Why is Midas Occurrence
Reporting Important?
• Midas is the communication tool used by the
organization that allows all departments to
share concerns in a secure system.
• Midas is the mechanism that documents how
we address safety issues and patient
complaints, which is a requirement of the
Centers for Medicare and Medicaid Services
(CMS).

Purpose of Reporting
• To create a culture of safety;
• Promote improvements in the quality of care delivered and to
promote a safe environment;
• To control/reduce loss in the event of an injury or unwanted event;
• Protect intangibles such as the organization’s standing in the
community with improved quality of patient care;
• Prompt identification of real or potential problems affecting the
safety of patients, visitors, and associates;
• Control/reduce the financial loss through early intervention and
remedy;
• To reduce the severity of potential claims and frequency of claims;
• Increase our customer satisfaction.

Click on ‘Risk’

Select the ‘Risk Form’ that best captures the nature
of what you are reporting.

To view options to
choose from, click on
the arrow down tab.
Select the Facility where the event occurred.
•Centegra Physician Care (CPC) – Includes both CPC and
Immediate Care Patients
•Memorial Medical Center – Includes both Centegra
Hospital - Woodstock and Centegra Specialty Hospital Woodstock
•Northern Illinois Medical Center = Centegra Hospital

When you open a field, the
help box will indicate how to
best complete the field.

Click on the calendar icon to
open a full view calendar. You
may then click on the date the
event occurred. The months
displayed can be changed with
the left and right arrows.

Select Patient or Non-Patient.
Patient Examples:
•Inpatient at CH-M, CH-W or CSH-W
•Outpatient Services (i.e. Same Day Surgery, Pre-Surgical Testing, Medical Imaging, Lab,
Cardiac Diagnostics, Cardiac Rehab, Sage Cancer Center, Physical Therapy)
•Home Health
•Wound Care Clinic
Non-Patient Examples:
•Visitor
•CPC Patient
•Immediate Care Patient
•Health Bridge Member or child of member

If you are entering a Patient
event, you can find the patient by
any of the options listed.
If you choose to look the patient
up by ‘Name’, you will be given a
list of all patients with the same
spelling.
*If you are entering a NonPatient event, go to page 15.

Select the correct Patient
Name with the correct
Encounter Start Date
(Admission Date) from the
list provided.

Risk Form
* Fields with a red
asterisk are
required fields

•Location should be the area
that is responsible for the
event or where the event
occurred. *This is not always the same
as where the patient is located.
•The ‘Event Type’ should be a
more focused description of
the event.
•The ‘Incident Subtype’ is not
available with every Risk Form.
However when it is available it
can provide more detail to the
report.

The look up field
displays your
available options.

The comments section
is used to enter the
details of the event.
This should be a BRIEF
description of the facts.

The individual reporting
the occurrence can
enter:
•Their Associate ID
Or
•The first few characters
of their last name and
then press the tab key.
Then select the correct
name in the ‘ Lookup
Results ’ section.
Click ‘Save’ and you are DONE!

If entering a NonPatient event, select the
Non-Patient ID by using
the Lookup Field.
Enter Non-Patient
Name
Click ‘Save’ and you are DONE!

Warning!
Midas/Remote Data
Entry will time-out after
20 minutes. If you need
more than 20 minutes,
please call Risk
Management at ext.
8111. If you do not
save within the allotted
20 minutes the
information will be lost.
The time remaining is in
the lower left corner.
