MISSION STATEMENT
To provide recreational opportunities that enrich the quality of life for all.
The Board of Park Commissioners and staff are very pleased to provide the programs and facilities listed in this brochure.
We try to offer a well-rounded selection of activities and amenities to choose from, and we want you to have an outstanding
experience. Please let us know of any suggestions or concerns regarding our programs and facilities. We hope you take
the time to enjoy the many parks, facilities, and programs we offer!
DIRECTOR’S UPDATE:
I am very excited to have this opportunity to encourage the citizens of our community to look through this brochure and to
take advantage of the many various programs and facilities that the Bettendorf Parks and Recreation Department has to
offer. The Park Board Commissioners, Mayor and City Council, and the many staff that help provide these opportunities are
all very proud of the contributions made to the quality of life for Bettendorf residents.
The largest development project that we will be undertaking in 2015 will be the construction of the first phase of Forest Grove
Park. This 100-acre site has been in the planning stages for several years now and this first phase will provide recreational
amenities to this rapidly growing section of our community. Future phases will continue to add to the park’s offerings and will
include larger amenities that will appeal to both local neighborhoods as well as the entire community.
I also encourage you to get out and walk, run, or bike the many trails that Bettendorf has to offer. Part of a rapidly growing
network of trails in Bettendorf and the Quad Cities as a whole, these trails offer tremendous connections for recreational,
fitness, and transportation opportunities that truly are on the cutting edge.

Registration Forms received by Mail, Dropbox, or In Person
at City Hall, 1609 State Street will be accepted starting at
8:00am.
This brochure is available on our website throughout the summer. If you’d prefer a paper copy, they
are available while supplies last at the following locations beginning March 11th: Life Fitness Center,
City Hall, Library, Community Center, and Family Museum.

Registration forms can be found on our website (www.bettendorf.org/parks) or you may pick one up at Bettendorf City
Hall, 1609 State Street. Please print legibly and include the program’s code number.
ALL custodial parents MUST sign the waiver. We accept Cash, Checks, MC, VISA, American Express and Discover.
Make checks payable to Bettendorf Park Board.
CONFIRMATIONS & RECEIPTS
If registering online, you will receive an email confirming your registration. If you do not receive this email, please call
563.344.4113 to verify your email address. Most program notifications (cancel, location/time change, field trips,
schedules etc.) will be sent via email, so it is extremely important that we have your correct email on file.
If registering by form, staff will contact you by phone if your choice is unavailable. If you would like a receipt, please
include a self-addressed stamped envelope or attach a note asking to have a receipt emailed to you.
Please Note: All emails from Bettendorf Parks & Recreation will say they are from ActiveNet.
CANCELLATION POLICY
If cancelling before the program’s coaches meeting OR before the activity start date (whichever comes first),
you will receive an 80% credit (per participant per program) on your Parks and Recreation account.
NO credit will be given for a cancellation after these dates.
Day Camp: If you need to cancel a week, you must notify City Hall (344-4113) at least one calendar week prior to the start of the
week you are cancelling to receive an 80% credit (per participant per week) on your Parks and Recreation account. NO credit will
be given for a cancellation after this date. NO credits are given for cancelling an early bird or late bird option.
Playgrounds: NO credits or refunds are given for missed field trips.
Splash Landing Season Pass Holders: You must show your pass to your counselor every time you field trip to Splash Landing
to receive a credit on your account for that field trip fee.
Family Museum Members: Per the Family Museum’s policy, you are not permitted to use your membership on field trip days, so
no credit will be given for FM members attending this field trip.
Credit on your account never expires and may be used for any future recreation program, a t-shirt purchase, a Community Center
rental, a shelter reservation, and any purchases made at the Life Fitness Center, the Palmer Hills Golf Course (PHGC), Splash
Landing (SPL), and/or the Family Museum.
If you are cancelling because you are moving out of the area or your child has a medical problem, you will receive a prorated
refund up to 50%. If the Parks and Recreation Department cancels a program, you will receive a 100% refund or a prorated refund
if the program has already begun. All refunds are issued in the form of a check and processed once a month.

Recreation Program Shirt Information
For our baseball, softball, soccer, and flag football programs, each child is required to have a
program shirt. Shirts may be purchased at the Herbert D. Goettsch Community Center, 2204
Grant Street, 344-4112. Our shirts are two-sided, high quality, orange and royal blue mesh and
cost $20 for kids, $22 for adults. Cash or check only.
Job Opportunities

Summer Volleyball
The summer volleyball team registration forms will be available February 27th online at www.bettendorf.org and also
at the Life Fitness Center and the Bettendorf Community Center. The fee is $110/team. The deadline for signing up a
team is May 1st. 7th and 8th grade teams will play on Mondays, Freshman teams will play on Thursdays, Sophomore
teams will play on Tuesdays, and Varsity teams will play on Wednesdays. It is an 8-week program that runs June 1st July 23rd. Contact Mike Jones for any additional information at 563-594-1898.
Adult Volleyball Leagues
Men’s, Women’s, and Coed volleyball team registration forms will be available May 1st online at www.bettendorf.
org and also at the Life Fitness Center and the Bettendorf Community Center. Women’s teams will play on either
Wednesday or Thursday nights beginning August 26th with a team fee of $185. Men’s teams will play on Monday
nights beginning November 2nd. Coed teams will play on Sundays beginning November 1st. The team fee is $220
for Coed and Men’s teams. Contact Mike Jones for any additional information at 563-594-1898.
Adult Kickball League
Online applications will be accepted via www.teamsideline.com/bettendorf beginning June 8th. Deadline is July
19th. Fees are $150/team. Games are played on Tuesday evenings at Crow Creek Park beginning August 4th. Any
questions, please contact Teresa at 563-344-4533. Rain dates will be Thursday of the same week.
Men’s and Coed Fall Softball
Online applications will be accepted via www.teamsideline.com/bettendorf beginning June 8th. Men’s deadline is July
26th and Coed deadline is August 2nd. Team fees are $320 per team. Any questions, please contact Teresa at 563344-4533.
Fall Soccer League
Still coached by parent volunteers, this youth league, offered to 4 year olds through 5th grades, is played on Sunday
afternoons in Crow Creek Park. Register online at www.bettendorf.org/register June 22nd - July 31st. Games begin
September 13th. Teams are based on schools, and requests will NOT be honored. Any questions, please contact
Chad at 563-528-1885.
Flag Football League
NEW NIGHTS! The 3rd/4th and 5th/6th Grade Divisions will play on Monday and Wednesday nights. The 1st/2nd
Grade Division will play on Tuesday and Thursday nights. All games are played in Crow Creek Park. Register online
at www.bettendorf.org/register beginning June 22nd - deadline is July 26th. Practices will begin August 10th and
games begin August 25th. Teams are based on schools, and requests will NOT be honored. Any questions, please
contact Teresa at 563-344-4533. Please be aware that a division(s) may be cancelled or combined due to
low enrollment. This league relies soley on parent volunteer coaches!
Adult Basketball “A, B, & C” Leagues
Online registration will be accepted via www.teamsideline.com/bettendorf beginning September 7th with a deadline
of October 18th. Games are played on Sundays at Bettendorf Middle School.
2015 MLB Pitch, Hit, & Run Event
Participate in this exciting baseball skills competition open to boys and girls ages 7-14. Showcase your pitching,
hitting, and running abilities. The MLB-PHR program is intended to encourage youth participation and emphasize the
“FUN” element of baseball. Since there is NO FEE, every child has the chance to participate. Boys and girls compete
together at all levels of competition. Participants will compete in each of the three components. April 18th from 9am
to 11am at Crow Creek 4-Plex.

Bettendorf Parks and Recreation and Davenport Parks and Recreation are partnering to
present a variety of recreational activities throughout the community.
Duck Creek Lodge
Duck Creek Lodge is a 40’ x 60’ indoor facility, with a maximum capacity of 200 people.
It is heated, air-conditioned, and has kitchen facilities, including a sink, refrigerator,
microwave and stove. Food and beverages may be catered. Seating capacity is
approximately 120. Reservations for the lodge must be made in person, on a first
come-first served basis at the Parks and Recreation Office at the River’s Edge, 700 W
River Dr, or at the Vander Veer Conservatory Store, 215 W Central Park. For questions,

The River’s Edge
The Quad Cities very own indoor ice and turf arena is located at 700 W. River Drive
in Davenport. Fall and Winter Activities include: Indoor Soccer, Public Skating, Flag

“For Kids, By Kids”
America’s second oldest children’s theatre is headed into its 61st season!
We help kids ages 3-18 learn to express themselves imaginatively, dream big, and
have fun. In addition to year round classes, camps, and workshops,
we also perform “Mainstage Productions” for the whole family to enjoy.

Facilitates communication and collaboration to promote, enhance, and support
access to recreational resources available to individuals of all ages and
abilities in the Quad Cities and surrounding communities.

Riverfront Golf Cart Tour
Sponsored by RiverAction Sept. 4th – The Mississippi River Trail tours offer participants an
opportunity to see and hear all that the riverfront has to offer from the front seat of a golf cart.
This is a great opportunity to gain some knowledge on the past, present, and future of many of
our riverfront amenities in the Quad Cities. There are five different tours from which to choose.

Ride The River
Sponsored by RiverAction June 15th – Annual Father’s Day bike ride that’s fun for the whole
family. Bikers ride on the Mississippi River Trail to Leach Park for a boat ride on the Celebration Belle. Route includes tour of Arsenal Island, family fun fair, and post ride party. Early bird

Mississippi Valley Track Club
For all area youth ages 8 to 18. Practices are from 6-7:15 p.m. Monday through
Friday at Bettendorf High School. The first practice is April 20th and season
finishes in late July. Competitions are sanctioned AAU and USA Track & Field

Bettendorf Trees Are Us

Bettendorf Senior Citizens Group
The Bettendorf Senior Citizens Group meets every Tuesday/Thursday from
12pm to 3pm at the HDG Community Center. All interested citizens 55 and
over are invited to attend. The group is sponsored by the Bettendorf Park
Board. Its many activities include Progressive 500, catered dinners, birthday
and anniversary parties, sing-a-longs, bus trips, and dinner and cards at Luther
Manor. $12.00/year due July 1st.
Bettendorf Park Band
Band rehearsals are Thursdays from 7:00pm–9:00pm in the Herbert D.
Goettsch Community Center. In case of bad weather, please listen to the
local radio/TV stations. If school is cancelled in Bettendorf on Thursday,
due to bad weather, the band will not rehearse. The band is open to any
instrumentalist from high school through post-high school age. Any high
school students wishing to join the band must participate in the instrumental
music program in their own school. Contributions may be made to Bettendorf
Park Band Foundation, City Hall, 1609 State Street, Bettendorf, IA 52722. The
only “fee” for membership in the Park Band is the purchase of a Park Band
Shirt. At present time, the cost of the shirt is $15.00. The Band Director is
Brian Hughes. Please contact Brian at maestroblh1@gmail.com for more

Points of Interest. Concerts are held at Veterans Memorial Park. In case of
rain, concerts will move indoors to the Herbert D. Goettsch Community Center,

NEW!! All reservations must be made online at
www.bettendorf.org/register. Pool rentals may be
made 60 days prior to your reservation date, and
are FIRST COME, FIRST SERVE. A $50 Non-Refundable deposit must be paid online when making
your reservation. The rental balance as well as your
concession option fees MAY be paid online as well,
but MUST be paid 5 business days prior to the event
date. Concession Stand Option (ran by the Hungry
Boy Deli) is $50. The Concession Area Option (you
bring in your own food) is $25. Rentals take place
Monday-Friday 8:30-10:30pm and Saturday-Sunday
6:00-8:00pm. Not ALL dates are available so check online for details. If you decide to cancel for any
reason, you must contact Splash Landing at least 4 hours prior to the event in order to receive a refund

POOL HOURS
Our main pool area includes a zero depth entry
with water geysers, a kiddie slide, two flume
slides, a drop slide, and a diving board, as
well as lap swimming lanes (Available during
scheduled times). The zero depth toddler pool
area features a slide, numerous spray features,
and ample deck and shaded areas for lounging.
The concession area also provides a large
shelter with seating and shade.

GENERAL POOL SAFETY GUIDELINES
Children 6 and under must be accompanied by an adult 16
years or older and will be required to wear a wristband and
must stay accompanied by an adult at all times and in both
pool areas. Children 7-10 years old must be accompanied
by someone 16 years or older to enter and remain at the
pool. Only “Coast Guard Approved” lifejackets/vests may
be worn into the pools. No water wings, blow up rings, food/
beverage or cameras will be allowed into the facility. Anyone
who requires a diaper must wear the appropriate swimming
diaper at all times. Child sizes are available for purchasing at
the Admissions Counter. Lawn chairs are welcome! Main pool
slides may require swimmers to complete and pass a swimming
test prior to their use. Parents/guardians are responsible for
supervising their own children’s actions and behaviors. Please
use common sense for the safety of everyone in your group.
The pool will NOT OPEN if the air temperature is below 65
degrees, weather conditions are threatening, or the number of
swimmers drops to below 20. If you are in doubt as to whether
the pool will be open or not, please call before you come! SPL

Outdoor Lessons at Splash Landing
Classes are 40 minutes in length and meet Monday-Thursday. We understand classes may
be cancelled due to weather or unforseen circumstances, but we do not offer any make-up
classes. You may register your child for 1, 2, or 3 sessions, but please know that
it may take a child two sessions to pass through a level. Parents observing lessons are

AAU BASKETBALL & USA VOLLEYBALL TEAMS
We offer discounted rental rates to organizations within the Bettendorf Community for volleyball and
basketball practices. Because our gym time is limited, a lottery drawing will be held August 3rd at
8:00am to determine the order in which coaches will be allowed to schedule practices. Practices may
be scheduled once a week from September 1st, 2015 through July 31st, 2016. Coaches must notify
Kim Radcliff at 344-4113 before August 1st of their intent to enter into the lottery. Rates are $10 per
hour M-F 3:30-5:30 and $25 per hour on weeknights and weekends.
Only Bettendorf Residents are allowed to participate in the lottery and reserve court time.

We have six shelters available for reservation.

or stop by City Hall weekdays 8:00am-5:00pm to
make a cash/check reservation. Special events
must be approved by the Bettendorf Park Board.

Regular Shelter Fees
$25.00 for a 4-hour time block plus $5.00
per hour for each additional hour.
Bandshell Shelter Fees
$35.00 for a 4-hour time block plus

Softball Fields
Reservations are taken for Crow Creek Park Fields
1-2-3-4-7 and the fields located at Veterans
Memorial Park (23rd St). All reservations are
1.5 hours and are open to Bettendorf residents
or teams playing in the Bettendorf Parks and
Recreation leagues. Reservations may be made
up to one week in advance. For tournaments,
events, or usage over 1.5 hours, the fields
must be rented. The fee is $25 per field per
day. Additional fees may apply depending on
the size of the event. All individuals requesting a
tournament must fill out a special use form and
have the event approved. This must be done at
the administrative office located at City Hall. The
event must be approved prior to the activity being
officially booked. Bases and lights at $15.00 per
hour are only available to groups renting the fields.

JAMIE HILL MEMORIAL GOLF TOURNAMENT
Friends of Bettendorf Parks Foundation will be hosting their 22nd
Annual Jamie Hill Golf Outing at Palmer Hills Golf Course on Saturday,
August 29th. This is a four-person preferred ball format with three flights.
Registration, for returning teams only, will be taken through June 30th. New
teams will be accepted beginning July 1st. This is a great fundraising event
for the Foundation and honors the memory of a former Parks and Recreation
Department employee. There are a number of pin prizes including a $10,000
hole-in-one contest and lots of raffle and door prizes. Contact the Palmer Hills

Senior GOlf Cart Tour
Enjoy this wonderfully relaxing scenic tour of Bettendorf’s parks and trails
as you caravan along in golf carts. The tour begins at Palmer Hills Golf
Course and lasts approximately 2 hours. This spring and fall event is hugely
popular and has two tour times: 10:00am and 1:30pm. Many even enjoy
lunch before or after their tour at the Palmer Grill. Spring Tour is May 18th
(rain date 5/19)Fall Tour is September 14th (rain date 9/15) Registrations

honored. Coaches meeting is Tuesday, April 14th at 6:00pm at the Bettendorf Community Center. Games
are played on Mondays and Wednesdays on Crow Creek Park Fields #4 and #7 at 5:15pm and 6:30pm beginning
May 4th. Registration begins February 23rd. Cost is $30 (after March 15th, fee is $40). Deadline to register is

Novice Girls Softball
This modified fast-pitch softball league is for all 9-11 year old girls. This will be a 10-game schedule with a season
ending tournament. Scores and standings will be kept. Games are played on Tuesdays and Thursdays at either
5:15pm*, 6:30pm* or 7:45pm* beginning May 5th at Crow Creek Park on Fields #7 and #4. Rainouts will be
made up when feasible. All new players or players wishing to be put into the draft and picked for a new team
must attend the player evaluation held April 6th at 5:30pm at Crow Creek #7. Players will only be accepted after
this date if needed to fill team rosters. Players are drafted to teams at the coaches meeting on April 7th at
6:00pm at the Bettendorf Community Center. All players wishing to remain on the same team as last year,
need to indicate on registration, and will not need to attend the player evaluation. Requests will not be honored.
Registration begins February 23rd. Cost is $40 (after March 15th, fee is $50). Deadline to register is March 29th.

close as possible. Games will be 55 minutes in length. There will be a home plate umpire
calling the game. This league relies on parent volunteers to serve as coaches. Without parent
volunteers, we will not be able to create teams, so please consider being involved! Games
will be played on Tuesdays and Thursdays at 6:30pm*, 7:45pm*, and 9:00pm* at Crow Creek
Park on Fields #4 and #7. PLEASE NOTE: *These days and times may be modified
based on enrollment. Season will begin Tuesday, May 5th. This will be a 10-game season

Tennis
lessons
Bettendorf’s summer tennis program is managed by our year-round Tennis Director, Brent Hargrave. Brent has
instructed tennis players of all ages for the past 10 years. He is a Certified Tennis Professional through PTR, and
ensures quality instruction and well-organized classes.
In addition to the fee listed on the schedule, each player is to bring one can of new tennis balls that will be kept by
the program. Private lessons are also available. Please call our Tennis Director, Brent Hargrave, at 563-344-4119
for more information. Cost is $35/hour. *Rates are good through June 1st. After 6/1, fees increase by $10.
Group Lessons, Doubles Tactic and strategy clinics using the ‘Three and Me’ (Tennis pro plus 3 players) are
available. Private and Semi-private lessons are available. We offer video analysis, ball machine workouts, National
Cardio Tennis classes and “playing lessons” with our tennis professionals. Weekly drop-in clinics are offered for all

Camper Guidelines
Your child is required to bring a sack lunch and drink
each day unless stated otherwise in your weekly
schedule. Children using the early bird option must be
dropped off at the camp site no earlier than 7:30am.
Children using the late bird option must be picked up at
the camp site no later than 5:30pm. Parents that are late
for pick up will be charged an additional $5.00 fee per
15 minutes late per day. Campers should be prepared
for the elements of the day. Children should wear “play”
clothes and comfortable tennis shoes. Also send along,
accordingly, a jacket or sunscreen. Label all your child’s
belongings. Day camp will not be cancelled or delayed
for any reason. Camp will either move to the Bettendorf
Community Center or the Bettendorf Life Fitness Center
in the event of inclement weather. Call our hotline for

Day Camp Payment Plan Policy
Parents must pay in full for the first week plus a $20 nonrefundable deposit for each additional week at the time
of registration. The balance owed must be paid in three
installments or less. The first payment will be due on May
22nd by 5:00pm The second payment will be due on June
12th by 5:00pm. The third payment is due on July 10th. All
transactions must be handled at City Hall, 1609 State Street.
Payment plan users must provide credit card or debit card
information to be used if payment is not received in another form
by each due date. If the card payment is declined, then the
child will be removed from all remaining weeks, and the payment
plan will no longer be an option for registration.
We are committed to making this a fun-filled summer for all our campers.
Parent orientations will be held at your campers site, May 28th from 5-7 pm.

Remember when you went to the neighborhood playground
program as a kid? Well, the program is still available to your
children, and it’s more fun than ever! Seven (7) locations are
staffed for 8 weeks, providing your child with a variety of
supervised activities including athletics, games, special visitors,
arts and crafts, picnics, community service projects, and interplayground activities. Let us help your child have a summer

This program is for mild-to-moderate, special population individuals. Pre-requisites for
admission include being toilet-trained, being able to independently walk short distances,
and being physically and behaviorally appropriate. These guidelines are strictly followed
for everyone’s safety and enjoyment. Participants enjoy a variety of activities such as
games and field trips that supplement their summer with just the right exercise, fun, and
new experiences. A summer at Camp Mac will prove to be a time of laughter, excitement,
new beginnings, and lifelong friendships.

There will be no camp the week of July 4th. Individual
aides accompanying campers (required by their IEP and
provided/hired by their school district) MUST provide
their own transportation. Special transportation must be
arranged for the child as well if they are to be with the aide
at all times (including transporting to and from field trips).
Weekly activity calendars will be sent to all participants
and aides prior to camp IF their information is sent to the
Program Manager in advance.

a helper or guardian to this event if one-on-one

INDEMNIFICATION WAIVER OF PARENTAL CLAIMS
THE CITY OF BETTENDORF REQUIRES THIS FORM BE SIGNED BEFORE PARTICIPATION IN A CITY SPONSORED ACTIVITY.
IF THIS FORM IS NOT SIGNED, YOU OR YOUR CHILD WILL NOT BE ALLOWED TO PARTICIPATE.
DO NOT SIGN IF YOU HAVE ANY QUESTIONS.
If you are signing on behalf of a minor, both parents must sign. In the case of a single parent home, the custodial parent must sign. If you are
registering your child for class electronically, completion of the registration will be considered your consent and agreement with this document.
1. You agree to release, waive, and hold harmless the City of Bettendorf, the Bettendorf and Pleasant Valley Community School District, agents, employees,
volunteers, and contractors of either entity (Collectively referred to as “RELEASEES”) from any and all liability which may accrue to you from your
child’s participation or your participation in the following activity:_____________________________________________________________________
2. This release waives all of your claims whatsoever, known or unknown, which may arise by virtue of your child’s participation or your participation in the
activity, including injury or death, and damage to property, however such claim may arise, including but not limited to breaches of duty (such as breach of
the duty of care) and acts of current or future negligence by Releasees, their respective officers, employees, or agents, including, but not limited to,
negligent care, supervision, or control. This release waives any of your claims whatsoever against the Releasees arising from the actions of any other
participant in the activity or any other third party. Further, this release covers all activities immediately before and after participation, including
transportation to or from the event, and waiting for rides from City or School facilities after the event.
3. You agree to defend and hold harmless Releasees, their respective officers, employees, or agents against any claim, cause, loss, cost, or damage
whatsoever, including attorney fees, that arise from the above-described activity or your child’s participation in the above-described activity, including,
but not limited to damages for injury to your child or another person. This means, in part, that in the event something happens to your child during his/her
participation in the activity, and your child decides to sue the Releasees, that you agree to pay any sums awarded to your child, and to pay the Releasees’
attorney fees incurred in the defense of any litigation. You are also agreeing to pay for any damage caused by your child during participation in the
activity.
4. Participation involves a risk of injury. By signing this form, you represent that you have considered the risks of your child’s participation or your
participation in the activity, have obtained any medical clearance necessary to participate, and you or your child are able to participate without harm to
your child, yourself or others. You represent you or your child will use any program equipment with care for himself/herself, yourself, other participants,
and the surroundings. You have evaluated the risks of participation in the event by your child, and grant permission for your child to participate.
5. In the event of injury, program supervisors shall administer basic first aid, and shall summon emergency services via 911. Supervisors are not trained to
provide detailed medical care, and shall not, without prior agreement, provide medications. All costs of emergency care are the responsibility of the
participant/ participant’s legal guardian(s). NO INSURANCE IS PROVIDED BY THE CITY FOR INJURY TO PARTICIPANTS. All claims for
reimbursement of medical care costs, including emergency transportation, for you or your child are hereby waived.
6. Parents or others may take audio or video recordings of the participants. The City does not supervise or restrict recordings of public activities by third
parties. You agree that photos or videos may be used in promotional materials produced by the City to encourage participation in the activity.
7. You and your child agree to follow all rules of conduct established in conjunction with the activity. Failure to follow the rules will result in removal from
the activity, and you and your child agree that any program fees paid will be kept and not refunded. In the event of a problem while engaged in the
activity, notify the activity manager/supervisor.
8. This waiver pertains to the activity or class listed on this registration form. Details regarding this activity/class such as name, location, dates, times,
instructors, lesson plans, etc. are subject to change without notice. Fees for any/all future exact or similar activities/classes are also subject to change
without notice.
9. This Permission to Participate and Hold Harmless is given in partial consideration of you and your child being allowed to participate in the activity
described, and binds yourself, your personal representatives, and any heirs or assigns.
10. In the event a court of competent jurisdiction declares any part of this indemnification agreement to be invalid, the balance of the agreement shall be
deemed to survive and be binding upon you.
Cancellation Policy:

If cancelling before the program’s coaches meeting OR before the activity start date (WHICHEVER comes first), you will receive an 80% credit (per
participant per program) on your Parks and Recreation account. NO credit will be given for a cancellation after these dates.

Day Camp: If you need to cancel a week of day camp that your child has already been registered for AND you have paid for, then you must notify City
Hall (344-4113) at least one calendar week prior to the start of that week in order to receive an 80% credit (per participant per week) on your Parks and
Recreation account. NO credit will be given for any cancelled early bird or late bird options.

Credit on your account will never expire and may be used for any future recreation program, a t-shirt purchase, a Community Center rental, a shelter

INDEMNIFICATION WAIVER OF PARENTAL CLAIMS
THE CITY OF BETTENDORF REQUIRES THIS FORM BE SIGNED BEFORE PARTICIPATION IN A CITY SPONSORED ACTIVITY.
IF THIS FORM IS NOT SIGNED, YOU OR YOUR CHILD WILL NOT BE ALLOWED TO PARTICIPATE.
DO NOT SIGN IF YOU HAVE ANY QUESTIONS.
If you are signing on behalf of a minor, both parents must sign. In the case of a single parent home, the custodial parent must sign. If you are
registering your child for class electronically, completion of the registration will be considered your consent and agreement with this document.
1. You agree to release, waive, and hold harmless the City of Bettendorf, the Bettendorf and Pleasant Valley Community School District, agents, employees,
volunteers, and contractors of either entity (Collectively referred to as “RELEASEES”) from any and all liability which may accrue to you from your
child’s participation or your participation in the following activity:_____________________________________________________________________
2. This release waives all of your claims whatsoever, known or unknown, which may arise by virtue of your child’s participation or your participation in the
activity, including injury or death, and damage to property, however such claim may arise, including but not limited to breaches of duty (such as breach of
the duty of care) and acts of current or future negligence by Releasees, their respective officers, employees, or agents, including, but not limited to,
negligent care, supervision, or control. This release waives any of your claims whatsoever against the Releasees arising from the actions of any other
participant in the activity or any other third party. Further, this release covers all activities immediately before and after participation, including
transportation to or from the event, and waiting for rides from City or School facilities after the event.
3. You agree to defend and hold harmless Releasees, their respective officers, employees, or agents against any claim, cause, loss, cost, or damage
whatsoever, including attorney fees, that arise from the above-described activity or your child’s participation in the above-described activity, including,
but not limited to damages for injury to your child or another person. This means, in part, that in the event something happens to your child during his/her
participation in the activity, and your child decides to sue the Releasees, that you agree to pay any sums awarded to your child, and to pay the Releasees’
attorney fees incurred in the defense of any litigation. You are also agreeing to pay for any damage caused by your child during participation in the
activity.
4. Participation involves a risk of injury. By signing this form, you represent that you have considered the risks of your child’s participation or your
participation in the activity, have obtained any medical clearance necessary to participate, and you or your child are able to participate without harm to
your child, yourself or others. You represent you or your child will use any program equipment with care for himself/herself, yourself, other participants,
and the surroundings. You have evaluated the risks of participation in the event by your child, and grant permission for your child to participate.
5. In the event of injury, program supervisors shall administer basic first aid, and shall summon emergency services via 911. Supervisors are not trained to
provide detailed medical care, and shall not, without prior agreement, provide medications. All costs of emergency care are the responsibility of the
participant/ participant’s legal guardian(s). NO INSURANCE IS PROVIDED BY THE CITY FOR INJURY TO PARTICIPANTS. All claims for
reimbursement of medical care costs, including emergency transportation, for you or your child are hereby waived.
6. Parents or others may take audio or video recordings of the participants. The City does not supervise or restrict recordings of public activities by third
parties. You agree that photos or videos may be used in promotional materials produced by the City to encourage participation in the activity.
7. You and your child agree to follow all rules of conduct established in conjunction with the activity. Failure to follow the rules will result in removal from
the activity, and you and your child agree that any program fees paid will be kept and not refunded. In the event of a problem while engaged in the
activity, notify the activity manager/supervisor.
8. This waiver pertains to the activity or class listed on this registration form. Details regarding this activity/class such as name, location, dates, times,
instructors, lesson plans, etc. are subject to change without notice. Fees for any/all future exact or similar activities/classes are also subject to change
without notice.
9. This Permission to Participate and Hold Harmless is given in partial consideration of you and your child being allowed to participate in the activity
described, and binds yourself, your personal representatives, and any heirs or assigns.
10. In the event a court of competent jurisdiction declares any part of this indemnification agreement to be invalid, the balance of the agreement shall be
deemed to survive and be binding upon you.
Cancellation Policy:
 If cancelling before the program’s coaches meeting OR before the activity start date (WHICHEVER comes first), you will receive an 80% credit (per
participant per program) on your Parks and Recreation account. NO credit will be given for a cancellation after these dates.
 Day Camp: If you need to cancel a week of day camp that your child has already been registered for AND you have paid for, then you must notify City
Hall (344-4113) at least one calendar week prior to the start of that week in order to receive an 80% credit (per participant per week) on your Parks and
Recreation account. NO credit will be given for any cancelled early bird or late bird options.
 Credit on your account will never expire and may be used for any future recreation program, a t-shirt purchase, a Community Center rental, a shelter
reservation, and any purchases made at the Life Fitness Center, the Palmer Hills Golf Course (PHGC), Splash Landing (SPL), and/or the Family Museum.

BETTENDORF DISC GOLF COURSES
Disc golf has similar rules to golf. Players count how many “strokes” (throws) it
takes to get from the tee to the basket. You can use any flying disc, although
players commonly use special golf discs for driving, putting, and approach
shots. Family members of all ages can enjoy the sport and best of all it’s free!

DEVILS GLEN DISC GOLF COURSE
This challenging 12-hole course is located in Devils Glen Park. Each hole has
one tee pad and three different basket replacements that are rotated often to
add variety. Hole one starts on the far side of the baseball field.
MIDDLE PARK DISC GOLF COURSE
Located at Middle Park (Middle Road and 23rd Street), this 18-hole course is
one of the premier courses in the area. With multiple pin placements on every
hole, this course will test your skill every time you play. Trees, hills, and water
hazards add to the fun on this course.
CROW CREEK PARK CROSS COUNTRY COURSE
The scenic and challenging running/walking trail officially opened in October
2001. The course, which features both a 2-mile and a 3.1-mile layout, starts
and finishes at the new soccer fields located in the southwest quarter of the
park. This can be accessed either by Tanglewood or Devils Glen Roads. The
course winds through the park and provides the user an enjoyable walk or run
through both wooded areas and open areas of the park, without the hassles of
traffic! There are restrooms and water fountains located throughout the course.
For all to enjoy, please follow posted rules which include: no motorized vehicles,
no bikes and all dogs MUST BE ON A LEASH (please pick up after your pet).
CROW CREEK PARK SKATEPARK
Located near the main entrance just off of Devils Glen Road, the skatepark was
built in 2003 and accommodates a wide variety of skill levels. Bikes are allowed
during the following times: Sundays, Tuesdays, and Thursdays from 10amNoon and again from 5pm-6pm. Also on Mondays, Wednesdays, Fridays, and
Saturdays from Noon-2pm and again from 8pm-Sunset.
CROW CREEK PARK DOG PARK
