Thank you for touching our lives by volunteering at

Dear Ridgeview Families,
At the end of this school year, I will be moving to Arizona to be closer to

End of the Year Medication Pickup
Dear Parents/Guardians,
Medications will not be sent home with students at the end of the school year.
•
Parents must pick up all medications on or before the last day of school. Medications must
be picked up by the end of the school day. Last Day for seniors is May 25th. Kindergarten last
day is May 26 and the All School Picnic May 27.
All medications will need to be picked
up by May 27th for all grades. If medications need to be picked up before the last
day of school, please make arrangements with the school nurse or health technician. The
Health Office number is 970-494-4620, Ext. 113
•
Emergency or as-needed medications (Epi-pens, Tylenol, Inhalers, etc.) must also be
picked up.
•
The school nurse will follow Colorado State Health Department procedures and deliver all
medications not picked up by May 27th, to be disposed of properly.
•
If medication will be brought back to the school in the fall, you may obtain a medical
permission form from the Health Technician at school or on the PSD website. We
encourage medication to be given at home if at all possible.
•
If your child will need medication for the 16-17 school year or will need a Health Care
Action Plan for health concerns such as Diabetes please see the website, Colorado Kids
with Diabetes Care and Prevention Collaborative website at
http://www.coloradokidswithdiabetes.org/ for your child's Diabetes Health Care Plans.
The Barbara Davis Center uses these care plans for your child's school Diabetes Care Plans.
The school will need an updated current care plan along with your child's current Doctor's
orders. If your child needs a Health Care Plan for asthma, permission to carry an inhaler,
seizures, serious life threatening food allergies or any other concern these forms can be
picked up in the health office before May 27th. These are available before May 27th if
you might be going to your child's health care provider over the summer break. The school
requires Health Care plans and Medication Permission forms to be signed by parent and
health care providers. Health Care Forms can also be obtained from the Poudre School
District website. If you have any questions or if you are not sure if your child needs a care
plan please call the Health Office at 970-494-4620, Ext. 113 and ask to speak to the Health
Technician or School Nurse.
•
This procedure is important to safeguard all students against improper or
accidental ingestion of medication.
