These facilities lack UNOCHA (ERF), Government of appropriate medical equipment, trained staff, Gilgit-Baltistan, Dept. of Health (G.B) modern data management systems. They were in dire need of extensive renovations keeping in Staff: context the weather, other regional requirements 190 National & International and international universal accessibility protocols (access to disabled and vulnerable population). The facilities were predominantly substandard and it was clear that a diverse and multifaceted approach to revamp these was the need of the hour. “ Did you know? In 36 Schools that Shifa Foundation visited, “ 122 students found to be suffering from eye disorders were completely unaware of their condition. Shifa Foundation responded by renovating, upgrading and developing six health facilities through installation of water heaters, geysers, construction of pathways, provision of equipment for wards, laboratory, wheel chairs and stretchers as well as training of local healthcare providers. Shifa Foundation also upgraded water and sanitation facilities, performed eye screening through mobile clinics thus identifying children who needed spectacles and provided them with these. Humanitarian Assistance Program (HAP) 09 Gilgit Baltistan Strengthening of Baltistan Health System and Renovation & Equipping Six Existing Government Health Facilities 10 SHIFA FOUNDATION making an effort Gilgit-Baltistan is one of the remotest regions in Pakistan. The area and its conditions are harsh and healthcare facilities are UNOCHA, WHO, scarce if any. Specialized medical facilities on the other hand are Department of Health almost completely non-existent. Any patient requiring such facility has no choice but to travel to Islamabad, which in of itself Staff: is a challenge partly due to the fact that it's a 20-30 hour drive, 120 National but mostly due to uncertainty that travel will be at all possible. & International For a good part of the year, the whole region is cut-off due to landslides blocking the roads that lead out of the region. Air travel, while inaccessible to the local poor population, also holds the same uncertainty factor due to weather conditions. Partners: To combat the fostering healthcare issues especially those requiring specialized tertiary level services, Shifa Foundation devised an elaborate and innovative strategy using state of the art high-tech equipment. Through the “telemedicine” project, Shifa Foundation catered to the needs of thousands of patients, providing them with readily available healthcare services while minimizing any hindrances. It is no coincidence that this project earned the title of “the most innovative project” during UNOCHA's internal global workshop. The project was nominated out 1,346 pooled fund projects around the world. Apart from this, Shifa Foundation renovated and re-equipped 3 health facilities and trained healthcare providers. Base stations were established at DHQ Khaplu and Civil Hospital Astore. Mobile health teams were sent to the community from these base stations for screening, health education and health promotion sessions to highlight issues of public health importance. In addition, Shifa Foundation has also improved maternal, neonatal and child health services by improving the system of availability of lady health workers (LHVs) and ensuring safe deliveries. Furthermore, information education & communication (IEC) materials, village health committees and public health awareness methodologies were developed in order to educate the local population on various aspects of health. “ Did you know? Since Gilgit-Baltistan is sealed off from the rest of the world during winters, Telemedicine is the most consistently available health service to the people. “ Shifa Foundation also established the DEWS system in collaboration with WHO in order to rapidly detect and control outbreaks through adequate preparedness and focused utilization of technical staff and rapid response. The project catered to the needs of more than 200,000 individuals and almost 1100 patients were provided specialized healthcare facilities through telemedicine. Humanitarian Assistance Program (HAP) 11 Gilgit Baltistan Provision of Integrated Primary Healthcare, Maternal and Child Health & Telemedicine in Astore and Ghanche “There is a growing need for telemedicine in Pakistan in regions where there are problems in accessibility for doctors, this technology plays a bridging role and can allow access to quality health care to even the most remote and marginaized population” - Dr. Zeeshan Bin Ishtiaque, Executive Director, Shif Foundation. 12 SHIFA FOUNDATION making an effort TELEMEDICINE: AN AWARD WINNING INNOVATIVE APPROACH TO HEALTHCARE Telemedicine is an IT based approach to providing healthcare from a distance. It combines technologically advanced tools in health and IT services to materialize the concept. The basic concept is that in remote areas where access is difcult for specialized medical professionals, telemedicine units are mobilized. These units enable consultants of different specialty to reach out to needy and deserving population in far flung areas of Pakistan. Shifa Foundation Telemedicine project was recognised by United Nations as “the most innovative project” The telemedicine unit is an online computer system with diagnostic equipment ranging from stethoscope; eye, ear, throat and skin cameras to ultrasound equipment etc. It connects the two ends of the telemedicine system i.e. patient's end and the doctor's end. Only a trained technician is required in the eld to use this equipment to provide real time images to a consultant in another facility. out of There is a signicant lack of health care facilities in the Gilgit-Baltistan region. The area is remote and access to it is limited especially during the winter season. Shifa Foundation saw this as a Patient’s End 1346 projects globally perfect opportunity to introduce a new idea of providing health care in the form of its telemedicine project. Due to recent advances in the telecom sector, wireless internet facility is now available in most of the country including Gilgit-Baltistan. The people of Gilgit-Baltistan welcomed this idea and greatly appreciated the facility that provided this service. With great coordination from the GilgitBaltistan government, Shifa Foundation was able to deliver the project's objectives while simultaneously assessing the prospects of expansion of this facility to a much wider audience in Gilgit-Baltistan and Sindh. Humanitarian Assistance Program (HAP) 13 14 SHIFA FOUNDATION making an effort Shifa Foundation had been working in the region of Gilgit-Baltistan. It had been gathering and assessing data about the prevalence of various diseases in this region. It assessed the data gathered from the health facilities in Baltistan and identied the high incidence of diseases such as cardiovascular diseases, stroke, chronic respiratory diseases, diabetes etc. Experts at Shifa Foundation suspected contamination of water used by people in this region. Water that was being used in this region for drinking purposes was then tested. It was found that in many villages it was contaminated with high concentration of metals especially Arsenic. Arsenic and many of its compounds are potent poisons. They occur naturally in mineral rich bodies Partners: of water and are known to contaminate these UNOCHA, WHO sources to the point of being a health hazard. Department of Health Symptoms of arsenic poisoning begin with headaches, confusion, severe diarrhea, and Staff: d r o w s i n e s s . A s t h e p o i s o n i n g d e v e l o p s , 120 National & International convulsions and changes in ngernail pigmentation called leukonychia may occur. When the poisoning becomes acute, symptoms may include diarrhea, vomiting, blood in the urine, cramping muscles, hair loss, stomach pain, and more convulsions. Almost all organs of the body are affected by arsenic poisoning. Arsenic poisoning at its worst can eventually lead to coma and death. Arsenic is related to heart disease (hypertension related cardiovascular diseases), cancer, stroke (cerebrovascular diseases), chronic lower respiratory diseases and diabetes. Shifa Foundation identied the village of Katpana as worst hit in this regards. The population displayed signs of severe arsenic poisoning with reported cases of death occurring regularly due to the contamination. Taking into account the serious health hazards in the village, including increase in mortality, Shifa Foundation, with help from WHO, established a clean drinking water supply scheme in this region. Apart from a clean water bore, a total of 21 subtanks and one main water reservoir were constructed, along with required plumbing, pipelining and primary and alternate water pumps. The results are extremely positive, with reports of signicant decline in arsenic related health issues and mortality in this area. “ Did you know? Arsenic, also known as the “Perfect Poison”, is attributed to the deaths of prominent historical figures such as King George III of “ England, Cleopatra and Napoleon Bonaparte? Humanitarian Assistance Program (HAP) 15 Gilgit Baltistan Provision of Clean Drinking Water in Katpana 16 SHIFA FOUNDATION making an effort Being a seasonal disease, diarrhoea / cholera during its peak, claim thousands of lives each year, majority of those children. During this time, the available facilities at government health centres are unable to cope with the influx of critical patients in the endemic prone areas. Partners: WHO, USAID, Department of Health Shifa International Hospitals Ltd. Staff: 115 National & International To lend a hand towards the unied efforts of major stakeholders in endemic prone areas, Shifa Foundation established its specialized Diarrhoea Treatment Centres(DTC) within the existing government healthcare facilities. These served as centres for identication and treatment of diarrhoea in Rajanpur, Alipur (Muzaffargarh), Skardu and Ghanche. Apart from providing direct treatment to the patients, this project aimed to educate the patients and their families on disease is the preventive measures to be taken at the household and community level. “ Did you know? Diarrhoeal second leading cause of death in children “ under five years old During the project tenure, more than 25,000 patients were treated in these centres and health education messages were given to more than 200,000 people. Humanitarian Assistance Program (HAP) 17 Punjab & Gilgit Baltistan Flood Relief Activities and response to Acute Watery Diarrhea in Rajanpur, Alipur (Muzaffargarh), Dambudas and Ghanche 18 SHIFA FOUNDATION making an effort The series of floods during the 2010 to 2012 period, brought unprecedented suffering and devastation to the country and exposed Sindh to one of the most ill-recognized but nevertheless deadly and morbid killer; malnutrition. In a country that is already on the top among the list of countries suffering from malnutrition, province of Sindh takes the title of the worst hit region. Recognizing the imminent threat to this region, Shifa Foundation mobilized its resources to do its part to combat malnutrition in Umerkot. Partners: UNICEF, WFP, Department of Health Government of Sindh through Nutrition Cell Sindh Staff: 300 National & International This intervention focused on alleviating malnutrition through a community based model. In this, Shifa Foundation is involved in developing the communities' capacity of understanding and combatting malnutrition, and at the same time providing direct therapeutic treatment to those in need. In this way, Shifa Foundation aims to address both the current crises as well as provide a sustainable solution to the future generations. This project is still in progress in Umerkot, where Shifa Foundation is providing these CMAM services in 24 union councils out of 27. Currently, more than 1100 villages have been reached where more than 54,000 children and 36,000 pregnant and lactating women have been screened for Malnutrition is the underlying malnutrition. Among them, around 15,000 cause of more than children and 8500 PLWs have been enrolled in ongoing home based treatment program. Those enrolled have been showing signs of steady improvement and are well on their way to recovery. “ “ Did you know? 50% of child deaths! Humanitarian Assistance Program (HAP) 19 Sindh Community based Management of Acute Malnutrition (CMAM) and Infant and Young Child Feeding Program (IYCF) in Umerkot 20 SHIFA FOUNDATION making an effort Sindh Community based Management of Acute Malnutrition (CMAM) and Infant and Young Child Feeding Program (IYCF) in Shaheed Benazirabad UNICEF, WFP, Department of Health Government of Sindh through Nutrition Cell Sindh Staff: 260 National & International Malnutrition is considered to be the underlying cause of more than 50% death in children and pregnant and lactation women (PLWs) in Pakistan. But even though the dangers of malnutrition have been identied and documented, the conditions mainly remain unaddressed especially in the rural context. The province of Sindh being the worst affected because of its historic and continuing division between classes of few extremely rich in comparison to the masses of very poor has been classied by WHO of having alarming nutritional indicators normally seen in the state of emergencies. This what attracted SF to divert its resources and focus on this health issue in 2010-2011 and it set off on analyzing the local and international assessment to identify the areas needing and lacking the most. “ Partners: Many locations were identied but it was decided that SF would initialize its CMAM services in Shaheed Benazirabad since it was not catered too Nutrition indicators of Sindh are by any other organization because of various concerns, security being the top most. However SF immediately went into dialogue with the local stakeholders and began community-Based Management of Acute Malnutrition (CMAM) intervention to combat this highly distressing state of malnutrition. It mainly targeted 23 union councils in two talukas i.e. Nawabshah and Daur. “ Did you know? worse than Sub Saharan Africa After a year-long intervention, Shifa Foundation screened more than 81,000 children and 51,000 pregnant and lactating women for acute malnutrition. It identied and enrolled more than 15,000 children and 6000 pregnant and lactating women in the treatment regimen. The recovery ratio among the enrolled was immensely positive. Apart from these, around 119,000 mothers and caregivers were imparted health and hygiene education. Shifa Foundation also played an instrumental role in polio eradication initiative by the government of Pakistan by identifying and referring children who missed the campaign and generating awareness on the matter. Humanitarian Assistance Program (HAP) 21 22 SHIFA FOUNDATION making an effort Sindh Provision of Emergency Health Interventions, Integrated Primary Healthcare and Nutrition in District Mirpurkhas Partners WHO, UNOCHA Department of Health Staff: Mirpurkhas was one of the worst affected districts in the series of floods that struck Sindh in the 2010 to 2012 period. After the catastrophes, most of the health facilities became inundated and access to primary healthcare for the already distressed population of Mirpurkhas became impossible. 75 National & International Realizing the need, Shifa Foundation, through a mobilebased health unit setup, initiated the provision of primary healthcare services including but not limited to: • Provision of integrated primary healthcare and nutrition services with focus on pregnant and lactating women and children under 5, • Taking emergency preventive health measures to reduce risks of epidemic outbreaks, • Antenatal and postnatal consultations, • Family planning consultations, • General OPD services During the project tenure, more than 11,000 patients received direct treatment while around 13,000 patients beneted indirectly through distribution of hygiene kits, referrals and counselling. “ Did you know? From 1947 to 2013, the average life expectancy in Pakistan has increased from approximately 33 years to 64 years Humanitarian Assistance Program (HAP) “ • Conducting deliveries, 23 24 SHIFA FOUNDATION making an effort Sindh Establishing Acute Respiratory Infections (ARI) Centres in District Badin, Tando Allah Yar and Matiari Partners: WHO, Department of Health Staff: 80 National & International In the aftermath of the catastrophic floods, the already distressed population of Sindh was about to face another peril. The shelter-less population was at a high risk of being exposed to acute respiratory infections (ARI). Children under the age of 5 were the most vulnerable. Statistics showed that the major direct cause of mortality among this age group was pneumonia and other respiratory infections. Shifa Foundation began mapping the high risk regions and identied the districts of Badin, Tando Allah Yar and Matiari in dire need of support and interventions. Shifa Foundation established specialized ARI centres in the District Headquarter Hospitals of the identied districts. The units aimed to provide both inpatient and outpatient services as required. Shifa Foundation ensured that the necessary human resources were available for treating patients in these ARI centres. more Trainings and on job coaching was provided to the local medical staff to people, manage patients in case of emergencies annually than and to make sure they have the knowledge and practical skills to effectively manage and patients suffering from ARI. Medicines and supplies necessary for the treatment of ARI patients were provided through close collaboration with WHO. During the project tenure, more than 5000 patients were treated for acute respiratory infections. “ Did you know? Pneumonia kills especially children, AIDS, malaria tuberculosis combined “ Furthermore, Shifa Foundation maintained and strengthened the emergency disease surveillance and the disease early warning system (DEWS) channel and implemented other outbreak control strategies at the community level. Humanitarian Assistance Program (HAP) 25 26 SHIFA FOUNDATION making an effort The government of Pakistan and other national and Partners: international stakeholders have been striving to reach the UNFPA, WHO millennium development goals (MDGs) set by WHO. In order to complement their efforts, Shifa Foundation sought to Department of Health establish a accessible facility for provision of emergency obstetrics and newborn care (EmONC). Shifa Foundation Staff: started providing its services in two districts of Sindh i.e. 98 National Mirpurkhas and Umerkot. It also mapped the health facilities & International that were partially functional and re-established them by providing them with adequate staff, medications and other supplies. In areas where all available health facilities were either damaged or otherwise not functional, Shifa Foundation instead opted to provide these services through mobile health units. It was also felt that these areas represented another social dilemma tied with the objectives of this intervention, i.e. the prevalence of gender based violence among the population. These cases remained highly unreported due to the social taboos attached to the issue, which is why, Shifa Foundation sought to create a safe environment for women and establish a safe pathway for them to report such cases. More than 42,000 individuals were provided the following services through this project. • Basic and Comprehensive EmONC services • General outpatient services • Distribution of reproductive health and hygiene kits • Health education • Establishment of referral linkages • Distribution of medications “ Did you know? Experts agree that every second woman will face some form of “ domestic violence in her lifetime Humanitarian Assistance Program (HAP) 27 Sindh Provision of Emergency Obstetrics and Newborn Care (EmONC) & Gender Based Violence (GBV) services in Mirpurkhas & Umerkot 28 SHIFA FOUNDATION making an effort After conclusion of Shifa Foundation's activities for acute respiratory infections in Tando Allah Yar, WHO, DHO, Health Cluster a shift in disease trend was observed in the region. Established disease observatory units Staff: such as DEWS (disease early warning system) 40 National & International reports began claiming multiple outbreaks of diarrhea and cholera. Since Shifa Foundation was already present and active in the region, it immediately took notice of the alarming situation and similar to its previous interventions in Punjab and Gilgit-Baltistan, devised strategies to combat the outbreaks. For this purpose it took support from WHO and district health ofce. Partners: “ Did you know? Globally, 1.7 billion cases of diarrhoea are reported each year, and if untreated Shifa Foundation established its specialized Diarrhea Treatment Centres in the District Headquarter Hospital (DHQH) in Tando Allah Yar and immediately began isolating and managing patients of acute diarrhea. Furthermore, it imparted health and hygiene education. “ Community based management methodologies were also implemented to combat the problem can potentially lead to through preventive measures. The project saw success by limiting the outbreak and through collaborative efforts of all stakeholders, no major outbreak occurred in this region. More than 1800 patients received direct treatment during the project tenure. death Humanitarian Assistance Program (HAP) 29 Sindh Establishment of Diarrhea Treatment Center in Tando Allah Yar 30 SHIFA FOUNDATION making an effort Sindh Strengthening the MNCH Services at 7 Govt. Health Facilities by introducing sustainable 24/7 CEmONC & BEmONC Services in District Sanghar Partners: UNFPA, Department of Health Staff: 60 National & International Sanghar is one of the most remote districts in Sindh, where obstetrics, gynecology and newborn health services are practically non-existent. Apart from most of the health facilities in the region being run down, some have not been operative for the last 15-20 years and the local beneciaries have to rely on expensive private run health facilities. The area has harsh terrain with scattered population and has grave security concerns that need to be tackled before any health interventions could be executed. Shifa Foundation, in an effort to establish a model district in terms of mother and child health opted to intervene in the district of Sanghar. It provided 24/7 basic and comprehensive emergency obstetrics and neonatal and child healthcare services as per the international protocols. Despite the hardships that were faced, the intervention led forward by Shifa Foundation was a success, reaching out to more than 45,000 patients. Shifa Foundation accomplished its targets specically: • Basic and comprehensive emergency obstetrics, neonatal and child healthcare services were established in 7 Government health facilities. • Through these sites, antenatal, postnatal and family planning consultations were provided and deliveries were performed by trained birth attendant. • In facilities that provided comprehensive EmONC services, skilled gynecologists were appointed to provide emergency and optional c-section and other advanced Obs/ Gyne services. • Hygiene kits, newborn kits and safe delivery kits were distributed • General out patient services were provided “ Did you know? In one of the Tehsil level hospital in Sanghar, not a single C-Section had been conducted in its history before Shifa Foundation's intervention 31 “ Humanitarian Assistance Program (HAP) 32 SHIFA FOUNDATION making an effort Due to unrest in the region, Malakand Division was facing a Partners: complex emergency state. Further aggravating the situation DHO (KPK), DHO (FATA) were the series of floods in 2010 and 2011. Due to this, people were displaced either to camp villages or with their FDMA, PDMA friend and families in the form of host communities. This movement put additional burden on the host communities Staff: and little work was done by humanitarian actors to cater to 35 National & International their needs. Owing to the harsh situation, the health indicators of the region were rapidly declining. At this point, Shifa Foundation intervened by provision of health services through a series of mobile health clinic. The main objective of this activity was to provide primary and maternal healthcare. Shifa Foundation also aimed towards providing health education to the uneducated people in this region As of and to create mass awareness of key health issues. The project aims to assess the alignment of community health needs and the health services that are available in the district. It documents any gaps identied for immediate intervention or future consideration depending on the severity. It also analyses the various trends observed in this regard. The project was based on coordination with government health facilities and on acquiring data from the District Health Information System(DHIS) and relating it to direct data gathered from the community (based on an exhaustive survey using statistically valid methodologies). Prevention of Rabies after Dog-bite Exposure (PRADE) Recently launched by Shifa Foundation, PRADE aims to educate general public and healthcare professionals on rabies and its prevention. It aims to help the victims of dog-bite with immunization when required. The program also aims to create a local registry of dog-bite cases. “ “ Rabies is a neglected disease in Pakistan, although the incidence of dog bites in the country is very high. In 2010, more than 97,000 recorded cases of dog bites were reported by basic health units alone. Research by WHO has revealed that most of the population are either unaware of the risk of rabies when bitten by rabid dogs, or do not seek the right treatment for its prevention. Did you know? Dogs are the source of 99% of human rabies deaths As part of PRADE, IEC (information, education and communication) material is being developed in local languages for general public and healthcare practitioners. Shifa Foundation is in process of developing collaboration with local authorities and hospitals to create a unison for the purpose. At the time of delivery, her husband, a janitor in CDA decided for home delivery. After attempting to deliver at home for approximately 20 hours, she was brought to the Emergency Room for the delivery of her fth child. By the time she arrived, her uterus had ruptured. It was her luck that the doctors in the Obs/Gyne department were well trained and well equipped to diagnose her condition and to operate quickly. Through their timely interventions they managed to save her life and that of her baby. Her family was supported nancially by Shifa Foundation at important juncture of their family life. Obstetrics & Gynecology Shifa Foundation has supported 21,613 deserving women during the last year, this included normal/ assisted deliveries, C-Section, gynecological procedure including hysterectomies, laparoscopic surgeries, laboratory services, ultrasound antepartum and intrapartum fetal monitoring. 510 Normal Deliveries Our doctors are well trained to carry out normal as well as assisted deliveries, caesarean sections, all major gynecological surgical procedures and diagnostic and therapeutic endoscopic gynecological procedures. Her pregnancy was normal and she delivered at home. After birth, Noor didn’t cry immediately and was rushed to a nearby hospital and was resuscitated. She was 3 months old when she started having generalized seizures and was brought us for the rst time. Here, owing to the expertise of the doctors in the pediatrics department and the diagnostic equipment available, she was diagnosed with Cerebral Palsy and her condition was explained to her parents. In a country where mental disability is considered a social taboo along with the nancial impact of the life-long care, this news was devastating for her parents. It was then that the doctors counseled them and offered them their full support for Noor who is currently undergoing treatment with the support from Shifa Foundation. Pediatrics Shifa Foundation supports deserving and poor pediatric patients from northern Punjab, KPK, and AJK as well as neighboring Afghanistan. Sub-specialty services are also offered at subsidized rates in the disciplines of pediatric gastroenterology and liver transplant, pediatric nephrology, urology, and renal transplant, pediatric hematology/oncology and bone marrow transplant. He works with his brothers as a low income laborer to support his family. Two months ago he started developing recurrent episodes of fever and nose bleeds. He went to a local hospital in Peshawar where he was diagnosed with Leukemia. His family was unable to afford the treatment and they began searching and contacting all forms of treatment facilities available. He was refused treatment by various well renowned cancer centers in the region. The family sold their home in the village but was still unable to raise money for his treatment. Disheartened by the response they heard about the Shifa Foundation from a family friend. With high hopes, Suleman and his brother Pir Syed visited Shifa Foundation and asked for their support. Pir Syed says he went into the doctor’s ofce, and said, “I don’t have much money and my brother has cancer. We have been all over the country looking for someone who can help us.
