movie at Warriewood Cinema.

Come and drive go carts
around a track as well as
having a play on all the fun
equipment at Rocketship
park. Plus we’ll have
a yummy sausage sizzle
for lunch!

Cheeky Monkeys OOSH Vacation Care General Information
What should we bring?
What to wear
As we provide ALL food there is no need to pack
morning tea, lunch or afternoon tea. If your child has
a fussy diet/likes to snack you are welcome to pack
snacks. Please make sure the snacks are nut free.
Please dress your children in t-shirts that cover their shoulders.
It is also advisable for them to wear long shorts or tights for use
on the slides. If you think it is appropriate,please
pack an extra set of clothes in their backpack.
Each child should have their own backpack
containing the following items:
Please wear covered shoes only
– thongs and most sandals
are NOT appropriate.
* Drink Bottle
* Hat
* Sunscreen
A drink bottle so staff can continually
refill throughout the day with water to
keep your child hydrated. If you forget
to pack a drink bottle we will give your
child a bottle of water and a $3 charge
will be added to your account. A hat and
sunscreen is essential for excursions.
If it is an outdoor activity and your child
does not have a hat they will only be
able to play in the shade.
Cheeky Monkeys OOSH is a sun Smart centre.
General Information
Vacation Care is for children up to 12 years old.
PG Rated, movies may be shown at the centre.
Please see staff regarding any concerns.
Please offer any suggestions or ideas you have to improve

Download School stream for regular updates on arrive
back at the centre on long days out.
If any of these are not packed
or your child/ren are not wearing
appropriateclothing it will mean
they have to play in the shade.
Please don’t bring valuables!
Cheeky Monkeys Staff will not be responsible for monies
left unattended or in a backpack. Please clearly label ALL of your
items with your child’s name. Toys (including mobile phones, ipads,
MP3 players) etc or expensive jewellery or items should not be
brought to the centre.
$
Children are permitted to bring a small amount of
money to spend at their parent’s discretion. Please
put the money in an envelope clearly marked with
the amount and your child’s name and ask a staff
member to look after it for them.
Payments Full payment is required with your booking,
permission and enrolment forms. There are no cancellation,
refunds, credits or transfers.
Late Bookings Late bookings are accepted depending on
availability. There is a $20 per family late booking fee on every
separate occasion a late booking is made.
Please be aware that Cheeky Monkeys Play House is no longer able to offer short day vacation care (9am-3pm).
New 2016 enrolments need to be completed and returned.

Cheeky Monkeys are heading to Warriewood Cinema to see a movie. We will be heading to United Cinemas in a Cheeky Monkeys Bus.
Afterwards we are off to Warriewood McDonalds to have a treat for lunch

Rocketship Park Callisteman Way, Warriewood
Cheeky Monkeys are walking down to Callisteman Way, Warriewood Valley (Near Mater Maria) to participate in some fun activities with
“Go Kart Party” battery operated Go Karts. We will be having a BBQ lunch and playing on the outdoor equipment.

Webers Circus Hinkler Park, Pittwater Rd, Manly
Cheeky Monkeys are heading to Webers Circus for the 11am show via a Cheeky Monkeys bus or a hire bus. The show goes for two hours.
Thursday 14th April 2016
Apex Park Surfview Rd, Mona Vale
Cheeky Monkeys are heading to Apex Park to play. All Children will be playing on the equipment, as well as fun games.
We will enjoy a picnic lunch in the park.

Today we are heading to Inflatable World with a hire bus company.

Today Cheeky Monkeys are heading to AMF Bowling. We will be heading to AMF Bowling either on a Cheeky Monkeys Bus
or a Hire Bus depending on number of bookings. Today children will participate in 1 Game of Ten Pin Bowling.

Pirates of Mini Golf 1500 Pittwater Rd, Narrabeen
Today Cheeky Monkeys are heading to Pirates of Mini Golf at Narrabeen on a Cheeky Monkeys Bus.
Children can either choose the Pirate or the Caves course.

Laser Tag at Apex Park, Seabeach Ave, Mona Vale
Today Cheeky Monkeys are heading to Apex Park near Mona Vale beach to participate in Laser Tag with the company Aim4Fun.
We will be heading to Apex Park on a Cheeky Monkeys Bus. We will be having a sausage sizzle at the park.

FX-Zone Uhrig Rd, Olympic Park NSW 2127 Please make sure you sign the waiver form.
Today Cheeky Monkeys are heading to FX-Zone via a Hire Bus to jump on trampolines and play a few games.
For 8 and over they will get a chance to head to the batting cages.

Water Bomb Day Pittwater Uniting Church Hall, 10 Jubilee Ave, Warriewood NSW 2102
Today Cheeky Monkeys will walk up to the church hall. After playing water splashing games the children and staff will walk back to the centre.
I give permission for my child/ren to catch a bus to and from all excursions. In the event of wet weather, I give permission for my child/ren
to stay at Cheeky Monkeys Play House. I have read and fully understand the above conditions and agree to abide by them.

Waiver, Release and Indemnification Agreement
In consideration for being allowed to enter the and/or participate in any activity and/or program at HIT & BOUNCE PTY LTD trading as FX Zone Sports
Centre & Trampoline Park (hereby known as “FX Zone”), the undersigned, on his or her behalf, and on the behalf of the Participant(s) identified below,
acknowledges, appreciates, understands, and agrees to the following:
1. I, the undersigned acknowledge and understand that there are inherent
risks associated with participation in activities at FX Zone, and the use of
the trampolines and sporting equipment including, without limitation, to
scrapes, bruises, cuts, and even more serious injuries, such as paralysis
or death, and I fully accept and agree to assume all of these risks
including risks arising from the negligence of other participants.
2. I, the undersigned, for myself and the Participant(s) named below,
knowingly and fully assume all risks, associated with participation in
activities at FX Zone, and waives all claims for damage to person or
property arising from such participation, including risks arising from
the negligence of other participants.
3. I, the undersigned, for myself and the Participant(s) named below,
agree to follow all the safety rules of FX Zone and agree that the
failure to do so may result in expulsion from premises.
4. I, the undersigned, for myself and the Participant(s) named
below, HEREBY RELEASE, WAIVE, AND GIVE UP ANY AND
ALL CLAIMS, KNOWN AND UNKNOWN, THAT MYSELF, OR
THE PARTICIPANT(S) MAY NOW OR LATER HAVE AGAINST
FX ZONE, FX BUILDING GROUP PTY LTD, ITS MEMBER(S),
OFFICER(S), INSTRUCTOR(S), OPERATOR(S), AGENT(S),
OR REPRESENTATIVES RELATED TO ANY ACT, OMISSION,
STATEMENT, OR OCCURRENCE DURING OR RELATED TO THE
ACTIVITIES AT FX ZONE; CLAIMS FOREVER RELEASE BY MYSELF,
OR THE PARTICIPANT(S) INCLUDE, WITHOUT LIMITATION,
LIABILITY FOR DIRECT, INDIRECT, VICARIOUS, CONSEQUENTIAL,
AND INCIDENTAL, PERSONAL INJURY, DEATH, ECONOMIC
LOSS AND OTHER DAMAGE OF EVERY KIND WHEREVER OR
HOWEVER IT MAY OCCUR.
5. By signing this form, I agree and understand that this agreement is
binding on myself and the Participant(s) named below and the heirs,
successors and representatives and next of kin.
6. The undersigned certifies that he or she and the participants are
physically fit and may participate in the activities available at FX Zone
and has not been advised otherwise by a qualified medical person.
7. I hereby consent to allow the use of voice, video, image or likeness
in photographs and/or video for my child(ren) The permission for
use of any of the media above is allowed for all media such as
Newsletters, Business Flyers, Facebook, Company Website and
Video by a third party (such as filming for a television commercial)
8. By signing this form, I give permission for FX Zone to use my
email address to advertise products or promotions related to
their company. I also give permission for FX Zone to use my email
address to respond to feedback given about FX Zone.
9. I certify that I am of legal age and mental competence and that I am
the legal parent or guardian of the child for whom I am signing or,
if I am not the parent or legal guardian of the child, that I have the
express permission of the child’s legal parent or guardian.
