First, we will cover how to approach patients with complicated and chronic pain complaints. Second, we will be introduced to the conditions that affect the orofacial region, and finally we will discuss some of the treatments which are used to help these patients. Alan Stiles, D.M.D., is an Instructor in the Department of Oral and Maxillofacial Surgery at Thomas Jefferson University. His clinical practice is limited to the management of head, neck, and facial pain. Dr. Stiles is a graduate of University of Pittsburgh. He received his dental degree from Temple University School of Dentistry in Philadelphia, Pennsylvania. He completed a residency in Orofacial Pain and Dysfunction at UCLA School of Medicine and Dentistry. He then completed his fellowship in headache at the Jefferson Headache Center in the department of neurology at Thomas Jefferson University in Philadelphia. There is a genuine energy, a hustle and bustle with people coming and going, that I find exhilarating and entertaining. Strange, I know, but over the years I seem to have spent, and continue to spend, a fair amount of time waiting in train stations, in particularly the Trenton train station. Both of my children have spent time in NYC and one of the Dr. Joanne Burrell easiest ways to get back and forth is utilizing the New Jersey Transit Line out of Trenton. Being the triple A type personality dentists often are, I am always early waiting for them, parked in the line of cars picking up passengers. This gives me ample time to “people watch.” I find the train station exhilarating because everyone is moving, going someplace, checking their watch, checking the board, walking and sometimes running. Everyone is on a mission, whether it is to get on the train or pick someone up; Entertaining due to the wide variety of people one observes: young, old, business people, students, parents, boyfriends, girlfriends – you name it. It is fascinating to watch the interactions of people, the various fashion senses and the individuals juggling luggage whether it is their own or someone else’s. The interactions are often heartwarming to observe; the girlfriend coming out of the station, seeing her boyfriend, watching the huge smile on her face develop; the parents waiting in the car, anxiously checking their watches. Dad gets out of the car, again looks at his watch; Mom bobs up and down looking over the crowds trying to catch a glimpse of the person they are waiting for. When all the family members see each other, it is touching: hugs, smiles, quick chatting and sometimes tears. Dad’s being Dad’s...they typically pick up the luggage and the party moves on. Luggage is another interesting item to observe at the train station. It often varies on what I suspect is the travel savviness of the individual. Who does not use luggage on wheels anymore?? You would be amazed. Then there is a wide variety of luggage on wheels. Some are easy gliders and roll right along, no hiccups. Then there are others...yes, the luggage might be on wheels, but they topple one way, are righted, than they topple the other way – amusing. Then there are the travelers who really are attempting to carry way too much, the rolling bag, the overstuffed bag over the shoulder and the back pack larger than life. All of this creates a Sherpa type of appearance. During the holidays, the train station is literally a hum of activity – almost as if on steroids. I have sat waiting for one of my children on almost every holiday. Thanksgiving is my favorite holiday. There are no expectations on this holiday except good food, family and friends. Typically our meal means as many carbohydrates that can possibly fit on a dining room table. My kind of meal. I suspect that this Thanksgiving I will be waiting at the Trenton station for Chelsea. The police will be directing cars which will most likely wrap around the block, and people will be darting across the street to waiting cars. Abington reports both the patients and participating dentists are very pleased with this worthy program providing basic dental services to the uninsured or underinsured residents of the greater North Penn community. This unique model of dental care is successful due to the partnership of Abington Health which administrates the program, the VNA Foundation of Greater North Penn’s grant of $100,000 to provide the funding, and Montgomery-Bucks Dental Society helping to coordinate recruitment of dentists. The goal is to continue to increase the number of participating dentists so those in need of dental care who may also have transportation issues can have a dentist proximal to them. Also, increasing the number of participating dentists will allow the program to serve more patients without taking too much chair time from each participating dentist. This is not a pro bono program. Compensation to participating dentists will be roughly half their regular fees. If you have already volunteered your dental services, thank you so much. Her office will serve as a consolidation packing area. From her office we plan to drop the candy off at the USO at the Philadelphia airport. Dr. Stiles is an active member of the American Academy of Orofacial Pain and the American Headache Society. He is also a peer reviewer for Headache: The Journal of Head and Face Pain and Cephalalgia. Dr. Stiles’s publications include many peer-reviewed articles, book chapters, an Atlas of Migraine, and Clinical Manual of Trigeminal Neuralgia.
