See Where We Are. — See How You Can Join Us. 2007 Report to the Community Dear Friends of Fraser, It is an exciting and critical time at Fraser. In 2007, I marked my 18th year with the organization. I have enjoyed watching Fraser grow and thrive and celebrating the impact we have made with your help. Through this annual report, I invite you to See Where We Are and See How You Can Join Us. 2007 brought significant growth to the organization. Fraser now employs some 600 staff members and delivers more than 35 programs through six divisions. Through critical services and in partnership with clients, staff and donors, Fraser accomplishes its mission of making a meaningful and lasting difference in the lives of children and adults with special needs. The mission of Fraser is to make a meaningful and lasting difference in the lives of children, adults and families with special needs. We accomplish this by providing education, healthcare and housing services. Along with growth come challenges. A skyrocketing increase in autism spectrum disorders has driven requests for Fraser programs higher than ever. Our current facilities are bursting at the seams and Fraser struggles every day to meet the growing need. Few days go by that I don’t personally receive a call from a desperate parent looking for answers and pleading for help. While we have made great strides (see accomplishments page 17) we must find creative ways to serve more families who need our help. In the spirit of our founder, Louise Whitbeck Fraser, we approach each day grateful for the work that we accomplish, yet yearning to do more. I thank each and every one of you for being a friend of Fraser and doing your part to save these families. Your volunteer and financial support have been critical. Fraser is a Minnesota nonprofit organization with a nationally renowned reputation for serving children of all abilities and adults who have special needs. Fraser provides a broad continuum of direct services, information and support in the areas of education, healthcare and housing. Autism is the fastest-growing developmental disability in the U.S. and is now more common than juvenile diabetes, childhood cancer and AIDS combined. The dramatic rise in autism spectrum disorders means that thousands of Minnesota families need direct-care and support services. As the longest-running, largest provider of autism services in Minnesota, Fraser provides diagnosis, treatment and support services to hundreds of children and adults with autism spectrum disorders and their families. Fraser uses a multidisciplinary approach and partners with each family to determine which services are the best fit. S ome of the services provided by Fraser include: early childhood education; ccupational, physical, speech, music and o feeding therapies; individual, family and group therapies; c are coordination and case management services; parent education, training and support; day treatment; respite care; diagnostic evaluations and referrals; PCA services; residential services; individual and group skills training; consultation and training to the community; and more! 3 4 Four Strategies We Have To Help Multidisciplinary Approach Because autism is a spectrum disorder, every individual is unique in his/her needs. A multidisciplinary approach considers what is best for each individual and designs services around individual strengths and weaknesses. 1 Lifelong support Fraser provides services to individuals with autism during early childhood, adolescence and adulthood. Our services change as an individual’s needs change. In 2007, Fraser Child & Family Center received an average of 22 calls per day regarding autism concerns (one-half of all calls received) and performed 45 autism evaluations each month. One-third of the clients currently receiving Fraser Residential Services and nearly two-thirds of the clients receiving Fraser services in their own homes have autism spectrum disorders. Streamlined services Through the Fraser ASD Model, children and families receive help sooner, more families can be served, parents are better informed and family stress is reduced. 2 3 4 Parental involvement Fraser believes parents are the experts on their children. We value parental input and work closely with parents when designing treatment programs. The GoldenTwins Autism Services “Jackson and Hunter now have a ‘voice’ to express themselves.” Jackson and Hunter were born happy, healthy, beautiful little boys. They were developing typically and hitting their milestones for the first year and a half. They loved going to the park, playing with toys, reading books and watching their favorite videos (doing their “happy dance” the whole way through!) They communicated well through gestures and sign language, pointing at things to communicate their needs. At 16 months the boys were only babbling and imitating a few sounds. 5 6 (What does a cow say? Moo! Moo! Sheep? Baa! Baa!) We asked our pediatrician if we should be concerned and were reassured that their speech was normal and we’d see more language develop by their 2-year check-up. The exact opposite was true! By their second birthday the boys had fallen completely silent. Their speech had regressed and in only a few months we saw regression in many other areas as well. They were no longer communicating by pointing, waving or blowing kisses. Going to their favorite places suddenly became stressful and caused anxiety. Both boys became increasingly dependent on videos and wanted less and less to play with toys. They would throw themselves to the floor and completely melt down with no concern for even their own safety! When we tried to comfort or even touch them, they became more upset. Once again we sought our pediatrician’s guidance. He suggested calling our school district for a speech assessment but was not overly concerned. The school district came to our home for a preassessment and mentioned concerns of autism. We took the advice of a friend and called Fraser the next day to seek out a medical evaluation. We thought the boys were just being extremely stubborn. We didn’t understand how one day they could have a skill mastered and the next couldn’t do it at all. We thought they were choosing to not blow kisses or to only want to watch television. Fraser explained what was happening to our boys. Jackson was diagnosed with autism and Hunter with PDD-NOS (Pervasive Development Disorder- not otherwise specified). The diagnoses brought mixed feelings. We were relieved to have answers, but scared of what was to come. We were angry that our boys would struggle and sad to have lost the typical little boys that we once knew. Rachel Gardner, an autism expert at Fraser, helped us sort through these emotions. She answered questions objectively and honestly and helped us figure out the best options for our boys. Rachel referred us to Fraser day treatment, skills training and PCA programs. When Jackson and Hunter first started day treatment, it was pretty rough! For the first few months the boys were so anxious about being someplace new that they weren’t able to start the day in their therapy classroom. The staff at Fraser was unbelievable! They helped the boys learn coping strategies to get them through the tough parts of their day. It took patience and persistence, but eventually the boys were willing to start in their classroom. Now when Jackson and Hunter go to Fraser they are excited to start their day! The one-on-one staff genuinely care about the boys and their progress. They have taken time to get to know the boys as individuals and work with each to develop core skills. The day treatment staff has exceeded our expectations! The boys have come a long way and will come even further with Fraser’s support! 7 8 We’ve also used the Fraser skills training program to help at home. Jackson and Hunter are nonverbal and unable to communicate with speech. Our skills trainer helped set up picture cards throughout our house. The boys tell us what they need by showing a corresponding picture. It has helped immensely with their frustration, and ours, and gives them a sense of control. Jackson and Hunter now have a “voice” to express themselves. The Fraser personal care assistant program has been helpful as well. Our PCA, Emily, worked closely with our boys in day treatment. She has been a consistent part of their lives since our first day at Fraser. She is a familiar face and so in tune to what each of them needs. What some may see as stubborn or strong-willed, Emily sees as determined and persistent. She always sees the positives and has truly been a blessing! Fraser has given us balance, something we depend on day after day. The people at Fraser have continually gone out of their way to support our family. They have a true concern for us all, but most importantly for Jackson and Hunter. They have been there to guide us from day one and we are confident they will continue to do so as long as we need them. In the unknown world of autism, you need a team that you can trust. We have that team at Fraser! The Golden twins came to Fraser when they were 2. Both were diagnosed with a pervasive developmental disorder and enrolled in the autism day treatment toddler program. They had difficulty tolerating transitions, communicating needs and playing with toys. While they continue to have challenges, both have made significant progress in transitioning between activities and using pictures to communicate. Jackson and Hunter are delightful little boys with many strengths, including an increased awareness of others and a growing motivation to engage with adults. Their parents have provided never-ending support and are excellent advocates for their needs. By Nick and Julie Golden Rachel Gardner, Licensed Psychological Practitioner, Fraser Autism Program I met Jackson and Hunter on their first day when I was a staff member in their day treatment classroom. Now, I’m a Personal Care Attendant (PCA) in their home. It is truly amazing to see how far their skills have come, especially in the area of communication. At home the boys work on using sign language, pictures and gestures to communicate. We integrate these skills into everyday activities such as choosing between grapes and bananas, requesting bounces on the therapy trampoline, or asking for a favorite DVD. Working in their home has given me new perspective on their personalities. Jackson is a laid-back, affectionate daredevil, while Hunter is determined, smart and loves to laugh. They never fail to make me smile! Emily Springer, PCA, Fraser Home & Community Supports Stephanie Abel Residential Services Fraser Residential Services provide caring and safe housing options for children and adults with special needs in single-family homes and apartments. Each year, Fraser provides housing to more than 200 people like Stephanie who need various levels of support. With Fraser Residential Services, individuals can achieve the highest level of independence possible while also being an important part of the community. “Living independently means a lot more to Stephanie than just living on her own.” Forty-two-year-old Stephanie Abel knows what it’s like to be part of a family – a big family. In 1991, she moved from California, leaving behind her parents and 10 siblings. With support from her family, Stephanie has found her independence in Minnesota. Fraser is a part of her life, and not just as a living space. One of Stephanie’s many ambitions is to work with kids. She gets to see this dream realized when she volunteers in the classroom at Fraser School. In 1993, she moved to Lyndale Court, an independent living apartment, and became part of a family again – the Fraser family. Living independently means a lot more to Stephanie than just living on her own. To Stephanie, living means reaching her goals. One of these goals is remaining active. Stephanie bikes whenever and wherever she can. She bikes for fun and to her job at Target where she restocks the shelves at night. She bikes for good causes, too. This year, Stephanie will participate in four charity bike rides, three of which span 150, 300 and even 400 miles. She has also run the Twin Cities Marathon twice and plans to run again this October. The affordable rent, comfortable living space and spacious back yard and patio make the apartment complex a nice place to live. But it is the friends she has made that she cherishes. A Fraser activities director, Holly, organizes events for the residents, like game night, bingo night and even computer education classes. Stephanie enjoys these activities with friends, some of which are the best she’s ever had. 9 10 She has pushed herself to extremes like these for the past several years, participating in multiple bike or walk events for different causes. “I don’t give up easily on life,” said Stephanie. “Sometimes life challenges me in many ways. Having a speech impediment myself, I can relate to people in the same boat at times.” Stephanie plays on a Minnesota Special Hockey League team and belongs to two Special Olympics teams: the Blues basketball and alpine skiing teams. “My motto is just go for it and don’t give up until you have reached your goal.” “Fraser has given him the tools to go to school.” Davion Mitchell Currie Preschool Mental Health Services 11 12 When Davion was 4, I had concerns about his behavior. We were going through a lot of changes. We moved three times in one year and the people in his life kept changing. His dad was incarcerated. His little brother was born. All of these things made his behavior worse. People kept saying he had ADHD. The public schools did an assessment and didn’t find anything. Then I got the Fraser number from his childcare. I called and talked to Jen who met with me first and then Davion. She diagnosed that he had a hard time coping with change. Davion was always an active kid, but he became louder. He would wake up in the night crying. He jumped off things and ran around like crazy. At the store he would run and hide from me. I had to tell him four or five times to do something. We enrolled Davion in the Fraser program provided through Head Start. Jen worked with him on expressing his feelings and listening. He learned how to use safety breaks when he needed to calm down. A few months after starting at Fraser he started getting better. He is still active but now he listens more. I tell him once not to do something. He gets rewards instead of punishment. Without Fraser, I would still be trying to find help. It would have taken me a lot longer to figure out what was going on. Jen has helped me understand what he’s going through. When I have a question, she makes time to break it down. I went to Fraser and peeked in one day. The kids were sitting around in a circle and Davion was focused and paying attention. I was like, “Wow!” I couldn’t believe it. Fraser helps Davion relate to other people. He helps with his younger brother and he has come so far that sometimes he even leads the other kids at Fraser. Davion will be starting kindergarten in the fall. Fraser has given him the tools to go to public school. I want to find the right school and teachers that will work with him. My advice to other parents is to be patient and don’t give up. Try lots of things, because every child is different. I would recommend Fraser to anyone having problems with their child. I’m so thankful. Life is good right now. It’s less stressful. I’m definitely not going to give up. By Camille Mitchell The partnership between Fraser and Community Action Programs of Ramsey and Washington Counties Head Start serves families whose children are having significant emotional and behavioral difficulties. Davion’s difficulties are common: negative attention seeking, frequent crying, difficulty following directions and aggression. He was enrolled in the program five days a week, three hours each day. Davion’s mom and I work together to provide consistency across environments and share what strategies seem to work or to be ineffective. With mom’s support, Davion’s behavior improved at home, daycare and in our program. When Davion starts kindergarten, he can use the strategies he has learned and build on his success. Davion is an intelligent, engaging boy with a bright future. Jen Sommer, Clinical Site Supervisor, Fraser/Head Start The Nolans In-Home Support Services Fraser Home & Community Supports for about three years. Anna, our respite care provider, comes to our house one weeknight and every Saturday morning. She is wonderful! She plays games with Jarrett, reads to him and helps with skills he is learning. “ Then we realized that Jarrett wasn’t hitting his developmental milestones.” Jarrett was an easy baby. Since he was our first, we didn’t have anything to compare, but he was smiley and happy and life was good. Our friend had a baby the same age as Jarrett. When we got together, we began to notice some differences. Jarrett wasn’t doing the things the other baby was doing. He wasn’t pulling himself up, playing with toys, beginning to walk. At first we thought it was natural differences from one child to the next. Then we realized that Jarrett wasn’t hitting his developmental milestones. Instead of playing with toys, he would spin a top over and over again. He was obsessed with ceiling fans and anything that spun. We needed professional help. We went to a number of places, but none would commit to a diagnosis. 13 14 When he was 2, we went to Fraser and Jarrett was diagnosed with autism. Fraser staff was educated and knew what to look for. They knew what tests to do and which questions to ask. The therapists didn’t miss a thing, it was like they had known Jarrett for years. We learned that his obsession with spinning, a self-stimulatory behavior, was common with autism. He also lacked basic speech and social skills, had challenges eating and low muscle tone. More recently, Fraser diagnosed that Jarrett also has obsessive compulsive disorder. Over the years, we have gathered a team to help Jarrett. Our team includes the school district, a social worker, various therapists and Fraser. We have been using respite services through Respite care gives us a break and allows us to get things accomplished (like balancing the checkbook, grocery shopping, getting some much-needed sleep for dad who works nights). It also gives us time to focus on our daughter Jenna, who is 6. So much time is focused on Jarrett’s needs, but Jenna needs attention too. Having Anna at the house allows us to take Jenna to her favorite parks or restaurants that Jarrett just can’t tolerate. If Anna is not available, Meera comes. All of the respite providers have been exceptional. They are not here just to do a job. They want to be here with Jarrett and to make a personal connection. We can’t use a typical babysitter and don’t have family nearby, so respite care is crucial. We’ve been impressed that Fraser goes the extra mile to screen and get the right person. (Caring for a child with disabilities is not something just anyone can do.) We are so thankful for our team. To us, there is no shame in asking for help. We have to think about our child and do everything to get him all the resources available. Jarrett is now a happy third grader. He enjoys reading Peanuts books, playing games and drawing. Our hope is that Jarrett continues to make progress. We know that it won’t be at the same rate as a typical child, but any progress is good. We have more hope now than ever. By Jim and Sandy Nolan I’ve worked with the Nolan family for more than a year now. During that time, I’ve witnessed Jarrett grow and learn and accomplish so many new things. Things like perfect spelling tests and mastering riding his bike. I knew working with children with special needs was what I wanted to do and working with the Nolans has confirmed it. They made me feel so comfortable becoming part of their team. My job brings me much joy. I feel so lucky to be a part of Jarrett’s (and Jim, Sandy and Jenna’s) life. Anna Brostrom Personal Care Assistant, Fraser Home & Community Supports “Her teacher at Fraser was the kind of teacher that changes children’s lives.” We learned about Fraser School and scheduled a tour. We quickly knew that this was the place for Maddie. She had just turned 4 when she began attending preschool. I would come a few minutes early to peek in on what they were doing. Without fail, the teachers were on the floor, actively engaging Maddie and the other children. Her first teacher at Fraser, Elizabeth, was the kind of teacher that changes children’s lives. We have also been impressed with Fraser Rehabilitation Services. Maddie gets physical, occupational, speech and music therapies. Her therapists Kay, Crystal and both Andreas are phenomenal. I learn something every time I talk to them. It’s a real gift when you can learn, take it home and continue to work there. Christi, her special education teacher, discovered that Maddie does much better paying attention when some physical movement is included. Now she begins by climbing the rock wall in the physical therapy room. She loves it and it helps her tune in to other tasks. Every two months, we get a written report on Maddie’s progress. After just six months at Fraser, she had already met 55 percent of her goals. She was talking, running and playing – being a kid! Maddie was doing so well that last summer she enrolled in the Fraser School Ready 4K program. Maddie Lewis Maddie came to Fraser a quiet flower. Thanks to the attention and nurturing of Fraser staff, she has blossomed. What we want for Maddie is to reach her potential and to know that she is okay being who she is. Her hill is a lot harder to climb than the average kid’s. But that’s alright. At Fraser, she is learning to soar! Education & Rehabilitation Services By Kelly and Paul Lewis Our first child, Madeline Rose, was born seemingly healthy. At 11 months, Maddie had her first seizure. While in her high chair, her head dropped for just a second. I wasn’t even sure I saw it. Other times, I would be holding her and she would jerk. We knew something was wrong but for several months there was no answer. I felt like we were on a tunnel slide, always wondering when we’d find the light at the end. By this time, Maddie was having 20-30 seizures each day. At 14 months, a pediatric neurologist diagnosed Maddie with generalized epilepsy. Each child 15 16 with epilepsy is different and it’s often trial and error with drugs. As a child’s brain grows, the seizures change. I felt like we were always one step ahead of a rolling snowball. Thankfully, Maddie’s condition stabilized and we could focus on her development. Maddie’s greatest delay was in speech and her least delay was in gross motor skills. The school district sent a special education teacher to our home. We also took her to speech, occupational and physical therapies. This seemed to go well at first, but soon we decided that there was a better match for her needs. Maddie has grown so much in speech-language therapy over the last year. It has been exciting to watch her discover the power of her words. Maddie can now ask for help, make requests and share information with her family, teachers and friends. She has learned to listen and talk about stories. She has developed social language skills that help her engage with her peers. As Maddie has learned to listen and express herself, her confidence and enjoyment for school have increased. Maddie’s family and teachers have worked to offer a variety of supports including adding sensory breaks, pictures and opportunities for practice. Andrea Nelson Fraser Speech-Language Pathologist Maddie came to Fraser School as a quiet observer in a busy classroom of 4 and 5-year-olds. It’s exciting to see the growth that has taken place – from needing extra assistance and encouragement to becoming a take charge leader. Maddie has been blessed with parents who go the extra mile. They researched techniques that would help her grow and learn; made sure the staff was aware of her goals; and supported efforts to achieve these milestones. Over a year has passed since Maddie’s first days at Fraser. That time has been used well by this strong little girl. It is amazing to see what can be achieved when you combine a supportive family, a special child and a team of people that know great things are ahead. Elizabeth Eng Teacher, Fraser School What We Accomplished Fraser measures success by the impact our efforts have on the children, adults and families we serve and their communities. Here are some of our accomplishments from 2007. Made a significant and lasting difference in people’s lives. Of the families responding to our client/consumer survey: • 91% said that Fraser programs met or exceeded their expectations; • 96% said that Fraser services made a positive difference in their family member’s skills or behaviors; • 97% felt that their family benefited from being connected to Fraser services; • 97% reported that they would recommend Fraser services. Provided critical interventions to more children by adding another psychologist to provide mental health evaluations and restructuring treatment space at Fraser Child & Family Center. Provided help to children who have autism as early as possible by formalizing and offering services to children younger than 24 months. Helped children gain the skills necessary to enter kindergarten by adding a new literacy, math and science curriculum to the Fraser School Ready 4K program. 1 1 17 18 22 Improved Fraser Rehabilitation Services for children by: • renovating occupational and physical therapy treatment space; • enhancing therapy equipment to provide state-of-the-art sensory integration services; p roviding tailored training for staff. Provided information to nearly 1,500 parents and professionals through The Fraser Institute seminars. Reached thousands more by publishing two books and a pediatric developmental brochure. Helped families receive specialized child care by providing scholarships to the Fraser STAR Program. This important program supports children with special needs who attend Fraser Academy, a public elementary charter school founded and sponsored by Fraser. Opened the first preschool mental health day treatment classroom housed in a Minneapolis public school. This classroom, along with those at Fraser, helps young children who have severe emotional and behavioral disturbances caused by neurological vulnerabilities; traumatic experiences (abuse, neglect, exposure to domestic violence); exposure to drugs or alcohol; and disrupted attachments to adults. Provided more support to families in their homes and communities by streamlining the intake process and expanding management support within Fraser Home & Community Supports. Helped individuals navigate the complex social service system by initiating a waiver case management contract with Hennepin County. Services include providing a comprehensive assessment of individuals’ healthcare, social, housing, educational and vocational needs. Planned for the future care and housing needs of clients receiving Fraser Residential Services whose needs continue to change as they age and require additional support to maintain their highest level of independence. Provided for the health, safety and security of clients and staff by developing organizational emergency plans for a natural disaster or community-wide health crisis. Provided residential clients with cutting-edge healthcare options by partnering with other services providers in the Partners Choice Network Managed Care pilot project.
