CoMMITTED To SERvING
Committed to Serving 2012 Annual Community Report Honoring Life Through Our Comfort, Care and Compassion The Haven Hospice Mission Haven Hospice honors life by providing comfort, care and compassion to those we serve. The Haven Hospice Vision Haven Hospice will be THE choice for end-of-life and palliative care through the passionate pursuit of excellence and innovation. At Haven, we believe every day is a gift. The generosity of your support helps us bring comfort before, during and after a painful loss. You help us provide care to those who are not covered by insurance. You help us offer counseling and other programs to support grieving loved ones and provide assistance to seriously ill patients who might otherwise be on their own. You help us teach families how to care for their loved ones and educate healthcare professionals about the benefits of end-of-life care. All of our programs are available to all who need them, regardless of their ability to pay. At Haven, we believe hospice is about living and that no one should suffer. As experts in relieving pain and symptoms, as well as providing emotional and spiritual support, our goal is to help our patients and their families focus on living maximizing the comfort and quality of life. We are dedicated to providing the care and support that meets the needs of our patients and their families. At Haven, we believe our staff and volunteers are the difference. Haven’s staff and volunteers go above and beyond to care for families in 18 North Florida counties from our offices in Bunnell, Chiefland, DeLand, Gainesville, Jacksonville, Lake City, New Smyrna Beach, Orange Park, Palatka, and St. Augustine. As Haven supporters, you share their calling to help provide extraordinary support during a time of extraordinary need. Accreditations Accredited by Accreditation Commission for Health Care (ACHC) Re-accredited by the National Institute for Jewish Hospice A portion of the cost to print this report was donated by our printing partner. 2 Letter from the President What a year we had in 2012. With the help of out dedicated staff, volunteers and all of you – our community partners who help us carry out our mission – we opened a new, state-of-the-art care center, learned from one of the leading experts in the United States on palliative care, hosted hospice and palliative care leaders from South Africa and, of course, continued to provide the highest level of end-of-life and palliative care to patients, while helping to support their families. I fully expect all of that to continue in 2013. At Haven Hospice, our staff and volunteers are passionate about our mission. We are dedicated to meeting the needs of those in our communities who are faced with life-limiting illness and loss. We all believe strongly in our mission that everyone deserves comfort, care and compassion. We remain strong in the belief that it is our honor and privilege to provide a variety of programs, services and support to the individuals and families for these last 34 years. However, our staff and volunteers have not done it alone. Without you, our community partners and donors, who have always been there to give generously, we would not have the resources to provide the scope of programs and services that we do. My thanks to all of you is beyond words. I cannot tell you how much we all appreciate that you have chosen to support Haven. As I always say in these pages, we believe that our staff and volunteers are what make us different and better. We also believe the same about our donors. Each and every one of you makes it possible for us to meet our mission. This includes home and inpatient hospice services, palliative care consultation services, transitions support, community education and resources, as well as grief support. Haven is honored and privileged to have served more than 60,000 patients and families in North Florida over the past 33 years and has always been committed to providing our programs and services regardless of an individual’s ability to pay. One hundred percent of the gifts and donations made to Haven remains in the communities we serve and helps provide care for patients unable to pay, as well as the many unfunded programs and services we offer. In 2012, Haven spent more than $2.6 million on unreimbursed patient care, family services and programs, community and health professional education and other community support. Community Benefits Bereavement & Children’s Programs $252,621 Transitions Program $63,549 Palliative Massage Therapy $355,310 Volunteer Services $630,114 Unreimbursed Patient Care provided at cost $1,201,872 Community & Health Professionals Education $103,484 5 Visit From South Africa Hospice Trio an Eye-opener for Haven Hospice Staff Haven Hospice staff and volunteers described the feeling as humbling, following a visit from Grahamstown, South Africa Hospice Director David Barker, Clinical Services Manager Trish Gillies and Assistant Corporate Fundraiser Sibongile Matambo. Haven has been partnering with and supporting the Grahamstown Hospice since 2001. That support includes a financial donation by Haven employees that is matched by the organization here. That donation is used by Grahamstown Hospice to pay for two hospice nurses in South Africa. The visit in early November was the first time members of both hospice staffs were meeting faceto-face. The story the three Grahamstown representatives brought was not only humbling, it brought tears to the eyes of many of the Haven staff members in attendance. Fundamentally, the two organizations do the same thing – provide end-of-life and palliative care to patients, along with comfort and support to family members – but the circumstances under which the Grahamstown Hospice staff completes its mission is far different. The challenges start with the vast poverty in and around Grahamstown. The unemployment rate is more than 40 percent. Home for most people is a shack or a government-issued home. Most have no running water and no indoor plumbing. Beds usually mean the floor, so one of the things provided by the Grahamstown Hospice is a real bed. While there are many similarities between the two organizations and those they serve, there are some striking differences. A full 64 percent of the Grahamstown patients are AIDS victims, while at Haven most of the patients are dealing with cancer, dementia or heart disease. And at Haven the vast majority of patients are more than 70 years old, while the largest age group in South Africa is between 18 and 45, and 23 percent are under 17. The challenges of providing end-of-life care in Grahamstown are complicated by personal beliefs, lifestyle and prejudice. Additionally, talking about death is taboo. “That makes end-of-life-care very complicated,” said Gillies. “We have to be careful because we are associated with death. Children often come to our support groups thinking their mothers have abandoned them because no one has told them their mother has passed away.” The challenges also include how they give service. In many instances, the case workers have to walk to the patient’s homes, so their average case load of 15 patients is generally in the areas where they live themselves. But the staff at Grahamstown remains dedicated to their mission and a belief that they follow and share “that there isn’t enough darkness in the universe to extinguish the light of one small candle.” When asked about the benefits of the trip for both hospices, Barker expressed his thoughts, and they could have easily been echoed by anyone in attendance. “We came here as strangers, but we are leaving as friends,” he said. 6 Commitment To The Community Patient and Family Care Haven serves patients and their families at home, in nursing homes, in hospitals and at our free-standing hospice inpatient care centers. No matter the location, Haven offers comprehensive care and support, as well as pain and symptom management. Our dedicated staff and volunteers are here to make sure patients and families have support, comfort and dignity each day. Donations helped offset $1,201,872 in unreimbursed patient care expenses in 2012. Healing Hearts Grief support services are available at no cost to Haven’s patients and families. Services are provided above and beyond general patient care and offered to individuals, families and groups to help anyone who is coping with grief, regardless of how the loss occurred. Throughout the year, Healing Hearts offers support programs for those who have lost a friend, mate, mother, baby or other family member, as well as those who will be facing the holidays after a loss. Last year, 3,841 individuals benefitted from the services provided by Haven’s Healing Hearts program. Your gifts in 2012 helped support $252,621 in unreimbursed expenses for bereavement services and children’s programs. That does not include the cost of our chaplains and social workers who spend thousands of hours providing supportive counseling for our patients and their families. Palliative Massage Therapy Comfort and compassionate care is part of the treatment for Haven Hospice patients, and nothing is more comfortable than the touch by Haven’s specially trained licensed massage therapists. Palliative massage therapy enhances our patient care by helping patients relax, relieving their pain, stress and anxiety, increasing their physical comfort and improving sleep. All together, we were able to provide more than 8,598 patient visits by our massage therapists last year. Your support of Haven in 2012 helped offset $355,310 in unreimbursed expenses for this program. 7 Commitment To The Community Haven Transitions Transitions is a unique program which offers information, education, resources and referrals to individuals and families who are dealing with potentially life-limiting illnesses. The Transitions program specializes in making caregivers aware of helpful resources and support groups. Community education programs help people interested in learning more about living with a chronic illness, preparing advance directives, and so much more. Transitions provided services to 864 individuals and their families last year. More than $63,549 of the unreimbursed expenses of those services was covered by donations in 2012. Children’s Programs: Camp Safe Haven No two people grieve in the same way. For children, the process is even more challenging. There is nothing more important than providing that special support for children who have recently experienced a loss. Our grief support services include Camp Safe Haven, a unique program for children who have lost a loved one. Camps are hosted every year for children, ages 6 - 12 and youth ages 13 - 17. With the support of Haven staff and volunteers, these children participate in fun, safe and therapeutic activities that allow them to work through their grief. They also find comfort in sharing their loss with their peers who may also be grieving. Camp Safe Haven served 204 children in 2012. 8 Commitment To The Community Volunteer Services Volunteers are a vital member of the Hospice Care Team. They provide companionship, support and assistance to patients residing at home, in assisted living facilities, nursing homes, hospitals and any of Haven’s care centers. They also reach out to grieving families, help staff the Attic Resale stores, and serve on special event committees. Haven is proud to have more than 800 community members serving among its dedicated volunteer ranks. In 2012, donations helped offset $630,114 in unreimbursed expenses required to recruit, screen, educate, train and manage volunteers. Community & Health Professional Education Haven Hospice’s commitment to all its communities extends far beyond providing hands-on clinical, emotional and spiritual care. Providing education and information on a variety of topics, including living wills and advance directives, coping with grief, caring for the caregiver and much more are all part of the services we provide. Haven Hospice also provided clinical education to future health professionals from area colleges, universities and community colleges by providing students with internships, clinical rotations, course instruction and lectures. Donations from you helped offset $103,484 in unreimbursed expenses for this program. Palliative Care During 2012, palliative care consultations were initiated at Orange Park Medical Center. Palliative care is specialized medical care that focuses on treating adults and children to provide relief from the effects of illness, regardless of the diagnosis or prognosis. The overall goal is to improve quality of life for the patient and family including the body, mind and spirit. In just over 6 months, Haven Medical Staff provided more than 130 consultations to patients experiencing pain, stress and other troubling symptoms associate with serious and chronic illnesses. 9 “We Honor Veterans” at Haven Hospice A “thank you” is a simple gesture. Combine a “thank you” with Haven’s desire to recognize, understand, care for, and appreciate the unique needs of the veterans of the United States Armed Forces and you have our “We Honor Veterans Program.” At Haven, our clinical staff participates in continuing education focused on veteran’s needs. Training is provided by experts on veteran’s care from the Veteran’s Administration (VA), former veterans, and psychosocial experts. In 2012, Haven offered classes on the special needs of veterans at end-of-life, veteran benefits at end-of-life, vet-to-vet volunteer training, and Wounded Warrior presentations. Haven sponsored Ms. Debra Grassman, nationally recognized author and VA nurse, to share her experiences in caring for veterans. In addition to education, we utilize a military history checklist with our veterans upon admission to ensure we know and understand their service history. We use this information to design a unique plan of care to address any concerns that may arise from our patient’s military service. 10 Haven celebrates and recognizes the contributions of Veterans in the communities we serve. During November, in celebration of Veteran’s Day and National Hospice Month, Haven offices and care centers throughout the area honored veterans with community events, held lunches in their honor, offered community breakfasts, participated in parades, and sponsored community appreciation and pinning ceremonies. We strive to partner with community organizations to better recognize and serve our veterans. Haven has partnered with several skilled nursing facilities to provide memorial wall plaques with residents’ names placed on the plaques, honoring their service to our country. In Volusia County, Haven and the Volusia Mall have created a one of a kind flag garden to honor community veterans. Haven volunteers assemble the flags and dog tags. As a veteran’s name is submitted, a volunteer places a flag and a dog tag in the garden in honor or memory of the veteran. This flag garden has been replicated at several skilled nursing facilities in the Volusia County area. Perhaps the most emotional part of our We Honor Veteran Program is the vet-to-vet pinning ceremony for those veteran patients who wish to be recognized. A Haven veteran volunteer presents a fellow veteran with a Certificate of Appreciation and a thank you card signed by the staff. Our volunteer then proceeds to pin and salute our veteran patient and their family. Rarely do we have a dry eye, even from our tough veteran volunteers. They are so honored to participate in this program. After a pinning ceremony, our volunteers, our patient, and their family have some time to talk about their experiences…often the patient shares stories about their service that their family never knew. Family members report how grateful they were to hear some of these stories. The experience is very healing for families and veterans involved in the recognition. Whether through recognition, volunteer and staff education, or community partnerships, we continuously strive to live our mission of honoring life by providing comfort, care, and compassion to those we serve…including, and most especially, our Veterans. Some numbers regarding Veterans in our care: In 2012, Haven cared for more than 600 veterans. Of that number, 533 of them identified themselves as having served during times of conflict and approximately 250 of them chose to receive veteran recognition from our volunteers. 11 THE COMMUNITIES’ SUPPORT Donors make it possible for us to provide all our services. In 2012, donors gave gifts to Haven Hospice to support the expansion of or the building of a care center, in honor or memory of a loved one, attended or sponsored a special event, responded to an annual appeal such as Light Up a Life or Forget-Me-Not, donated items to or shopped at our Haven Attic resale stores or remembered Haven in their wills. Their generous contributions provided $4,610,009 in total support. These donations are vital to Haven and allow us to provide many programs and services to our patients and their families that otherwise would not be available. The sources of gifts and pledges in 2012 include: Major Gifts, Honor/Memorial Gifts, Special Events, Annual Appeals, Planned Giving, the Haven Hospice Attic Resale Stores and Other Gifts (including foundations, the University of Florida Community Campaign and more). 2012 Sources of Gifts & Pledges Totaling $4,610,009 Attic Sales 36% Major Gifts 39% Other 6% Special Events 8% Planned Giving 2% Annual Appeals 2% 12 Figure 1 Honor/Memorial 7% The Community’s Support Special Events Honor/Memorial Gifts As they always have, Haven supporters embraced the opportunity to uphold our mission through fundraising and volunteering in 2012. Many supported our events through their attendance and participation. As we have strengthened the partnerships in each of the communities we serve, organizations in those communities have stepped up to host special events. These events, vital to our fundraising efforts throughout North Florida, raised $354,753 in cash to benefit Haven in 2012. Haven received $300,270 in gifts and pledges in honor or memory of a loved one in 2012. These donations are a primary source of support for unfunded and unreimbursed programs and services offered by Haven. Many families and friends request that donations be made to Haven Hospice in lieu of flowers, or they decide to memorialize their loved ones along the Memorial Walkways located at our care centers. Annual Appeals Thanks to the generosity of our donors, Haven Hospice received $115,023 in 2012 through our spring Forget-Me-Not appeal, our fall Light Up a Life appeal, our In Touch newsletters and in response to our Annual Community Report. The Memorial Walkways are dedicated to your loved ones. A permanent legacy is created through the walkway donors. Bricks, granite pieces and benches along the Memorial Walkways are inscribed to honor loved ones according to your wishes. 13 The Community’s Support Planned Giving Planned giving is a significant way to provide major support for Haven Hospice. We received $110,000 from donors who supported us with new Charitable Gift Annuities or named us in their wills, estates or other planned giving vehicles in 2012. These donations are vital to the patients and families who benefit from the many unfunded programs and services Haven Hospice offers. Planned giving enables donors to arrange charitable contributions to Haven Hospice in ways that maximize their personal objectives while minimizing the after-tax cost. Depending on the asset given to Haven Hospice and the gift arrangement selected, a donor can generally expect to obtain some or all of the following benefits: • Fulfill philanthropic goals • Reduce income tax through a charitable deduction • Avoid capital-gains tax on gifts of long-term appreciated property • Retain a stream of income for life for yourself and/or other beneficiaries • Increase spendable income • Eliminate potential federal estate tax on property that passes to Haven Hospice • Reduce costs and time in estate settlement Community Campaigns In 2012, Haven Hospice received $65,168 from generous individuals throughout our communities that designated Haven as their charity of choice in the associated employee campaign. Donors to the University of Florida Campaign for Charities (UFCC), the Combined Federal Campaign (CFC) and the Florida State Employee Community Campaign (FSECC) have designated their gifts to Haven. Additionally, even though Haven is not a United Way agency, anyone participating in a United Way employee campaign can name Haven as a designated agency of choice. 14 Sustainers: Making It Simple to Provide Your Annual Gift to Haven Hospice You can now make your annual gift to Haven Hospice even easier by becoming a Haven Sustainer. The Haven Sustainers program allows donors to make convenient, automatic, and ongoing monthly contributions that can be changed or stopped at any time. Haven Sustainers have chosen to contribute at least $5 per month through automatic deductions from their credit cards. This provides a consistent stream of income to help Haven provide critical programs and services to patients and families regardless of their ability to pay. Membership in the Haven Sustainers program is automatically renewed each year until you choose to make a change in your giving level or your participation. It is very easy to become a Haven Sustainer. You can call 1-800-727-1889 and ask to speak with someone in the Development Department. Or you can go online to www.havenhospice.org, fill out our online donation form and click on the monthly donation option. However you choose to make your donation to Haven Hospice, we want you to know how much it is appreciated. Without you we cannot do most of what we do for families impacted by the passing of loved ones. Endowment Fund The Haven Hospice Endowment is a separate account of funds designed to provide future long- term support to Haven Hospice. These funds are intended to be used to provide for the unfunded patient care and many community and family support programs and services offered by Haven. The Haven Hospice Board of Directors maintains oversight of funds associated with the endowment. As of Dec. 31, 2012, the balance of the endowment was $1,824,365. Haven Hospice Attic Stores You can support Haven’s four Attic stores in Gainesville, Lake City, Orange Park and Chiefland in multiple ways. Shop We are thankful for our customers who love shopping for all those unexpected treasures at our Attic stores. We restock with new treasures daily; you never know what you will find in your Attic. Partner At the Attics, we also partner with community organizations to provide much needed clothing, housewares and more to the homeless, displaced homemakers, victims of fire and many others in need. We host Kids’ Night in December to help children shop for family members who otherwise could not afford to exchange gifts. Donate Donations brought to the stores create the inventory we need. We appreciate our generous donors who bring their new or gently used treasures to us. Without them we would have a great challenge helping the families and communities we serve. Sales from these donations benefit the patients and families who cannot pay for care, along with those served by programs such as Camp Safe Haven for children who have experienced a loss. Countless individuals, businesses and organizations generously donate items ranging from housewares and furniture to clothing and children’s toys. Thanks to your generous donations, the Attic stores combined in 2012 to generate $1,662,212 in total sales. Host Another way to support the Attic is hosting a donation drive or bringing the Rollin’ Attic truck to your place of business or neighborhood. Volunteer It is the Haven Attic volunteers who make the stores a success. They spend time sorting donations, stocking the floor and making friends. But beyond that, they help people throughout the Haven Hospice community — patients and families, as well as people who have challenging economic circumstances and need an affordable place to shop. We are always seeking energetic, conscientious volunteers who are mindful of the Haven mission. We could not do what we do without our volunteers. 15 The Community’s Support CHIEFLAND A Special Night for Kids in Chiefland It’s a big annual event for Haven Hospice, and at first glance, it has little to do with the organization’s mission. However, providing comfort, care and compassion can take on many different forms. And for the first time in 2012, the kids in Chiefland were able to experience it, just like those in Gainesville and Lake City. With a new Attic Resale Store having opened last year in Chiefland, youngsters in the tricounty area experienced their first Kids Night at the Attic. In December, hundreds of kids were 16 able to shop at the store for family members or themselves using Santa Bucks provided by the Attic. Children were treated to a free photo with Santa, gift wrapping and refreshments, which helped many families who are financially struggling during the holiday season. Area businesses such as Toys for Tots, Dollar General, Family Dollar and Wal-Mart supported the effort. “We are thankful to all the Haven staff, donors and volunteers who participated,” said Michael Morse, vice president of organizational advancement for Haven Hospice, “as well as the countless individuals in the community who generously support the Attic stores with their donations and purchases. Their support goes a long way toward helping the families and communities we serve.” A Beautiful Setting for Haven Hospice’s Fall on the Prairie The Whitehurst Ranch & Prairie served as the perfect backdrop for Haven Hospice’s Fall on the Prairie. The hot, humid later summer weather broke just in time for this annual fundraiser in the tri-county area. Music was provided by Dottie South and the Slackers. Food was catered by Ivy House. As always, the evening included a live and silent auction with some great products. There was even a cowboy poet to provide entertainment during dinner, which was served outdoors adjacent to a majestic lake. And a widescreen TV showing the Gator victory over Vanderbilt added to the festivities. All proceeds from the event benefit the Haven Hospice Tri-County Care Center and unreimbursed patient care, grief support and community education programs in the area. Approximately $50,000 was raised with 400 guests and volunteers in attendance. “As one of our signature events, we look forward to Fall on the Prairie every year,” said Haven Hospice Vice President of Organizational Advancement Michael Morse. “It is a great opportunity to visit with our friends in the tricounty area in a breathtaking venue. Our sponsors and donors make this event possible, and for that we are extremely grateful.” Nature cooperated with a late afternoon of pleasant temperatures, followed by a great sunset and stars. 17 The Community’s Support GAINESVILLE ViVA! Goes Carnival A sold-out crowd of 650 guests, along with 130 volunteers, walked along the stylized vintage 1920s boardwalk on a beautiful Saturday evening for ViVA! 2012 at the Rembert Farm in Alachua. A new theme this year – a Coney Island Carnival – was a smash hit for attendees at the eighthannual Viva! Guests rode the Ferris wheel, played carnival games, and enjoyed the gaming tent all while snacking on gourmet classic carnival treats prepared by Embers Wood Grill. In addition, more than 120 items were on display in the Remberts’ barn – all part of the silent and live auctions. After dinner guests were able to witness an extraordinary acrobatic and juggling display. 18 Clearly ViVA! 2012 was a big success, raising substantially more than in 2011, as well as creating awareness and educating people about the mission of Haven Hospice. Proceeds went to pay for unfunded Haven Hospice programs and services like Camp Safe Haven, Transitions, pre-hospice programs and community grief and loss support. Run for Haven Doubles in Size A little rain did not dampen spirits at the thirdannual Run for Haven, as more than 700 runners and volunteers participated in the 5K/10K race on Saturday, March 17th. The St. Patrick’s Day event, held in the Tioga Town Center and Town of Tioga, included the race, food, beer, live entertainment, children’s activities and a great costume contest. In addition, booths were set up by several of the race sponsors and provided runners and their families with free giveaways and services. The fundraiser for Haven Hospice more than doubled in size from 2011, and in only three years has become the second largest race in the Gainesville area. The race raises money for all of Haven Hospice’s unfunded programs, such as the bereavement program, Camp Safe Haven, the massage therapy program and community memorial services for anyone who has experienced the loss of a loved one. Karen Paige, a 5K participant, said in 2005, when her grandmother passed away, Haven was there for her and her family. This was Paige’s first year participating, along with her son and his fiancé. “We are all walking for the same reason. Sooner or later, everyone needs Haven,” she said. 19 The Community’s Support Fashion Show a Big Hit There were some changes in 2012 for the annual Styles for Life Luncheon, Fashion Show and Holiday Gift Gallery presented by the Haven Hospice Alliance, a group of volunteers who promote and support Haven through fundraising, special events, volunteerism and community education. The more than 250 people in attendance were treated to an outstanding event at the Paramount Plaza Hotel. As the attendees ate a delicious lunch in the hotel’s main ballroom, members of the community modeled some of the hottest fashions – all of which came from Haven’s Attic Resale Store. This year’s event included a gift gallery, which featured discounted prices on luxury brands like Michael Kors and Jones New York. Attendees raved about that new addition. The constant, of course, was the presence of the emcee – WKTK radio personality Storm Roberts. Proceeds from the event will benefit Haven Hospice’s Camp Safe Haven, a free day camp for children ages 6-12 and teens ages 13-17 who have lost a loved one. In the fun and therapeutic setting, children learn to cope with their feelings of grief, meet other children dealing with similar issues and understand that it is ok to laugh and have fun again. JACKSONVILLE Making Camp Safe Haven a Reality While coping with the loss of a loved one is a challenge for any family member, it is often true that none feel that loss more than children. Unlike adults, children are often dealing with emotions that they simply do not understand. At Haven Hospice, we have come to realize that the best way for young people to effectively deal with these emotions is by sharing them with others going through the same thing, and that is what Camp Safe Haven is all about. But like many of our programs, this one would not be a reality without the generous donations of our supporters. Thanks to you, we have been able to provide this program for the last 15 years. In September, we were fortunate enough to be able to hold Camp Safe Haven in an ideal setting – the Jacksonville Zoo. Supported by our staff of professionals and volunteers, and led by Vonceil Levine, MSW, children were able to participate in fun and therapeutic activities to help them work through their feelings of grief. 20 Camp Safe Haven is open to children 6-12 who have suffered the loss of a loved one, whether that loved one was cared for by Haven or not. At the zoo we offered a day of hands-on sessions that allowed young people to express themselves and cope with their feelings in a safe and secure environment. The activities created an atmosphere where the children began to understand that it is OK to grieve and have fun at the same time. Traditionally, a day at Camp Safe Haven ends with a moment to love, honor and remember their loved ones. The goal was for the children to understand how to manage and express feelings of loss and recognize that grieving is a natural process. ORANGE PARK It was an exciting time in August as Haven Hospice opened a new, state-of-the-art care center in Orange Park. The facility includes 18 private inpatient rooms, multiple living rooms and porches, a family kitchen, a non-denominational chapel for memorial services, prayers and meditation and a large community room in the administrative building that can be used by local groups and organizations to host events and meetings. The Homer J. Sr. & Fern O. Custead Care Center was a great accomplishment on multiple levels. First, it allows us to dramatically improve the level of hospice care available in Clay County and the surrounding area. Second, the construction of the care center was a community effort in every sense. The 20,000-square-foot facility is located on nearly nine, pristine acres off of Blanding Boulevard in Orange Park. The new center’s design takes inpatient hospice care to a new level. Haven received an honorable mention from the Long-Term Living and Healthcare Design Magazine. Senior-living experts recognized the center for its connection to the outdoors and thoughtful integration into the surrounding natural environment. Its careful design saved more than 80 percent of the specimen trees on the nine-acre property. Named for the Custead family, who has longstanding ties to the Clay County area and helped make the facility possible through their generous gift, the state-of-the-art Custead Care Center is providing expanded service to the patients and families in Clay County and throughout Northeast Florida. The center was also built to the Florida Department of Emergency Management’s standards for a special needs shelter and can withstand the impact of a category 3 hurricane. No other freestanding hospice inpatient facility in North Florida has been built to these standards. Custead Care Center Up and Running The Custead Care Center, one of five freestanding hospice inpatient facilities Haven operates, provides a wealth of resources for individuals, their families and the community. Among its services are inpatient palliative and end-of-life care; respite care support for families and caregivers and grief support. “Haven Hospice is pleased to expand its direct service and care to meet the needs of residents in Clay County and the surrounding area,” said Tim Bowen, president of Haven Hospice. “When we develop each of our centers we, in essence, form lasting partnerships with the local communities we serve. That starts with fundraising and it extends to the beautiful completed facility we see today,” he added. 21 The Community’s Support Poker Run Raises $6,000 for Haven Hospice Custead Care Center It was not your normal poker run. All sorts of vehicles – two-wheeled, three-wheeled and fourwheeled – participated in the Ride for Haven Poker Run in May to raise money for the Haven Hospice Homer J. Sr. & Fern O. Custead Care Center. While it started as a motorcycle poker run, by the time the planning was done it was opened up to include three-wheeled and four-wheeled vehicles to allow more people to participate. There were motorcycles, cars – even six Mini Coopers. About 120 people come out for the event, with 106 riders participating on the course. The highlight of the day was the after-party at Whitey’s Fish Camp which included auctions, raffles and a lot of fun. The poker run’s success came from the participants and the sponsors, which included the host stops along the way. The day started at the Orange Park Kennel Club, and the stops included La Nopalera in the Oak Leaf Town Center in Jacksonville, Ronnie’s and Wings and Oyster Bar both in Green Cove Springs, Prevatt’s Sports Bar and Grill in Middleburg and Tap’s Bar and Grill in Orange Park. The finish was at Whitey’s. In addition to the support of all these establishments, Guardian Pharmacy was a major cash sponsor and the Silverbacks Riding Club provided a number of volunteers. Run Around the Mall for Haven Hospice It was a beautiful evening as more than 80 runners participated in the second-annual Run for Haven at the Care for Clay 5K at the Orange Park Mall in September. The twilight 5K and post-run party, sponsored by the mall, was a fundraiser for the Custead Care Center in Orange Park. All proceeds from the run, which raised $7,500, will stay in Orange Park to support the care center and other programs designed to benefit the thousands of patients and families served by Haven Hospice. The post-run party included live music, food, drinks, expo booths, awards ceremony and more. “In only its second year, the community has embraced the Run for Haven at the Care for Clay 5K,” said Kelly Wells, administrator of the Custead Care Center, located in Orange Park. “We hope the run continues to grow in numbers and enthusiasm in future years.” LAKE CITY 9-Year-Old Girl Makes $2,600 Donation to Haven Hospice Seeing it as a way to make the world a better place, 9-year-old Natalie Green turned over a check in November to Haven Hospice for $2,600. Natalie decided to raise money for Haven because “we had learned in school about making the world a better place,” she said. “I thought about how I could make the world a better place; so when I got home I asked my Mom and Dad if I could make the world a better place by doing something for Haven Hospice.” 22 Natalie’s goal of making the world a better place will be achieved in part because this donation is being used to help pay for patient care for those who are unable to pay themselves and to provide grief support and bereavement camps for children. Natalie’s original goal was to raise $1,000, but with the help of her church, the First Full Gospel Church in Lake City, which got behind her by selling dinners and having cake auctions, she managed to collect $2,600. FAM Fest: A Healthy Way to Support Haven Hospice Sunlight glinted off Lake DeSoto as community members gathered to participate in Haven Hospice’s third-annual Fitness, Art and Music Festival (FAM Fest). More than 70 people participated in the 5K run/ walk that was the focal point of FAM Fest. The event, which was in Wilson Park, raised more than $7,000 benefitting Haven Hospice. The race winners were Cary Blakely for the men and Anna Busby for the women. In addition to the 5K, attendees enjoyed live music by Patchwork, sales of artwork at nearby booths and the Lake DeSoto Farmers Market. Patchwork singer and guitarist Cathy DeWitt encouraged runners to listen while they cooled down after crossing the finish line. “I’ve known a lot of people they’ve helped through hard times,” DeWitt said of Haven Hospice. “They provide a great service.” FAM Fest also featured an instructor-led dance routine, a fitness station provided by the American Family Fitness Center, free cotton candy and several other interactive displays. Sponsors of the event included Rountree Moore, Edgley Construction, FPL, GulfCoast Financial, Columbia Bank, First Federal Bank of Florida, Home Depot, Allstate, Baya Pharmacy, Southwest Family Dentistry, the city of Lake City, The Demas Law Firm and Fit2Run. All of the funds raised support patient care, services and other programs provided by Haven Hospice that are otherwise unfunded. 23 The Community’s Support PALATKA NEFAR Anonymous Donor Pays for Care Center Expansion NEFAR Donates $18,546 to Haven Hospice An anonymous donation of $1.7 million will go a long way to making a much-needed expansion of the Haven’s Roberts Care Center in Palatka a reality. “Five fish, five alive!” announcer Mike Oglesbee yelled out before weighing each boat’s catch at the ninth-annual bass fishing tournament for Haven Hospice. The annual event is hosted by the Northeast Florida Association of Realtors (NEFAR). The donation will enable Haven to add six beds to the current 12-bed center. Since, at any given time, all the patient rooms can be full, and there is a waiting list, this is a real boon to end-of-life care in the Putnam County area. In addition to the patient rooms, the expansion will include support space – a private counseling room and family visiting areas. Right now, there is no real private space to meet with families and patients, and the family visiting area is limited. Since resources for end-of-life care are limited in this area, this donation is a great gift, and it will enable Haven to better complete its mission. Planning for the expansion is underway. Groundbreaking is expected to occur in the next few months, and the expanded facility should be open before the end of 2013. The tournament began at safe-light on a Saturday morning, and the boats started to come back to the dock at 3 p.m. Each boat had a team of two fishermen. There were 57 boats entered in the tournament, each paying $110 to participate. The winners of the tournament were Lee Stalvey Jr. and Jason Caldwell with their five bass weighing in at a whopping 28.04 pounds. They received a $2,500 prize for their victory, but their success didn’t end there. The pair also had the largest single bass fish, weighing in at 8.55 pounds. They received another $1,000 prize for this. Second place winners Stanley Hunt and Shawn Jannotti also won $1,000 with their catches totaling 21.57 pounds. Third place winners David Davis and Kevin Dufur won $500 with their 21.10 pound catches. The day was a successful fishing day all around. It was a strictly catch-and-release tournament, and all fish were put back in to the river after being weighed. In fact, if a team brought back a fish that was no longer alive, the team was penalized .25 pounds. 24 The event took place at Butler Park East, starting with a 5K race on the beach. There was also a 1-mile Kid’s Costume Fun Run for the junior ghouls and goblins. The post-run party included live music, food, drinks, costume contest, expo booths, awards ceremony and more. In addition to the fishing festivities, there were also several raffles throughout the day. There was a 50/50 raffle as well as several other prizes. From a fishing rod to a picnic set, countless gifts were raffled off. There was also a $250 cash prize giveaway. All proceeds from the day went to the Haven Hospice Roberts Care Center in Palatka. NEFAR was able to raise $18,546 for Haven this year, bringing the total donated for the nine years to more than $161,519. ST. AUGUSTINE Ghouls and Goblins Hit the Beach in St. Augustine Usually known for making appearances at night, the ghouls and goblins were out early on St. Augustine Beach this year in early November for the first-ever Monster Dash & Creepy Crawl benefiting Haven Hospice. All proceeds from the event remain in the community to benefit the thousands of patients and families served by Haven Hospice. It’s About Living… A Haven Hospice Community-Education Series No one knew what to expect when Haven Hospice kicked off its seven-part, community education series in St. Augustine in June. Seven months later, after the last of the monthly It’s About Living… programs were completed, it was easy to say, they were a success beyond anyone’s expectations. The monthly sessions were themed to help people deal with end-of-life issues, and they were open to anyone in the community. The sessions’ topics included living free from pain, memory loss, advanced life-care planning, complementary therapies, spiritual care, caring for veterans and coping with the holidays. Special expert speakers were called on for each session, and attendees gained valuable insight. While all the sessions were special in their own way, the highlight may have been in November with the focus on caring for veterans. Not only did local veterans participate in the program, which included a special showing of the documentary Korea: Forgotten War Remembered Heroes, it also served as the launch date of a special partnership between Haven and the St. Johns County Public Library – The Veterans History Project. The Veterans History Project allows local veterans to record their military service as a special memento for their family, with a posting of the recording also being placed in the Library of Congress. 25 The Community’s Support VOLUSIA/FLAGLER Honoring Veterans at the Volusia Mall Haven Hospice’s ties with veterans and veteran programs was made abundantly clear in November, when we kicked off a special program on Veteran’s Day that will run through 2013. Honor a Veteran will be on display at the Volusia Mall as a salute to veterans in the Daytona area. To show appreciation for veterans, Haven Hospice, in association with the mall, is planting American flags for community members to view throughout the year. The Veterans Flag Garden is located in the center of the mall (formerly the fountain). The idea was originally suggested by DeLand Patient Care Manager Elizabeth Moran, who also designed the dog tag and yellow ribbon on the veteran American flag that is planted at the location. Volunteer Brenda Gormley is making all the dog tags. Participation by the public is free. Anyone who has a veteran they wish to honor can go online to www.havenhospice.org/veteransgarden and fill out the form or call (386) 734-7600. Haven volunteers will then place a flag in honor of your veteran and notify you when the flag is in place, generally within three days. While the cost of this program is covered by Haven Hospice, anyone wishing to donate can go online to www.havenhospice.org/donate to learn more. BUNNELL/ NEW SMYRNA BEACH Haven Hospice Opens New Offices in Bunnell, New Smyrna Beach In order to more effectively achieve our mission in the Volusia and Flagler counties area, Haven Hospice opened two new offices in 2012 in Bunnell and New Smyrna Beach. By joining forces with the already existing office in DeLand, we can more effectively provide services in this part of our area. “While we have been providing support to this community for some time, opening these offices allows us to enhance those services,” said Haven Hospice President Tim Bowen. “We are committed to this community; just as we are committed to all the communities we serve. And we take that responsibility seriously.” Haven Hospice has been greeted warmly in both communities. Flagler County Commissioner Alan Peterson welcomed Haven by pointing out the need for the organization. With 27 percent of its residents retired, Flagler County has one of the largest retirement communities in the state. “Quality of life is important to all of us,” said Peterson at a ribbon cutting for the opening of the Bunnell office. “It is great to have Haven here to provide care for our residents. We get a lot of businesses opening here, but this opening will provide a very important support service. We welcome you to the county and wish you success.” The new office in Bunnell is located at 608 E. Moody Blvd., and the one in New Smyrna Beach is located at 509 Canal Street. Both new offices have been providing Haven’s full complement of services to residents in the two counties including end-of-life and palliative care and support programs to those with a life-limiting illness, as well as grief support for loved ones. 26 DONOR RECOGNITION All gifts to Haven remain in the community and help make it possible for us to pursue our mission regardless of a person’s ability to pay. The following pages are dedicated to those donors who generously gave in support of the programs and services provided by Haven. Thank you for the tremendous caring and commitment to Haven but, more importantly, for helping the thousands of patients and families we serve. In addition to the donors included in this report, many individuals have donated to Haven through the United Way Campaign, the University of Florida Community Campaign, the Florida State Employees Charitable Campaign and the Combined Federal Campaign. Some donors have chosen to remain anonymous or have not released their names to us, so we are unable to include them in this report. However, their gifts and generosity are just as appreciated and vital to the success of Haven. If you support Haven through one of these campaigns, we encourage you to specifically direct the campaign (on your pledge form) to release your contact information so that we can personally acknowledge your gift and thank you. 27 Friends For Life Over the years, the generosity of individuals, families and businesses has provided the necessary support for us to meet the needs of thousands of patients and their families.
