Unfortunately, there is nothing we can do about it since the deter~ination that such allowances to personnel assigned to Guatemala should cease September 1, 1947 was made by representatives of the President and ver,y definite regulations pertaining to payment of living allowances promulgated and printed in the Federal Register. I know adjusting your several budgets to this change in policy will be diffioult but, believe me, we in Washington are going through the sa~e thing - with eggs selling at $1,00 a dozen, butter $1.00 a pound and ever.ything else rising in cost acoordingly. For the benefit o! your staff, I would like to say that the decision to cut off Guatemala from the high cost list was made after much deliberation by the Board which had complete information on basic living oosts. No evidence was presented which, in ~ way, reflected higher cost in Guatemala as compared to Washington. Please tell Dr. Levitan, Mr. Portnay and Miss Vlalker how very sorry we are that we are unable to 11ve up to the agreement made at the tilDe they conaented to go to Guatemala. Cutler Sanidad Publ ica 9& Ave. Sur y 15 Calle Oriente Guatemala City, Guatemala Dear John: We have just completed & telephone conversation with Doctor Heller in regard to the discontinuance of the foreign service maintenance allowance for personnel in Guatemala. We were unable to find any legal way of circumventing the ruling or of making a compensatory financial arrangement. The big problem at the monent is an ' estimate of the impact which the ruling will have upon the Guatemala unit. The effect upon Portnoy and ~;iss Walker can be f ores em • We have canvassed the possibility of securing an increase in grade for them as a means of compensating for the financial loss. This innovation, however, has been vetoed by the Washington group. The matter places us in a position of not. living up to our part of the agreement which was made with the personnel at the time they consented to go to Guatemala. In the event of their expressing a desire to return to the States, we can ally feel that they are entirely justified. At the moment not any replacement peraorme1 are ava.i1able who would agree to go to Guatemala on the baaia of the present financial arrangement, especially in view of the fact that any agreement which might. be effected today might not be valid tomorrow. You will receive an official notification of the ruling in the very near future, if it has not already reached you. I enjoyed my visit to rour home city and have seleoted the spot in the city square where they will eventually erect a monuJIIent to you. The APHA meeting Was entirely painless and very informative. Harris and Thayer had papers and both did very well. In a recent note Doctor Heller expressed a desire to visit Guatemala. He suggests a date early in February. The plan is to attend a meeting in Monterey, Mexioo, on February 6th and 7th and then to proceed to Guatemala City. I will probably accompruly him. I keep forgetting that Doctor Spoto is permanently in Guatemala. Have not been sending oorrespondence through him but will do so in the future. 1 haYe the highest regard for his judgment and ability and 1 teel very fortunate in having him on the grounds. We regret very much the loss of the essential parts ot the Barnstead still. It is not the financial loss involved as much as the difficUlty encoWltered in replaoing the parts. We will contact the makers and have the items of equipment and the necessary intormation torwarded to you as soon as possible. Would adTise the utmost caution in the handling of electrical equipment . in order to avoid further difficulties in this regard. }/..r. Morrison is forwarding some torms to be used for requisitioning supplies. Many ot your recent requests have been contained in the body of letters and we have difticul ty in following through on their purohase. 'rhe requisition form will simplify the entire procedure. I suggest that an effort be made to arrange this matter through the Pan We would be more than glad to act as American ~n1 tary Bureau. an agent in the reatter. A shipment of penicillin is going forward to you today. Suggest that you use it in any manner which best fits the general situation. 1 would also suggest the advisability of not entering into a too comprehensive program which may involve the use of more of the drug than we are able to procure. The embargo on shipping appears to be at an end and the remainder of the laboratory equipment will be placed on board ship here on November 20th. It should reacil you shortly thereafter. The items of heat controlling equipment whioh you mention are being purchased and we will forward them to you as soon as J;lossible. Between the trucking strike and the maritime strike the harbor of New York is stUl practically olosed and we are unable to obtain any precise information as to when the acoumulated cargo ma:,r be moved. We have oontaoted the Army Transport Service in regard to employing that facility in moving freight to you. We were i ntormed that the Army does not maintain a fl~ng field in Guatemala which was contrary to my impression, BS I thought that the Air Corps had a field in Guatemala City. The Army did have regular service from West Palm Beach to Panama at a cost of 60p per pound. I wonder if you would check on the situation and 88 to the possibility of moving material up from Panama City to Guatemala City. \Ie were all pleased to learn that Joe Spoto is returning to Guatemala Oi ty. He can hardly fail to be of real assistanoe to us as our work unfolds. I do not know of any man 19ho could be more helpful inaLatin American country than Joe. Your show is already attraoting rather wide and favorable attention up here. We are frequently asked as to the progress of the work. Doctor T. B. It concerns the study of the final phase of the prorhylaxis subject. As you know, we have delayed setting up a field trial of the prophylactic agent in the hope that the Guatemala work would give precise data which would support, even in a smll way, the experimental findings in anirmls. From your reports both Arnold and myself are somewhat pessimistic as regards the possibility of setting up a ccntrolled e.xperimmt in gonorrhea within the range of the patient material which will be available to us. Also, in syphilis, unless we can trar.::;rdt the infection readily and without recourse to scarification or direct implantation, the possibilities of studying the subject are not bright. Both of the latter procedures are drastic, are beyond the range of natural transmission and will not serve as a basis for ~he study of a locally applied prophylactic agent. The Army now desires that we join them in guiding a field study :in prophylaxis. The first inclination was to select field organizations in Germany. This has been changed to Italy. We have agreed to undertake a pilot study, which, if successful, will be followed by a statistically sound investigation to be conducted entirely by the Army. The work of organization is going on but the timing of the onset of the study is not in sight. Arnold feels that either you or himself should guide the laboratory's part of the show. This detail should not require a prolonged stay. The work will be done by a joint commission of Army and Public Health Service men, with the Army doing the official part of the job. A new angle has developed in regard to whioh I hope that you and Doctor Spoto will make a mental note and then destroy this letter. Dootor Van Slyke made a hurried trip trom Washington recently to tell us that llarry Eagle is about to complain to the Surgeon General that I have not been extremely enthusiastic about allowing him to enter the Guatemala study. As you may know, he has done oonsiderable animal work in prophylaxis in syphilis by use ot penicillin and can only prove the thesis by a human experiment. He discussed the matter with me several weeks ago but I could not see wherein a study ot that kind would have other than an academio value it an injection technio was employed. I thought it would be ot still less importance if an oral preparation of penicillin was to be studied as a prophylactic agent. Further, I do not care much about having anyone join the stUdy who could not share in the responsibility tor the entire program. We are soheduled to have a meeting in Washington later in the month, at which time the entire matter will be discussed. I shall intorm you as to the outcome. Vie also gather the impression that Doctor lleller would feel considerably more seoure it we were to set up an advisory group of leading figures in the world ot science to serve as a background tor the study. I have never been a believer in this type of thing and I do not think that an advisory committee would help us greatly, but we might have to deter to political expediency. There are several men whom I would not mind being aSSOCiated with the work. There are several other leading figures wilo, I think, would be a distinct detriment • . Dear Mr. Van Herpe: In reply to your note of the 29th, it would seem inadvisable to grant an unlimited travel authority to Doctor Cutler if the expense of such travel is to be borne by the graat-in-aid fund. These tunds were made available tor a specific purpose which centers in Guatemala and difticulty would be encountered in justitying expense incurred in travel elsewhere. No objection would be interposed for Doctor Culler being of any assistance possible to the Pan American Sanitary Bureau in other Central and South American oountries provided the travel involved would not interfere with the conduct of the Guatemalar.project, and that the expense inv~lved be defrayed fran funds other than those earmarks d tor venereal disease work.
