Allopathic Private Medical Care Registration Registration(Hospital (Hospital Registration)-Department Registration) Allopathic Private Medical Care Registration request is applied by the citizen through the Meeseva centers centers.. This request can be accessed by the DMHO and processed. The processing steps and screens are provided below. The DMHO initially provide Temporary Certificate which is validate for 90 Days. After that DMHO Provide Permanent certificate which is validate for 5 years. Once 5 years completed Applicant has to again Renewal the Hospital Registration. Category Type B User Charges INR 35/ 35/- Service Charges INR 10,000/ 10,000/- Documents Required 1. Application Form* 2. Photo Copy of Lease Document* 3. Doctor Particulars in Annexure A* 4. Nursing Staff Particulars in Annexure B* 5. Paramedical Staff Particulars in Annexure C* 6. Supporting Staff Particulars in Annexure D* 7. Details of Specialties Available* 8. Details of Equipments and Furniture* 9. Audit Report of Last Two years* 10. Declaration on Stamp paper* 11. Demand Draft Towards Registration Fee* 12. One set of photographs of the premises with its functional areas (hard copy and Soft copy)* 13. Self addressed envelope, acknowledgement and affixed registered postage fee stamps* SLA 30 Working days Output Temporary and permanent Certificate Note: The asterisk (*) denotes mandatory requirement of documents. Step:1 Procedure for Processing the Request at DMHO (District District Medical and Health Officer) End: - ♦ Select “Hospital Hospital registration registration” from Application Processing. It has been depicted in figure 1. Figure 1: Hospital registration Process link under Application Processing Note: - DMHO can also process Requests through the Dashboard in the DMHO Login. ♦ After selection of “Hospital registration” link, DMHO processing window will be displayed. It has been depicted in figure 2 2. Figure 3: Selection of dates in DMHO processing window ♦ Select “Status (either All/Not Viewed/Open Viewed/Open/Temporary /Temporary Certificate issued etc...)” from request status drop down list as depicted in figure 4. NOTE: - Here, At First the Request is in “Not Viewed” Status.. Here I am selecting “Not viewed” request. Figure 5: Get Details button in DMHO processing window ♦ All “Not Viewed” Requests are displayed as depicted in figure 6. Figure 8: Request details page of Local Candidate in DMHO Login ♦ Download all the attached documents from “Documents Attached” panel by clicking on links as depicted in figure 9. Figure 9:: Attached Documents in Hospital registration details Page Note: - After downloading all Verification/Proceeding documents, DMHO Verifies all Details and Take Action (Action Taken)) either Temporary Certificate issued / Reject based on enquiry Report as depicted in figure 10. Note: - Based on DMHO action (either Temporary Certificate issued /Reject), status (Temporary Certificate issued /Rejected) is generated. Figure 14:: “OK” button selection in Certificate window ♦ After clicking on “OK” button, private exchange key popup will generated as depicted in following figure. ♦ Now clicking on “OK” button as depicted in following figure. ♦ After clicking on “OK” button ““Hospital registration” is generated as depicted in following figure. Note: - Now the request is in Temporary Certificate issued status. Note: - After Temporary Certificate issued /Rejected, Customer Received Message. Like you are Request is approved for Temporary Certificate issued /Rejected /Rejected. ♦ Once the request is in Temporary Certificate issued status, then Hospital registration Certificate is available at KIOSK for Printing. ♦ In Rejected case rejected Endorsement was generated with Remarks Step:2 Procedure for Processing the Request at DMHO (District Medical and Health Officer) End: - ♦ Select “Hospital Hospital registration” from Application Processing. Note: - Now the request is in Temporary Certificate issued status. Note: - After Accepted/Rejected, /Rejected, Customer Received Message. Like you are Request is Accepted /Rejected.
