Purchases must be made between April 1 – June 30, 2016. Redemption form must be received by
July 15, 2016. All documents needed must be received at the same time. This offer is only valid
in Canada only. Customers will receive a $75 check in Canadian funds. Please allow 6-8 weeks for

INSTRUCTIONS: Please follow the instructions listed below completely and accurately in
order to qualify for the rebate. To receive your rebate by mail from BAK, you MUST:
• Buy any BAKFlip or Revolver X2 truck bed cover between 4/1/16 and 6/30/16 from a
participating retailer/installer.
• Fully complete the mail-in redemption form
• Attach your original sales receipt*
• Attach the original UPC from the product
Mail all of these items (redemption form, sales receipt, UPC)
