TRIP DESTINATION: Gig Harbor DAY/DATE/TIME: ANNOUNCE DATE SEATTLE SINGLES YACHT CLUB Version 4.2, March 2015 SIGN-UP SHEET SKIPPER: PHONE: BOAT NAME: LOCATION: F/C SIGNATURE TRIP CREW EMAIL: BOAT MAKE: COMMENTS: THE UNDERSIGNED, who is a Member of Seattle Singles Yacht Club, a Washington nonprofit corporation (“SSYC”), participating as a guest on a boat skippered by a Member of SSYC, agrees to hold its Members, Officers and Directors, including, but not limited to Club Skippers, harmless from any and all liability, whether known or unknown, which may arise out of Skipper’s negligent acts while acting as a Skipper on an outing sanctioned by SSYC. The undersigned further agrees to indemnify SSYC from any and all claims and causes of action which Member or third parties may have arising from the undersigned being a Member of SSYC and participating as a guest on a boat skippered by a Member of SSYC. The undersigned acknowledge that SSYC and its skippers do not allow any firearms or illegal drugs on any boat during any boating activity. The terms of this Indemnity shall also serve as an agreement for the undersigned heirs, personal representatives and for all members of the undersigned's family, including any minors. THE UNDERSIGNED, acknowledges that photographs taken during SSYC events may be placed on the SSYC photograph bulletin board, shown on the SSYC internet website and printed in the SSYC newsletter. The undersigned may reserve the right to request in writing for the removal of any photographs. In part, this Indemnity is provided as a recognition that SSYC is a non-profit corporation and without protection of its assets and its leaders, SSYC would not be able to offer the boating activities. The undersigned recognizes that boating activities may involve certain dangers, including unforeseen forces of nature, and the actions of participants and other parties. Member personally assumes all risks in connection with these activities.
