Monthly meeting. Developed a LUEZ (Loading, Unloading, Exclusion Zone) Safety Project Plan. Review of WorkSafe Victoria claims statistics for LUEZ injuries for the past 2 (two) years – Transport & Storage, and Manufacturing. Physical site visits. Developed / conducted an Industry survey into LUEZ. Industry Workshop on LUEZ – 28th May 2010. MUARC Study & Trials. LUEZ – WorkSafe Statistics LUEZ Journey 1. 2. 3. Claims statistics for LUEZ injuries for the past 2 (two) years (07/07-06/09) for the Transport sector. 10% sample of LUEZ for Manufacturing over the same period. Findings: i. ii. iii. iv. v. vi. Data did not allow easy reporting on LUEZ. Transport – Total Claims 2,855, with a min 43 LUEZ & max 65 LUEZ incidents (1.5% - 2.3%). One fatality in above statistics. Manufacturing – sample of 1,500 from total of 13,000 Claims, with a min 5 LUEZ & max 8 LUEZ incidents (0.3% - 0.5%), equating to min of 43 & max 69 of the total claim (based on sampling numbers). Major Bodily Location: Hip/Leg/Ankle, Foot & Toes, Hand & Fingers. Major Injury Type: Fractures and breaks. LUEZ – Physical Site Visits LUEZ Journey 1. 2. 3. 4. 5. 6. In excess of 30 sites visited by LUEZ Committee members. Site visits commenced from November 2009 – April 2010. Includes sites within Australia & New Zealand. Incorporated Transport, Distribution, Manufacturing & Stevedoring operations. Sites were of varying age i.e. very old to new premises. Findings: i. ii. iii. iv. v. 7. 8. Some good systems. Some poor to non existent systems. Great ideas. Poor compliance. Extract DO’s and DO NOT’s. The above was used in formalising Industry guidelines. Approx. 300 survey results to date (240 completed packs – 4 characters per company and 60 individual surveys). Responses - from Qld, WA, Tas, SA, NSW & Vic (majority). Age of site/s – Min 1 year to max 90 years, average 23 years. Employee numbers per site - Min 4 to max 1000. Average 157 per site. Number of loads per day – Min 0 to max 2000, average 86. Q17 – Near Miss – 81 near miss reports. Q18 - Potential seriousness NMI – 14% Severe & 24% Substantial nature. Q19 - Contributing Factor – People 85% (evenly split between drivers and mobile equipment operators). Q22 – How to improve • Training – 49% • Engineering solution – 26% • Supervision – 8% • Other – 17% Overall ranking (1-10): score of 7+ equated 80%. Leadership (Blue) ~ Simon Skazlic Other Activities LUEZ Journey • K&S - Monash University Accident Research Centre (MUARC) – Report Essential Elements of a Best Practice System of Control – Designated exclusion zones for the sole use of the forklift, or other loading equipment, demarcated in a manner which is fit for purpose for the nature of the loading environment; – Designated driver safety zones located so that the driver is kept away from the line of fire and can be kept under surveillance by the forklift operator at all times; – Clear and effective systems of communication between the operator and the driver which are fit for purpose for the nature of the loading environment and the equipment/vehicles involved; – Effective methods of providing loading in progress warnings to other operators, drivers, and pedestrians. • K&S, AkzoNobel & BOC - Pilot Trials. Separation of People and Equipment – The greater the strength of separation the greater the control and the less likelihood of an incident occurring. 2. The green light activates when the forklift handbrake is applied (orange operational lights turn off).
