telecommunication closet improvements, wireless and network electronics upgrade with 10GE WAN and IP video
systems and display devices plus IP security enhancements.
Keshequa Central School District – Nunda, New York
District Wide Telephone System Replacement Project— Assessment of existing NORTEL PBX and voicemail systems
and scope and budget cost to replace with a new Cisco telephone and voice mail system. IP telephones will be
installed throughout the district and new cabling and network electronics upgrades will be included. Converged data
and voice IP network to achieve district wide communication across the existing 100MBS WAN. Telephone PSTN
services will be revised to reduce operational cost.
York Central School District – Restof, New York
VOIP Network Readiness Study & Pre-Referendum Planning for a District Wide Telephone System Replacement
Project. Assessment of existing NORTEL PBX and voicemail systems and scope and budget cost to replace with a
new Mitel VOIP and Cisco network solution.
SUNY Brockport – Brockport, NY
Campus Wide Network Electronics and Wireless Upgrade to provided robust wired and wireless data capabilities to
support growing demand for bandwidth across the campus. Faculty and students are typically bringing 3-4 devices on
campus, which all require connectivity to the network. The new network electronics and wireless solution will be
based on a Cisco platform and utilizing the new 802.11ac wireless access points to significantly enhance wireless
connectivity in all facilities and outdoor venues on the campus. Project cost is approximately $2M and ECC
Technologies will also assist College IT staff with implementing the solution.
ECC Technologies is also assisting the College IT Department and Facilities Planning Department with the
development of a migration strategy to implement Voice Over IP (VOIP) to approximately 2000 staff and students

Project Scope
The scope of the project is fully defined in section 5G Description of Service. Below is an estimated timeline of these tasks for a
single district building, shown for both Basic and Enhanced.
The time scheduled for these activities is directly related to the studies that are selected. The 5-year long-range plan would be
completed 2 weeks after the completion of the BCS for each building. All additional “study” or enhanced survey items will be
complete within 3 weeks of the completion of the BCS. Final input of the BCS into the State web-site will be complete within 2
weeks of the completion of the BCS or per the State’s directive on web availability.

By signing this form, the insurance carrier identified in Box "3" on this form is certifying that it is insuring the
business referenced in Box "1a" for disability benefits under the New York State Disability Benefits Law. The
insurance carrier or its licensed agent will send this Certificate of Insurance to the entity listed as the certificate
holder in Box "2". This certificate is valid for the earlier of one year after this form is approved by the insurance
carrier or its licensed agent, or the policy expiration date listed in Box "3c".
Please Note: Upon the cancellation of the disability benefits policy indicated on this form, if the business continues to be named
on a permit, license or contract issued by a certificate holder, the business must provide that certificate holder with a new
Certificate of NYS Disability Benefits Coverage or other authorized proof that the business is complying with the mandatory

DISABILITY BENEFITS LAW
Section 220. Subd. 8
(a) The head of state or municipal department, board, commission or office authorized or required by
law to issue any permit for or in connection with any work involving the employment of employees in
employment as defined in this article, and notwithstanding any general or special statute requiring or
authorizing the issue of such permits, shall not issue such permit unless proof duly subscribed by an
insurance carrier is produced in a form satisfactory to the chair, that the payment of disability benefits
for all employees has been secured as provided by this article. Nothing herein, however, shall be
construed as creating any liability on the part of such state or municipal department, board, commission
or office to pay any disability benefits to any such employee if so employed.
(b) The head of state or municipal department, board, commission, or office authorized or required by
law to enter into any contract for or in connection with any work involving the employment of employees
in employment as defined in this article, and notwithstanding any general or special statute requiring or
authorizing any such contract, shall not enter into any such contract unless proof duly subscribed by an
insurance carrier is produced in a form satisfactory to the chair, that the payment of disability benefits for
all employees has been secured as provided by this article.
