We are committed to providing compassionate, high quality healthcare to the communities in which we work, live and play. Our first commitment is quality care for our patients – a commitment that earned both our hospitals special recognition from The Joint Commission in 2012; for the second consecutive year, each hospital was named a Top Performer for Key Quality Measures. Fort Walton Beach Medical Center was recognized for quality measures of heart attack, heart failure, pneumonia and surgical care, while Twin Cities Hospital was recognized for quality measures of pneumonia and surgical care. While this recognition speaks well of our quality care, we are also focused on promoting patient safety and quality through a variety of initiatives you can learn about on pages six and seven. Along with our commitment to quality is our commitment to compassion not only for our patients and staff, but also for our communities. As one of the area’s largest private employers, with 1,345 employees, we are proud of our employees’ investment of their time, talents and finances to non-profit agencies, including United Way and it’s various agencies, American Cancer Society, American Heart Association, Okaloosa County Schools, Chambers of Commerce, and more. Although reimbursement for services rendered is critical to the operations and stability of our hospitals, we recognize not all individuals have the ability to pay for medical services. In keeping with our commitment to serve the community and in respect to healthcare, we continue to provide free and reduced care to the indigent and to persons covered by governmental programs at below-cost, such as Medicaid and subsidized health services. Last year this amounted to over $20,672,025 in uncompensated care. In addition to our uncompensated care and $18,550,734 in taxes, we also continued our tradition of community education, health screenings, support services, charity care, and health professional education. We thank our Boards of Trustees, Medical Staffs, Employees, and Volunteers for their dedication in providing the best care, service and support to our patients, visitors and their families. Our partnership with area nursing programs continued to play a role in the education and development of the next generation of healthcare professionals. In 2012, hundreds of nursing, pharmacy, radiology, dietary and paramedic students trained at Fort Walton Beach Medical Center and Twin Cities Hospital under the guidance of our nurse and staff preceptors. We also provide meeting space to many area support groups. In 2012, individuals throughout the area improved the quality of their lives through the following support groups: Bosom Buddies, Emergency Management Services, Folks with Strokes, Look Good Feel Better, Man 2 Man, Ostomy, Smoking Cessation, Trauma Intervention Program, Twin Cities Ministerial Association, and more. Volunteering Our staff and physicians believe in going beyond one’s personal duties to help others in need. From providing free medical care within our communities and in areas outside our country where medical needs are non-existent or underserved, to serving on local United Way Agencies or other non-profit organizations that benefit humanity and our communities, we are committed to making a difference in the lives of others, both inside and outside our HCA facilities. Improving the healthcare of our community is part of our hospital mission. Our hospitals actively participated in area health fairs, often providing blood pressure checks and glucose checks free of charge to thousands of individuals. We also partnered with the area Sickle Cell Foundation to assist in screening individuals who are genetically predisposed to carry the disease, as well as speaking at their seminars and classes to educate students and patients. Our speakers bureaus educated community groups on a variety of health topics, including heart disease, cancer, stroke, diabetes and drug safety to name a few. In addition, Fort Walton Beach Medical Center hosted its annual Cancer Survivor Day. Fort Walton Beach Medical Center and Twin Cities Hospital partnered with Northwest Florida State College to host the inaugural Women on Wellness Symposium—a one day event dedicated to enriching the health and wellness of women in our communities. Our hospitals also played active roles in the United Way Days of Caring, American Heart Association Heart Walk, March for Dimes March for Babies, and American Cancer Society Relays For Life. Dean, Career and Technical Education 6 2013 COMMUNITY REPORT Quality and Patient Safety Despite our achievements, Fort Walton Beach Medical Center and Twin Cities Hospital recognize the importance of patient safety and continue to make it paramount. Our initiatives ensure the safety and quality of care given to our patients each day. BCTA — Barcode-enabled Transfusion Administration Barcode-enabled Transfusion Administration provides our hospital teams with barcoding capability for blood products, with automated reporting and trending of blood utilization in accordance with regulatory guidelines as well as automated reporting of blood transfusion reactions. BCTA implementation began in 2012 and will be completed by mid-year 2013. It is an important service to provide our teams because it enables bedside patient and product verification for blood product administration for enhanced patient safety. In addition, it improves monitoring and reporting regarding blood administration and utilization, while decreasing errors. Severe Sepsis Program Fort Walton Beach Medical Center and Twin Cities Hospital have programs aimed at the challenge of severe sepsis. Both facilities practice goal-directed therapies, instituted a Sepsis Alert, and invested a combined $75,000 for a total of six Vigileo monitors and interface equipment needed to support the therapies. Efforts included ongoing support for FloTrac arterial line sensors and PreSep Oximetric Central lines. Leaders formed coordinated Sepsis Teams at both Fort Walton Beach Medical Center and Twin Cities Hospital. Program managers believe continued mortality reductions are possible and say they will remain diligent in detecting and managing this difficult disease. O U R C O M M I T M E N T T O O U R PAT I E N T S Radiation Right Studies have shown that in addition to the benefits of diagnostic procedures that involve radiation, there might also be risks that should be considered by patients and those treating them. Some government agencies and medical organizations have stressed the adoption of two guiding principles of radiation protection for patients: appropriate justification for ordering each procedure and careful optimization of the radiation dose applied during each procedure. Radiation Right incorporates these baseline recommendations but adds additional steps, including adoption of education and competency standards for clinical professionals, meeting and achieving nationally recognized certification standards and adopting “As Low As Reasonably Achievable” or ALARA dose protocols. Radiation Right also follows The Alliance for Radiation Safety in Pediatric Imaging’s “Image Gently” protocol for pediatric patients. Our radiation-reduction efforts focus on preventing accidental or isolated incidents of excessive exposure, reducing cumulative radiation dose, and focusing on nationally accepted guidelines to assure that we always give the lowest effective dose of radiation possible. 7 Modified Early Warning System (MEWS) Monitoring Process In a busy, complex healthcare environment, recognition of deteriorating patient’s condition can be delayed. Identification of negative changes in a patient’s condition can decrease the number of failure to rescue events and resuscitation efforts through monitoring and timely intervention. The MEWS system has been proven to show a significant relationship between the MEWS score and transfers to the ICU or death. The score is comprised of five measures: 1. Blood pressure 2. Heart rate 3. Temperature 4. Level of Consciousness 5. Respiratory rate MEWS transforms this physiological data, and using weighted criteria, determines an aggregate score that may be used to categorize clinical deterioration and direct care. The score is determined electronically and displayed on each patient’s status board. It is important to note that MEWS is a monitoring aid to clinical judgment, a marker of critical illness, and an indicator of physiologic trends. It is intended to trigger critical thinking and assessment prior to making care decisions. Anticipated benefits derived from utilizing the MEWS scores include timely response to changing patients’ conditions, decreased resuscitation events and decreased patient mortalities. 8 2013 COMMUNITY REPORT Specialized Care Emergency Excellence Destin Emergency Care Center Highly skilled emergency physicians and nurses, combined with best practices and the latest technology, benefitted _____ residents last year. Our protocol-driven care that begins in the field, through direct communications with local EMS teams, meets stringent standards ensuring emergent medical needs are provided timely. Destin Emergency Care Center (DECC) is located at 996 Airport Road, less than one mile north of Highway 98, across from the Destin Airport. Our strong focus on enhancing care and improving ER processes has also dramatically reduced average wait times to below the national average. In fact, we take so much pride in our reduced average ER wait times, our current Average ER Wait Times are posted on electronic billboards throughout our communities, available on our websites at www.FWBMC.com and www.TCHospital.com, by texting ‘ER’ to 23000, and through iTriage® smartphone technology for iPhone and Android. Both Fort Walton Beach Medical Center and Twin Cities Hospital are leading the way in making this important time decision criteria available to residents and visitors of our communities through these innovative technologies. The DECC assesses and treats all ages, from neonatal to geriatric patients. With an emphasis on timely assessment and treatment, the DECC staff are skilled in the specific age-related needs of these diverse populations in this clinical setting. The DECC has developed mechanisms for triage, treatment and stabilization as defined in department policies and procedures. The DECC is equipped to handle medical, pediatric and/or psychiatric emergent health care needs. Patients requiring an advanced level of care will be stabilized and transported to the facility of their choice which has the required services available. OUR COMMITMENT: SPECIALIZED CARE 9 Pediatric Emergency Room Heart Care Fort Walton Beach Medical Center opened its new Pediatric Emergency Room in April 2010. It is the only one of its kind in the tri-county area including Okaloosa, Walton and Santa Rosa Counties. The Pediatric ER is a separate emergency room open 24/7 that is dedicated solely to treating the emergency healthcare needs of infants, children and adolescents from birth to 15 years of age. The Heart Center at Fort Walton Beach Medical Center is the Tri-County’s only comprehensive cardiovascular program. The Heart Center at Fort Walton Beach Medical Center provides the latest and most advanced treatments in cardiac care, including cardiac catheterization with coronary angioplasty and stents. These advanced cardiac treatments ensure that life-saving technology and treatments are available right here in our community. The Pediatric ER recognizes that children’s medical emergencies require a special level of care. Our child-friendly clinicians are trained to help children cope with medical emergencies. The American College of Cardiology and the Centers for Medicare and Medicaid Services have established guidelines for how soon a hospital must treat a patient having a heart attack once they arrive at the hospital. This is often referred to as the “door to balloon” time. Fort Walton Beach Medical Center’s Emergency Department and Catheterization Lab Team work closely together in meeting this standard of care. During 2011, the Medical Center’s heart attack response team’s average “door to balloon” time was 53 minutes, much better than the recommended 90 minutes. The team’s motto is “our hustle saves your heart muscle.” In partnership with the Gulfarium and its staff of artists, the Pediatric ER provides a child-friendly ocean environment of vibrant sea creatures. Each of the ER treatment areas has a theme supported by dolphins, sea lions, sea horses, turtles, puffer fish, eels and crabs. We are truly appreciative of the artwork the Gulfarium and its artists have done to make our Pediatric ER so pleasant for the children in our community. For patients recovering from cardiovascular surgery and interventional cardiology procedures, Fort Walton Beach Medical Center offers cardiac rehabilitation programs which assist the cardiac patient in achieving and maintaining a state of optimal health and daily living activities through education, regular exercise, dietary counseling and lifestyle changes. 10 2013 COMMUNITY REPORT Comprehensive Oncology Program Stroke Care In May of 2012, the FWBMC Cancer Care Center became an affiliate of the University of Alabama at Birmingham’s Cancer Care Network. The UAB Cancer Care Network, developed by UAB Medicine and the UAB Comprehensive Cancer Center, is a network of hospitals across Alabama, Florida and Georgia that provides access to the latest discoveries in cancer research and delivers an unmatched level of expert cancer treatment. The UAB Cancer Care Network is dedicated to building and fostering collaborative relationships between leading UAB physicians and scientists and community hospitals. These relationships will help connect UAB’s expertise in cancer research and clinical excellence with partnering communities. The UAB Cancer Care Network provides a structured program to support community-based oncology services to foster collaborative physician relationships, provide continuing medical education and offer local patients the opportunity to enroll in clinical trials managed by leading scientists and physicians, and access to UAB’s best practices in cancer care at a local level. Fort Walton Beach Medical Center is the Tri-County’s only Advanced Primary Stroke Center certified by the Joint Commission. The Medical Center also received the American Stroke Association’s Get With The Guidelines™ Gold Performance Achievement Award recognizing the Medical Center’s success in implementing a higher standard of stroke care. In achieving these designations, the Medical Center has demonstrated that its stroke program follows national standards and guidelines that can significantly improve outcomes of stroke patients. When a patient is suffering from a stroke, the time a hospital takes to administer the drug tPA (clot dissolving drug) is critical. Fort Walton Beach Medical Center, in cooperation with EMS (Emergency Medical Services), has demonstrated they can rapidly diagnose stroke victims and administer treatment within three hours of when the symptoms begin— the maximum time period for administration of tPA, a proven life-saving intervention. Fort Walton Beach Medical Center also received re-accreditation from the American College of Surgeons Commission on Cancer and is the only hospital in Okaloosa County with this accreditation. Administrators and Physicians celebrate the affiliation of FWBMC and the UAB Cancer Care Network. Twin Cities Hospital provides state-of-the-art teleneurology services for the evaluation and stabilization of stroke patients. When necessary, patients are transferred to Fort Walton Beach Medical Center for further treatment. OUR COMMITMENT: SPECIALIZED CARE Orthopedic & Spine Surgery Programs Twin Cities Hospital offers the area’s first Certified Joint Replacement Program. The disease-specific accredited program is designed with the patient in mind and consists of a preoperative educational class. The class allows the patient to prepare for and be knowledgeable of the care they will receive during their hospital stay. A multidisciplinary team of physicians, nurses, physical therapists, and case managers work together to ensure quality care throughout the patient’s stay. In addition to general orthopedic surgery, our orthopedic surgeons offer advanced procedures including minimally invasive knee and hip resurfacing to relieve pain and help patients return to normal activities. Additionally, Twin Cities Hospital offers comprehensive inpatient and outpatient rehabilitation services for recovering orthopedic surgical patients. Twin Cities Hospital’s O-arm Surgical Imaging System is a multi-dimensional surgical imaging platform that is optimized for use in spine surgery. It provides real-time, intra-operative imaging of a patient’s anatomy with high quality images and a large field-ofview in both two and three dimensions. 11 By integrating O-arm surgical imaging technology with Medtronic’s StealthStation surgical navigation systems, our spine surgeon is able to perform less invasive procedures and confirm the precision of advanced surgical procedures before the patient leaves the operating room. The StealthStation with the O-arm reduces x-ray exposure, increasing safety for both the operating room staff and patients. Fort Walton Beach Medical Center’s Spine Surgery Program is recognized by Becker’s Review as a Top 101 Spine Program in the nation. Our team of highly trained Neurosurgeons, operating room staff, nurses and rehabilitation specialists work collaboratively to ensure spine surgery patients receive the high caliber of care and outcomes. Robotic Surgery In July 2012, Fort Walton Beach Medical Center purchased a $2.2 million da Vinci Si robotic surgical system. The da Vinci® Surgical System provides surgeons with an alternative to both traditional open surgery and conventional laparoscopy, putting a surgeon’s hands at the controls of a state-of-theart robotic platform. The da Vinci System enables surgeons to perform even the most complex and delicate procedures through very small incisions with unmatched precision. For the patient, benefits may include significantly less pain, less blood loss, less scarring, shorter recovery time, a faster return to normal daily activities and, in many cases, better clinical outcomes.
