Community Health Resources (CHR) 2013 Annual & Quality of Care Report A Year of Spreading Hope CHR is the most comprehensive behavioral health provider in Connecticut, committed to helping people find Real Hope for Real Life through a wide range of high quality, mental health, substance use and supportive housing services for children, adults and families. Our Mission is to provide treatment, support, rehabilitation, prevention and education through a well-trained, dedicated staff of professionals committed to recovery. These efforts are completed in an environment which utilizes resources to ensure that care is delivered to those most in need, utilizing up-to-date service technologies with effective outcomes. Our Vision is to be the first choice for high quality mental health care, addiction services and supportive housing in Connecticut. Known for clinical excellence, exceptional customer service and a culture of flexibility, CHR will be a valued partner in the healthcare system. Our goal for every individual and family member served by CHR is to restore hope, provide care and celebrate progress. CHR’s history dates back to the 1960s, and since its creation it has become a well-respected service provider and trusted community resource. Today, we employ more than 650 experienced staff members who provided services to over 16,000 individuals in the past year. We offer effective, accessible, and affordable behavioral healthcare through our 80 programs and 32 locations. CHR is also one of the only community-based behavioral health providers to offer same-day access to care. 1 CHR’s behavioral health programs have spread hope to more than 16,000 adults, children and families this past year. Our commitment to excellence is demonstrated in the positive outcomes of our clients and the improved health of our communities. Fifty years ago President Kennedy signed the Community Mental Health Act, establishing a bold new approach to mental health. He envisioned a community system focused on prevention, treatment, education and recovery. Voices of Hope “ “ I am so happy that I found CHR’s Community Support for Families program. When I started I was standoffish and distant, but the staff at CHR treated me like someone they cared about and that really helped me open up and gave me more confidence as a mother and as a person. I was losing hope and they helped me turn my life around. ” I was overwhelmed by the generosity and support I received from CHR. Everyone I have worked with has been very kind and empathetic and I can’t say enough about the great job they are doing. Sometimes life throws you a curveball and you need someone to help you and tell you that everything is OK. That is what CHR has done for me. ” 4 Child & Family Services We know that life brings real challenges for children and their families. At CHR, we offer real hope. Every day, we see positive outcomes unfold. Children discover ways to cope with difficult issues, strengthen relationships and succeed in school. Families learn how to better communicate and discover that new beginnings are always within reach. No two children or families are the same. That is why CHR’s Child and Family Services offers a full range of treatment options from office-based interventions to in-home programs. Our clinical services include, Child and Family Outpatient Services, Emergency Mobile Psychiatric Services, Multi-Dimensional Family Therapy, Adolescent Substance Abuse Treatment, Integrated Family Violence Services, Foster Care, and Intensive In-Home Child and Adolescent Psychiatric Services. In the past year, these services have produced outstanding results, including: • 94% of families expressed overall satisfaction with the services their children received through the CHR Child and Family Outpatient Clinics. • 85% of families felt their children got the help they needed. • 84% of families stated their children’s behavioral symptoms had stabilized or improved through CHR’s services. 5 In addition to clinical treatment, CHR’s Child and Family Division also provides foster care programs, residential care, community supports for families, and many other home and community-based services. These services also saw numerous positive outcomes in 2013 including: • 95% of caregivers who were served by the Community Support for Families program felt that as a result of the program they were better able to manage their families and care for their children. • 92% of the children served by CHR’s Reconnecting Families program were reunited with their families after their parents received services through the program. • 80% of the children and youth served had no psychiatric emergency visits or hospitalizations while receiving services, and 95% were living at home by the time they were discharged. • The number of children placed in foster homes through CHR’s Therapeutic Foster Care program more than doubled. • Children and adolescents enrolled in CHR’s in-home programs saw a 53% reduction in the number of days spent in inpatient psychiatric care or hospitals. Child and Family Services Leadership Team 6 Adult Mental Health & Substance Use Services Opportunities for Hope and Recovery That Last 97% of the adults served by CHR said that our services helped them feel more in control of their lives. “ “ Voices of Hope I am so grateful to have the staff at CHR in my life. If it weren’t for CHR, my future would not have been so bright! ” 7 1 in 4 adults in the U.S. will struggle with mental illness or substance use at some point in their lives. At CHR, we know that recovery is possible because we see positive, lasting changes made every day. CHR offers the most comprehensive mental health services available in Connecticut, including outpatient treatment, partial hospital/intensive outpatient programs, crisis intervention and emergency services, community-based outreach services, supportive housing, supported employment services and residential programs. Working together, we help each person define his or her unique needs, identify goals, define recovery and create a personalized treatment plan. Of the more than 7, 000 individuals treated by CHR's Adult Behavioral Health Services in the past year: • 95% expressed satisfaction with the services provided and 93% stated they would recommend the agency to friends and families. • 95% said that they found CHR staff to be supportive of their efforts to recover and that our staff had been successful in helping them secure information to take charge of their illnesses. • 94% felt more capable of dealing with daily problems. CHR also has a wide range of treatment options for those looking to break the cycle of dependency, make healthier choices and overcome substance use. Our Substance Use Services include, outpatient services, residential programs, housing and support services and intervention and pre-trial programs. Individuals who turned to CHR for Substance Use Services saw many positive outcomes in 2013. Highlights of their success include: • 90% of clients in CHR’s New Life Center, a residential substance use treatment program for expectant mothers and women with children also successfully completed treatment. • 89% of clients in CHR’s Milestone program, a 30 day intensive residential treatment facility designed to accommodate women with a history of substance use issues, were not re-admitted into the program or similar levels of care following treatment. • 87% of clients in CHR’s Pathways program, an accredited opiate treatment program, successfully completed treatment. Served 20% more clients than last year. Created the Center of Excellence for the Treatment of Women / Women and Children in Putnam. Granted bonding dollars from the State of CT to develop a Center for Health and Wellness in Manchester. Developed partnerships with primary healthcare providers to integrate care. Expanded Same Day Access to services. Opened offices in Hartford and Middletown for child and families services. ASSESSMENT CENTER CHR provides support 24 hours a day, 365 days a year and offers simple access to treatment. It begins with a single call to our Assessment Center. Our qualified staff of professionals, including licensed and Master’s level clinicians, learns about a person’s needs and then partners with them to select the best treatment options. We answer questions, discuss program eligibility requirements, process insurance information and schedule an initial appointment.
