Adult Mental Health
Services
in Waterloo Wellington
Timely Access to Adult
Mental Health Services
Timely access to adult mental
health services are available from
WWCCAC for adults and seniors
who live in the Guelph and Waterloo
Region who have a major mental
illness as their primary diagnosis
or as a complicating factor of a
primary diagnosis.
Specialized community mental
health care coordinators and
hospital-based care coordinators
work with clients referred from the
community or from adult mental
health beds in area hospitals.
Care coordinators and service
providers with expertise in the area
of mental health, such as nurses,
occupational therapists, personal
support workers and social workers,
provide support services and work
in collaboration with individuals,
their families, other formal and
informal supports.
Eligibility Criteria
Client Goals
• Client is 18 years of age and older.
Clients are encouraged to achieve
their full potential through goaloriented outcomes, and to take
responsibility for their health and wellbeing. Although WWCCAC recognizes
that some clients require ongoing
support, the goal is to provide linkages
with community supports where
appropriate and available.
• Client has a Primary Mental Health
Diagnosis such as Schizophrenia,
Major Depressive Disorder, Bipolar
Disorder, Anxiety Disorders/Obsessive Compulsive etc.
• Client meets general criteria for
CCAC service.
• Client has ongoing mental health
issues requiring assessment and
intervention, rehabilitation and
recovery services.
Role of the Mental Health
Care Coordinator
Conducts assessments with client
recognizing the complexity of physical,
psychological, social and cultural
implications of mental health and
illness.
Develops a care plan in collaboration
with the client and family as well as
the formal and informal caregivers
(e.g. health care teams, community
supports, physicians etc.)
Supports the implementation of the
care plan and re-evaluates ongoing
needs.
Promotes education about mental
health and addresses issues of stigma
and discrimination which create
barriers to accessing health care and
services.
How Do I Get a Referral?
The CCAC program is accessed through
hospital and the CCAC intake line. If
you are in hospital please speak with
a hospital care coordinator, if you are
in the community please call the CCAC
