We are very proud to announce Medicare
has selected Coastal Hospice for a new model
program that will allow many patients to receive
the supportive care typically provided by hospice
and continue treatments to cure their disease at
the same time.
Beginning January 2016, Coastal Hospice
will launch “Coastal Care Choices,” designed to
improve care for people with cancer, HIV/AIDS,
COPD or congestive heart failure.
Patients no longer have to make the difficult
choice between treating their disease and receiving
comfort care. Support is a phone call away 24 hours
a day, 7 days a week. Patients can now stay in their
own homes, determine their own health care goals
and improve their quality of life.
On July 20, 2015, Health and Human Services
Secretary, Sylvia M. Burwell, announced Coastal
Hospice’s selection for this model. HHS selected
only 140 hospices across the country to test the
model for a period of five years and expects as many
as 150,000 eligible Medicare beneficiaries will
benefit from the flexibility of care.
The model is part of a larger effort at HHS to
transform the nation’s health care system to deliver
better care, spend taxpayer dollars in a smarter way,
and put patients in the center of their own care.
To qualify for the Coastal Care Choices model,
individuals must be diagnosed with cancer, HIV/
AIDS, COPD or congestive heart failure. They must
be living in a traditional home, not a nursing home

Having end-of-life discussions in nursing homes is critical
Long-term care facilities — like nursing homes and assisted living homes — offer their residents much
needed care and services. Unfortunately, they often get bogged down when it’s time to discuss what their
patients want to happen when they approach the end of their life.
This is a very difficult conversation to have but a very important one.
In July, a leading advocate for patient-centered end-of-life care, Dr. Ira Byock, visited Salisbury and
told a group of doctors and nurses, “The fundamental nature of illness isn’t medical. It’s personal. The best
care for a person has to be consistent with their personal values, preferences and priorities.
“There are worse things than having someone you love die; there’s having someone you love die badly,”
he said. “We ought to be talking about being able to die with a sense of well being.”
What Dr. Byock is saying is that we need to talk about what we want at the end of our life and not be
afraid of the conversation.
In long-term care facilities, there are several barriers that keep end-of-life conversations from
happening. Family members may live a distance away and not be aware of the medical condition of their
loved one. Staff may regard this as a medical situation, expecting their doctors to handle it, while
physicians may view dying as an emotional issue, expecting social workers and chaplains to deal with it.
The patient may be receiving aggressive medical treatments even though they may have a low likelihood of success. Or the family may be arguing among themselves about what the best next step is for
Mom or Dad.
It’s important to ask the patient — before it’s too late — “What do you want your end-of-life
experience to be?” Most will say they want a peaceful, pain-free death, surrounded by those who love
them, rather than being in a hospital connected to machines and tubes. But this is something we must ask
and then put in writing.
Coastal Hospice has a team of palliative care consultants who are now working at Genesis, Deer’s
Head and Anchorage nursing homes. This team has experience in getting the end-of-life conversation
started, guiding the discussion and offering suggestions for future care.

Patients must be enrolled in Medicare Parts
A, B and D and meet hospice eligibility requirements under the Medicare or Medicaid Hospice
Benefit. They must not have elected the
Medicare or Medicaid Hospice Benefit within
the last 30 days prior to their participation in
the new program. There are a few additional
restrictions that apply.
The patient’s primary care physician will
make the initial recommendation to the program and remain active in directing treatment
and care. The care provided will be specialized,
focusing on providing patients with relief from
the symptoms, pain and stress of illness. The goal
is to improve quality of life and provide greater
coordination of services among all providers
involved in the person’s care.
Like hospice, Coastal Care Choices will take
a team approach that becomes an extra layer
of support. Once the patient is enrolled in the
model, a team of registered nurses, aides, social
workers and chaplains are available to help.
Patients are now in charge of directing their
own health care. They can leave the Coastal Care
Choices program or can enter full hospice care at
any time.

Noted physician/author shares end-of-life message
In July, Coastal Hospice along with
MAC, Inc., The 50+ Network and the
Richard A. Henson Cancer Institute at
PRMC invited Ira Byock, M.D. to speak
to the local health care community about
his compelling end-of-life care message.
Dr. Byock is a leading palliative care
physician, author and public advocate for
whole-person health care and improved
end-of-life care. He visited Salisbury on
July 23-24 for a pair of presentations.
He presented his topics, “The Best
Care Possible: Clinical and Cultural
Leadership for the 21st Century,” during
a dinner at PRMC, then addressed 35
PRMC physicians and guests during grand
rounds with “What Are Doctors For? The
Physician-Patient Relationship through the
End of Life.”
Dr. Byock’s visit was sponsored by the
partner organizations and grants from the
John B. Parsons Foundation, the
Community Foundation of the Eastern
Shore and the Medical Staff of PRMC.

“Kicks for Kids” kickball tournament and fundraising event and raised
more than $4,000 for the pediatric program of Coastal Hospice.

Capobianco, coming in second.

University a success in its first year

6-12 struggling with grief. Sixteen children
who had lost a parent, grandparent, sibling or
other loved one in the past year participated
in the 3-day Camp Safe Harbor, which was
funded by a grant from the Community
Foundation of the Eastern Shore.

For the 4th year, Coastal Hospice provided the kids’ activities
at the Taste of Cambridge event. This year, 111 children ate watermelon, entered the hula hoop contest, had their faces painted and
played games. Thank you to staff members Nancy Stewart and
Mary Katherine Gray for coordinating, and to Bob Stewart for
building the exciting new games.
