BENLEE PRINCIPLES SAFETY Quality EFFICIENCY Growth Four Key Principles-SAFETY #1 Safety Roll off Trailers & Trucks if operated correctly are very SAFE. If incorrect, they are DANGEROUS! Many accidents happen due to people “know better” and get complacent #1 Cause of Accidents! Sadly “Preaching to the choir” Seriously-you have to be smart to work in a Scrap Yard -It is a dangerous place In past 18 months there have been a number of roll off deaths/accidents Three deaths in 2013/2014 that we know of -All were operator error There are a few major types of accidents -They are preventable!!! BENLEE trailers try to out think the user to make him/her safe Training, Policies & Enforcement are Key Case Study #1-Multiple deaths and known accidents Hitting Overhead Power lines -Electrocution We know of one recent death & one recent close call & “Sparky”-1999 Cause-Drivers did not Look Up! -Dark surroundings-early AM What to do -Look up! Not that tough to do What to do cont’ -Install large “LOOK UP” sign at controls -Install lights on tarp system Face lights up to see wires, etc. -Lights and sign can be purchased at low cost from BENLEE -Simple to install on older units -Train team to understand issue -BENLEE – Sign & lights standard An Absolutely Preventable Accident Case Study #2-Death and multiple known accidents Hitting overhead bridges, rail crossings, parking lot trusses Cause-Drivers running with hoist up -Crazy but true What to do -Ensure hoist up alarms operating What to do cont’ -“Most” roll off trucks that come into BENLEE service have hoist up alarms disconnected-literally kills people Drivers short them out/cut wires -Most trailers (until now) did not have alarms -Install hoist up flasher (“alarm”) directed at driver’s mirror Can be purchased at low cost Simple to install on older units What to do cont’ -BENLEE trailers have hoist up Flashers (“alarms”) as standard Ensure alarms are engaged An Absolutely Preventable Accident Case Study #3-Death Raised hoist and went behind box, which slid down Cause-Box was not attached to cable What to do -Hoist never to be raised if box is detached -Never get behind a box, even if cable attached Cable could snap! Never get behind box when hoist up! An Absolutely Preventable Accident Other Safety Issues and Solutions! 1) Tires coming off while driving Cause-Loose nuts -Solution-Regular torque checks and install Wheel Checks Wheel Checks are simple, low cost and effective -Identifies loose lug nut. Spins out of place Standard on all BENLEE units Standard at WM & Omni 2)Flat tires shredding causing accidents as tire disintegrates -Solution-Regular tire check and install Auto Inflate system Auto Inflate-Keeps tires inflated & activates warning light if leak -Also saves tires and fuel THE ONLY OPTION RECOMMENDED 3)Sheaves freeze up and snaps cable -Box falls -Solution-GREASE, GREASE, GREASE Ensure replacement sheaves have grease grooves in bushings Trailer will last and be SAFE 25+ years if Greased 4)Trailer tilt over -Operate on uneven ground &/or Jackknifed -Solution-Pretty simple-Do not do it! CRAZY Thing to Do! 5)Roll off box door hits driver as opens -Solution-Stand as clear as possible Consider using door “tool” wear hardhat and safety glasses Pressure on Door A Major Unknown at times 6)Hit person as backing up truck -Solution-Ensure back up alarms working Consider installing electrical or mechanical alarm Consider installing back up camera system-Wired or new Wireless Camera systems may soon be an option 7)Cable snaps-dropping box and/or hitting person -Solution-Inspect cable daily for wear And GREASE sheaves wear hardhat and safety glasses Cables are a wear Item Inspection is Critical 8)Material flying out of box while driving -We get calls weekly from around U.S., due to our name on mud flaps -Solution-Tarp box when in doubt Never drive with material sticking out of box Many States have Tarping Laws 9)Employee falls while climbing to hand tarp load -Solution-Install Automatic or Semi Automatic Tarp system Climbing is dangerous Can use on roll offs, luggers, open tops & dumps “Window Shade” Systems Require some climbing, but are low cost 10)Hoist falls during maintenance -Solution-ensure use of safety stands, body props Consider using “Dump Locks” -Stand-between frame and hoist Chain from Ceiling Hoist Can Snap-give Way 11)Operator hit by high pressure hot hydraulic fluidhose failure -Solution-Inspect hoses daily Use 3,000lb.-4,000lb. hoses 2,000 lb. hoses-common standard -Problem-Old products have valves on fender-Be Careful. -Install shield in front of valve Quite a Few Cases of this Failure - we moved Valve 12) Leaf Spring failure-U bolts come loose -Solution-Inspect daily. Easy to Inspect 13)Accidents due to drivers with bad driving record, on payroll -Solution-Personally ensure driving history is checked Almost Crazy, BUT REAL ISSUE! 14) Driver gets hurt climbing in and Out of Tractor (Power Unit)-If Trailer, Driver must go in and out of unit. -Solution - Install ‘Dead Man’ button eliminates second trip and/or Run away unit Can be Installed in most Trailers 15) Ensure accurate Pre & Post trip reports are being done Solution-Audit yourself-get under truck! -Dangerous & DOT issues missed?! Do Not Assume Being Done Correctly SUMMARY -Roll off Trailers Can Be SAFE OR CAN KILL. -Follow Safety rules -Inspect and Maintain -“Experienced people” can be Dangerous OR VERY SAFE! -Install-Signs, Alarms, Lights, Button, Cameras, Tarp System, Air Inflate INVEST IN SAFETY
