In response to an unprecedented shortage of medical staff, which jeopardized the very survival of the only pediatrics department in the entire Outaouais, le Copain was created in January 2001. To optimize the use of the expertise available from the limited number of pediatricians in the Outaouais and to assist in recruiting doctors and nurses, we organized the work of the active members of the pediatrics department around a coordinated regional team: Le Copain. The hierarchy of medical care is the guiding principle underlying the organization of the work of our solid team of pediatricians, now 11 active members. We have concentrated the specialized pediatric medical activities at Gatineau Hospital by creating an outpatient clinic in addition to the existing inpatient pediatric and neonatal units. In this clinic, the «pediatric internists» of Le Copain treat patients who often require multidisciplinary pediatric expertise, in addition to and in support of primary care. The primary goal of Le Copain is to meet the priority medical needs of their target clientele . Therefore, we believe that, in terms of equivalent quality of health care, people in the Outaouais benefit from receiving treatment in Quebec. The importance of the bio-psycho-social integration of health care is now a widely accepted concept. Page 3 The target clientele of Le Copain are: 1. Patients 0-17 years old in the Outaouais who suffer from disabilities or chronic illnesses that are sufficiently severe to make them vulnerable to the point where, for example, the slightest concurrent infection could threaten their safety. For this clientele, the pediatricians of Le Copain serve as attending physicians. 2. Patients 0-17 years old who have been referred by a doctor. For this clientele, the pediatricians of Le Copain act as consultants or co-attending consultants, as required. 3. Patients 0-17 years old from the Outaouais who require hospitalization under the care of pediatric specialists. Le Copain and inpatient pediatrics Patients in the 0-17 age bracket in the Outaouais whose condition requires pediatric expertise in an inpatient hospital setting are cared for by the team at Le Copain, whether for: •• treatment, •• investigations (with or without sedation), •• medical or diagnostic intervention, •• education and support for at home intravenous antibiotic therapy, •• intensive functional rehabilitation, •• hospitalization of severely disabled patients, •• treatment of medical complications resulting from eating disorders, •• hospital palliative medical care, •• medical stabilization prior to transfer to another hospital (university tertiary care centre, Centre hospitalier Pierre-Janet, palliative care centre, etc.) Page 4 Le Copain and neonatal pediatrics Newborns in the Outaouais whose condition requires level 2+ intensive care are treated at the neonatal unit. Generally speaking, Gatineau Hospital looks after premature births after 32 weeks. We also take sick newborns who were born elsewhere in our region. We provide a place for an early return for patients whose condition required level 3 care in a university setting, once the critical period on mechanical ventilation has passed. The pediatricians of Le Copain also provide prenatal consultations. Such consultations are essential to prepare for the birth of fetuses suffering from specific conditions or risk factors that require medical intervention within the first few hours of life. We strongly advise that mothers with at risk pregnancies be referred as early as possible to allow the pediatrician to complete the research required for the organization of these specialized birth plans. Le Copain and ongoing professional training The pediatricians of Le Copain are involved in an educational role of the CSSS Gatineau: •• Complete two-year cycle of courses and workshops covering aspects of general pediatrics, offered to residents of the UMF (family medicine unit) of the CSSS Gatineau. On request, presentations could be broadcast via teleconference to other CSSS in the Outaouais. •• Clinical rotations for residents in family medicine. •• Clinical rotations for McGill pediatric residents and optional rotations for pediatric residents from other faculties of medicine. •• Teaching or lectures on invitation by physicians and other professionals in the Outaouais. •• At regular intervals and on request, we offer a three-hour structured workshop on periodic examinations of children («Healthy ABCs» Page 5 from Université de Montréal) to small groups of doctors and nurses. •• At regular intervals, we offer a day of NRP provider certification or recertification (AHA/AAP/CPS). •• We periodically organize PALS certification days in Gatineau. •• We respond to requests from community health workers who call upon our expertise to improve the health care available to children in the Outaouais. •• General practitioners, specialists or residents. •• Other professionals in the health and social services system. •• Other community workers (police, teachers, etc.) Le Copain and outpatient consultations The ambulatory department of Le Copain is located on the 4th floor of the Gatineau Hospital. The pediatricians of Le Copain respond to requests for consultations from doctors, dealing with acute, sub-acute, chronic, prenatal or developmental health problems. The services offered are described in greater detail below. Le Copain and rural outreach consultations We offer rural outreach consultation services at the following institutions: •• CSSS Vallée-de-la-Gatineau (Maniwaki) •• CSSS du Pontiac (Shawville) •• CSSS Papineau (CLSC Petite-Nation, St-André-Avellin) •• Centre régional de réadaptation La RessourSe (La RessourSe regional rehabilitation centre) •• La RessourSe Service Point, Fort-Coulonge (CSSS du Pontiac) •• La RessourSe Service Point, Buckingham sector (CSSS Papineau) •• La RessourSe Service Point, St-André-Avellin (CSSS Papineau) Page 6 Pediatricians who are active membrers of Le Copain All pediatricians who are active members of Le Copain contribute to a rotation of 24/7 on-call duty and to hospital duty in both pediatrics and neonatology. Furthermore, they are all involved in ambulatory pediatrics, where they respond to requests for consultations from physicians in dealing with acute, sub-acute, chronic, prenatal or developmental health problems. As required, the pediatricians collaborate with: •• other Le Copain professionals (clinical nurse, social worker, nutritionist, physiotherapist, occupational therapist, psychologist, speech therapist, and audiologist), •• the institutions in the region (La RessourSe, Centre hospitalier Pierre-Janet, Les Centres jeunesse de l’Outaouais, etc.) and •• visiting subspecialists from Le Copain (in Gatineau or by telemedicine).
