Message from the Chair
Dear Friends & Supporters,
It is a privilege to share with you the amazing work and
exciting new opportunities that Donate Life Connecticut
encountered in 2012. Our mission to increase awareness of
organ and tissue donation and increase the donor registry is
always at the center of everything we do, and 2012 delivered
many innovative ways to carry out that mission. It is only
through your continued support that these opportunities are
made possible, and together, we are saving lives.
Donate Life Connecticut provides our state’s residents with a unique opportunity
to participate in awareness and outreach activities within our own communities,
with local transplant professionals, local transplant recipients, and local donor
families. We so often receive feedback that those exposed to our messages
are inspired to join the registry after hearing a story of how members in their
local communities were impacted by the miracle of transplantation or donation.
With nearly 19 of our neighbors, friends, and family members in the U.S. dying
each day while waiting for that life-saving transplant, it is increasingly critical
that we, as an organization, continue to provide this outreach.
We are incredibly honored to perform our work through the tireless efforts of
our Director, volunteer Board of Directors and volunteers, all of whom share an
unwavering passion and energy for sharing the message of organ and tissue
donation that is so close to our hearts. Together, we continue to save lives, and
we thank you for your continued support.

Nothing speaks louder for our organizational accomplishments than the
number of dedicated VOLUNTEERS that share our message to the public. NINETY
-ONE individuals gave their time to further organ and tissue donation
awareness in Connecticut!

Our Volunteers spent approximately 852 hours educating the public at 125
events around the state.

More than 4040 High School and College students were educated about the
importance that one decision can make in so many other lives.

This year, 77 Connecticut municipalities raised a Donate Life flag outside town
or city hall to call awareness to National Donate Life Month. Creating donation
friendly communities contributes to increasing the number of registered donors.

Donate Life Day at the
UNIVERSITY OF BRIDGEPORT
We celebrated the 2nd Annual University of
Bridgeport Donate Life Day this past
February. Head coach, Stephanie Del Preore
created the day to bring awareness to a
cause that helped lengthen her father's life
by seven years following a liver and kidney
transplant. The players wore green Donate
Life tees and bracelets, cheerleaders tossed
frisbees to the crowd and the UB community
raised $3000 for organ and tissue donation
public outreach!! Go Knights!
Donate Life Connecticut honored Coach Del
Preore for her outstanding donation outreach
through
athletics.
As
part
of
the
national initiative, to register 20 Million new
donors in 2012, special individuals from
athletic communities around the country
were honored for their ability to impact and
inspire others to consider registering as
donors.
Stephanie has certainly brought
donation awareness to the Southwestern

Training & Appreciation
VOLUNTEER SUMMIT
Donate Life Connecticut hosted our annual
Volunteer Summit at J. Christian's restaurant in
March. The half-day event was attended by fortyone individuals who were taught how to best
communicate their personal transplant or donation
story by Transplant Speakers International. This
nationally renowned public speaking organization
has trained thousands of Donate Life affiliated
volunteers how to best address the public to
consider registering as organ and tissue
donors. Attendees enjoyed a delicious hot lunch
and left with volunteers who are now ready to inspire their Conna made-in-Connecticut goodie
basket!
We are proud and thankful to our
prepared and enthusiastic ecticut neighbors to
save lives!
We Passed
LEGISLATION!
In June, Governor Dannel Malloy
signed a bill into law that Donate
Life Connecticut authored and
lobbied for. There is now a
Governor's Council of private and
public sector individuals who
evaluate best practices in other
states and look to apply them in
Connecticut to increase the
number of registered donors. This
Organ and Tissue Donation
Awareness Advisory Council will
meet quarterly and submit an
annual report to the Governor.
Many of our collaborative
pro-donation organizations and
dedicated volunteers took time to
give oral and written testimony on
the behalf of the bill. We thank
everyone for their support!
NATIONAL DONATE LIFE MONTH
and High School Arts Contest
Donate Life Connecticut's 2012
special guest speaker at the
annual
State Capitol event
kicking-off National Donate Life
Month was Jen Panzer. Jen spoke
eloquently about her deep
gratitude for her
donor, Angie
and her decision to donate life.
Angie saved Jen's life through her
gift of double lungs. At the event,
Jen was joined by Angie's parents
(Bruce & Jane) and Angie's heart
recipient (Tom). The close ‘family’
they have created from a great
tragedy
moved all who
witnessed their interactions.

This annual event is attended by many legislators, community leaders,
volunteers and high school students. The attending high school students
submitted artwork promoting the message of organ and tissue donation and
received awards for winning artwork in the Annual Donate Life Poster Contest.
Hundreds of students participated in the contest and were educated about
their opportunity to register as organ and tissue donors.
Flags Across America
The Flags Across Connecticut program (part
of the national program, Flags Across
America) grew by 20% since 2011. Seventyseven towns had at least one Donate Life
flag flying during the month of April! This
program honors those who gave the gift of
life through donation, celebrates the lives
saved and brings awareness to those who
still wait.
It is our hope that every
Connecticut town will fly a Donate Life flag
in the future.

BOWL FOR LIFE
The DiVincenzo family celebrates the life of their
young son, DJ, who was given a
heart transplant at the age of 6
months old. The family held their
3rd Annual Bowl for Life at the
Milford Lanes this past May! This
year they filled 30 lanes of
bowlers - of all ages! Special
prizes were given to randomly
selected bowlers from the Wheel
of Prizes! This year’s event was a
huge success again! Thanks to
the DiVincezo family!
BILLBOARD OUTREACH
Ten years ago, Peter Kupczak’s twenty-two year old daughter, Jessica passed
away during a severe asthma attack. In the midst of his unbelievable grief,
Peter supported his daughter’s last wish to be an organ and tissue donor.
Since that life-changing event, Peter has been a strong advocate for
increased donation education, inspiring others through his personal story.
Peter always looks for ways to increase donor awareness and had a unique
idea to create a billboard that would both honor his daughter and illustrate
what and who donation affects. He knew he had to raise approximately
$3000 to pay for the creation and rental cost of putting a billboard in the high
traffic area on Interstate 95 in Bridgeport.
Peter wrote a heartfelt letter to his friends and family asking them to support his
idea. The donations poured in. Not only was he able to fund the billboard, but
he raised an additional $3000 that he donated to Donate Life Connecticut for
our public outreach campaigns. Thank you, Peter!
20 MILLION in 2012
As part of the national campaign to register 20 Million Donors in 2012, national
Donate Life created the "I Am Hope" campaign. Each state had one week to
promote local stories across the country through social media. October 14-20
was Connecticut's week. Volunteer, Greg Novak donated his expertise and
time as owner of video production company, Our Past is Now (OPIN), to create
two beautiful public service announcements of local individuals impacted by
the gift of life. The first video profiled Dr. Emre, local transplant surgeon who
transplanted a liver into Jax, a young boy whose mother is a Real Housewives
of New York star Heather Thomson. The second video intertwined the stories of
a young Seymour boy, Aaron waiting for a life saving transplant and a young
Milford woman battling a disease that her own mother passed from, waiting for
a kidney transplant. These videos were shared virally across the country
through Facebook, Twitter, YouTube and other types of social media.
CELEBRATE
EAST LYME
Sandy Clarke has
coordinated Donate Life Connecticut’s participation in
the Celebrate East Lyme annual event for many years.
This event draws thousands of guests from around the
state. This year Sandy was unable to join the regular
volunteers - as she was recovering from her long
awaited lung transplant! Congratulations, Sandy!!!
5 BORO BIKE TOUR

Tour! Zvi raised over $3,000 for
his organization, our organization! Thomas Carley and friends

BRAD SHWIDOCK
Brad was one of our wonderful volunteers who proudly talked
about his liver transplants. He inspired others to consider
giving the gift of life. He was the driving force in Donate Life
Connecticut's participation in the 2009 Stamford Thanksgiving
Parade. He is survived by his loving wife and two daughters.

group invited Donate Life
Connecticut with the Red Cross
and Be the Match organizations

Donate Life Rocks
MIKE CUSATO BAND
Donate Life Connecticut hosted a Holiday
Party at the Black Bear in South Norwalk on
the last day of November 2012. The talented

provided rockin'
entertainment to the crowd who enjoyed
FUEL ROCK CONCERT
Donate Life Connecticut was invited to be the beneficiary organization for
104.1 RockFest 2012 with nationally known band, Fuel at the Black Bear in
Hartford. This event was both a fundraiser and awareness event. We hope to
be back Summer of 2013- and we hope you join us for a Rockin' good time!
5th Annual Toast To Life
Donate Life Connecticut held its 4th
Annual Toast to Life event at
Gouveia Vineyards in June. Guests
were treated to a beautiful sunset
while sipping on delicious wines. The
sold out event was attended by 200
Donate Life CT supporters who
allowed the event to raise over
$20,000.

awards were given to three
exceptional volunteers: Bruce
Adams, David DiVincenzo and Peter
Kupczak.

DMV Ambassador & Outreach Program
Donate Life Connecticut with
LifeChoice Donor Services has
created a new DMV outreach
program, the Rewards Program. Each Connecticut DMV Branch has at least
two volunteer ambassadors assigned to visit the staff thanking them for 'asking
every customer, every time' to join the Donor Registry. The Ambassadors bring
delicious treats and small trinkets (pens, pins) showing our appreciation to the
people who register more than 95% of the registered organ and tissue
donors. In addition to the Appreciation portion of the campaign, the branches
compete against each other to achieve higher and higher designation rates
(number of registered donors from the number of license transactions) each
quarter. The branches with most improved and highest overall rate receive
extra appreciation gifts including cakes, snack baskets or lunches. We are
closing out our first year of the program and have seen some dramatic
results. For example, the Wethersfield DMV had a designation rate of 33% in
early 2011. As we conclude 2012, the designation rate has shot up to
39%! Great work, DMV Ambassadors. We are looking to start a similar program
with the AAA offices in 2013.
ING Hartford Marathon
Donate Life Connecticut's involvement in the 2012 ING Hartford
Marathon was a HUGE success! The running team consisted of more
than 115 supporters who wore Donate Life shirts! Together we raised
over $25,000 and registered 17 new donors! Two of our runners were
chosen to be Elite Inspirational Runners - part of the VIP team! These two
amazing women were chosen to be part of a small team of individuals
who have overcome adversity with a positive outlook! Dara Brooks and
Leslie Schlemmer were part of this team! Dara donated a portion of her
liver to her infant son who suffered with liver failure. Unfortunately her
son, Tyler passed away a few months later and Dara then chose to
donate his organs to save other babies. Leslie unexpectedly suffered
liver failure as the mom of two young boys. Without receiving a liver
transplant Leslie’s prognosis was not good. Fortunately, a liver came in
time...and Leslie’s family was saved. Undoubtedly our Donate Life team
inspired others on the race course in our team shirts, in the participant
packets with the Elite, Inspirational Team member stories and with our
unwavering spirit of sharing life through organ and tissue donation.

Connecticut Donor Registry.

Connecticut’s Donor Designation Rate (the percent of registered donors 18+) of 41% falls short of the National
Goal of 50% of adults registered as donors. This is a 4% increase over 2011 but more work is to be done.
Financials:
Every small, volunteer driven, non–profit organization struggles to find effective
and diverse ways to obtain sufficient funds that support the mission of the
organization. Donate Life Connecticut continues to seek new methods to
broaden our community outreach activities to raise awareness of the
importance of organ and tissue donation. In 2012, due to the tireless and
creative efforts of the volunteers, board members and our director, new
programs were conducted as noted in this report. In addition, the fundraising
activities were so successful that this organization had a 37% increase in
revenue. This was also due to the careful efforts of management to control and
reduce expenses.
Our 2013 challenge is to sustain this growth in financial support, to leverage our
past experience, and to establish new strategic relationships to save lives by

The Mission:
The Connecticut Coalition for Organ and Tissue Donation, dba Donate
Life Connecticut (DLC), is dedicated to the purpose of public education
