T
he New York State Dental Foundation
will host “The Current State of
Children’s Dental Health in New York:
Reviewing the Best Practices for
Reducing Childhood Caries” on
Thursday, June 18, and Friday, June 19. The workshop
will bring together dental professionals, policymakers
and others with a vested interest in children’s dental and
overall health to discuss the current best practices for
reducing and treating early childhood caries and to
develop a statewide approach to preventing and managing ECC.
The ADA recommends that children visit the dentist for the first time within six months of their first
tooth eruption and no later than their first birthday. Yet,
few infants enrolled in the Medicaid Program in New
York State are being seen. In 2013, 1 in 266 infants
enrolled in the program received oral health services
from a non-dental provider; even fewer, 1 in 355,
received oral health services from a dentist. By the time
these children are in Head Start, two in five have a history of tooth decay. ECC are preventable, yet if not
treated, they end up being detrimental to the daily life
of a child and his or her future dental and overall health.
Day one of the workshop will feature presenters

Day two of the workshop will feature small group
discussions of incorporating interventions into communities throughout the state, a facilitator-led large group
discussion that will bring together the ideas of the small
groups and brainstorming an action plan.

Barnum’s Gift of
the Rubber Dam
For inventing an indispensable
dental tool, Sanford Barnum received
accolades, but not a penny.

A 19TH CENTURY MEMBER of the Dental Society
of the State of New York and the First District
Dental Society gave a gift to the dental profession that is found today in every dental practice
in America and is utilized countless times, worldwide, every day: the rubber dam.
Sanford Christie Barnum was born to
George and Caroline Barnum in upstate New
York, in Oakland Valley, Sullivan County, on
August 24, 1838. Barnum received much of his
early education at the Monticello Academy,
which, at that time, was a well-known private
educational institution. At the age of 18, he entered
the office of his uncle, Dr. Joseph Clowes, who
had a dental practice in New York City. Barnum

is discouraging to
learn that there is a
dearth of certified
dental assistants in the Fourth District and around the
state, I commend Dr. Waldman and Mr. Balacky for
clearly defining the supportive services a certified dental assistant can legally provide to a dentist. This is
invaluable information for dentists and should go a
long way toward protecting them from inadvertently
violating the law.

Clifton Park Bugs are Beautiful
I
Central New York dentist teams up with daughter to dispel fears and
false notions about things that crawl, fly and sometimes swim.
f the thought of cozying up to a bug makes
your skin crawl, it’s time you were introduced
to “Dr. Zach’s Amazing Catskill Critters” (Mr.
Nick Productions LLC; 2014). Ten minutes
with this bright and colorful book and you’ll
be devouring and enjoying tales about personal encounters with all manner of crawly things.
The book is a collection of articles which originally
appeared in the regional magazine Kaatskill Life that were
written and illustrated by Central New York prosthodontist Nicholas Zacharczenko and his then-teenage daughter, Brigette. Dr. Zach, as he prefers to be called, is an
amateur naturalist who has been collecting insects and
mounting butterflies practically all his life. Brigette, who
inherited her father’s enthusiasm for things that live
under rocks, went on to study applied zoology at McGill
University and is currently finishing her Ph.D. in entomology in the Department of Ecology and Evolutionary
Biology at the University of Connecticut.
If there is anything to be learned from this book it’s that
with knowledge comes respect, in this case, for insects, bugs,
snakes, toads and turtles. Even spiders, generally at the top
of many people’s loathsome list, don’t seem so bad when
viewed under the clinical microscope of a collector—which
is not to say all insects are good. The Zacharczenkos include
in their writings mites, chiggers and ticks, acknowledging
that while these critters are fascinating to observe, they do
possess a dark and destructive nature.
There are fun facts here—did you know infant
mice are called kittens? Facts to counter erroneous
assumptions—earwigs do not crawl into people’s ears.
Surprising facts—even the longest millipede has only
200 legs. And just plain facts—being a true bug means
having mouth parts fitted for piercing and sucking.
How is it that some people find bugs fascinating
when others are downright repulsed by the sight of
them? Dr. Zach explains he developed an affinity for
insect collecting as a child growing up in Delhi, Delaware
County. Absent the Internet, computer or cable TV, he
occupied himself roaming through “field and stream”
honing his skills as an amateur naturalist. He received
encouragement—and instruction—from his friend and
mentor the Rev. Dr. Cameron Reed, Dr. Zach said, as
well as from his mother, Valencia, who he said always
encouraged him to look for “bugs and slugs.” “Dr. Zach’s
Amazing Catskill Critters” is dedicated to the author’s
mother, as well as to his wife, Sherelyn, and son, Alex,
who, Dr. Zach said, put up with “a myriad of creepy,
crawly pets in our home over the years.” (All captured
critters were relegated to “Zach’s Nature Center” from
where they were rotated back into nature after a period
of study and photography.)
The fourth person in the dedication is, of course,
Dr. Zach’s daughter, Brigette, the author and co-author
of many of the tales in the book. Dr. Zach describes his
daughter as a “born naturalist.” She herself recalls spending her earliest years looking under rocks for pill bugs or
patrolling a backyard pond for dragonflies and frogs.
She’s been writing about insects since she was in grade
school, she said, and harbors the hope that through her
writings and public outreach work she can “inspire budding naturalists of all ages to go outside and find some
of their own Catskill critters.”
Nicholas Zacharczenko took time out from bug collecting to get degrees from Albany College of Pharmacy
and New York University College of Dentistry. He completed a three-year master’s degree in prosthodontics at
Manhattan VA Hospital. He sees patients at his practice
in Cooperstown, but is semi-retired from dentistry,
which leaves him with more time for his other passion—
participating in competitive tennis tournaments, and
track and field events in his age group. z

spent the next four years as a dental student/apprentice
to his uncle. At the completion of his service, he
returned home to Monticello to start his own dental
practice.
Barnum knew his dental education was lacking, so
he returned to Manhattan and enrolled as a student at
the New York College of Dentistry, forerunner of the
New York University College of Dentistry. He would
become one of the college’s first graduates when he
received his Doctor of Dental Surgery degree on
December 2, 1868.1
After graduation, Barnum remained in Manhattan
and opened a practice in his uncle’s house. He was a very
skilled practitioner and soon developed a
lucrative dental practice. It was during
this time that he discussed with his uncle
an invention that he had made a few
years prior while practicing in Monticello.
It was a rubber dam, meant to keep the
field free of saliva during operative dental procedures. Barnum first used his
device on a Monticello jeweler, Robert C.
Benedict, in 1862. Prior to Barnum’s
invention, practitioners had to try to keep
the teeth dry using napkins, cotton and
other absorbents.2
Both Barnum and his uncle saw
the great financial possibilities of such
an invention. However, Dr. Clowes
requested that his nephew relinquish
any possibility of pecuniary gain and
present the rubber dam to the dental
profession as a gift. Barnum agreed
wholeheartedly; he never received any
financial reward from his invention.
The rubber dam was readily and
eagerly accepted by the better dental
practitioners of the mid-19th century.
And its inventor was the recipient of
many honors, bestowed upon him by a
grateful dental profession. Among these
honors was a large gold medal presented by the American Dental
Association for “the invention, perfection, introduction, and donation of the
rubber dam to the dental profession.”1
He was also honored by the American
Dental Society of Europe and the
California State Dental Association,
among other dental organizations.
Barnum practiced both with his
uncle and by himself for the remainder
of his career. He was afflicted with
chronic meningitis during the last 10
years of his career, which was never
properly diagnosed and caused him
excruciating pain. He was said to have
stoically managed the symptoms of his
disease for years until it finally forced
him to retire from his beloved profession.
Barnum returned to his father’s
home in Monticello to live out his
remaining, painful days. Unfortunately,
it was during his time as in invalid that
Barnum was confronted by jealous
detractors, who tried to take credit for
his invention and who circulated a
vicious rumor that Barnum was in need

of financial support and had requested that every member of the profession send him a dollar. Barnum denied
this in writing and returned whatever money sent to him
whenever he could summon the strength.
Sanford Barnum, the man who gave the dental
profession one of its most useful gifts, died on Christmas
Eve of 1885 at the age of 47. He was buried in St.
John’s Cemetery in Monticello. z

A SECOND Community Water Fluoridation (CWF) Spokesperson training
session sponsored by the New York
State Dental Foundation is set for Friday,
July 24, in Syracuse. Instructors are Matt
Jacob of the Children’s Dental Health
Project in Washington, D.C., and Meg
Atwood, R.D.H., M.P.S., associate professor in the Department of Dental
Hygiene at Orange County Community
College and successful advocate for
CWF in Middletown.
Dentists, dental hygienists and
representatives from local health
departments and water systems are
invited to attend to prepare as future
spokespersons for CWF, learning how
to best communicate with town councils, media personnel, community
members and legislators. This training
may serve as an effective resource as
questions are raised about the validity
of CWF after the April 27 recommendation of the Department of Health
and Human Services to reduce the
level of fluoride in drinking water to
0.7 parts per million.
The session runs from 8:30 a.m. to
2:30 p.m. and is free to attendees.

on the science and myths of fluoridation.

have been granted accreditation by the
Commission on Dental Accreditation
for a newly created general practice dental residency
program. Students in the one-year program will rotate
between the UB Dental and RPCI Dental Services.

A GUIDE FOR PARENTS
What Not to Say When I am Treating Your Child

A QUICK GLANCE into your waiting room, or a whispered comment by your dental assistant, and you realize
you might have a rough time with your next patient—an
obviously anxious child.
So, you and your dedicated team put on your most
kindly faces, project a tranquil and friendly demeanor,
establish a quiet and relaxed environment, and try to
mesmerize the child into being a good patient.
And then, has this ever happened to you?
Suddenly, just when you have succeeded in getting
the child to be composed, compliant and cooperative,
the parent blurts out a loud, inappropriate comment that
ruins everything?
In order to avoid such happenstances, I present the ultimate list of what parents should not say in front of their
children in the dental office.
1. “Don’t worry; he’s not going to do
anything today.”
Until you uttered that very unhelpful phrase, it may
never have occurred to the child that he, in fact, should
worry.
And do you realize that by telling the child not to
worry because I am “not going to do anything,” it
implies that if I ever have to actually “do something,” it
should be a legitimate cause for very real and tangible
worry?
2. “He wants you to open your mouth.”
When we ask little Suzy to open her mouth, we really
don’t need you to translate our request into English. By
your speaking up, you break the spell; the child, instead
of being focused on us and listening to our voice,
focuses instead on you.
3. “Don’t worry; I won’t let him hurt you!”
Is that comment really helpful? Is it you and the child
united against the big, bad dentist?
Instead, you should be teaching
your child that you and the dental team
are partners. And that we can be trusted
to take good care of the child.
4. “Have you ever done a
procedure like this before?”
No, I never have. But, I did review that
chapter in the dental textbook last night.
5. “Are you giving him a shot?”
When we tell your child that we are putting “sleepy water” on the tooth, we are
not trying to be silly. We chose our words
carefully, because we realize (and so
should you!) that the spoken word has a
powerful effect.
So, humor us and play along with
us, when we clean (not “drill”), or wiggle
(not “pull out”) a tooth.
6. “Is this going to hurt a lot?”
What scale of parental comprehension
leads you to believe that such a comment
helps create a relaxing atmosphere for
your child?
7. “I told you not to eat candy!
It’s your fault that this is
going to hurt!”
Does that help relax the child? Do you
really want him to feel that our treatment
is a punishment?
And, by the way, since your child
doesn’t have an independent source of
income for purchasing candy, we tend to
blame his supplier (you!).
I realize that my list is not definitive,
and I encourage readers to submit additional suggestions.
It is so very easy for parents to hinder the rapport that we are trying to
establish with their children. An ill-considered comment by a parent can completely destroy the relaxed mood that we
attempt to create.
In order to avoid such thoughtless
comments impacting negatively on a child’s
conduct, many dentists do not allow parents into the operatory. In our office, we
encourage parents to stay with their children in the treatment room, but we advise
that “a silent, smiling, parental presence is
extremely helpful.” z

NYU and Columbia Dental Schools Partner on Test of
Nonsurgical Model for Early Childhood Caries
N
ew York University College of Dentistry
has received a sub-award in the amount of
$967,235 from the Center for Medicare
and Medicaid Innovation to coordinate a
consortium of pediatric dental residency
programs that, together, will provide “dental homes” for
children enrolled in a program to test a nonsurgical disease
management model for early childhood caries.
The model uses family-level, peer-counseled and
technology-assisted behavioral risk reduction strategies to reduce the following: new caries; the progression of existing caries; caries-associated pain; and,
ultimately, the number of children seen in the operating room for highly invasive and costly treatment.

The nearly $4M total grant was awarded to Columbia
University College of Dental Medicine, where the
principal investigator is Burton Edelstein, professor of
dentistry and health policy. The NYUCD portion of
the grant is led by Amr Moursi, associate professor
and chair of the Department of Pediatric Dentistry.
“The project aims to divert children with early- and
advanced-stage early childhood caries (ECC) from highcost surgical treatment to low-cost, nonsurgical disease
management,” Dr. Moursi said. “Together, parents and
community health workers will use MySmileBuddy
(MSB), a mobile, tablet-based health technology, to
plan, implement and monitor positive oral health behaviors, including dietary control and use of fluorides,
which can slow or arrest ECC’s progression.”
Through home visitations, community health
workers will meet in person with parents of children
with early-stage ECC bimonthly for one year, and with
parents of children with advanced-stage ECC weekly for
the first four weeks, then bimonthly thereafter for the
remainder of the year. In addition, community health
workers will provide tailored telephone intervention

THE NEW YORK STATE DENTAL
Association has selected 10 dental
hygiene students from across New York
to receive its Albert H. Stevenson Award.
The award, which is presented each
year to a graduating student of each of
the dental hygiene schools in the state,
was established by the Dental Society of
the State of New York to commemorate
the work done by Albert H. Stevenson in
the field of dental hygiene.
Dr. Stevenson was a tireless advocate in the oral hygiene field and communicated its importance throughout the
country. He developed the School of Oral
Hygiene at Columbia University and early
on became its director. NYSDA honors
students who demonstrate leadership qualities in the field of dental hygiene and who
display the same level of enthusiasm in the
field as Dr. Stevenson did.

NYSDA Members Can Save on
Workers’ Compensation Insurance
with New Safety Group
WORKERS’ COMPENSATION INSURANCE is one of
two coverages that New York State requires most
employers to provide their employees. The second is
New York State Disability Insurance. “Workers’ comp”
provides financial benefits to workers injured on the
job, while NYS Disability Insurance provides temporary
cash for illness or injury that occurs off the job, and for
disabilities arising from pregnancies.
New York State began requiring workers’ compensation coverage in 1914. At the same time, it established the New York State Insurance Fund, a quasi-
governmental insurance company. The fund was created to guarantee the availability at all times of workers’
compensation protection at a reasonable cost to any
employer seeking coverage. As the years went on, the
State Insurance Fund became one of the most competitive and accessible carriers of workers’ compensation
insurance in New York.
Fast forward to 2015, and the competitive edge
the State Insurance Fund once enjoyed has dissipated
for many groups. There are now many more insurance
carriers that write workers’ compensation policies and
many that offer more competitive
premiums, especially for certain
occupations. The State Insurance
Fund, in fact, now insures less than
half the total New York State market,
with more people choosing to insure
with private carriers.
NYSDA recently endorsed a new
workers’ compensation program offered
through Arthur J. Gallagher & Co. to
provide one of the most competitive
workers’ compensation programs for
New York State dentists. The program
is offered through a limited “safety
group” that not only offers lower rates
but is also “dividend-eligible.” That
means if the claims experience is
favorable, employers may receive a
return of premiums in the form of
annual dividends.
Many businesses, among them
dentists, may be part of the State
Insurance Fund because it is the only
option their broker offers, or they
may have a past history of claims
that forced them into the fund. No
matter what the reason, you owe it to
yourself to get a quote for this new
NYSDA program to see if it can save
your practice money.
It is important to keep in mind
that if you are insured with the State
Insurance Fund and want to receive
a quote, you need to send the fund a
“Reservation of Rights” letter at least
30 days prior to your renewal date.
For example, if your policy renews
on July 1, you must send a signed
Reservation of Rights letter sent to the
State Insurance Fund by June 1. This
letter does not signify you are canceling coverage or leaving the State
Fund, it simply is letting the fund
know that you are reserving the right
to look elsewhere. If a quote is not
competitive to your current premium,
you can still opt to renew your policy
at the same expiring date even
though you have sent the letter. There
is no penalty for sending the letter;
there is only a penalty if you don’t
send the letter and decide to move
the policy to another carrier.
