The West Third Street Reservoir is a circular, below ground concrete reservoir constructed in 1914 with a concrete roof, which was added in 1921. The reservoir is 180 feet in diameter and 41 feet deep with a rated capacity of approximately 7,500,000 gallons. There are 32 buttresses around the reservoir. The top slab is 7” thick and is supported by the exterior wall of the tank and 52 interior columns. The columns are braced at approximately mid-depth by concrete beams. The floor is 6” thick placed on rock. Approximately, the top 3’-0” of the tank is above ground level. The tank was designed in 1914 by Dabney H. Maury, Consulting Engineers of Chicago, Illinois. 2.0 PROJECT DESCRIPTION Professional services for the Inspection Report and Plans for any corrective action of the Interior & Exterior of the West Third Reservoir and Control Valve Housing Building and Pit. 3.0 PROJECT SCOPE OF SERVICES A professional engineer (P.E.) will be present for the tank inspection. The P.E. must demonstrate extensive experience and training in concrete tank design, tank inspections, tank construction, sanitary requirements, re-coating, and repair or modifications of several tanks. This includes but is not limited to specification preparation, failure analysis and warranty inspections. The Engineering Consultant will provide the following inspection reports based on the water reservoir inspection criteria, referencing the Ten States Standards and all applicable Iowa DNR and OSHA requirements. -4- A. Minimum items inspected will include: (not inclusive) Roof structure & condition Vents Sediment depth (& removal if necessary) Safety considerations Capacity level & head range Fill (Inlet) / Discharge (outlet) pipe Drains Floor structure Leak detection Interior floor & wall coating Overflow weir & pipe Support column Safety devices Screens on vents & overflows Wall structure Ladders, platforms and handrails B. Inspection report and plans: The Engineering Consultant will provide the owner with written reports/plans following the completion of the inspection. This report shall contain a narrative, video, photographic documentation, and plans for corrective actions that warrant repair, servicing or replacement by the owner, or any issues believed to constitute conditions that warrant continued observation by the owner for potential future repair, servicing or replacement. The Inspection Report shall include descriptions of the locations of any relevant observed problem, any faults in the structure and include recommendations, prioritizations and cost estimates for the maintenance, repair, service and/or replacements. The final Inspection Report shall be submitted to the owner no later than October 1, 2009. C. Reservoir Water Circulation: The existing inlet/outlet pipes and pumps are configured to fill and empty from the same location. There is insufficient water turnover in the reservoir. The Engineering Consultant shall provide alternatives and costs for reconfiguring the present pump and valve system. The Engineering Consultant shall also provide alternatives and costs for most cost-effective and sustainable method to circulate water within the reservoir. D. Means of Inspection: Alternate #1: Dive, The Engineering Consultant shall provide interior inspection by means of an underwater diver equipped with two-way communication devices. The diver shall record the inspection with a video camera and provide the owner with the recorded footage. If necessary for proper inspection of the reservoir floor, sediment shall be removed (vacuumed). Alternate #2: Remotely Operated Underwater Vehicle (ROV), The Engineering Consultant shall provide interior inspection by means of a remotely operated underwater vehicle. The inspection shall be recorded with a video camera. The Inspector shall provide the owner with the recorded footage. -5- If necessary for proper inspection of the reservoir floor, sediment shall be removed (vacuumed). Note #1: Dive technicians and in-tank equipment shall be fully disinfected in accordance with AWWA standard C652-02. All system entries shall be conducted in accordance with applicable OSHA regulations pertaining to Diving and Confined Space entry. Note #2: Complete copy of the 1988 inspection report (74 pages) is available upon request. 4.0 SUBMISSION REQUIREMENTS Before submitting a proposal, each Proposer shall make all investigations and examinations necessary to ascertain site conditions and requirements affecting the full performance of the contract and to verify any representations made by the City upon which the Proposer will rely. -6- PROPOSAL SUBMITTAL INFORMATION Submittal Deadline: Submittal Mailing Address: Submittal Copies: July 27, 2009 on or before 2:00 p.m. CST City of Dubuque Water Department C/O Jacqueline Vanek 1902 Hawthorne Street Dubuque, Iowa 52001 Two (2) hardcopies and one electronic (pdf) copy of the proposal shall be provided. Submit one (1) original signed proposal and one (1) copy all labeled INTERIOR & EXTERIOR INSPECTION REPORT OF: WEST THIRD STREET RESERVOIR. Submitted proposals must be delivered in printed format. In addition, supply one (1) electronic version of the proposal in Adobe Acrobat format. No faxed or e-mail proposals will be accepted. The original proposal document shall be signed in blue ink by an officer of the company who is authorized to legally bind the Proposer to its provisions. Proposals are to contain a statement indicating the period during which the proposal will remain valid. A period of not less than sixty (60) calendar days from the proposal closing date is required. Each engineering firm assumes full responsibility for delivery and deposit of the completed proposal package on or before the deadline. Any proposals received after the submittal deadline will not be considered, and will be returned unopened to the Proposer. The City of Dubuque will not be responsible for any loss or delay with respect to delivery of the proposals. The City reserves the right to reject any and all bids or to waive any technical ties and irregularities. By virtue of statutory authority, a preference will be given to products and provisions grown and coal produced within the State of Iowa and to Iowa Domestic Labor. The City of Dubuque is not liable for any cost incurred by any Engineering firm prior to the execution of an agreement or contract. Nor shall the City of Dubuque be liable for any costs incurred by the firms in responding to this RFP and those not specified in any contract. All results from this project will remain the property of the City of Dubuque. The engineering firm must provide Proof of Insurance, which complies with the City of Dubuque’s Insurance Schedule C (attached). The certificate must be issued to the City before the start of the project and insurance shall be maintained throughout the duration of the contract. All policies of insurance required hereunder shall be with an insurer authorized to do business in Iowa. All insurers shall have a rating of A or better in the current A.M. Best Rating Guide. 2. All policies of insurance shall be endorsed to provide a thirty (30) day advance notice of cancellation to the City of Dubuque, except for 10 day notice for nonpayment, if cancellation is prior to the expiration date. This endorsement supersedes the standard cancellation statement on the Certificate of Insurance. 3. ____________________ shall furnish a signed Certificate of Insurance to the City of Dubuque, Iowa for the coverage required in Paragraph 6 below. Such Certificates shall include copies of the following endorsements: a) Commercial General Liability policy is primary and non-contributing. b) Commercial General Liability additional insured endorsement. c) Governmental Immunities Endorsement. ____________________ shall also be required to provide Certificates of Insurance of all subcontractors and all sub-sub contractors who perform work or services pursuant to the provisions of this contract. Said certificates shall meet the same insurance requirements as required of ________________________. 4. Each certificate shall be submitted to the contracting department of the City of Dubuque. 5. Failure to provide minimum coverage shall not be deemed a waiver of these requirements by the City of Dubuque. Failure to obtain or maintain the required insurance shall be considered a material breach of this agreement. 6. Nonwaiver of Governmental Immunity. The insurance carrier expressly agrees and states that the purchase of this policy and the including of the City of Dubuque, Iowa as an Additional Insured does not waive any of the defenses of governmental immunity available to the City of Dubuque, Iowa under Code of Iowa Section 670.4 as it is now exists and as it may be amended from time to time. 2. Claims Coverage. The insurance carrier further agrees that this policy of insurance shall cover only those claims not subject to the defense of governmental immunity under the Code of Iowa Section 670.4 as it now exists and as it may be amended from time to time. Those claims not subject to Code of Iowa Section 670.4 shall be covered by the terms and conditions of this insurance policy. 3. Assertion of Government Immunity. The City of Dubuque, Iowa shall be responsible for asserting any defense of governmental immunity, and may do so at any time and shall do so upon the timely written request of the insurance carrier. 4. Non-Denial of Coverage. The insurance carrier shall not deny coverage under this policy and the insurance carrier shall not deny any of the rights and benefits accruing to the City of Dubuque, Iowa under this policy for reasons of governmental immunity unless and until a court of competent jurisdiction has ruled in favor of the defense(s) of governmental immunity asserted by the City of Dubuque, Iowa. No Other Change in Policy. The above preservation of governmental immunities shall not otherwise change or alter the coverage available under the policy.
