Our great sympathy goes out to Dr. Darwin’s wife Bev and their two sons who are suffering due to this great loss. Information regarding an FMC memorial service will be announced in the days to come. Bring your es! dancing sho Welcoming New Chief Information Officer to FMC FMC is pleased to welcome Alan Greenslade, M.B.A., as the new Chief Information Officer. Greenslade, a native of Cleveland, joined the FMC staff on Nov. 2. Greenslade has more than 30 years of experience in information technology and comes to FMC from Parkland Health and Hospital System in Texas, where he served as the chief technology and interim chief information officer. While at Parkland, Greenslade directed 180 employees and outsourcing firms supporting 10,000 end-users at 50 different facilities. He also managed multiple application installations and upgrades, and co-directed both meaningful use and ICD-10 initiatives. In his new role at FMC, Greenslade will manage the hospital’s computer system and information technology strategy. He also will be heavily involved in the implementation of the new electronic medical records system. “Alan is an incredibly talented and experienced information strategist who has come to Fairfield Medical Center at the perfect time,” said John R. “Jack” Janoso, Jr., president and CEO of Fairfield Medical Center. “In light of our organization’s strategic rebirth and movement toward a new electronic health record provider, Alan will be the chief architect of the information and services needed to support the FMC brand of care we are intent on providing. I am excited to have Alan as part of our team.” Greenslade has a Master of Business Administration from Western Governors University and 15 years experience working in healthcare. He said he looks forward to frequently rounding the hospital to talk to employees and gather feedback on any issues they may be experiencing. Clinical Update on FMC Psychiatry Coverage Due to unforeseen circumstances the, psychiatry coverage at Fairfield Medical Center is understaffed effective immediately. The inpatient psychiatry unit coverage will remain unaffected. Consults that are placed on patients currently admitted outside of the 6N inpatient unit will not be seen at this time. Assessment of patients who have potential to be admitted to the inpatient 6N psychiatry unit will continue to be seen. Ideally, these consults should be requested once the patient is medically stable. Effective communication with the on-call psychiatrist will ensure there are no lapses in patient care. Fairfield Healthcare Professionals Welcomes Dr. Scott Baker Scott Baker, M.D., has been a part of the medical community at FMC for many years. As of Nov. 16, his practice will join Fairfield Healthcare Professionals. He will contiunue to practice out of the same office located at: FHP Old Schoolhouse Family Practice 1955 Lancaster-Newark Road Lancaster, OH 43130 740-689-2820 – office 740-689-2830 – fax MEC Updates from October • The FMC Chief of Strategy, Business and Development Officer position has been posted externally; the key duty will be to guide implementation of the Strategic Plan. • EMR System Selection will be announced within the next few weeks pending contract negotiation. • Dr. Terrence Welsh is announced as the new Chief of Medical Affairs; he is scheduled to begin working at FMC by the end of the year. 2016 Benefits Open Enrollment Attention Employed Providers and Staff Open Enrollment to elect 2016 benefits is now available in Employee Self-Service (ESS). The enrollment will be open through Monday, Nov. 30. All full-time and part-time employees are required to complete their benefit enrollment in Employee Self-Service and meet with a Colonial Enrollment Specialist to receive your Benefits Statement. My Committment to You for 2016 My goal as Medical Staff President will include promoting continued improvement in physician relationships, emphasizing communication and selfgovernance. Most importantly, our success as a medical staff and in our practices has become linked to successful outcomes requiring effective interaction between physicians, administration and all members of the medical delivery team, resulting in improved experiences for our patients. Respectfully, David Conley, M.D. Clinical Documentation Improvement Tips Altered Mental Status is a common reason for a patient to be admitted to the hospital. There are many causes for AMS. You will see a new query asking for more specific documentation regarding the patient’s altered mental status. We are looking for specific documentation to ensure we have an accurate portrayal of the patient’s severity of illness and the treatment that he or she is receiving. 1.) The nature of the AMS-delirious, demented, confused, stuporous, psychotic and comatose. 2.) Severity of the AMS-acute, chronic or acute on chronic. 3.) Etiology of the AMS. This will include specific neurodegenerative diseases, psychiatric illnesses, a focal structural problem of the brain and, of course, encephalopathy with five specific types of encephalopathy named on the query with their etiologies listed. Our staff neurologists Drs. Walz and Staley have worked with us on this project and it has been approved by the Neuro Unit Committee. The goals focus on problems in health care safety and how to solve them. Identify patients correctly NPSG.01.01.01 NPSG.01.03.01 Use at least two ways to identify patients. For example, use the patient’s name and date of birth. This is done to make sure that each patient gets the correct medicine and treatment. Make sure that the correct patient gets the correct blood when they get a blood transfusion. Improve staff communication Get important test results to the right staff person on time. NPSG.02.03.01 Use medicines safely NPSG.03.04.01 Before a procedure, label medicines that are not labeled. For example, medicines in syringes, cups and basins. Do this in the area where medicines and supplies are set up. NPSG.03.05.01 Take extra care with patients who take medicines to thin their blood. NPSG.03.06.01 Record and pass along correct information about a patient’s medicines. Find out what medicines the patient is taking. Compare those medicines to new medicines given to the patient. Make sure the patient knows which medicines to take when they are at home. Tell the patient it is important to bring their up-to-date list of medicines every time they visit a doctor. Use alarms safely NPSG.06.01.01 Prevent infection NPSG.07.01.01 Make improvements to ensure that alarms on medical equipment are heard and responded to on time. Use the hand cleaning guidelines from the Centers for Disease Control and Prevention or the World Health Organization. Set goals for improving hand cleaning. Use the goals to improve hand cleaning. NPSG.07.03.01 Use proven guidelines to prevent infections that are difficult to treat. NPSG.07.04.01 Use proven guidelines to prevent infection of the blood from central lines. NPSG.07.05.01 Use proven guidelines to prevent infection after surgery. NPSG.07.06.01 Use proven guidelines to prevent infections of the urinary tract that are caused by catheters. Identify patient safety risks NPSG.15.01.01 Find out which patients are most likely to try to commit suicide. Prevent mistakes in surgery UP.01.01.01 Make sure that the correct surgery is done on the correct patient and at the correct place on the patient’s body. UP.01.02.01 Mark the correct place on the patient’s body where the surgery is to be done. UP.01.03.01 Pause before the surgery to make sure that a mistake is not being made. This is an easy-to-read document. It has been created for the public. The exact language of the goals can be found at www.jointcommission.org. Meet our Community Hospitalists Dr. Talal Sabbagh joined Community Hospitalists in September of this year. He has proven over and over to be a big asset to the team. In late September, our Board of Directors formally approved the amendments to the Medical Staff Bylaws to address the proposed succession plan for the officers of the Medical Staff. It’s now official that we have formal Medical Staff Leadership succession! Dr. Sabbagh attended Lake Erie College of Osteopathic Medicine in Pennsylvania. He was enrolled in Fairfield Medical Center’s Residency Program prior to coming to Community Hospitalists. ICD-10 Transition a Success – Thank you! Talal Sabbagh, D.O. In Dr. Sabbagh’s spare time, he enjoys hiking, playing drums and keyboard and watching soccer. He likes Italian food. You will always see a smile on Dr. Sabbagh’s face, and he always takes time to listen to patients, as well as family members. Amanda Slater, D.O. Dr. Slater was a third year resident at FMC prior to joining our Hospitalist Group. She attended Lake Erie College of Osteopathic Medicine in Pennsylvania. Dr. Slater goes the extra mile to make sure our patients are well taken care of and has proven to be a great asset to our team. In her spare time, she enjoys spending time with her darling daughter, Evelyn. She also loves gardening and proves it by sharing her goods with our entire team. We are so happy to have Dr. Slater on our hospitalist team. Visit the Community Hospitalists On-Call Suite We have an onsite office for the hospitalists at Fairfield Medical Center. The office is located near the Cath Lab and across from the service elevators on the first floor of the main building. ICD-10 is well underway and we want to thank you for a job well done throughout this transition. The transition has been a success for FMC. Below are a few updates to keep you informed of our progress. • The hospital and physicians are both receiving payment on accounts with ICD-10 codes. • There have been minimal denials related to ICD-10 coding. • Providers have done a wonderful job of sending patients with orders that have ICD-10 codes on them. • There have been a few minor system issues that needed tweaked, but nothing major. • The command center has been utilized to help physicians on the floors with questions. • The training modules are available until Dec. 31 for anyone who needs a refresher.
