One “wind” filling our sails continues to be the huge differentiation we gain from our XRO system, with its proprietary vacuum carts. No one else has this. A second “wind” is the pressure from all hospitals to reduce costs. Xanitos does this. A third “wind” is the increasing focus by hospitals on the reduction of Hospital Associated Infections. Xanitos is the leader amongst EVS companies in reducing the risk of HAIs. A final “wind” is the growth and maturing of systems of hospitals and their focus on a single provider system-wide for support services such as EVS. Xanitos, with its cost savings, its state-of-the-art EVS systems and focus on HAIs, coupled with a new insourcing approach, is very attractive to systems. While a lot of our sales focus has been with systems, we have added six individual hospital clients this year; Kaiser Permanente Santa Clara Medical Center; Kaiser Permanente Richmond Medical Center; St. Mary’s Hospital, Athens, GA; Cortland Medical Center, NY; Albert Einstein Medical Center Montgomery, East Norriton, PA; and St. Josephs Hospital, Chicago. This brings our tally of clients up to 41 and our managed volume to in excess of $180 Million. A key factor to achieving growth is account retention. We may be small compared to our competitors — but we move quickly — we are indeed dancing between the feet of elephants! On a recent visit to a facility we serve in the Midwest, I was stunned (and so proud) as each XRO Team shared with me the most recent Top Box Always HCAHPS score for the unit(s) they clean. If that was not enough, they walked me through the past 6 months trend and shared with me their specific plans to improve upon the high Top Box scores they were already achieving. One of the Teams even took the time to explain to me the purpose and function of the HCAHPS Survey. The level of staff engagement I witnessed was awe-inspiring. It highlights several of the strategies we have hardwired into our operations to ensure we deliver high quality service each and every day. Thank you for helping to save lives, Mike Bailey 4 THE HCAHPS CHALLENGE: Making a Difference in the Patient Experience With HCAHPS scores being a key part for Medicare’s Hospital Value Based Purchasing Program, HCAHPS scores are now even more important to our clients. One of the questions in the HCAHPS questionnaire to patients is, “was your room and bathroom always clean?” What Does a Patient Think When They Hear “Clean”? We have analyzed this, and believe that there are four key factors that make up a patient’s view of “clean.” They are cleanliness, visibility, communication, and neatness. Our XRO system itself goes a long way to affect these four influences of a patient’s view of “clean.” Its proprietary team cleaning process and vacuum-cart help as follows: CLEANLINESS: The use of an ULPA filtered vacuum in place of the traditional dust control tool significantly reduces the air particle count and improves the air quality, so much so that our vacuum has been informally named an “air scrubber” by a third party research firm. Correct use of microfiber cloths and mops further remove dirt, particles, and pathogens from patient room surfaces. The patient room is indeed cleaned much more thoroughly than by using traditional cleaning methods. Poor cosmetic maintenance including walls that need painting, torn wall coverings, chipped door jambs, stained ceiling tiles, and aging base tiles all combine to make a clean room look dirty. Thus cooperation with the maintenance department or, as is happening frequently now, having Xanitos being responsible for cosmetic maintenance, is important to ensure a “clean look.” VISIBILITY: By daily vacuuming of the floors, soft furniture, air vents, and high ledges, the room is not only clean but also makes the patient aware that it has been cleaned thoroughly. The patient knows their room is clean. Further, the three separate visits by a housekeeper under our XRO team cleaning makes it highly probable that the patient will be aware that their room was cleaned. Use of “while you were out we cleaned your room” tent cards also helps. 5 “You can’t do it all in a day’s time, so choose wisely what your priorities are. Know the difference between what’s urgent and what’s important. Don’t let others determine how to use your time.” — R. C. Slocum COMMUNICATION: Under the XRO team cleaning approach, each staff member is trained (scripted) on what to say to the patient to describe how the tasks they are performing are part of the total clean. We select team members who are not awkward in talking to a patient, and have a pleasant manner. NEATNESS: Xanitos has a “Picture Perfect” program in which each housekeeper has on their cleaning cart a set of pictures of the different types of patient rooms they clean, showing exactly how the furniture, over bed table, side table, etc. are to be arranged. An over bed table that is not at right angles to the bed; a side table that is not aligned exactly with the wall and side of the bed; a visitors chair that is placed in an inappropriate position all combine for a lack of “neatness.” A further factor in neatness is establishing a partnership with Nursing in which they agree to bear some responsibility, along with EVS and maintenance, for the appearance of the patient room. For example, picking up debris from the patient floor (a la Disneyland) and being tidy themselves, such as not leaving spare linen on window sills, etc. In every hospital where we have implemented the XRO system the HCAHPS scores have increased significantly from an average of 61.2% “Top Box” in early 2008 to an average of 74.1% today, four years later. These averages are pulled down by new accounts where it takes time for our improved cleaning and systems to affect HCAHPS scores. Of the accounts we have held 18 months or longer our average HCAHPS score is 76.8%. 6 THE XANITOS DUCK Maintaining and improving hospital cleaning standards is like the progress of a duck paddling up a fast flowing stream. The duck moves serenely upstream, but underneath it is paddling hard and efficiently. This includes policing the room, removing any trash, trays, or soiled linen that is cluttering up the room; a second clean of high touch points; and provides the housekeeper with a chance to address any patient concerns or questions. swept downstream. Increased Burnishing The frequency of burnishing of the walkway areas of the patient room is increased to twice a week, using a newly available high speed burnisher with a built in HEPA filtered vacuum to minimize ccreation ea o of dust. If the patient is not present during the process a note card is left to info inform them that the housekeeping staff visited their room. Effect of Enhancements Xanitos has implemented these enhancements at several hospitals where our XRO system was in place and the HCAHPS scores were already high. The enhancements further increased the HCAHPS scores at these hospitals by an average of 6.5 percentage points. 7 Going Green at Metro With a plant covered roof and a variety of environmentally friendly technologies, Metro Health Hospital is at the forefront of the Healthcare Industry’s “green” movement. Of Metro’s “Green” alternatives, one of its greatest accomplishments is its 48,500 square foot garden roof that includes a variety of plants that bloom into burgundy, pink, amber, and purple flowers. These plant beds provide a natural and effective insulation for the hospital building, nearly doubles the life of the roof, and provides the patients with a beautiful view of the roof-top garden. In addition to its green roof, Metro has introduced water conserving fixtures; rain gardens to filter off pollutants from storm water; motion sensitive lights to conserve energy; uses both paints and furnishing that improve indoor air quality; and has implemented an aggressive recycling plan that both has a positive effect on the environment and has resulted in a 45% waste cost savings this past This beautiful hospital campus, located in Grand Rapids, Michigan, has taken an innovative approach to hospital design and incorporated several green features, becoming the first hospital with over 200 year at Metro. Parts of the new building have even been constructed using recycled building materials. Xanitos too has joined in the “Green” movement at Metro Health. In cleaning every room with its proprietary ULPA filtered vacuum, Xanitos significantly improves the environment’s air quality. Additionally, the standard microfiber mop system uses drastically less water than the bucket and mop cleaning method, cutting annual water use by 43,000 gallons and providing savings to the hospital by reducing the chemical use by 90%. At Metro, Xanitos also uses all Green Seal Certified cleaning products instead of the standard disinfecting chemicals that are harmful to people and the environment. Metro’s green program goes beyond just benefiting the hospital; they have a lasting effect on the beds to earn LEED certification environment as a whole. We welcome the following hospitals who have recently selected Xanitos to provide their EVS service. St. Joseph Hospital, Chicago Tracing its roots all the way back to 1868, this 360 bed facility is an award winning educational hospital that has served the Chicago area for nearly 150 years. A member of the Presence Health Care System, St. Josephs joins as Xanitos’ second Resurrection Health Care client. “There’s no one grand stroke that does it. It’s a lot of little steps.” — Peter A. Cohen Xanitos completed the startup process with all XRO teams fully implemented on February 10th. The EVS department at St. Joseph’s has been handed over to the permanent management team led by Philip Griffith. Cortland Regional Medical Center This acute care hospital in Cortland, New York has been serving the area for over 120 years. Cortland houses 166 beds and has over 5,600 inpatient visits each year. Xanitos was awarded the Cortland contract in March of this year, with the implementation of the XRO system following immediately. When Cortland toured Xanitos’ Olean hospital during the decision process it was so clean they couldn’t believe it and requested a tour of another Xanitos hospital that was not so clean! After seeing Wilkes Barre Medical Center, almost as clean as Olean, but where we had served for only 12 months, they made the decision to award the contract to Xanitos to manage their EVS Department. 9 Einstein Medical Center Montgomery Opening in September, 2012, this full service 146 bed acute care facility is tranquilly located within 30 acres of open green space near Philadelphia. The new building is LEED certified, reducing water usage by 30%, energy by 25%, and using ‘green’ housekeeping materials. Xanitos has been selected to provide EVS management services to the facility and will both be involved in the pre-opening cleaning and providing Xanitos’ high standard of cleaning once the facility opens. Our manager will be Sarah Maisey, our former Training Director. We anticipate that Einstein Medical Center Montgomery will become a training hospital. St. Mary’s Hospital, Athens Ranked as both #1 in Georgia for Stroke Care, and rated in the top ten hospitals in Georgia, St. Mary’s Hospital has provided top quality service to the Athens region since 1906. This hospital is part of the Catholic Healthcare East system and houses 196 beds. Xanitos started at the hospital on August 1st, with the start up being led by Evan Sirlin and Geoff Robinson. Our manager, Renzo Medrano, who has many years experience and was recommended to us by the hospital’s CEO. Kaiser Permanente – Santa Clara Medical Center The Proof of the Pudding is in the Eating After several years of Xanitos’ services, many of Xanitos’ partners are coming back for more! The following hospitals have signed multiyear extensions to continue Xanitos’ EVS services at their facilities: In August we commenced our housekeeping management services at this 330 bed, 1.3 Million square foot medical center and medical office buildings. Our management team, led by Bill McKenzie, is managing over 200 housekeeping employees. Santa Clara is Kaiser’s flagship in Northern California, and we are proud to be associated with it. • Froedtert Memorial Lutheran Hospital • South Bend Memorial Hospital • South Shore Medical Center Kaiser Permanente – Richmond Medical Center This Kaiser Permanente 50 bed Hospital is located in the East Bay Area for Northern California. Xanitos started serving this facility in January 2012, and things have gotten off to a great start. Richmond has a reporting relationship with Kaiser’s Oakland Medical Center, which Xanitos already serves, and our performance there has led to this opportunity at Richmond. After a successful startup, Richmond continues to improve under Chris Swinton’s management, with HCAHPS just recently hitting 80% top box score. • Altoona Regional Hospital • Adventist LaGrange Memorial Hospital Thank you to those hospitals for their continued confidence in Xanitos. The single most significant outcome we have seen relates directly to patient satisfaction scores that are attributed to our hospital. Frequent visitors to our hospital have made comments about the hospital being notably cleaner. Another outcome has been the consistency of the services provided. We still put out fires but so much less frequently than before. Systems are now in place to ensure efficiency of operations. 2 How was the initial transition? The original transition actually went much better than I anticipated. The Xanitos team came aboard several weeks before conversion date to ensure a successful transition. I give a lot of credit specifically to the team leader Jose Feleciano who engaged not only the EVS team but the Nursing Leadership as well. His professionalism and drive to make this a successful startup was so appreciated by our Executive Team as well as our entire leadership team. In my mind, the XRO system has changed the way our patients, staff, physicians and visitors see Resurrection Medical Center. I had a visitor recently comment to me that she knew something had changed because the hospital is so much cleaner that it has been. She says she knows this because she has been coming to RMC for many years so she “knows.” I am excited about the changes I have seen, have been impressed with the flexibility of Xanitos to adapt to some of our specific needs and appreciate their willingness to find a leadership team that function well with our staff and hospital leaders. 14 EMPLOYEE SPOTLIGHT Sean Shapert In 1991 Sean Shapert had a choice, environmental services in the healthcare industry or laundry services management. After a brief visit to a hospital in Atlantic City, New Jersey, the choice was clear – Sean began his career in environmental services. While at Marriott, Sean developed strong customer relations skills and learned the value the front line associates bring to an EVS department and to the company as whole. “You need to treat people with respect, empower them to make good decisions, let them know you value their input,” Sean states, “I could not have asked for a better organization to learn the basics.” Following several years at Marriott, Sean joined Crothall Services. They were a specialist company at that time, focused solely on EVS management – something that appealed to Sean. By March of 2001 Sean was overseeing six clients and the Cleanroom Division as one of Crothall Services’ “Sean leads from the front. I am fortunate to have an understanding wife and children. Sean’s passion and drive inspires all of us.” 16 AROUND TOWN Going from Good to Great at Cortland Regional Medical Center Every quarter Cortland Regional Medical Center awards one of its departments the Good to Great in Patient Satisfaction award to recognize the department that achieved the greatest improvements in HCAHPS scores. For the first time ever the Environmental Services Department at Cortland has received this award by increasing HCAHPS from 60% to 79%. Safety Moment: Push, Don’t Pull The XRO cart is constructed such that it only needs 8 lb. of push force to move it along corridors, well within the 33 lb. max acceptable limit, making it an ergonomically low risk work activity. The cart is designed to be pushed, not pulled. If you pull a cart while walking backwards, you risk running into things or people. If you pull a cart while walking forward, you can catch your heel on the wheels or bottom of the cart. Additionally, if you have to push a tall cart or object, stand slightly to the side to see around it, or have a co-worker walk in front to be your guide. Employee Satisfaction an All Time High Froedtert Lutheran Memorial Hospital gauges its employee satisfaction using Morehead’s survey that ranks employee satisfaction in tiers, with Tier 3 being overworked and disengaged and Tier 1 representing the most engaged and functional work units. When Xanitos arrived at Froedtert the EVS employee satisfaction ranked at a very low Tier 3. Within 18 months that ranking rose to Tier 2. Now, after another year and a half of rounding, staff meetings, and huddles every day, every shift, the Froedtert EVS team has finally reached Tier 1. This, despite the fact that Alan Ferris, V.P. of Moorhead, stated that attempting to reach Tier 1 required a three to five year culture change. Xanitos did it in 18 months! Gary Colpaert, Vice President Perioperative Services at Froedtert, stated that without EVS scoring as well as they did, the improvements that the hospital as a whole has reached would not have been possible. Practice makes perfect. Over the past five months, seven of the Kaiser Permanente facilities we serve had Joint Commission Surveys. There were no recommendations at any of them, and the Joint Commission had positive comments on the cleanliness of the hospitals. Of course this is what our clients expect of Xanitos, but with so many surveys in such a short time at hospitals where our program has only been established for a few months makes it noteworthy.
