Welcome to the 2014 CDA Presents in San
Francisco. We have prepared an outstanding
educational opportunity for you and your staff
to experience.
Besides taking advantage of the versatile continuing education
and large exhibit hall, I encourage you to pursue additional
aspects of the meeting. Developing and pursuing relationships,
both professional and personal, is an enormous support to each
of us. Encouragement and understanding from the people who
surround us can often be the difference in moving forward or

President’s Dedication
Over the 35 years of my education and career in
dentistry, I have had many fine role models who
have been inspirational as they taught me how to
serve the profession. One such person in my life is
Dennis Shinbori, DDS. He was my instructor at the
Pacific Dugoni School of Dentistry, is now a friend and colleague,
as well as a fellow CDA volunteer. He possesses qualities that have
made him stand out as one of those unique individuals who can
seemingly do it all and makes it look easy.
As one of my instructors, he embodied the humanistic model in
dental education. He never gave a critique of a student’s work
without finding something positive in the effort, even when others
may have seen it as hopeless. As a colleague, he is a constant
positive friend who is always willing to share his considerable
wisdom and experience. Always self-effacing, he never misses an
opportunity to point out the positive attributes and contributions of
his colleagues and co-workers. And as a CDA volunteer, he has
been a tireless contributor to the success of CDA Presents and
nationally with the ADA Annual Session.
Dennis has been a stellar volunteer in the management of our
flagship membership benefit, CDA Presents. For nearly 25 years,
he has been a stalwart on the Board of Managers, serving from
1990 to the present and as chair in 2001-2002. At the ADA, he
has also contributed his expertise to our tripartite’s national meeting
as our ADA representative to The Council on Annual Sessions
(2005-2008) and was council chair for the 2008 meeting in San
Antonio. Besides the official positions on the Board of Managers,
he has been a patient mentor to new associates and board
members. He has been one of the constants in the incredible
success of CDA Presents.
I have been fortunate to have the opportunity to be guided by
visionaries such as Coach Bill Walsh and Dr. Art Dugoni. They
have been influential in how I have pursued my career. However,
when I look around for a person who quietly embodies the servant,
volunteer, educator, steadfast colleague and a positive influence on
all with whom he serves, Dennis stands out.
It is an honor and a privilege to dedicate the 2014 CDA Presents
San Francisco meeting to Dr. Dennis Shinbori.

Strollers are not allowed on the exhibit floor. A stroller/coat check

Course Audio Recordings
Selected recordings of lectures are for sale at the Digitell booth
located on the Esplanade level. Presentations that will be recorded
are marked with an audio symbol by the program.
First Aid
A first aid room is located across from Room 106 by the exhibit
hall entrance. A registered nurse is on duty during convention
hours. Any CDA Presents board member, staff or guard can
direct you.
Child Care
For the safety and productivity of all attendees, family hours on
the exhibit floor are from 9:30 a.m. to noon daily. KiddieCorp is
providing traditional day care services for children up to 12 years
of age at the Marriott Marquis. Note: A family lounge is located
behind aisle 300 on the exhibit floor, conference room 4.
Shuttle Service
Shuttle service is provided from most of the hotels on the CDA
housing list. Check outside the convention center entrance or with
the front desk of your hotel for a schedule and pick-up locations.
BART
The nearest BART station is about an 8-minute walk from the
Moscone Center. It is located at Powell and Market streets
between Fourth and Fifth streets. For a list of departure times,

Reserved Seating
Guidelines
Below are guidelines for those who purchased a reserved seat.*
This information was included in the Preliminary Program and
in your confirmation email.
• Seats will be held up to 15 minutes after the program begins,
after which time the seats will be released if the room is full.
• A separate entrance is available for reserved seat ticket
holders.
Workshops
Tickets can be purchased on site if space is available.
Some workshops have recommended or required prerequisites,
noted in a gray bar below the course title. If a course has
requirements or supplies, they are noted below the speaker’s name.
The deadline for ticket refunds was Aug 6. Refunds will not be
given for tickets purchased after Aug 6. All on-site sales are final.
Lost or forgotten tickets will not be replaced so please treat your
tickets like cash.
Express Lectures
These free, nonticketed lectures feature up-and-coming speakers
who are new to CDA Presents. Be among the first to hear
them! These speakers have not been scouted by the Board of
Managers and have accepted the invitation to present and be
scouted at this meeting without an honorarium.

Corporate-sponsored courses may or may not be ticketed.
General Information
• Courses must be attended in full; attendance is verified by
scan-in and scan-out times. Unverified attendance is not
eligible for credit.
• Ticket must be presented and is nonrefundable if lost, stolen
or forgotten.
• All courses have limited seating and may fill up and sell
out quickly.
• Reserved seating is grouped together in a designated section
so we can provide better service.
• Some courses do not provide C.E. units. Please check each
course description for C.E. details.
Note: This opportunity is optional and only available for select
lectures. Beyond limited reserved seating options, all lectures are
on a first-come, first-served basis. Plan for future meetings and take
advantage of reserved seating.
• Videotaping, photographing or audio recording with

Helpful Tips to Receive Your C.E.
Required Units for License Renewal
License numbers matter — License numbers and formal
For every renewal cycle, California state law requires licensed
dentists and allied dental health professionals to complete 2.0
units in infection control and 2.0 units in the California Dental
Practice Act. See Page 25 for available courses.
names of all licensed attendees are required for registration.
Plan ahead — Arrive at least 15 minutes early to all courses
and plan an alternative in the event your preferred course is full.
Late arrivals will not receive C.E. credit. Please take traffic into
consideration.
Scan in and out of each course — Arrival and departure
times are used to issue C.E. credits. Scan upon entry and exit
and remain in the course the entire time. Partial credit will not be
granted and credit will not be given for overlapping course times
or incomplete course attendance.
Write down course codes — During a course, the host will
provide attendees a three-digit code, an additional way to assist
in verifying your attendance. Write it down and keep it safe until
you’ve received your official C.E. certificate within three weeks
after the meeting.
Verify your C.E. units — After attending your courses:
• Use the CDA app through Sept. 11.
• Go to cdapresents.com through Sept. 11.
• Visit the on-site C.E. Pavilion located in the Esplanade and
The Spot on the exhibit hall floor.
Print certificates online — C.E. certificates will be available
three weeks after the meeting. Licensed attendees will receive
an email notification with a link to log in and access your C.E.
certificates. They can also be accessed via cdapresents.com
or mailed upon request by calling CDA at 800.232.7645 three
weeks after the convention.
C.E. Regulations
Educational Requirements for
Unlicensed Dental Assistants
Unlicensed dental assistants (who include any unlicensed
individuals in the dental office who perform the duties of a
dental assistant) hired on or after Jan. 1, 2010, and employed
beyond 120 days must complete the following courses one time
within 12 months of hire:
• California Dental Practice Act
• A specific 8-hour course in infection control (to include clinical
evaluation)
Additionally, they are required to maintain a current basic life
support certificate.
Dentist employers are responsible for ensuring that any
individual performing dental assisting duties complies with
these requirements. Dental assistants who have completed these
courses should keep evidence of completion in their files for all
future employers’ records.
Note: For a list of 8-hour infection control courses approved by
the Dental Board of California, go to cda.org/elearning.
If you are unable to attend one of the required courses offered
at this CDA Presents, these courses are also available online at
cda.org. For programs in your local area, you may view the
C.E. calendar online at cda.org/practicesupport.
To help you comply with the Dental Board of California
regulations for C.E., CDA identifies each course as “Core,”
“20%” or “non-eligible.”
Core — Courses that directly enhance the licensee’s knowledge,
skill and competence in the provision of service to patients or the
community. Core courses must comprise at least 80 percent of the
credits in a renewal cycle.

Infection Control
The Dental Board of California mandates continuing education
in infection control and the California Dental Practice Act for
license and permit renewal.

section 1005, the Dental Board of California Infection Control
Regulations.
