THE PLAYERS Center for Child Health at Wolfson Children’s Hospital 2015 Annual Report 2015 Annual Report THE PLAYERS Center for Child Health at Wolfson Children’s Hospital serves as a community outreach center focused on improving the health, wellness and quality of life of families living within the North Florida and South Georgia region. Thanks to a $1 million gift from THE PLAYERS Championship, the Center was established in 2012 as a major child advocate and injury prevention center. The Center promotes health and safety while also representing the interests of medically underserved children at the local, state and national government levels. Our comprehensive approach to these focuses include: • Community Education • Healthcare Access • Unintentional Injury Prevention • Legislative Advocacy • Health and Wellness 2015 BY THE NUMBERS This report outlines the Center’s continuous efforts to promote 10,829,436 media impressions with important information on children’s health from our fourth year of programming. In 2015, our core approaches of injury prevention, asthma management, THE PLAYERS Center for Child Health participated in 277 nutrition, hygiene and health care access. events, educated 35,780 children and caregivers, and made Community Education THE PLAYERS Center for Child Health offers six community-based programs with the goal of improving health and wellness through children’s learning development. Our programs are developmentally and age-appropriate, encouraging and empowering children to make better choices regarding hygiene, eating habits, safety and overall health. The School Health Advisory Council for Duval County approved our community educators to teach the programs in Duval County schools. The programs have been well-received and requested in classrooms, faith-based organizations, after-school programs, child care centers, YMCA Healthy Living Centers, City of Jacksonville Parks, and summer camps in Duval, Baker, St. Johns and Clay counties. Our efforts have expanded in 2015 with the addition of “Countdown to Drive” as one of our curriculum offerings, a program geared toward promoting driving safety for soon-to-be driving teenagers. In total, community educators from THE PLAYERS Center for Child Health have educated 7,487 children in 401 classes. Each year, roughly 8,000 families lose a child due to a preventable injury. Safe Kids Northeast Florida, led by THE PLAYERS Center for Child Health, works to provide families in the Northeast Florida region with services and education to keep their children safe. As the lead agency for Safe Kids Northeast Florida, we provide education on comprehensive pediatric safety at community events and programs throughout the region. Safe Kids Event Interactions 17246 17823 14273 18114 17958 2013 2014 18814 16000 In 2015, we provided 250 free swim lessons, including specialized lessons for 10 autistic children in partnership with the Florida Swimming Pool Association, Wolfson Children’s Rehabilitation and Swimming Safari. With help from a Safe Kids Worldwide grant, we were able to conduct and administer events and education for Safe Kids Day, pedestrian safety, home safety, medication safety, laundry room safety, water safety, sports safety, bike safety, and Safe Kids Buckle Up child passenger safety program. Safe Kids educators from THE PLAYERS Center for Child Health have been established as the go-to resource for the local news media. In response to accidents that gained media attention, media outlets rely on our child safety experts to provide valuable education to the community. 8424 2008 2009 2010 2011 2012 2015 2015 Unintentional Injury Events by Specialty Child Passenger Safety Home Sport Water Bike/Pedestrian Safe Sleep/CPR Child Passenger Safety Motor vehicle crashes are the number-one cause of death among children ages one to 19 in the US. A child’s risk of death in a car accident decreases by roughly up to 71 percent if properly secured in a correctly installed car seat. However, 73 percent of all child passengers either sit in an incorrectly installed car seat or none at all. Certified Child Passenger Safety technicians at THE PLAYERS Center for Child Health facilitate monthly car seat checks and classes to educate parents about the importance of motor vehicle safety throughout childhood. Offered at both our office location and Baptist Clay Medical Campust, more than 1,300 car seats were inspected by our technicians this year. Special needs-trained Child Passenger Safety technicians also provide much-needed education and services to our community. Working closely with the Newborn Intensive Care Unit, rehabilitation services and the orthopedic program at Wolfson Children’s Hospital, THE PLAYERS Center for Child Health provided 20 car seats on a temporary basis for special-needs patients this year. Using referrals from Healthy Start, the Ready, Set, Sleep classes have been very well attended. Additionally, in 2015, we expanded our reach with a pilot Spanish class with translated materials and a community Baby Shower offered to families in our local region. Participants in the program are taught important information about general baby safety, breastfeeding, safe sleep techniques and CPR to help keep their children safe and healthy. Our Ready, Set, Sleep program increased its total reach to 6,841,764 through programs, events, social media, local media outlets and distributed materials. Health and Wellness Community-Based Asthma Workshop Participants Community Asthma Partnership at Wolfson The Centers for Disease Control reports that asthma rates continue to rise in the United States. Asthma is the numberone reason children miss school and the leading cause of hospitalization for Children in Northeast Florida. The Community Asthma Partnership at Wolfson Children’s Hospital (CAP-W) offers programs and resources that teach parents, children, educators and health providers about asthma so they can successful manage the disease and break the cycle of repeated asthma attacks. The CAP-W team provides seven-day coverage for asthma education at Wolfson Children’s Hospital. Registered respiratory therapists visit asthmatic patients who have been admitted to the hospital and in the Children’s Emergency Center seven days a week. Educators provide important information on asthma symptoms, triggers and medications. The educators also encourage families to participate in one of two free monthly workshops held at THE PLAYERS Center for Child Health. Asthma educators provide continuous care coordination for Wolfson Children’s patients that meet National Institute of 379 240 53 2010 133 140 2012 2013 91 2011 2014 2015 Health High Risk Guidelines. Asthma patients with four or more ER visits, two hospital admissions within 12 months, or one Pediatric Intensive Care Unit admission lifetime are considered high-risk patients. Educators work with the patient’s family, social services, asthma specialists, primary care physicians, and school nurses to provide individualized assistance. In 2015, the CAP-W team implemented a High Risk Care Coordination Clinic day at Nemours Children’s Specialty Care. The team additionally developed an age-appropriate asthma program to provide education to middle and high school students, expanding to more children in the community. THE PLAYERS Center for Child Health has been able to expand its partnerships in the community through CAP-W collaboration with St. Johns County Head Start, Wolfson Children’s Hospital Social Services, Lunch and Learns for Southeast Georgia pediatric pulmonologists, and in-house services to the Baptist Beaches Emergency Department staff, Wolfson Children’s Hospital Respiratory Department, and Wolfson Children’s Hospital Emergency Center staff. A2 Asthma Action Our educators teach A2 Asthma Action, a six-session curriculum focused on asthma management and geared toward asthmatic children. It is designed for students 2nd through 5th grades in local elementary schools. For the third year in a row, the Center has also worked with the City of Jacksonville to offer a modified curriculum to summer camp participants. Easy Breathing® THE PLAYERS Center for Child Health’s asthma educators work closely with local pediatricians to facilitate the Easy Breathing® Program. The community-based asthma management program modifies the national asthma guidelines into a usable format for pediatricians and other health care providers. In 2015, 1,185 children were screened for asthma. Of those screened, 246 children were diagnosed with asthma. Easy Breathing® 4405 Surveyed Diagnosed 2725 2675 1830 483 156 2009 708 2010 944 2011 1300 629 2012 616 2013 279 2014 1185 246 2015 Advocacy Through participation in the Children’s Hospital Association and the Florida Association of Children’s Hospitals, THE PLAYERS Center for Child Health at Wolfson Children’s Hospital works to advocate for children’s healthcare issues. In June 2015, Wolfson Children’s Hospital patient Journey James and her family traveled to Washington, D.C., to participate in Speak Now for Kids Family Advocacy Days. The James family met with legislators to discuss important pediatric issues, including ACE Kids, which is focused on improving care for children with medical complexity enrolled in Medicaid. At the federal level, HR 2, the Medicare Access and CHIP Reauthorization Act of 2015 was enacted on April 16, extending the Children’s Health Insurance Program through 2017. Health Care Access With the belief that access to health care is one of the biggest predictors of health outcomes, THE PLAYERS Center for Child Health has a priority focus on enrolling children in health insurance. THE PLAYERS Center for Child Health continues to help identify and enroll families in Northeast Florida through outreach and events. In 2015, new partnerships have been instrumental in generating new referrals to our center. Partnerships with Jacksonville’s Center for Language and Culture, Wolfson Children’s Hospital and its NICUs, Wolfson Children’s Hospital and Nemours social workers, and CareSpot have helped us almost double our enrollments in Florida KidCare. THE PLAYERS Center for Child Health was pleased to receive funding through the Jacksonville Children’s Commission Cover Jax campaign, Florida Healthy Kids for a Back To School grant, and the University of South Florida for a Florida Covering Kids and Families grant. In 2015, THE PLAYERS Center for Child Health educators spoke to roughly 5,500 individuals about the importance of coverage. The outreach team enrolled 656 children in Florida KidCare or Medicaid, and assisted 57 families in coordination of their insurance enrollments. Additionally, THE PLAYERS Center for Child Health has become a service location for AmeriCorps. For the 20142015 year, we had our first AmeriCorps Member join our team and have already welcomed a new member for the 2015-2016 year.
