Annual Report 2010 - Southington Care Center
2010 Annual Report Connecticut Center for Healthy Aging • Jerome Home • Arbor Rose Mulberry Gardens of Southington • Marian Heights The Orchards at Southington • Southington Care Center Not for profit members of Central Connecticut Health Alliance. Message Dear Friends, We are pleased to present you with this third annual report of Central Connecticut Senior Health Services. Last year, we shared with you some of the heartwarming stories that reflect the true spirit of caring that burns brightly in each one of the communities that make up our senior care continuum. This year, we wanted to carry that story further in sharing with you, our donors and friends, some of the ways in which you have made a difference through the generous giving of your time, talents and treasure. We chose to spotlight the ACES Life Stories initiative that began this year as a result of receiving a grant from AmeriCorps through the Foundation for Long Term Care associated with the New York Association of Non-Profit Homes and Services. The Life Stories initiative involves interviewing seniors and their family members and then recording that story through a variety of different media. Each person has a wonderful personal life story to tell. Our ACES volunteers, together with many other dedicated baby boomers, will ensure that these stories are told and shared for generations to come. Thank you for your support in this past year, in the prior years and hopefully in the days ahead. Our ability to make a difference in the lives of seniors throughout our region is directly related to your caring and generosity. Kind regards, Trish Walden Vice President Clarence J. Silvia President Our Mission Our mission at Central Connecticut Senior Health Services is to provide and promote wellness and independence to ensure optimal quality of life for the people we serve. Sherer www.ctseniorcare.org Testimonials When families need support, referrals, medical advice, compassionate professionals, answers to questions or respite from caregiving, we are here to help. Whether it’s a skilled nursing community, assisted living, adult day program or a resource center specializing in senior care, follow these remarkable journeys that portray success stories of senior living. Connecticut Center for Healthy Aging Pictured at right: For Thomas and Sandra Stone, an ordinary grocery shopping outing turned out to be a serendipitous event when they approached Connecticut Center for Healthy Aging’s kiosk and met Marc Levesque. Dealing with the Alzheimer’s and dementia issues of Thomas’s 89-year-old mom was proving difficult for the Stones, and they needed help. After a lot of research and discussion with Marc, Thomas and Sandra and their family members found the best place for mom was Mulberry Gardens. “Now the time we spend with her is fun, and not stressful like it used to be.” To read this complete story, log onto www.cthealthyaging.org or www.mulberrygardens.org. Arbor Rose at Jerome Home At left: Barbara and Charlie Lorch joined the Arbor Rose community shortly after Charlie was diagnosed with dementia. Barbara explained Charlie’s continued ability to lead an active lifestyle: “He has done very well here... He calls Bingo and still drives to church and bowling.” To read this complete story, log onto www.jeromehome.org. www.ctseniorcare.org Annual Report 2010 3 The Orchards at Southington Testimonials For George Sward (at right with daughter Donna Morin), making the decision to join The Orchards community was simple: “It was just a change of address, not a change of lifestyle.” Being near family in Southington and living somewhere that gave him peace of mind, made The Orchards the right home for him. For the complete story, log onto www.southingtonorchards.org. Jerome Home At left: Sadie Albino, a nine-year resident of The Attwood Residence, came to the community after having a stroke. Although it was difficult to leave her home, she made the best of it. She now entertains fellow residents playing the organ and is a member of Good Life Fitness. “It’s a hard thing to get through, and some people don’t get through it,” Albino said. “I was one of the lucky ones.” For the complete story, log onto www.jeromehome.org. 4 Annual Report 2010 www.ctseniorcare.org Southington Care Center Testimonials Pictured at right with Exercise Physiologist Mark Colello: Diane Loukopoulos, who has been to Southington Care Center twice for post-surgical care and physical therapy, has just one word to describe her experience — “outstanding.” “The response of the staff, day or night, was immediate,” Loukopoulos said. “I really felt like they were all my family — they took care of me that well.” To read this full story, log onto www.southingtoncare.org. Mulberry Gardens of Southington Two years ago, the Tellerico family not only had to deal with the acceptance of their mother Lena’s Alzheimer’s and dementia issues, but also the fact that she could no longer live on her own. According to David Tellerico, Lena’s son, “We’ve had a good experience at Mulberry Gardens. They do as much as they can to make sure it’s as much like a home environment as possible.” With the help of a full-time aide, Tellerico’s mom, now 94, is doing well. He and his siblings make daily visits to see Lena (at left), and her life was recently chronicled through the Life Stories project. “We’re pleased that our mom is safe, and that’s important. We go to sleep at night and don’t have to wonder if she is okay.” To read this full story, log onto www.mulberrygardens.org. www.ctseniorcare.org Annual Report 2010 5 ACES Initiative Brings Volunteers to Record Seniors’ Life Stories Life Stories is the most rewarding part due to the relationship building aspect,” Stachelek said. Eventually, Stachelek would like to offer residents either a CD or DVD that will contain interviews, photos and music. By Pamela Morello Mentone Everyone has a story to tell, and the Life Stories project is going a long way toward recording those stories for history. With a grant from Americorps through the initiative called Americorps in Eldercare Settings (ACES), Life Stories is able to record seniors’ stories in a variety of ways in order to preserve their legacies. Four of the five ACES The Orchards at Southington “I feel so honored and privileged to have listened to John share so much of his story with me.” Lynn Patrick and the eight volunteers she has recruited, have been working with the first three residents of The Orchards to participate in Life Stories. Patrick has been working with John to record and transcribe his history, and choose photos that will be included in his book. “I feel so honored and privileged to have listened to John ~Lynn Patrick volunteers are charged with coordinating the Life Stories projects. Bill Stachelek works at Jerome Home and Arbor Rose, Lynn Patrick at The Orchards at Southington, Marie Olson at Mulberry Gardens of Southington, and Donna Storms at Southington Care Center. Each coordinator has gathered a group of Baby Boomer volunteers to help with the initiative. “Most volunteers want to do the interviewing, which is the most rewarding part due to the relationship building aspect.” ~Bill Stachelek 6 Jerome Home & Arbor Rose At Jerome Home and Arbor Rose, Bill Stachelek and his staff have recently completed the first Life Stories book. Residents receive a hardcover keepsake book for free that features photographs and text that tell their stories. So far, Stachelek has recruited seven volunteers. “Most volunteers want to do the interviewing, which Annual Report 2010 “Residents will receive a personalized photo and story album — easily accessible to residents as well as family and staff members.” ~Donna Storms www.ctseniorcare.org Life Stories for family members. Marie Olson, ACES volunteer at Mulberry Gardens explains, “It’s gratifying to create such a beautiful keepsake for the residents and their families. It’s a gift for generations to come.” Southington Care Center “It’s gratifying to create such a beautiful keepsake for the residents and their families.” ~Marie Olson share so much of his story with me,” Patrick said. “It has been such an enriching endeavor for me as the interviewer.” Residents receive a Life Stories book that includes a detailed account of their lives with accompanying photos. At Southington Care Center, the development of the Life Stories project has been a combination of best practices. Volunteers have created a set of interview topics and questions, and developed a training program for volunteers covering interview techniques and working with residents who have memory problems. “Residents will receive a personalized photo and story album. We chose this format because it will be easily accessible to residents as well as family and staff members,” said Donna Storms. Fran Geschimsky, the fifth ACES volunteer, works with Connecticut Center for Healthy Aging. She takes the kiosk into the “It’s wonderful to provide blood pressure screenings free of charge.” ~Fran Geschimsky community, and as a nurse, she is involved in the many blood pressure screenings offered by the center. “It’s wonderful to provide blood pressure screenings free of charge,” she said. Mulberry Gardens With nearly 100 residents, Mulberry Gardens is planning to complete a Life Stories project for each senior. Jennifer Doty contacts the family of the resident to start the storytelling process. The family compiles photos, then meets with a volunteer to discuss the resident’s life. The volunteer also meets with the resident to garner more information. Volunteers plan to create a DVD for each resident, and will also offer a memory book option www.ctseniorcare.org Left to right: Mulberry Gardens’ resident, Frances Fontana with ACES volunteer, Marie Olson. This new fund known as The Endowment for Healthy Aging will provide benefits for senior innovations and initiatives for years to come. Trish Walden, vice president of Central Connecticut Senior Health Services, explained, “We partnered with Community Foundation of Greater New Britain to help create this fund because of their expertise in endowment building and their understanding of its importance to our area’s growing senior population.” In June, at the Garden Party held to introduce the fund, supporters of CCSHS were given the opportunity to become members of the Founders Society, expressing their care and concern for seniors through generous donations.
