JULY 2007 FOR MEMBERS ONLY Unforgettable and Picturesque Terengganu The recently concluded 29th Congress of the MPA in the serene Primula Beach Hotel & Resort started with a sombre skit on palliative care as we do not want to see it being practised during the opening ceremony. Acted solely by nursing students, the audience was quick to forgive, but not forget with all the ‘backstage’ discussion that followed. It was a pivotal discussion point among all delegates. The rest of the congress was upbeat with exhibition booths crowding the tight hotel corridors, 403 registered delegates, an exciting social programme, a stimulating scientific discourse and personal memories of reunions spanning all three days from 8th to 10th June. The pre-congress workshop on epilepsy management on 7th June was exceptionally well-attended. Conducted by Drs Hussain Imam, Khoo Teik Beng and Wong Sau Wei with support from the other paediatric neurologists, active participation from the more vocal of the paediatricians made the whole-day session lively. Starting with simple classifications, the workshop went into high gear after morning tea break and shifted even higher after lunch. Lucid video and EEG recordings of seizures were shown and participants were encouraged to speak and contribute to the discussion. The opening ceremony was officiated by the Chief Minister of Terengganu, who gave a frank outlook of the state and the plans that are afoot to improve the whole state to be at par with the best in the world. The honour of our annual Bill Marshall Memorial Lecture this year was given to Dr Catherine Peckham who talked on Prevalence and Pattern of Chronic Illness in Childhood. Let me start by saying a sincere and humble thank you to the members of the association for entrusting me with the task of serving as your president. As I had mentioned during our banquet night at Kuala Terengganu, it is good that the presidency has crossed the South China Sea, a symbol of MPA coming of age and truly encompassing the country, on the 50th year of our country’s independence, and the 29th year of our Association. Another area that we can and should get more active in is the arena of social service. The Association gives 10% of the profit from our annual conferences to the Malaysian Paediatric Foundation. This money has been fruitfully rechanneled into various training and educational activities over the years. As was discussed in the recent AGM in Kuala Terengganu, perhaps it would be good if the Association in conjunction with the Foundation, could increase its activities in the realm of social/welfare projects. In the past, we have given grants to medical volunteers who have participated in various medical relief programmes overseas. I would like to see this dimension of social conscience strengthened and developed. We have been blessed with bountiful finances as a result of successful annual conferences. We should now look more actively into giving to the community through more welfare oriented projects, either through the Foundation or directly through the activities by the various state committees. Firstly, I should thank our outgoing president Dr. Zulkifli Ismail for his impeccable term of service. He has been instrumental in the success of many key programmes of the Association, chief of which are the Positive Parenting and the Make It Safe For Kids programmes. Despite being new in private practice, he has invested a lot of his personal time and attention into the many activities these have involved. He will be a hard act to follow. The recently concluded 29th MPA Congress went on smoothly and successfully in picturesque Kuala Terengganu. In addition to an involving scientific program, we had many broad smiling faces toting shopping bags filled with batik, keropok and lekor. Many thanks to the local organising committee, and of course, the very able secretariat under the leadership of the tireless Datin Saadiah. We have successfully bidded for the 2009 International Association for Adolescent Health (IAAH) World Congress. This will be a major meeting on the world congress calendar addressing the issue of adolescent health. This comes at an appropriate time in the development of our services as we are in the midst of persuading the Ministry to officially extend the paediatric age coverage to 18 years. We will have to work hard over the next two years to make this congress the resounding success that we have come to expect from ourselves. What directions do I see for MPA in the coming years? I hope to see more active involvement by the various state committees. Each committee is given seed money of RM2,000 to start their activities at the local state level (with option for more funding on a need-to basis), but not every state has taken up this opportunity. Undoubtedly, the various paediatricians have been organising various activities on their own initiative at their local level, often in conjunction with various NGOs and government institutions. I hope that they will choose MPA as their partner in these activities. This will be a win-win situation whereby the administrative support and experience of the central MPA secretariat is made available to the various states, and in return, MPA is strengthened and made relevant to the medical community, and the community at large. That So, let’s all put our shoulders to the wheel and work at the challenging tasks ahead, or perhaps a more appropriate phrase for our present age would be Captain Kirk’s “engage warp engines, full speed ahead”.2 Soo Thian Lian President 2007-9 (continued on page 3) BERITA MPA – JULY 2007 2 Report We Are 7 Years Old! March 24th, 2007, Sime Darby Convention Centre, Kuala Lumpur Yes, we are seven years old and we celebrated the occasion with over 300 guests, which included members of the programme’s expert partners, representatives from caring corporations, young parents and parents-to-be. The launch ceremony featured the unveiling of the all new Positive Parenting Guide and website www.mypositiveparenting.org. In this regard, we find Positive Parenting an ideal complement to our efforts. Of interest to us is Positive Parenting’s efforts to inculcate skills and best practices in parenting, expert medical and nutritional information, injury prevention, coverage of common concerns like financial planning and children’s education, and even prevailing social issues, like divorce, communication breakdown and child abuse. Day 1 A very interesting session began with a presentation by Dr Kamariah, a traumatologist from HKL who enlightened us on the gory details of the cases that came to the emergency department. This was followed by ASP Hj Hamza from the police department who showed video clips of actual scenes of `Mat Rempit` in action. He gave a police perspective of the issue and how his department has managed to reduce the problem around Kuala Lumpur by strict enforcement. He is hopeful that the new bill to charge not only ‘Mat Rempit’ but onlookers as well will enable the police to handle the problem more effectively. This is because without admiration and adulation from their audience, there is no point in risking themselves doing all the antics! Another presentation by Dr Normah gave a very good profile of ‘Mat Rempit’ and what makes them tick. Jointly organised by: Adolescent Health Committee, MMA Malaysian Association for Adolescent Health (MAAH) Ministry of Health Malaysia (MOH) Lembaga Penduduk dan Perancang Kesihatan Negara (LPPKN) Malaysian Paediatric Association (MPA) Day 2 On the second day, our guest speaker Professor Susan Sawyer, Attentive audience of healthcare professionals and Professor in Adolescent Health from the University of Melbourne, adolescents Australia, shared her expertise and experience when she presented ‘The Adolescent with Chronic Illness’. Many adolescents with chronic illnesses can live up to adulthood with advances in treatment and this poses a problem when they reach adolescence. Not only do they have to face the challenges that normal adolescents have, but they have to contend with their own illnesses as well. Adolescents with chronic illnesses have a higher prevalence of risk taking behaviours. Addiction in the adolescent poses a serious problem. Not only are they more prone to addiction but their addiction tends to last into adulthood and currently substitution therapy has not been recommended for this group. The answer lies in prevention. He was very supportive of adolescent health and recognised that more needs to be done in this area of medicine and promised to support application from doctors to further their studies in adolescent medicine. His presence brought media coverage to our event. Datuk Dr Teoh in his speech congratulated the organising committee on the success of the symposium and pledged MMA support for all activities related to adolescent health as well as the formation of the Malaysian Association for Adolescent Health (MAAH). Dr Nazeli gave an overview of adolescent health issues in Malaysia. The real challenge is in addressing these issues. Currently there is a lack of reliable data, and solutions are knee jerk reactions to complex inter-related problems. There is a need for a central coordinating body to maximise limited resources. She hoped that with the formation of the Malaysian Association BERITA MPA – JULY 2007 4 Report adolescents at risk’. She was truly in touch with adolescents having been a teacher for many years and her presentation kept the audience glued to their seats wishing we could have more of her. for Adolescent Health (MAAH), there will be better coordination amongst all interested in adolescent health. There were three concurrent symposia addressing three groups of participants. The symposium on addiction and sexually transmitted diseases including HIV AIDS were targeted at medical personnel, and the Reproductive Health of the Adolescent Module (RHAM) targeted the adolescents themselves. The parenting skills workshop targeted councillors and teachers as well as parents. The audience participation in all symposia was encouraging. The other topics covered were ‘Beautiful Skin, Beautiful Image’ touching on skin care, posture and poise, in order to project the image of a confident and beautiful person. Weight and diet management was given by Ms Mary Easaw and Ms Wee. ‘Beautiful Mind and Soul’ was jointly conducted by Dr Aminah and Assoc Prof Haniki and ‘Beautiful Smile’ by Dr Hou. The 1st Adolescent Health Learning Market: Look Good, Feel Good The sessions were very well attended but unfortunately some speakers took too much time and thus there was less time for discussion. The adolescent health learning market was jointly organised by the MMA and the Malaysian Association for Adolescent Health (MAAH). The MAAH is a newly formed organisation whose members comprise doctors, paramedical personnel, councillors, parents and most importantly, adolescents themselves. In fact, anyone interested in adolescent health is a potential member of MAAH. The main objective of the 1st MAAH Learning Market was to help build the ideal image of the adolescents. A holistic approach to image building includes not only physical appearances but ‘feeling good’ as well. Ms Mary Easaw stressing a point on nutrition A Great Success! Overall the symposium was a great success with a record attendance of 347 participants from all over Malaysia including Sabah and Sarawak. The National Symposium on Adolescent Health has turned out to be an event that brought together all doctors and personnel interested in care of the adolescent. The participation of adolescents themselves gave a refreshing mood of youthful vigour to the event, a respite from the normally sedate structure of medical conferences. The committee worked very hard to make it a successful conference. The committee would like to record its thanks to the Director General of Health for making time to attend our function despite his busy schedule. Participants were divided into four groups and move from one `market` to the next to `pay` attention to the speakers and facilitators. These sessions aimed to be as interactive and informal as possible with free flow of ideas and discussion between the participants and facilitators. The first session given by Hjh Noor Rezan Bapoo, the Director of Student Health, Ministry of Education was very interesting and informative. She spoke on ‘Reaching out to A job well done and the organising committee has every reason to smile! Covering almost all aspects of children with chronic illnesses, no disease was left out and no subspecialty ignored. From the six selected, Dr Toh Teck Hock walked away the winner with his presentation on the the use of botulinum toxin for lower limb spasticity in Sibu District Hospital. The judges were unanimously impressed with the usefulness of the paper to our community. As always, the entertainment during all the dinners were good. Dr Syed Zulkifli led the crowd two evenings in a row with his antics, including getting a former MPA president, Dato’ Dr Azizi Hj Omar and the outgoing president, Dr Zulkifli to do a duet. Dr Soo Thian Lian, the incoming President, also had to sing. Our Treasurer, Dr Musa Nordin was not spared and he sportingly came on stage to sing his uniquely funny school song! Ballet and cultural dances provided the rest of the entertainment. As usual, our 29th congress was a success and managed to put Terengganu among the memories of our members. Credit is due to the local organizing committee led by Dato’ Dr Jimmy Lee, the scientific committee led by Dr Hussain Imam and the secretariat steered by Datin Saadiah Ahmad. We can now look forward to our 30th congress in Melaka before the World Congress of the International Association of Adolescent Health in 2009 that will coincide with our 31st annual congress. Till then, the memories of Terengganu as displayed on these pages will stay with many for some time to come.2 Huge crowds at the exhibition booths... Thank you for your e-mail regarding MPA's Pneumococcal Disease (PnD) Awareness Public Education Campaign. For your information, and that of other members, MPA Executive Committee made a decision to start on the awareness programme in 2006 based on the following reasons: 1. The sequelae of PnD can be devastating 2. With Haemophilus influenzae type b (Hib) vaccine in the National Immunisation Programme, pneumococcus as a major aetiology of meningitis in childhood is expected to increase in importance 3. There is a very low knowledge of Streptococcus pneumoniae or pneumococcus and the diseases that it causes amongst the public (only 3% of a population sampled knew or had heard about it in 2006) 4. To encourage the public to discuss the diseases caused by this bacterium with their health care provider 5. Almost all of us have treated patients with invasive pneumococcal disease and would hope to see less of it through the use of the conjugate vaccine. The public education campaign was discussed at our Executive Committee meetings and later during our Annual General Meetings in 2006 and 2007. It was approved during these meetings by a majority of the members. The first lot of advertising material was adapted from the Singapore Paediatric Society (SPS) Fight Pneumococcal Disease education programme. The SPS had their logo side by side with the Wyeth logo. In Malaysia, we made it very clear that the MPA logo is to stand on its own without any company logo next to it. The text of the media advertisement was also screened by key committee members, viz. President, Secretary and Treasurer with the rest being sent pdf copies via email, and had to be referenced. It aimed to inform about the disease and directed parents to discuss it with their doctors. With feedback from our colleagues and after the 2006 AGM, it was jointly decided to change the approach of the campaign. Hence the new advertisements to highlight that the presentation of pneumococcal disease can be innocuous and that both parents and doctors have to be vigilant and have a 9 Creating public awareness about pneumococcal disease is in line with other countries in the region. The Pediatric Society of Thailand had a big publicity blitz on television, radio and press with stronger proposal on the use of the vaccine by senior paediatric infectious disease specialists, the Philippine Pediatric Society had their UNITE for pneumococcal prevention public education programme and the SPS had the programme stated above. It is also in support of the World Health Organisation (WHO) Weekly Epidemiological Record No. 12, 2007, 82, 93-104 dated 23 March 2007. This report, reviewed and endorsed by WHO’s Strategic Advisory Group of Experts on immunisation, recommends that WHO works through national professional societies to inform the public on the availability of the pneumococcal conjugate vaccine in an effort to reduce the morbidity and mortality from this disease. WHO also considers that it should be a priority to include this vaccine in national immunisation programmes. All of us are concerned about the high cost of the 7-valent pneumococcal conjugate vaccine. In view of the manufacturing processes involved in production of the vaccine, the only way that the price can be reduced is to help move its use and sale. Without educating our colleagues and also the public, this will take a much longer time to happen. While we recognize that there will be parents who cannot afford to pay for the three or four doses required, we cannot assume that they are not willing to pay for the added protection conferred by the vaccine. We must remind ourselves that one of the main aims of the MPA is to advocate for and on behalf of children, and disease prevention through immunisation is one of the ways to achieve this. The author’s positive perspective drives home the point that special children are truly gifts to be treasured and with proper love and care, you can help them grow to the fullest of their potential. Look at the author’s list of famous people who’ve achieved greatness despite their disabilities (which stretches an astonishing nine whole pages!), that initial “Why me?” reaction may very well turn into “Why not?” Baby Handbook: Caring For Your Child’s Health From Day One 101 Questions You Forgot To Ask Your Paediatrician: A New Handbook for Parents Author: Dr Tan Poh Tin Pages: 280 pages Language: English Authors: Dr Iean Hamzah-Sendut, Professor Dr Zulkifli Ismail Pages: 163 pages Language: English The blurb on its cover “Current medical information made simple for parents” describes Baby Handbook to a T. Written as a medical reference book, the information is presented in a straightforward, almost textbook manner – ideal for those who prefer to get right to the meat of the matter (the medical matter, that is). Dr Tan takes pains to explain in laymen terms all the conditions parents may need to deal with and this covers the simple (bathing baby) to the more challenging (common and not-so-common childhood illnesses). Targeted primarily at parents, Baby Handbook provides much more detail than the average baby care book, in the hope that it answers the questions that doctors may not have time for. The verdict?
