children’s Health Pediatric Clinical Nurse Specialist Pre- and Post-Natal Care and Teaching Pediatric Assessment, Care and Teaching (post-acute and high-tech) High-Risk Pregnancy Management IV Nutrition, Hydration Linkage with CompassionNet, if needed Call 1-585-214-1000 or 1-800-724-1410 to learn more, or visit us at lifetimecare.org Leading-Edge Patient Care Technology Telemed Systems Advances in medical technology for the home can increase patients’ ability to monitor their own condition, reduce rehospitalizations, and improve collaboration between patients, physicians and Lifetime Care nurses. • Daily Home Monitoring of vital signs to identify changes in condition; reporting vital signs trends to physicians • Video Visits provide face-to-face contact with patients and physicians for high risk patients • Zoe Monitor measures fluid status on patients with CHF • State-of-the-Art Digital Cameras assist in wound care treatment Home Support Services Enjoy the independence of living in your own home, with a little extra help assuring your safety and reducing anxiety. • Lifetime Personal Health Alert is by your side in case of an emergency any time of the day or night • Dispense-A-Pill (DAP) ensures accurate and timely dosing of medications
