Thank you for joining us at the first-ever National Skilled Nursing Investment Forum. We are excited by the response to this new event and hope you find the content and networking opportunities valuable. With the average skilled nursing facility now 30-40 years old and continuing to lose occupancy each year, this year’s theme—Moving From Yesterday to Tomorrow: Capital & Reimbursement Strategies—is an important one. The opening general session will focus on the business case for modernizing your facility. Breakout sessions will feature case studies that present approaches for modernizing and repositioning your facilities, plus discussions on best practices, accessing capital and reimbursement strategies. SNIF We’re all wondering how potential healthcare legislation, regulatory and reimbursement changes, and budget cuts may impact long-term care providers. Sessions will examine the possible effects, both at the national and state levels. During the Tuesday Luncheon—The Impact of Healthcare Reform on Long-Term Care and Especially on Skilled Nursing Facilities: The View from Washington D.C.—a panel of experts will discuss what may emerge from the health care debate and RUGs-IV. Later that afternoon during the Medicaid Funding and Policy Directions: The View from Individual States general session, four state nursing home association presidents will discuss the policies and programs in their states and what trends they see at the state level. In addition to these exciting sessions, you will enjoy multiple opportunities to network with fellow operators, lenders and investors in the skilled nursing sector. Join us Monday evening for the Opening Reception, where you can enjoy cocktails, hors d’ouevres and a casual networking environment. On Tuesday evening, join Regional Symposium attendees for a joint networking reception. To aid in your networking efforts, skilled nursing operators will be identified by the “SNF Operator” ribbon on the top of their badges. Thanks again for joining us at this inaugural event. Enjoy! Doug Korey Managing Director Contemporary Healthcare Capital, LLC Planning Committee Chair 29 Tuesday SessionS 9:00 a.m. - 10:30 a.m. Opening General Session Indigo AB SNIF The Business Case for Modernizing Your Facility: Moving From Yesterday’s Skilled Nursing Model to Tomorrow’s How do you combine an aging structure with today’s available capital to provide a product that competes and meets the needs and desires of the modern resident? Find out in this session, where you’ll hear current data that describes the state of the nursing home sector, including age of properties, revenue, market trends, bed needs and design obsolescence. Two very different case studies—one from national operator Genesis HealthCare Corporation with Formation Capital LLC, and one from California based regional operator Plum Healthcare Group with GI Partners LLP—will demonstrate their individual business cases and explain their rationales for renewing their facilities. You’ll learn how both the operators and equity partners evaluated the merit of investing in older buildings, calculated return on investment and analyzed the market and environmental factors used to determine which types of replacements/renovations would be most beneficial. You’ll get real numbers that explain why modernization is a logical choice, in addition to strategic approaches designed to help you assess, prioritize and implement your own improvements. Moderator: John J. (Sean) Nolan, CFO, Fundamental Panel: Alexander Fraser, Director, GI Partners LLP George V. This includes details on capital structures and reimbursement considerations. You’ll hear how StoneGate’s new skilled nursing model leverages a patient-centered environment, outstanding medical outcomes and the lowest cost for post-acute rehab to become a leader in short-term transitional stays in each of its markets. Learn how combining a high-acuity medical model with a superior hospitality component—including enhanced programming, a variety of dining options, an emphasis on wellness and amenities such as private showers, flat-screen TVs and spas—significantly improves marketing and referrals for potential residents, adult children, discharge planners and case managers. Citibank will explain why this next generation business model is attractive for loan financing. Moderator: Mike Stephen, Senior VP-Marketing, Health Care REIT, Inc. You’ve heard and read about healthcare reform, but what could it mean for the long-term care provider? A panel of experts—including a leading analyst, two major operators, the president of The Alliance for Quality Nursing Home Care, and a capitol hill veteran—will examine effects of potential legislation and RUG IV on long-term care. These knowledgeable experts will discuss what may emerge from the health care debate, including employer insurance requirements, post-acute bundling, transparency requirements, the CLASS Act, and the merging of payment mechanisms with quality measures. In addition, panelists will focus on the potential “game changing” impact of RUG IV for skilled nursing providers. This experienced panel will take all the news about healthcare reform and drill it down to the bottom line, so you’ll learn how you’re likely to be affected as a result of the possible healthcare legislation. Moderator: Brian Fortune, Chief Political Strategist, Marwood Group Panel: Eric Gommel, Vice President, Stifel Nicolaus Greg Miller, Senior Vice President-Corporate Development, Kindred Healthcare, Inc. This continuum is comprised of skilled nursing, assisted living and memory care on a campus that is either newly developed or that has been repositioned. The session will focus on the evolution of Trilogy’s site and market evaluation, facility design, construction, marketing and programming over the past five years to meet ever changing resident needs and desires. You’ll also hear the perspective of an active industry lending institution on why the Trilogy model is compelling from a senior debt risk perspective. SNIF Moderator: F. Donald Kelly III, Principal & Managing Partner, Energy Court Holdings, LLC Panel: Peter J. Kane, Managing Director, The PrivateBank Steven A. Van Camp, Executive Vice President and CFO, Trilogy Health Services, LLC 35 Tuesday SessionS 2:00 p.m. - 3:00 p.m. Breakout Session Indigo H SNIF Repositioning Existing Nursing Home Beds Come learn about the 2003 Skilled Nursing Facility bed capacity reduction plan proposed in the State of Oregon, which was crafted to manage the declines in SNF caseload throughout the state at that time. The plan was designed to create a positive method of eliminating existing capacity—which was mostly older, outdated SNF facilities—as opposed to forcing these operators out of business. In addition, the plan also gave the Oregon SNF industry more opportunities to create new capacity in the future as demand warranted. This session will describe the key points of the plan in detail, as well as the positive and negative outcomes the plan has had in Oregon. Also learn about New York State’s program, the Health Care Efficiency and Affordability Law for New Yorkers Capital Grant Program (“HEAL” NY Program). This program funds the development of long-term care initiatives as alternatives to residential health care facility beds to effectuate downsizing of RHCF bed capacity. ElderWood Senior Care applied for grants to convert skilled nursing facility beds to assisted living under the HEAL program and received 7 grants and licensure approvals for projects in Western NY. Come hear how the program enables the State of New York to reduce Medicaid spending, while offering providers economic incentives to resolve excess capacity and to provide a variety of alternative community-based services to meet community needs. Moderator & Panelist: Steve Fogg, Chief Financial Officer, Marquis Companies & Consonus Healthcare Panel: Jim Carlson, CEO/President, Oregon Health Care Association Dick Herrick, President & CEO, New York State Health Facilities Association, Inc. Come find out how a Pennsylvania facility did so by switching to a higher paying class. Learn how to utilize market research to identify niche markets, and discover the tools for analyzing your cost per patient. Get explanations of the cause and effect between targeted expenditure and increased revenue, as well as the components necessary for creating a successful high-acuity program. In addition, you’ll hear about marketing techniques that can boost occupancy and attract high-level patients. Owners and operators who want to effectively analyze outcomes and remain in their community spotlight will benefit from this session. Rosenbloom, President, Alliance for Quality Nursing Home Care 38 TUESDAY SessionS 3:30 p.m. - 4:30 p.m. Breakout Session Indigo H Rehab/Renovate Case Studies Before you initiate renovations, you need to formulate a strategy for your project. Do you hope to preserve or expand your market share? Is it better to rehab the entire facility or just a section of it? How do you identify “product lines” for development? In this session, case studies will illustrate scenarios to help you answer these important questions. You’ll also learn tactics for evaluating market demand and assessing the need for development or expansion of your facility, as well as how to pro forma payback and return analysis. At the same time, many states are looking for ways to move more of their Medicaid dollars into home and community-based services and away from institutional care. In this session, you’ll hear insights from four state nursing home association presidents and learn about the trends they’re seeing. They’ll respond to the first two general sessions with specifics about their individual states. In addition, they’ll look at their new local policy initiatives, such as the push for patient-centered or person-directed care, and the impact of budget constraints on their local Medicaid rates. You’ll discover how these policies may have applications beyond their individual states—and how they may relate to your own circumstances. Find out what these pros are seeing locally, what these trends mean for aging nursing home stock and the need for renovation or replacement, and whether these policies are making modernizing easier or more difficult.
