The Tuberculosis Laboratory Network in the Philippines (2007) William Mac Kenzie, MD & Aysel Gumusboga Phase 1: The socio-demographic factors that affected treatment default were age, sex, educational attainment, family support, key TB knowledge, treatment side-effects, the relationship between patients and health workers, alcoholism, and smoking. The socio-economic factors, on the other hand, were travel cost to TB treatment facility, geographical accessibility, type of occupation, and identifcation of main breadwinner of the family, while socio-cultural risk factors included poor knowledge, attitudes arising from the social stigma, and risk-taking behaviors. This study was conducted by United States Agency for International Development (USAID) upon the request of the Centers for Disease Control and Prevention (CDC), with the aim of assessing the quality assurance for sputum smear microscopy and evaluating the laboratory capacity to support Programmatic MDR-TB Management. TDF, in partnership with DOH, had been successful in combating MDR-TB in GFATM Rounds 2 and 5. A new testing and treatment policy, to be instituted in the next few years for smear positive patients with more than one month of previous treatment, will become an important step in the reduction of MDR-TB transmission. The NTRL, as structured during the study, was administratively under the RITM. However, this structure led to difficulties in NTRL's delivery of service and programmatic mission. Private laboratories capable of performing culture and DST for TB were available, but had no quality assurance measures in place. Public laboratories, on the other hand, were subjected by NTRL to quality assurance measures, but full implementation was hindered by lack of resources. Funding was mentioned as dependent on the interest of the Local Government Unit (LGU). Identification of Clinical, Radiographic and Demographic Characteristics of Smear Negative X-ray Positive TB Citizens (Iloilo) Coalition Against Tuberculosis, Inc. (CICAT) A study was conducted to identify the clinical, demographic, and radiographic characteristics of all patients diagnosed by the CICAT TB Diagnostic Committee of Iloilo as smear negative with active TB and to identify clinical and radiographic improvements of all smear-negative patients referred by the TBDC. The top five clinical symptoms reported were cough, chest/ back pain, weight loss of more than 10%, sputum production, and fever. The top three radiographic descriptions included apical/ upper lobe disease, cavity, and pleural effusion. Radiographs of 75% of the patients improved while 21% remained the same and only one case was found to have worsened after the DOTS treatment. Clinical symptoms of cough, hemoptysis and weight loss significantly improved in 25 patients. Malaria Prevalence and Knowledge, Attitudes, and Practices (KAP) Surveys in Priority Project Provinces John Q. Wong, MD, MSc Jose Miguel V. Vergara, MD A cross-sectional study of malaria prevalence and knowledge, attitudes, and practices in seven (7) priority project provinces in the Philippines. Results showed that 50% of Indigenous People (IP) respondents and 66% non-IP respondents have knowledge on malaria transmission, symptoms, and prevention. IP participants of the survey also revealed that their lack of financial resources inversely affected their attitudes towards practices of malaria prevention and treatment. Existing risk factors for IPs were their source of livelihood, home structure, and geographical proximity to health facilities. Contrasting practices between IPs and nonIPs from the survey included the increased use of prescribed medicine and the bringing of suspected malaria patients to a health facility in the IP population compared to non-IPs. Practices related to maintenance of mosquito nets showed that although the proportion of nets with holes was similar between IPs and non-IPs, the latter showed more seam failures. The LLINs also revealed fewer holes and seam failures compared to other types of mosquito nets. Recommendations on malaria prevention included mandatory re-treatment of mosquito nets. Future studies were recommended to be held on the same time that this study was conducted, keeping the sample size relatively small and using rapid diagnostic tests. 2009 83 84 External Dr. Kevin Palmer Evaluation of the and Dr. Eva Maria GFATM Support Christophel to Malaria Control in the Philippines (February 2004) The Philippines through the Tropical Disease Foundation was granted the GFATM Malaria Round 2 grant in 2003. In 2005, an external assessment was conducted as part of the PR's evaluation and final reporting for Phase 1 and preparation for Phase 2. The assessment covered areas such as project design, implementation of interventions, performance, operating environment, project management, and structural issues. The study was focused in 4 provinces: Apayao, Mindoro, Palawan, and Isabela. The external evaluation reported that the GFATM project made a significant difference for the at-risk populations in the Year 1 provinces. Effective malaria control was implemented on a significant scale, and malaria cases in 2004 already seemed to have decreased in some areas. Awareness about malaria control has increased, and municipalities had ownership of the project. Overall the project was well managed.i Second External Evaluation of the Global Fund Round 2 Support to Malaria Control in the Philippines (April - May 2007) As a follow-up of the assessment conducted in 2004, the Tropical Disease Foundation again engaged the WHO Regional Office for the Western Pacific (WHO-WPRO) in 2007 to conduct an external evaluation to assess the project outcomes and project effectiveness in Year 4. Findings were categorized into three areas: impact, outcome, and project effectiveness. As for impact, a notable decrease in cases and deaths had been recorded in areas where implementation had started in 2003; however, the observation could also be a result of external factors which could not be assessed by the evaluation. In terms of achieving the project goals, a 70 % reduction in morbidity had been achieved in 10 out of 26 provinces by the end of 2006, and a 50% reduction in deaths had been achieved in 23 out of 26 provinces. The evaluation also reported that overall, the project achieved significant outcomes in diagnosis, treatment, and prevention and control. In terms of project effectiveness, while the project generates many data, the issue lies on who will and how to manage the data and whether these are being utilized for decision making. Lack of coordination between the project and the NMCP has improved but is not totally resolved. Dr. Eva Maria Christophel Data Quality Audit for Global Fund Tuberculosis Round 5: Philippines (September 2008) Dr. Marijke Bleumink, Dr. Jerod Scholten, and Dr. Thomas Chiang The aim of the data quality audit (DQA) was to: 1) verify the quality of the reported data for key indicators at selected areas; and 2) assess the ability of the data management systems to collect and report quality data. The results of the DQA suggested that the program is running quite well. Data management and reporting systems are functioning quite well. The strongest categories identified were the indicator definitions and reporting guidelines. The weakest category was in the data management system process in various audit sites, including the national monitoring and evaluation (M and E) unit, Region III, and Region VI. The national M and E program scored lower than the intermediate and service delivery sites. Third External Evaluation of the Global Fund Round 2 Support to Malaria Control in the Philippines (October November 2008) Dr. Jeffrey Hii and Dr. Robert Condon The aim of the third external evaluation is to assess how close the project is to attaining its goal and objectives and to provide recommendations to guide implementation during the RCC phase. The report concluded that generally, the project activities had been successfully implemented. External Evaluation of GFATM Round 5 HIV Component - Round 5 Phase I Program Evaluation (March 2008) 86 Dr. Angela Chaudhuri, Technical Support Facility South East Asia and Pacific (TSF-SEAP) The GFATM HIV/AIDS Round 5 Grant was awarded to the Tropical Disease Foundation and commenced in October 2006. To continue funding for the program's second phase, an external evaluation of Phase 1 was needed. The evaluation aimed to assess the accomplishments of the GFATM HIV R5 Phase 1 (first 17 months) in preventing the further spread of HIV/AIDS and reducing the impact of the disease, as well as suggest strategies and recommendations for the implementation of Phase 2. The evaluation was conducted by the TSF-SEAP, along with representatives from WHO-WPRO, WHO-Philippines, DOH, and UNAIDS. The evaluation utilized quantitative and qualitative methodologies, such as the review of documents, individual interviews, focus group discussions, and field observations. The evaluation was conducted in 9 prevention sites and 3 treatment, care, and support sites. Findings were divided into two areas: project design and project implementation. For the project design, the report stated that the overall project design and the strategies are relevant, much needed, and aligned with the national needs and goals. However, the contribution to the national response, in terms of coverage, reach, and financial expenditures of the overall need, remain largely inadequate, making the contribution rather insignificant. The report noted that the implementation of the project is largely done well. Within the given constraints, the PR is on track to achieving most of its objectives. Areas of success are Procurement and Supply Management, capacity building of medical and allied medical staff, monitoring and evaluation, and documentation.
