CNAP Prior to FFY 15
• CNAP group consisted of only SNAP-Ed
funded partners
• Focused primarily on coordination of
sites where education was provided
• Little coordination of nutrition and
physical activity messaging and
resources across agencies in Santa
Clara County
Integrated Work Plan
Objective
• By September 30, 2015, collaborate with
at least five partner agencies in the
community to coordinate consistent
nutrition and physical activity
messaging to the SNAP-Ed eligible
population in Santa Clara County.
Selection of Monthly
Themes
• Promote fruit and vegetable consumption,
physical activity, and key messaging from the
2010 Dietary Guidelines for Americans.
• Many tied to national health observances,
NEOPB signature campaigns, and/or holidays
• For Harvest of the Month Theme, Second
Harvest Food Bank provided input on produce
items typically distributed by the food bank
each month, and which items clients had most
difficulty knowing how to prepare

• Increase in number of individuals promoting
each month
• 38% increased nutrition and physical activity
promotion as a result of the monthly themes
• Most common ways of promoting the
monthly themes
– Distribute handouts to participants – 75%
– Conduct taste tests/recipe demonstrations – 42%

• 8% response rate
• Increase in number of individuals promoting

• 61% increased nutrition and physical activity
promotion as a result of the monthly themes
