Activity Vitality Points
Health Assessment (HA)
HumanaVitality calculates your Vitality Age with a brief online health
questionnaire. Members earn Vitality Points for completing the HA
for the first time each program year.

Once-in-a-lifetime reward for the first-time HA completion.

Calculators
Online tools that measure aspects of your health, like “Are you at risk
for a heart attack?”. They can help you take steps to lead a healthier
life. There are 19 different calculators, and adult members can earn
Vitality Points for each calculator they use – up to four per program year.
CPR certification
First aid certification
An adult member must send a completed CPR Form or First Aid Form,
available online, to HumanaVitality with the copy of certification within
90 days of completing the event. The form can be submitted while your
certification is still valid, if you completed your certification before your
HumanaVitality effective date.

The Health Assessment provides

Activity Vitality Points
Verified workout Members can earn 15 Vitality Points for a workout through partner
health clubs, tracking with a pedometer or heart rate monitor,
or by using smartphone activity-tracking apps.

Sports league
A member must be an active team member in a qualified, organized
sports league, such as baseball or basketball. The minimum number
of games or matches that must be played is eight. Members must
complete a League Participation Form, available online, and submit
within 90 days of league completion to HumanaVitality.

A member must register for and complete a fitness event or race approved
by a fitness, athletic, or sporting organization recognized by HumanaVitality.
The member must complete the Athletic Event Form, available online, and
submit it within 90 days of the event completion to HumanaVitality.

Please fast 9-12 hours before your screening (nothing to eat or drink besides water). Some exceptions for
fasting: People who have been diagnosed with diabetes mellitus or hypoglycemia, women who are pregnant
and people taking prescription medicines that must be taken with food. If you decide not to fast, your
screening results may not be accurate.

How to connect your compatible fitness device to HumanaVitality
If you have a Humana g1.0 or g2.0 Pedometer:
1. Sign in to HumanaVitality
2. Plug in your Humana g1.0 or g2.0 Pedometer
3. C
lick the View Calendar button and you will be automatically
directed to the new Workout Calendar
4. I f you don’t see View Calendar, go to the Connect Fitness Devices
page and click the prompt to begin registering your device

How to reconnect your fitness device to HumanaVitality:
For all other compatible fitness devices:
1. Sign in to HumanaVitality
2. Visit the Connect Fitness Devices page under Get Healthy tab → Fitness and Exercise
3. Find the link under your device logo to reconnect your device directly to HumanaVitality
4. App connects directly to HumanaVitality
How Vitality Points are awarded:
Steps: Take 10,000 steps in a calendar day to earn 15 Vitality Points.
Calories: Burn 200 calories in a single workout, at a pace of at least 200 calories per hour, to earn 15 Vitality Points.
Heart rate: Exercise at an average of 60 percent or more of your maximum heart rate for a minimum of 30 minutes in
a single workout session to earn 15 Vitality Points. Your maximum heart rate is calculated by subtracting your age from
220. A compatible chest strap transmitter needs to be worn during a workout for the heart rate data to be captured.
