CHECKING ACCOUNT
All teams need a team checking account. KC Select strongly recommends keeping team funds
separate from your own personal funds in a team specific checking account. You may bank at the
bank of your preference. Most banks require the following documentation to open a checking
account. You should check with your bank to see what they need.
1. Letter confirming that you are a member team of KC Select. You will need to request this form
from the KC Select Registrar by sending an email to Jennifer Mathis at registrar@kcselect.org.
Include your Name, Team Name, Team Gender and Team Birth Year.
2. Fictitious Name Form (DBA – Doing Business As) – This form can be obtained online through
the Missouri Secretary of State office at https://bsd.sos.mo.gov/loginwelcome.aspx?lobID=1
You must set up an account and log in. Go to the Fictitious Name section and select Register a
Fictitious Name. Enter the name of the team (Example: KC Select Academy 2011). Next, enter
your personal information for the address. Go to the Owner page, select “Owner is Individual”
and put ownership as 100%. Enter your personal information as the owner of the account. Got
to Review the Filing and select the check box to acknowledge the information is correct. Save
the filing and select the check box regarding false statements. Select to go to the shopping cart.
Pay for the filing. The cost for doing a Fictitious Name Filing is currently $7. After you pay for
the filing, a printable copy will appear. Be sure to print a copy for your records. You should
also save a copy. You will need this to provide to the bank.
3. Employer Identification Number (EIN) – This form can be obtained online through the IRS.
These numbers are no longer just for employers but are also used for organizations, teams, etc.

 Select Begin Application.
 Select “View Additional Types, Including Tax-Exempt and Governmental Organizations”

 Select “Sports Teams (Community)”, Select Continue
 Enter your personal information including your social security number and select “I am
a responsible and duly authorized officer…” and select continue.
 Enter your personal address where you would like to receive mail.
 Accept the database version of your address if one is found.
 Enter your team name, doing business as or DBA team name, enter your County, State
and Month and Year.
 Select “No” on all questions.
 Select “Other”.
 Select “Organization (such as Religious…athletic, etc.)
 Select “Athletic” and enter “Soccer Team”
 Select “Receive Letter Online”
 Select “Submit”
 Print and Save your IRS Letter that appears.
Take all of your documents to the bank of your choice. When setting up your account, you should
request checks and a debit card for the account. If there are charges for opening your account, then
those costs should be passed on to your team. If you collect a Team Administrative fee, then you can
use that money to cover your bank fees.
TEAM REGISTRATION – Due to Registrar by Monday, June 27, 2016
Once your coach has determined the team’s player roster, you need to
collect and organize the player’s address, phone numbers and email
addresses. You will need to fill out a Team Registration Form. This
can be found on the website under Team Manager Forms & Information.
This form will be used by the KC Select Registrar to card players to your
team and create your team roster so it needs to be accurate. Players can
be listed as primary (P), secondary (S) or club pass (CP). Primary
players are players that are primary to a KC Select team. Secondary
players are players who are primary to a team outside of KC Select and
are dual carding with your team. If you are utilizing a secondary player,
then you need to fill out the Dual Roster Form for that player which is
found on the website. Incomplete forms without all signatures will not be
accepted. Club Pass players are players who are primary to another
KC Select team but are also playing (or dual carding) with your KC Select team. Discuss with your
coach and be mindful of any league rules that may apply with regards to dual carding between teams.
If you are unsure what designation to put for a player, then contact the Registrar.

TEAM PAPERWORK –Due to Registrar by Monday, June 27, 2016
KC Select teams will be registered and carded through Missouri Youth Soccer Association (MYSA) and
we will be using GotSoccer to process rosters and player cards for the 2016-17 soccer season. MYSA is
a state organization that is part of the United States Youth Soccer Association (USYSA). USYSA
follows the standards set by US Soccer. Should you need assistance with player cards, rosters or
injury forms, you should contact your Club Registrar first before contacting MYSA. MYSA should
only be contacted as a last resort when your Club Registrar cannot help you.
You should collect the following required paperwork from each of your
players:
•
Copy of an official state birth certificate (no hospital certificates
or passports are acceptable)
•
USYSA Parental Consent and Medical Waiver Form (provided
to all players at tryouts)
Team managers are responsible for collecting all paperwork and
submitting a COPY ONLY of the medical waivers and the birth
certificates for all players. This paperwork should be submitted
with the Team Registration Form by no later than Monday,
June 27, 2016.
COACHES/MANAGER BACKGROUND CHECK
All coaches, assistant coaches, managers and parents who are working closely with a team in games or
practices need to register with KC Select using GotSoccer and do a risk management background
check. This is done through a link provided by the KC Select Registrar in July. There is no cost to
you to do this. Once the Registrar contacts you that registration is open, please do this as soon as
possible so your card can be processed and make sure your coach does it too. The risk management
check is good for two years.
GOTSOCCER PLAYER REGISTRATION
This year, players will need to register with KC Select in GotSoccer. Missouri Youth Soccer
Association (MYSA) uses GotSoccer as their source to create rosters and player cards for teams with
the help of Club Registrars. An email will be provided to
coaches/managers that can be sent to each of the players
who have verbally accepted a position on a team.
Players must register and pay the non-refundable initial
deposit fee to accept a position on the team. Players will
also sign up for a payment plan that allows them to pay
the remainder of their registration, league and coaching
fees. A list of fees and the payment plans can be found
below in the Appendices. If you have a player who is
requesting a scholarship, then please contact the KC

Select Registrar at registrar@kcselect.org to request a Voucher # and how to use it. Vouchers will
waive specific fees for a player if they have preliminary approval as a scholarship player. All
scholarship players must receive final approval from the KC Select Board after submitting their
scholarship application. Information on scholarships are located later in this document.
Managers will be provided with a GotSoccer team account by the KC Select Registrar. This account
contains a list of players who have been placed on your team roster and all of their information.
Please note that if you are utilizing Club Pass players to supplement your roster then those players
cannot be added to your team until after August 1 when MYSA resets their dates in the GotSoccer
system. Club Pass players have to be added manually by the Registrar so they will be added as soon as
possible to finalize and print rosters and cards after August 1. Teams will receive their Official Roster
and Player Cards from the KC Select Registrar after August 1. You will need to pick up your roster and
player cards at the following address: 3035 SW Saddlewood Drive, Lee’s Summit, MO 64081. There is
a box on the porch for pickup and you will be notified when your team is ready to be picked up.
Your GotSoccer account may also be used by some tournament directors if they are using GotSoccer
to run their tournament. It is also used to register for State Cup, President’s Cup and the Missouri
Olympic Development Program (ODP).
Detailed instructions for registering in GotSoccer are found below:
Complete the Official Gotsoccer Registration
To officially accept a position on your team, please register in Gotsoccer at the link below. An initial
non-refundable deposit is due at the time of registration. This registration is different from the
registration for tryouts. Go to the following link: 2016-17 Gotsoccer Registration
 If you are new and do not have a Gotsoccer then you will need to go to the New User section
and Create Account
 If you have a Gotsoccer account, then login under the Registered User section. If you do not
know your username and password, then select Player Password Lookup and enter your email
address. An email will be sent to you.
 Once you are logged in, enter or confirm the accuracy of your player’s personal information
including all parent information. This information is for club use only and will not be shared.
Select Continue.
 Enter your player’s emergency contact information and insurance information. This
information is for club use only and will not be shared. Select Continue.
 Select your payment plan for your registration and coaching fees. Your initial deposit will be
deducted from your account immediately. The remainder of your payment plan will start on
July 15 (or October 15 if High School Boys). A list of these KC Select fees can be found on the
tryout page at www.kcselect.org. Teams will continue to collect team administrative fees,
tournament fees (including coach travel expenses), practice fees and all winter fees as
determined by the individual team. Select Continue.
 Select Pay by Credit Card or Pay by E-Check
 Enter your Credit Card or E-Check information
 Review your information and the Registration Agreement. Electronically sign the agreement
and select the check box agreeing to the terms. Finish and Submit the Application.

 On the final page, you can print out the Registration Agreement for your records. You should
also go in to your Gotsoccer account and load a 1x1 head shot only picture of your player. This
will be used on their player card.
TEAM FINANCES
There are various fees due throughout the year for your team. Some of those fees will be collected
directly by KC Select while others will need to be collected by the team manager from each player on
your team. It is best to keep a spreadsheet or use software to track the fees coming in from each
player. You should provide players with a monthly or quarterly statement that shows
the payments that you received and what each player paid for such as tournaments,
practice rental, winter league, etc. You can contact the KC Select Registrar for a mock team
finance spreadsheet if you need one. Below is a list of fees that the team may have each year. Team
funds that are collected should not be used for your own personal financial obligations.

This year, KC Select will be collecting the annual registration fee, annual coaching fee and fall and
spring league fees for our teams through the Gotsoccer system. Players will sign up for a payment
plan when they do their online registration in June. All players, including High School Boys, will
register and pay an initial deposit to officially accept a spot on a team in June. This deposit will go
towards payment of the annual registration and coaching fees. See Appendix 2 for payment plan
options.
Fees Collected Directly by KC Select Soccer Club
Annual Registration Fee
The annual registration fee covers carding fees, coach carding fees, Registrar services, one league fee
per season (fall and/or spring) and other administrative costs of running the club.
Annual Coaching Fee
The annual coaching fee covers payments to coaches for training teams within KC Select. The current
rates for the 2016-17 soccer season is posted on the tryout page at www.kcselect.org. Teams
playing up in age will pay the fee for the age group that they are playing in and not their actual age
group. For example, U12 9v9 team playing up in U13 11v11 will pay the U13 11v11 rate.
Fall and Spring League Fees
The club will be paying one league fee per season for every team, except high school boys and girls
where one league fee will be collected and paid. Teams that would like to play in multiple leagues will
need to collect for payment of the second league that they register for on their own.

Fees Collected by Teams
Annual Team Fee
This fee is recommended for team supplies, an extra uniform for guest players, bank fees, etc. This is
an optional fee and can be set at whatever rate you feel is right for your team but normally runs from
$10-$25 per player. If you collect this fee and do not use it during the year, then it should be returned
to the player at the end of the year. This fee is charged to primary team players only.
Tournament Fees
Tournament fees are set by each individual tournament and should be paid directly to the
tournament. Tournament fees are divided out amongst all participating primary team players.
Normally, guest players are not charged for tournaments unless it has been worked out in advance
with the player. Club pass players are charged for tournaments if they participate.
Practice Field Rental Fee
KC Select currently provides discounted rates for field rental at Franke Fields and/or Raytown Soccer
Complex. Coaches will be notified on when to sign up for practice dates and times in the Fall and
Spring. The current cost for those fields are $40 per player for two nights a week and $20 per player
for one night per week. Teams can also choose to utilize an indoor facility or their own private soccer
fields if they so choose. They are responsible for working with the facility to agree on payment terms
for use of the facility and for paying their bill in a timely fashion.
SCHOLARSHIPS
KC Select does provide a limited amount of scholarships for players who have a financial hardship. If
a player approaches you regarding a scholarship, please provide them with the KC Select Scholarship
Application. An application can be requested from the KC Select Registrar at registrar@kcselect.org.
Depending on the situation and need expressed, KC Select may waive fees for the Annual
Registration, Annual Coaching Fees, League Fees and Uniforms. Tournament fees and tournament
travel fees are normally not provided as part of a scholarship plan without specific approval from the
KC Select Board.
UNIFORMS
Uniforms are required to be purchased every 2 years. We are on the second year of our two-year
uniform cycle. You will need to either assign numbers, if it is a new team, or find out the numbers
that are currently on your team from each player. Team managers in each age group should work
together to assign numbers especially if you are utilizing club pass players to round out your team’s
roster so that you do not have duplicates. Teams that play in Missouri State or President’s Cup should
take special care to not duplicate numbers on your team.
Uniforms will be available for sizing throughout tryouts. Players should be encouraged to
try on uniforms during this time and can fill out the Player Uniform Order Form that they can use to
complete their online purchase process in the MyUniform system. The MyUniform system will open
for purchases on Friday, June 10, 2016. Soccer Master provides an open order period in June. The
open order period provides the standard club pricing on all uniform items. Order placed during the
closed uniform order period will incur a 20% processing fee added to the cost of each item in your

order. Parents should order uniform items now during the open order period to avoid additional
charges. An open order period will be provided for high school boys in October/November so they
can place uniform orders at that time. There may also be an open order period in the spring but that
is to be determined.
Open order period ends Monday, June 27, 2016.
Team Managers are responsible for loading their team
roster in the MyUniform system. The system link is
http://myuniform.soccermaster.com/. The KC Select
Registrar will set up your team account in the
MyUniform system and you will receive an email for
you to set up your username and password for the
system. Once in the system, you should view the site
tutorial. The tutorial will show you how to enter your
team roster in to the system. When you enter your
team roster in to the system, the system will send out an email to each player. This email will instruct
the player on how to set up their username and password and give them access to order their uniform.
Team managers need to make sure that their parents are aware of the items that they need to
purchase either the basic kit or the travel kit. There are additional items available for purchase such
as practice jerseys (royal or silver - $10 each), full sized red backpacks ($50), navy sack packs ($20),
training jackets ($50), training pants ($40) and extra socks ($10).
Soccer Master will not accept returns for any merchandize once it has been personalized with a club
patch and/or player number. KC Select will accept returns but a 25% restocking fee will be applied to
each item returned and all items must be returned to the KC Select Registrar prior to receiving a
refund. Parents should take special care to order correct sizes when placing the order
for their player.
Uniform – Basic Kit includes:

Red primary jersey, white conflict jersey, navy short, white sock…… $125.00
Uniform – Travel Kit includes:

red primary jersey, white conflict jersey, navy short, white short, red sock, white sock,
long sleeve jersey ……$195.00
All players will need to purchase all pieces of the kit that the team has decided to purchase. The only
exception is for a team Goalie who can purchase a modified kit that includes the Goalie jersey ($25)
instead of the regular jerseys.
ALL TEAM UNIFORM ORDERS MUST BE IN THE MYUNIFORM SYSTEM BY NO
LATER THAN MONDAY, JUNE 27, 2016 OR YOU WILL NOT HAVE UNIFORMS IN
TIME FOR LEAGUE PLAY.

OUTDOOR LEAGUES
There are several different leagues where a team can play. You will need to check with your coach to
determine where you should register for league.
Heartland League
Heartland Soccer Association league is based out of Kansas. They primarily play games at Heritage
Soccer Park, Swope Soccer Village, Overland Park Soccer Complex and Legacy Park. Since KC Select
teams use Missouri Youth Soccer Association (MYSA) for rosters and membership player cards,
MYSA requires teams to obtain travel permits to play games outside of the state of Missouri.
Heartland league is considered an out of state league so you will have to request travel permits to play
in that league. Detailed instructions on how to obtain travel permits are provided later in
this document.

9v9 (U11-U12) $895.00
11v11 (U13-U19) $995.00
You can register for league at www.heartlandsoccer.net. Select the “Non-Member Team” option. You
will need to list any conflict dates that your team or coach will have with the schedule. The calendar
dates for the fall (spring) season are located on the league website. Please check with your coach for
conflicts with any of these dates. You will also need to list any tournament weekends as a conflict
date. Keep in mind Heartland will only let you list 3 conflict weekends for the fall (spring). You
should also select “Pay by Check” on your registration and forward your confirmation
email to the KC Select Registrar at registrar@kcselect.org. KC Select will pay for one fall
and one spring league registration per team. Once your registration is received, you will start getting
emails from Heartland.
HEARTLAND SOCCER DEADLINE FOR REGISTRATION IS NOON ON JULY 1, 2016. DO
NOT MISS THIS DEADLINE OR YOU WILL NOT BE ALLOWED TO PLAY.
The office for Heartland Soccer Association is located just west of 135th and Antioch in a strip center
just behind the Blue Moose restaurant. (9161 W. 135th St., Overland Park, KS 66213). Hours are
Monday –Friday from 10am-3pm.
Heartland will need a copy of your state roster before the season starts. This will be provided to
Heartland by the KC Select Registrar as a group for the entire club. The same roster will be provided
to your team. You must have a copy of this roster at every game. Player cards will need to match this
roster. Heartland will have team packets for you to pick up at their office that contain your league
game day roster cards along with coupons, flyers, etc. They will send all managers a note about when
and where to pick these up. You will have to have a game day roster card for every game. You should
have them filled out ahead of time for your game. The referee will want the game day roster card, any
guest player forms and the team’s player cards at the beginning of each game. You can give them to
your coach before the game and remember to get them at the end of the game. You should familiarize
yourself with the Heartland league rules. These can be found on the Heartland website along with all
the information listed above.

KC Metro League
KC Metro League operates differently in that you register for league through the organization that you
want to play your home games. Fees vary from organization to organization so you will need to check
their website for their fees. For example, to play games at Raytown Sports Complex then you would
register and pay fees through Raytown Soccer Club for that location or if you want to play at the
Western Missouri Soccer League (WMSL) then you would register through WMSL. A list of league
members who accept registrations for league can be found on the KC Metro League website at
www.kcmetroleague.com.
KC Metro league has similar rules on submitting conflicts for games on their registration, having
rosters, game cards and player cards at all games. For a complete list of rules, please familiarize
yourself with their league rules as well.
Missouri Cup Qualifying League (MQL)
New this year is the Missouri Cup Qualifying League. This league is being provided for teams that
plan to play in Missouri State Cup. In the past, Heartland Soccer Association league has been a MYSA
sanctioned league for Missouri State Cup teams but this is no longer the case. Teams must play
against other Missouri State Cup eligible teams in a MYSA sanctioned league to qualify for Missouri
State Cup. This league will offer that opportunity. You should check with your coach to see if your
team will need to register for this league. This league should be used in addition to another league
since it only provides 4 games per season. This league is a self-scheduling league similar to Midwest
Regional League. The league fee is not covered by KC Select and must be paid for by your team in
similar fashion as a tournament. For details on the MQL visit the following link:

Midwest Regional League (MRL)
Midwest Regional League is a regional league that is conducted for high level teams in the Midwest
region. Some KC Select Academy teams may play in this league when they are eligible. Check with
your coach to see if your team will be participating in this league. Information on MRL can be found
at www.midwestregionalleague.com. The KC Select Registrar can also answer questions regarding
this league.
INDOOR LEAGUE
There are several options for indoor play. Please discuss with your coach where they would prefer to
play. Most indoor facilities have a website where you can find registration information and a couple
of those locations are listed below. This information is normally released by the first of October.
Most facilities have a Winter I and Winter II session for indoor soccer. Games usually start the first
weekend in November and run thru the first of January. There will be a second session that usually
runs from mid-January thru March.
You will register and pay the facility directly for your team.

TOURNAMENTS
Normally most coaches will have an idea regarding what tournaments they would like your team to
play in. You will need to get this information as soon as possible by working with your coach. If you
know ahead of time, you can use these dates as conflict dates when you register for league play. If you
do not know the dates then you need to find out as soon as possible so you can plan for them. You will
have to find out when they are, how much the fee is, and get it sent in before the deadline.
You can Google most tournaments to find fees and registration deadlines. Fees for the tournament
should be divided out by the number of players. We encourage our KC Select teams to play in our
club sponsored tournaments. Specific dates and registration information can be found at
www.kcselect.org under Programs/Tournaments. Some additional websites for local tournaments are
www.challengersports.com; www.heartlandsoccer.net; www.gsisports.net. Local clubs also have
tournaments that you can participate in for various division levels.
You also need to check with every tournament to see what forms they require at check in for your
team to play. You do not want to find out you do not have something the day of check in. Most
tournaments want a copy of your state roster, medical waivers and player cards. For teams carded
through MYSA, you will need to provide travel permits to tournaments outside of Missouri (see travel
permit section). Some tournaments may also need a copy of birth certificates for their tournaments.
You should keep a file folder with all of this information in it and keep it secure.
For out of town tournaments, you will need to work out compensation for coach travel
with your coach. All KC Select coaches should be compensated for travel. Coach travel
can be done in different ways. One way is to charge each player a per player fee that will be paid to
your coach often $20-30 per player. The coach is then responsible for paying for all of their travel
expenses (i.e. gas, room, food) out of that amount. Another way is for the team to reserve the coach’s
room and pay for it and then provide the coach with an amount for food and gas. The total of all those
expenses are then spread across the players on the team. You can also ask your coach to cover their
own travel expenses, submit receipts to you for actual expenses and reimburse them for the amount.
GUEST PLAYER/PLAYER LOAN
Teams can utilize guest players for some leagues and most tournaments. Guest players are players
who are assisting your team or are taking the place of a player on your team for a game or event.
Guest players require specific paperwork. If you are utilizing (borrowing) a guest player from another
team then you will need to go to the following website www.thetournamentcenter.com and fill out a
guest player form. Once you have completed your request, you should print the form and have your
coach sign the form. You will need to get the player card and a copy of the medical waiver from the
other team to use during the league or tournament games. If you are loaning a player to another
team, then you are responsible for getting the player card to them. If you are loaning a player, then
you should also provide a copy of their medical waiver and birth certificate if requested. Instructions
for creating an account and requesting a guest player form are below.

Go to www.thetournamentcenter.com and click on the Register option in the Site Login box.
Select Register under the Coach or Team Administrator section.
Enter your team and personal information as requested.

 Once you have a team account, click on the Guest Player/Player Loan option at the top of the
page.
 Enter your username and password.
 Select “Start a New Guest Player Form”
 In the Borrowing Team/Contact Information box, select “Yes” in the State Policy Box after
reviewing the state policy on guest players.
 Enter your team state as Missouri
 Enter your personal information
 In the Borrowing Team Information box, enter your team’s information. Your team id number
can be found on your roster or on your player cards.
 In the Event Information box, enter information about the tournament or league. Information
on your tournament or league can normally be found on their website. Review the definitions
for the event type to select the correct event you are attending.
 In the Guest Player List box, select “Add Guest Player” and enter the state the guest player is
from, the player’s id number which can be found on the player card, the player’s team name,
the player’s name and the loaning coach’s email.
 Select “Save”.
 Read the Electronic Signature agreement and sign the form with your personal information.
 Select “Submit” and your form will appear.
 Print your form and have your coach sign it.
Forms for league games are normally turned in at the start of the game to the referees. Forms for
tournaments are submitted at tournament check in as directed by the tournament.
NOTIFICATION TO TRAVEL/TRAVEL PERMITS
If your team is carding through Missouri Youth Soccer Association (MYSA) and you will be playing in
a league or tournament outside the state of Missouri, then you are required to get travel permits to
gain permission to “travel” outside of the state. This is MYSA’s approval that they will provide
insurance for your team outside of the state. Travel permits should be done for Heartland League
after you apply for league and done for tournaments after you have been accepted to a tournament.
The cost for a travel permit is $15 each and no refunds will be given should you request
them early and then not get in to a tournament. If you delay and wait until 48 hours prior to
your event, then you will be charged a $50 fee by MYSA.
Travel permits can be requested at www.thetournamentcenter.com. You must log in to your team
account for the system (see how to create an account in the Guest Player/Player Loan section above).

Create a team account if you do not already have one.
Select “Notification to Travel/Travel Permits” at the top of the page.
Log in to your team account by entering your username and password.
Select “Start New Travel Notification Request”
Read the directions from MYSA at the top of the page.
Select the type of event that you want to attend.
Select “Yes” after reviewing the state association’s travel policy
In the Team Contact box, enter all of your personal information
In the Team Information box, enter the information for your team. Your team id can be found
on your roster or player cards.
In the Event Box, enter you event information. Most information for tournaments and leagues
can be found on their website. Event dates should cover the entire period that you will be
traveling. Example: Heartland League should be 8/6/2016-10/23/2016.

Enter your credit card information and your card will be charged $15.

Once your Travel Permit loads, print a copy for your event. You can also save it to a file on
your computer as well.
Forms for league games should be submitted to the Heartland League office by the deadline that they
request. Forms for tournaments are submitted at tournament check in as directed by the tournament.
MANAGER MONTHLY DUTIES
During the month of June, you will be the busiest. You will have to collect everyone’s paperwork,
monitor your Gotsoccer team account to make sure player have signed up, take photos (if parents do
not upload them to Gotsoccer), monitor uniform purchases, get signed up for league, prepare for
tournaments, etc.
Staying Organized - Buy a binder or folder of some kind to keep all of your team paperwork. You
want to have the hard copy available should you need it. Make sure you make about 3 copies of your
state roster to have on hand should you need one.
Collecting Fees – There are various ways to collect fees. Cash, check or credit card can be accepted.
When accepting checks, you should document your check numbers and the date of payment. When
accepting cash, it is helpful to provide a receipt to parents or have them provide it in an envelope that
they confirm with you the amount provided to you. For credit cards, there are some commonly used
websites that teams can use such as paypal.com (send as friends and family for free), venmo.com and
teamsnap.com. To pay by debit card or a bank account for a low transactional fee ($0.95) you can use
www.popmoney.com to pay directly to a team.
Uniforms – Purchase an extra club uniform kit for your team for guest players or new additions. It
can be the very basic kit but at least you will have a uniform available immediately if you are adding a
player to your team or you are utilizing a guest player.

SPIRITWEAR
There will be a link to our spirit wear on the home page of MyUniform. Soccer Master will open up
order periods at various times throughout the year. Notification that the order period is open as well
as a link to the order website will be sent to team managers for distribution to their teams. A
deadline will be provided for the end of the order period and orders will be processed by Soccer
Master and delivered directly to individuals after the order period ends.
