AFCCA CONVENTION INFORMATION:
REGISTRATION FEES INFORMATION:
A basic registration fee must be paid by all attendees, but will cover CEUs, lunch, breakfast and breaks. Golf, Bunco Night and the
Thursday Banquet Dinner are a separate price per person. See the registration forms for the costs for both member and nonmembers.

EXPO HALL • JUNE 3 & 4
The Supplier Exhibit Room will be set up for time and space to visit with suppliers. You can earn 1 CEU Hour each day that you
visit.
CREMATION OPERATOR TRAINING SEMINAR - FRIDAY, JUNE 5
A separate seminar will be held within the structure of the convention. “Cremation Operator Certification Seminar” will be offered.
Anyone attending the seminar must pay a separate fee. Students may attend but a registration fee is required.
HOTEL INFORMATION
The Carefree Resort & Conference Center is a great place to bring the family to enjoy the pool and other North Valley sites.
Rates are $89 plus tax for the Resort Room.
Be sure to identify your reservation as being part of the Arizona Funeral, Cemetery & Cremation Association Convention room
block to receive the convention rate.
You may make your reservation by calling the main telephone number at 480-488-5300 or online at bit.ly/AFCCA-Rooms.
Reservations must be made by May 19, 2015 in order to reserve at this special rate. Credit card guarantee is required.
CONVENTION ATTIRE
Casual attire or business casual is encouraged for all sessions and events.
FOOD
Continental breakfasts, breaks and lunch are included for attendees only. Lunch on Wednesday and Thursday will be served in the
Exhibit area. Coffee, tea, lemonade and light snacks will be offered for breaks.
FOUNDATION FOR CHILDREN SILENT AUCTION • JUNE 3 & 4 • ENDS AT 1:30 PM JUNE 4
A silent auction will be set up through the entire convention for individuals to bid on baskets and other prizes. All proceeds benefit
the AFCCA Foundation for Children. If you would like to donate an item for this auction, please see form included in packet. All
prizes must be received by Tuesday afternoon.

FOUNDATION FOR CHILDREN BUNCO NIGHT • TUESDAY, JUNE 2 • 6 – 9PM
Once again, the Foundation for Children, the philanthropic branch of the AFCCA will hold a fund raising event on Tuesday evening.
A Bunco Tournament will be the highlight of the evening. This is a fun event great for networking and a chance to win a cash prize!
SUPPLIER NIGHT RECEPTION • WEDNESDAY, JUNE 3 • 4 – 6PM
The Supplier Night Reception will be a fun party atmosphere in which you can enjoy visiting with the suppliers in attendance.
Everyone is welcome. Heavy hor’s douvres and a cash bar will be available.
AWARDS BANQUET DINNER • THURSDAY, JUNE 4 • 6 – 9 PM
Awards will be given during the Dinner. Cocktail hour begins at 6:00pm and 7:00pm for the buffet dinner. This event requires extra
tickets for all. Prices are listed on the registration page. The Friday Night Dinner evening is “Denim & Diamonds” themed, so wear
you best blue jeans and dress it up with your favorite bling!
ALCOHOLIC BEVERAGES
Please be advised that attendees must be 21 years of age to order alcoholic beverages. All bars will be on a cash basis unless drink
tickets are provided.
NAME BADGES
Name badges will be issued to registrants and guests. There will be a $25 per badge requirement fee if lost. Badges are nontransferable. Badges will be required for the Exhibit Hall entrance.
CONVENTION ATTENDANCE POLICY:
Please plan to take a seat in the presentations well in advance, as doors close promptly at the scheduled start time to ensure
compliance with state board continuing education requirements. Continuing education forms will be punched at the end of each
seminar.
CANCELLATION POLICY:
Registrations cancelled by May 15 will be refunded in full less $25 processing fee. Cancellations made May 16-May 30, 2015 will be
refunded 50%. No refunds June 1, 2015 or later and no refunds for no shows.
