Item 6. Limits of Insurance
Aggregate Limit of Insurance:$2,000 per Named Insured or two
(2) occurrences within a 12 month
period, whichever comes first.
Occurrence Limit of Insurance:
$1,000 per Occurrence for each Named Insured

The deductible will be the amount corresponding to the noncontract, non-subsidized retail price of the Named Insured’s
wireless device when initially purchased.

• Claims may be fulfilled with new or reconditioned equipment. Reconditioned equipment may have
minor cosmetic flaws. Replacement of ‘accessories’ may be with generic or non-original manufacturer
accessories. If the exact model is not available, the replacement will be of like kind and quality.
• You are not required to purchase insurance to activate wireless service. This coverage may provide a
duplicate of other sources of coverage. You may cancel at any time.
• The Program is a replacement service provided to subscribers of Appalachian Wireless. This coverage
is being provided by the New Hampshire Insurance Company, through eSecuritel Agency, LLC and is
administered by eSecuritel Holdings, LLC.

How do I enroll?
You can enroll in the program at anytime. A fee of either
$4.99, $5.99, or $9.99 based on the make and model of
the insured handset, will be added to your monthly bill.
When am I covered?
To be eligible for coverage immediately, purchase the
Program at the same time you purchase your new
handset from Appalachian Wireless (Point of Sale). If you
select coverage after the Point of Sale, no claims may be
made under the Program for thirty (30) days following
purchase of coverage. Please note, your phone must be
operational at the time of Program activation in order
to enroll.

What is the deductible?
Your deductible will be based on the make and model
of the handset for which you make a claim, ranging from
$25 to $200. Please consult the Deductible Schedule by
visiting appalachian.esecuritel.com for handset model
specific information. The deductible must be paid
before you receive your replacement phone.
Call eSecuritel
In the case of accidental damage, loss, theft or
malfunction (after the manufacturer’s warranty expires),
call eSecuritel’s Customer Care Center at (866) 249-3186.
Claims must be reported within 60 days of the incident
and proof of loss and/or ownership must be provided, if
requested. Only the account holder may file the claim.
Please have the following information ready:
• Your wireless phone number
• The account holder’s billing address
• Wireless phone manufacturer/model and unique serial number
also called an ESN or IMEI
• Date, time, location and detailed description of the
incident
• Police report number (if theft occurred)
Step 3
Pay Deductible & Receive Replacement
If your claim is approved, you will be directed to a
participating Appalachian Wireless location to pick up
your replacement handset within 30 days. A deductible
will be collected based on the make and model of
the handset, ranging from $25 to $200 (please visit
appalachian.esecuritel.com for handset model specific
information). Claims may be fulfilled with new or
reconditioned equipment. Reconditioned equipment
may have minor cosmetic flaws. Replacement of
‘accessories’ may be with generic or non-original
manufacturer accessories. If the exact model is not
available, the replacement will be of like kind and
quality. Please keep the receipt, original box, manual and
all packaging materials for warranty purposes.

The program covers the handset (device, standard
battery & wall charger) you purchased from
Appalachian Wireless from accidental damage, loss,
theft and malfunction (after the manufacturer’s
warranty expires). Normal wear and tear and cosmetic
damage to equipment is not covered.
Call Appalachian Wireless Customer Care
If your handset was lost or stolen, call Appalachian
Wireless Customer Care at 800-438-2355 to suspend your
service and protect yourself against unauthorized use
of your account. If your handset was stolen, file a police
report where the theft occurred. This report number may
be required to process your claim.
$2,000 per Named Insured or a maximum of two (2)
equivalent replacements (whichever comes first) will be
permitted in each consecutive twelve (12) month period
beginning with the date of the first loss.
