These are the most common blemishes affecting the skin of adolescents.
Blemishes, especially showing on the face, can cause a situation of
psychological distress, which can often lead to very negative habits, such
as the use of detergents having a strong delipidising power or, even worse,
trying to eliminate spots by squeezing them out. Most severe acne forms
can lead to difficulties in relating with other people.
Impure,
acne-prone skin
Nearly all skin is covered with hair, showing more in some areas, less
or basically invisible to the eye in others. Every hair, whatever the
size and visibility, emerges from a microscopic cavity - the follicle,
which is also where sebum is discharged and helps to keep hair
soft and protect skin. Sometimes, sebum is produced in excess and
accumulates inside the follicle. Blending with the dead cells on the
skin surface, sebum can lead to the formation of a “plug” and take
a brownish-blackish appearance in the outer section of the follicle
and yellowish on the inside - this is a typical blackhead. If squeezed,
a small cylinder of solid, yellowish material will come out. If sebum
accumulates in significant amounts, it puts pressure on and irritates
follicle walls, which get inflammed and infected, swelling up and
taking the appearance of a pimple.
Skin imperfections
and adolescence
During adolescence, the amount of circulating hormones in the body increases, sebaceous
glands grow in size and, consequently, their activity becomes stronger. A greater amount
of sebum is produced, which tends to build up, forming a sort of “plug” at the outlet
of the gland. Oxidised by air, the plug rapidly turns brownish and forms the comedo,
commonly known as a “blackhead”. In the follicle “clogged” by the blackhead, a number of
bacteria species normally found on skin finds favourable conditions to proliferate and to
create conditions for infection and inflammation.
Pimples and blackheads more likely occur on the forehead or the neck, areas that, being
partly covered by hair, can create poorly aired micro-environments.
Other factors leading to or worsening oily, acne-prone skin can be:
• a number of medications, like cortisone for instance, can cause or aggravate acne. Such
drugs can influence hormone secretion and thus make sebaceous glands more active;
• fatty and occlusive substances that are deposited on skin and can promote the onset of
acne, since they clog follicles from the outside, hindering the normal leakage of sebum;
• stress, which may facilitate the onset of acne. It is no coincidence that pimples come
out, for example, in the days before a school test, prior to menstruation, and anyway in
any situation where one feels to be under stress.

How much
can a diet
rich in fats
affect skin?
No food is responsible for acne, not even
chocolate and cold cuts are able to trigger it.
They may rather exacerbate pre-existing acne
lesions, by inflaming them.
Resorting to correct eating habits is however
always advisable.
Imperfections
and tendency to acne:
is it the same for
everyone?
No, acne is not the same in all adolescents. Clinically, there are different
forms on the basis of lesion morphology (type) and severity of the
condition. Around the age of puberty (age of development), mild
forms of acne, especially the “comedonal” form, where blackheads
and whiteheads show more, and the papular form are more frequent.
After puberty especially, though it can occur during puberty as well,
acne shows in more severe forms, with inflammatory lesions being of a
pustular or cystic type.
An individual can be affected by just one form of acne or show a
number of clinical forms, one following the other.
Prevention and
treatment
Usually the first instinct with a pimple or a blackhead is to squeeze it.
Nothing could be more wrong. Squeezing it will transfer the infection to other follicles
and form new pimples. In addition, though hands may be washed and clean, they can still
hold bacteria that may penetrate the lesion, thus delaying lesion healing. Finally, you can
damage the skin of the face, which could lead to scarring.
The following are some practical tips to avoid the aforesaid problems:
• good personal hygiene is key: a good cleanser used twice a day, morning and evening,
ensures effective cleansing. If you use soap or other cleansers, it is better if they are not
too aggressive to not overstimulate sebaceous secretion (rebound effect);
• keep skin areas most likely to be affected by acne exposed to the air. Preferably keep
hair short or tied, so they do not cover the face. Better avoid hats or other similar
clothing accessories that won’t let skin “breathe” and can actually carry bacteria, being
accessories which are not often washed;
• acne-affected skin can benefit from sunlight. Acne often worsens in winter and improves
in summer. However, always wear appropriate sun protection.
