f you are looking for excellence in FOOT and ANKLE care; if you are looking for a doctor who practices state-of-the-art Medicine;
if you are educated enough to understand that you may only want an ORTHOPAEDIC SURGEON to touch your feet, then you
owe it to yourself to make an appointment.
There are many people with very serious problems of the foot and ankle who go from doctor to doctor and remain undiagnosed or
poorly treated. The typical patient coming to see Dr. Samotin has been evaluated by many other doctors. The patient usually finds
that Dr. Samotin is the last doctor they have to see.

any
people
have
been
there ... staring at a future that
may never be, longing for a
change that may never come, seeking a solution
that may never be found . Leonard Narracci has
been there. He, too, was informed that his life was
coming to a close. How could this have happened?
It wasn 't long ago that he could have done
whatever he wanted. To say he lived an active lifestyle barely illustrates how this fireball of a man
enjoyed life. By day, he was a high school English
teacher. During the weekends and summers, a
farmer. In his spare time he performed laborintensive hobbies. When it came time to close the
schoolbooks and retire to Venice, Florida he was
not satisfied with simply moving into a condo or
pre-built home, but began making plans to build
his retirement home. But, then it happened.
Lethargy settled in. An overwhelming feeling of
being run-down melted the energy from his once
active body. Daily tasks felt daunting. After his
extreme fatigue smothered him, Mr. Narracci knew
he needed help.
His doctors told him that he had a heart disease
called cardiomyopathy. To his dismay, he learned
that his condition was incurable. His life would
never be the same. His energy would never return.
His fire - gone. He was sentenced to a life of
routine medications, decreased productivity, and
eventual heart failure. For four years Leonard
struggled through apathy and battled depression
due to his condition. He described these years as
the worst of his life. But, Leonard Narracci's life
was not over. He was determined to find answers
for a better quality of life. Leonard began to
research his disease and for any possibilities of
further treatment. What he found was a novel
treatment using his own stem cells.
Mr. Narracci discovered Regenocyte Therapeutic,
an organization leading the world in regenerative
cell therapy, on the Internet. Dr. Zannos Grekos,
Director of Cardiac and Vascular Diseases in
Bonita Springs, Florida, was conducting a seminar
at the Collier County Regional Library discussing
the use of adult stem cells and their regenerative
nature. Mr. Narracci attended the seminar and his
interest was piqued. Listening to former patients
speak on their success with adult stem cell therapy,
only made Mr. Narracci more interested in learning
about this natural way to regenerate the human
body. Before arriving home from the library, he
decided that Regenocyte therapy was for him.
(ejected) with each beat of the heart, was at 20%.
Prior to his treatment he expressed a personal goal
of hoping to obtain an Ejection Fraction (EF) rate
of 40% within one year of undergoing adult stem
cell therapy. This type of increase would be a significant improvement over his severely dysfunctional 20%. A general breakdown of the values is
as follows: 40-50% is considered a mild dysfunction; 25-39% is moderate dysfunction; less than
25% is severe dysfunction.
Before

Leonard 's
heart
was severely dysfunctional.

At his one-year check-up
in October 2009, Mr.
Narracci was stunned to
find his EF rate registering 52%!

The treatment process, which led to such high
results, began when Leonard 's blood was drawn
and then couriered to a state of the art laboratory in
the country of Israel. The stem cells were then
separated from the blood, engineered into 40-80
million stem cells, and then educated to reinforce
Leonard 's heart. Next, the cells were sent to the
Dominican Republic where they passed through
many quality assurance checkpoints. Approximately one week after his blood was drawn,
Leonard made his way to the Dominican Republic
to receive his stem cells.
The delivery of the stem cells is what makes
Regenocyte's therapy so unique. A team of physicians injects the stem cells into the heart through a
specialized catheter that allows precise delivery of
the stem cells to the exact location they are needed.
As the stem cells are injected they begin to repair the
damaged area. Due to the ease of the catheter procedure, patients are released from the hospital the next
day. Every patient's body has a different response
time to the treatment. Some may begin to notice
improvements within the first month and then
optimal improvements by month six. Mr. Narracci
began discovering improvements after the first
week. He said, "I was expecting improvements, but
not so quickly. After two weeks my Ejection Fraction
rate had gone up 8 points - I was amazed, and I think
Dr. Grekos was too! " With each check-up Mr.
Narracci continued to improve and at his one-year
mark he had climbed from 20% to 52%!
Mr. Leonard Narracci, the life-long educator had
become the student - a student of his own heart. For
Mr. Narracci, the answer to his problem was not
found in a textbook, but rather in his own stem cells.
Because he was proactive in researching a better
Celebrating with his wife, Betty, on his mother's 1DOth birthday, Leonard may now look forward to a longer and
healthier life of his own based on the results of his new therapy
quality of life even though he had an incurable
disease, he found a way, through adult stem cell
therapy, to revitalize the energy and vitality of his
former self. He can now say, with much pleasure, "I
can do what I want to do, when I want to do it!"
Dr. Grekos spends his career in the field of stem cell
therapy. Working in collaboration with major international treatment and research centers, he has
developed many of the protocols used for adult stem
cell therapy throughout the world. He was recently
appointed to the Science Advisory Board of the
United States ' Repair Stem Cell Institute and is a
member of the International Society for Stem Cell
Research (TSSCR). Regenocyte Therapeutic is

he looks no different than you or me.
To look at her, you wouldn't know she
was a fierce warrior, a relentless fighter,
and a hero's champion. But the battle lines have
been drawn, and Irene Gargiulo isn 't backing down
- even if her opponent catches her by surprise and
attacks again. Irene is determined to keep fighting.
She is strong-willed and defiant - a force to be
reckoned with.
In 2005, Irene visited her physician because she
was concerned by minor palpitations she thought
were brought on by stress. However, a chest x-ray
told a far different story. "When my doctor told me
that I had lung cancer, my legs came very close to
giving out," recalls Irene. "How was I going to tell
my husband? How could I tell my children? Would
I see our youngest son marry? How much would I
miss? How much time?" Deciding to research her
odds, Irene found out that lung cancer patients
have a meager 15% five-year survival rate.
Only a month after diagnosis, Irene underwent
surgery at Charlotte Regional Medical Center to
remove her Stage IB Non-Small Cell Adenocarcinoma. Her upper right lobe was removed. "I
believe time was important to my survival. I see
God's hand in the when, the where, and even the
why," confides Irene.
On October 10th of this year, Irene celebrated a
birthday. Not a birthday in the traditional sense, but
her birthday celebrating four years cancer free . "I
have cancer, but cancer does not have me" is
Irene's battle mantra. The cancer is now gone,
hopefully for good, but Irene's battle continues.

This cancer crusader does not sit back and let
cancer get the best of her. No way. Noticing the
lack of a lung cancer support group in the area,
Irene has accepted the challenge of creating one.
Soon, the group will hold its first meeting, offering
support and encouragement to lung cancer patients
and survivors. "Studies have shown that psychosocial support can have a beneficial impact on quality
oflife for cancer patients," reveals Irene.
"It is with gratitude that I see the improved
survival rates of breast, prostate, and colon

Determined to win her battle, Irene has a strong
supporter in her husband Carlo. Each year, Irene
and Carlo take part in the American Cancer
Society's Relay For Life of Punta Gorda. Relay is
a celebration of life in honor of those touched by
cancer. The event raises funds for the America
Cancer Society's programs of research, education,
advocacy and service - programs that can reduce
cancer deaths and cancer incidence, and improve
the quality of life for those touched by cancer.
"Do you want to know why I Relay?" asks Carlo.
"I Relay in the hopes that my grandchildren will
not have to." Well said.

For Life of Punta Gorda
April 10 -11, 2010
at Charlotte High School's track
elay for Life is an overnight (sunset
to sunrise) event (3 pm till 9 am the
next moming). The overnight stay is
symbolic - cancer never sleeps. Relay begins at
sundown with a survivor walk, signifying one's first
diagnosis of cancer and the beginning of one's
cancer journey. The darkness ofthe night represents
the treatment - the walk of survival. The next
morning's rising sun and participant victory lap represent the victory that a survivor has over cancer.
Relay is a true community event where people from
all ages and from all walks oflife come together for
a common cause. Relay is not a race - participants
will take turns walking relay-style around the Charlotte High School track. Friends, relatives, local
businesses, hospitals, schools, faith based organizations, service clubs and other organizations organize
teams of 10-15 people. Food, fun and camaraderie
are also part of the Relay For Life experience.
Relay For Life - it's about providing

WHY DOES MY
KNEE AND HIP
HURT?
I
By Ociel Betancourt, PT
During our rigorous schedules, we may feel that the
aches and pains associated with everyday living are
acceptable. Tftolerable, we tend to put pain in the back
of our minds and expect it to heal itself; and most cases
it does. On occasion, a little back ache, a slight pain in
the hip region or an elbow that just doesn't bend as
easily as it should, can be cause for concern. As a
Physical Therapist, I can evaluate your condition and
advise you regarding your concerns.
Bursitis is a common problem caused by swelling and
pain in the bursae; one of the more than 150 small bags
that keep muscles, bones, tendons and skin gliding
smoothly and that cushion each other while you are
moving. Bursitis can be acute or chronic and vary from
moderate to severe. Bursae of the knee, hip, elbow, and
shoulder cause most problems. Bursae usually occur on
one side and in most cases is caused by repetitive
motions or direct, minor impact on the area.
Other
causes of bursitis include gardening, raking, carpentry,
painting, scrubbing, tennis, golf and incorrect posture.
Certain diseases such as rheumatoid arthritis, gout, psoriasis and thyroid disease can contribute to bursitis; and
although rare, infection as well.

INFLAMMATION OF THE SOFT-TISSUE
TENDONS AND BURSAE OFTEN
WILL BE MISTAKEN FOR ARTHRITIS.
conditions such as leg length discrepancies,
improper posture or poor technique in sports or
work must be corrected.
Tendonitis means inflammation, irritation and
swelling of the tendon. Tendonitis can be caused by
sudden intense injury but is most often the result of
a repetitive minor injury of the affected area.
The strategies for preventing these disorders are
very similar to the ways of preventing just about
any inJury:
13
Tendonitis produces pain along the affected tendon
and may be associated with local inflammation and
decreased joint mobility. Several factors cause this
disease which include but are not limited to:
mechanical factor, traumatic injuries, or loss elasticity due to age.
In addition, Iliotibal Band Syndrome is another
overuse condition observed in individuals that participate in running and bicycling. The symptoms
are: pain over the lateral knee or hip on the affected
side or tenderness of the lateral knee to touch. The
Iliotibial band at the hip lies lateral to a bony bulge
on the femur called the greater trochanter. This is
an overuse syndrome. Potential predisposing
factors are: excessive bow-Ieggedness, overpronation of the foot and ankle, leg length discrepancy
and improper training.
IF THE PAIN IS SEVERE, MAKE AN APPOITMENT TO SEE YOUR DOCTOR!
Bursitis and tendonitis can be treated based on the

AVOID STRESSFUL
OVEREATING
THIS
SEASON
I
By Debra A. Florio, LMT, CIIM, MA
s we approach the Holiday Season, we
must remind ourselves daily that this is
not the time to pack on the pounds. For
many, this can be a very emotional time of the year. This
season generally brings on the joyous feeling of family and
closeness, however, for some it is an unhappy time, and yet,
for others it is an eating frenzy. Yes this time of year can be
a little stressful. During all the hustle and bustle, please take
time to remember what this season is all about.
To prepare for this joyous
season there are a few tips
we would like to offer to
help you avoid stressful
over eating.
I. Practice eating slowly during the holiday season. No
matter what you are eating, practice eating slowly. Do not
eat on the run. Research shows that people who eat their
food slowly are more likely to eat less than those who
quickly want to finish their food.
2. Eat more fruit and veggies. Do not go all day without
eating so you can indulge in the Christmas party food.
Eating more frequently will boost the metabolism. So snack
all day on healthy fruit and veggies. Reduce your grains and
carbs, you will eliminate cravings for carbs by eating more
fruits and veggies and won't crave fatty and greasy foods.
3. Drink green tea. Drinking green tea stimulates your body
to burn fat for energy. Drink three cups per day to fight
sugar cravings. Try to drink the tea 30 minutes before
physical activity to increase the effect.

4. Do not indulge in alcoholic beverages. Studies
have shown that you tend to overeat when you have
consumed multiple alcoholic beverages. Avoid at all
cost the high caloric beverages, such as egg nog and
creamy drinks. Have your holiday toast with a wine
spritzer, 2 ozs of wine and 6 ozs of club soda.
5. Plan your diet. Some discipline is needed here.
You should know what your trigger foods are and
what makes you put on weight. Choose wisely, you
will feel better this season if you do.
6. Eat your proteins first, it takes longer and burns
more calories to digest. You will feel fuller longer,
than if you eat sweets or carbs. Then eat your low
carb vegetables. Then if absolutely necessary eat a
SMALL amount of dessert. Do not forget, you are
what you eat.
When everyone else IS
making that New Years
resolution to lose weight
you might be surprised
that by making good
choices you may even
drop a few pounds. We tend to

little and take some nice deep breaths. Remember it
is a joyous season do not stress yourself, stress will
put weight on you. Be happy, healthy and lighter
this season.
Also, please DO NOT DRINK AND DRIVE, and do
not let your friends either. All of us here at Contour

very year a fair number of patients
present complaining mostly of hand
numbness and pain. Many of these
patients are assumed to have nerve
compression in the neck area and come to the office
with an MRI of the cervical spine in hand. Some
have even had surgery performed in the neck area
with little or no relief. The reason the pain persists is
because they have been given the wrong diagnoses.
What they really have is Carpal Tunnel Syndrome, a
compression of the median nerve at the wrist and
palm of the hand. The Carpal tunnel is a pathway
made of ligaments creating a tunnel thru which
nerves travel. Together with the nerves there are
some tendons that also travel alongside the median
nerve. If something makes tbe tunnel to become
narrow then in turn this will cause compression of
the nerve and also of the tendons. For example if the
ligament forming the tunnel gets swollen, inflamed
or thickened that will limit the space available for the
nerve and tendons to go thru and the person may
become symptomatic. Women who are pregnant
sometimes develop symptoms because of water
retention and swelling, which after delivering the
baby seem to improve.
What are the symptoms associated with Carpal
Tunnel Syndrome? Like I said at the beginning, a
feeling of numbness in the bands is a very common
symptom of CTS. Patients often complain that this
sensation wakes them up at night and that by shaking
tbeir bands or hanging tbem over tbe side of the bed
alleviates the sensation. Others report dropping
things or having trouble opening jars or making a
fist. Tbis could be secondary to squeezing of tbe
tendons in the tunnel. When patients wait too long to
seek treatment and they present with atrophy or loss
of muscle in the hands and weakness of the thumb.
There are several ways to diagnose CTS . By the
history of the patient and related complaints
together witb tbe medical exam, the doctor will
suspect the condition, which then can be verified
with a nerve test such as an EMG & Nerve conduction test. MRIs of the wrist are sometimes also performed to ensure a proper diagnosis.
The treatment of CTS can range from mild, tbe
placement of a wrist-hand brace, to severe requiring
surgery. The goal of surgery is to relieve the compression of the nerve by cutting tbe tissues responsible for it and opening the tunnel. This can be
achieved in various ways including endoscopically
or by microsurgery. Each way bas its own set of pros
and cons. Either way is very succesfull in relieving
symptoms and improving the condition. The operation is done on an outpatient basis under sedation or
nerve block and usually is a short procedure.

YOUR"ARCH"ENEMY
I By Myles Rubin Samolin, MD - Board Certified Orthopaedic Surgeon, Fellowship Trained in Fool and Ankle
oak down at your feet right now! Do
they seem different from ten years
ago? Does your foot arch look the
same as il did, or are you imagining that it has changed?
Does your arch seem flatter to you? Well, maybe it has
become flatter. And quite possibly that flatness may
result in a 101 of severe foot problems for you.
First, try the wet test. Wet your feet, then go and
stand on a flat, dry surface that can leave an imprint
of your foot. A normal footprint has a wide band
connecting the ball of your foot to the heel, with an
indentation on the middle, inner side of your
imprint. A foot with a high arch has a large indentation and a very narrow connecting band, and conversely, a flatfoot leaves a nearly complete imprint
with almost no curve.
There are several reasons for having flatfeet. They
can be present at birth (congenital), caused by
ongoing stresses to your foot, obesity, diabetes,
Osteoarthritis, Rheumatoid arthritis, hypertension,
or traumatic injury to your foot or ankle.
You may have flatfeet and be pain free. But you
must understand that the arch in your foot is there to
help distribute your weight across your entire foot.
Painless flatfeet can deteriorate over the years and
become painful. By that time, it may be too late to
treat conservatively.
extremity can help. Physical therapy may be necessary to strengthen and stretch the surrounding
tissues, helping to alleviate the stress placed on to
your affected foot.
However, frequently the only way to correct your
painful foot is through surgery. Surgical procedures
can help reduce or eliminate the pain and can improve
bony alignment. They can include Arthrodesis, or
welding (fusing) two bones together, Osteotomy or
cutting or reshaping a bone to correct alignment,
Excision or removal of extra bone or spurs, Synovectomy or cleaning the sheath of a tendon, and/or
Tendon Transfer, to replace a worn or ruptured tendon.
examine your feet, and if necessary, your gait. Why
an orthopaedist? Simple!! The foot has twenty eight
bones that need and should be evaluated by a bone
doctor!!! Be sure to bring your regular shoes so that
these may be examined. X-rays of both feet should
be done for comparison and to determine, ifit exists,
the severity of the flatfoot. Muscle and tendon
strength should be evaluated by your Orthopaedic
Foot and Ankle Specialist.
Treatment can vary depending upon the cause. Conservative treatment can include shoe modifications,
arch supports and custom orthotics. An injection of
corticosteroid may be used to calm an inflammatory
pain in your flatfoot. Resting and icing the involved
Having flat feet can be a very serious matter. If you
are experiencing foot pain and think it may be
related to flatfeet, see an Orthopaedic Specialist as
soon as possible. This is a problem that often
worsens over time with treatment becoming more
and more complicated. With 28 bones in your foot,
you need to be evaluated by a Board Certified
Orthopaedic Surgeon with a Sub-specialty, Fellowship Trained in Foot & Ankle surgery. In fact T am
the only surgeon with these qualifications in our
area. I believe this makes me uniquely able to deal
with these problems in a state-of-the-art atmosphere
and method that will keep you in good hands and
provide you with the most desired result.
The signs and symptoms of flatfeet problems can
include lower leg pain and weakness, pain around
the inside ankle, sometimes with swelling. You may
have weakness inverting (pointing the toes inward)
the foot. You may find that you have uneven shoe
wear and notice that your regular shoes seem to be
collapsing. You may also find that you have the
inability to stand on your toes. Sometimes flatfeet
can contribute or exacerbate other problems such as
plantar fasciitis, posterior tibial tendonopathy,
achilles tendonitis, shin splints, bunions, stress fractures and calluses.

NO PREP LUMINEERSVS REGULAR VENEERS:
PROS AND CONS
hrough the years I've had many
patients ask about the difference in
veneers. Lumineers™ is a trademarked name for a brand of veneer
that is made by Den Mat laboratories in California.
Den Mat has been a dental manufacturing and supply
company for many years. Den Mat has invested a lot
of money into advertising and has successfully trademarked the name of tbeir veneer to be a housebold
name. Much like "Kleenex" is for tissues. This is the
first time that a dental product of this sort bas been
marketed to tbe general public.
Porcelain comes in a bottle. It is a powder. The
powder is sold to laboratories all over the world. If
the bottle of powder is made into a porcelain veneer,
it is called a "DaVinci Veneer" in the DaVinci laboratory, or a "MAC Veneer" in tbe MAC laboratory or
a Lumineer in the Den Mat laboratory. Each one of
these laboratories is trying to trademark their
veneers, even tbough they are often times using tbe
same porcelain.
What makes a porcelain veneer different is the skill of the laboratory
technician and dentist.
The Den Mat company advertises their Lumineer™
as a veneer that does not require removal of tooth
structure. If a patient has straight, nicely aligned
teeth and just wants a color change, the teeth do not
bave to be prepared or "altered". If the teeth are
crooked or bave gaps between tbe teetb at tbe
gumline, tooth preparation must be done in order to
achieve great results. Through advertizing, the
public has been led to believe that tooth preparation
for a veneer is dangerous to the tooth. Let's clear up
some confusion.

Dr. Jill Morris - Cosmetic & Restorative Dentistry I Dr. Burr Bakke - Implant Surgery & Restorative Dentistry
If a patient has high esthetic expections, would like a
more natural looking veneer that has internal characterization built into the porcelain, smooth transition
at the gumline, and no limitations as to length and
design of the smile, a regular veneer is recommended. The preparation for a regular veneer is only
0.6mm removal ofthe enamel off the front and biting
edge of the tooth.
Let's talk about sensitivity. Much of the sensitivity
which is mentioned is usually because of tbe technique the dentist used during the bonding process.
Special training is imperative by the dentist to learn
to properly bond and desensiti ze the teeth. Drs.
Morris and Bakke use ozone and lasers to desensitize
the teeth and kill bacteria. They have taught thousands of dentists the correct protocol for bonding of
veneers. Ifthere is sensitivity, it is minor and usually
lasts less than 6 weeks. Some sensitivity to cold can
be expected for a few weeks. Dr. Morris lectures to
dentists nationwide about correct adhesive techniques. Both Drs. Bakke and Morris bave taugbt
dentists all over the world correct preparation, design,
and adhesion protocols. Dr. Morris is the only accred
ited cosmetic dentist in the area. The American
Academy of Cosmetic Dentistry has awarded
accreditation to only 300 dentists worldwide.
In summary, there is a place in cosmetic dentistry for
both"no-prep" and "regular prep" veneers. TalJdng
about goals and smile design will help the patient and
dentist determine which veneer type is best. Viewing
pictures of both types of veneers is belpful as well as
seeing the actual model of tooth preparation fully
informs the patient before the best decision is made.

MAKING 'YOUR' DECISION ON
RECEIVING SWINE FLU VACCINES
I
By April S. Brown. Doctor of Pharmacy
he Centers for Disease Control
(CDC). county health departments
and the media have been impressive
in their efforts to disseminate information which, by nature, is rapidly
changing as new data emerge regarding the HINI
virus and the new vaccines.
Physicians are fielding an onslaught of questions
from worried patients- as individuals struggle to
decide for themselves whether or not they are appropriate candidates for the HINI vaccine ... or whether
to get one or not if the vaccine is available to them.
As a Doctor of Pharmacy, I am aware that patients
often share comments with their pharmacists that
they would not otherwise share with their physicians. This is likely because many patients do not
view pharmacists as true health care providers, and
therefore feel free to share an opinion. In fact, many
people are surprised to discover that pharmacists are
now required to eam Doctorate degrees which
entails six to eight years of university study- more if
a pharmacist undertakes a residency or fellowship.
To help people decide on an individual course of
action in terms of flu vaccines, some unbiased data
may help allow for rational and confident decision
making. This will allow individuals to determine for
themselves where they fit in and whether they wish
to receive the vaccines.
THE OBSERVED DATA
The following graphs relate patient age to infection
rates, hospitalization rates, and death rates. Note
that not all people seek medical care when developing symptoms, and not all people seeking care have
specimens collected; therefore, the overall numbers
themselves are greatly underestimated.
Q.4 'ITs
While it may be true that the goverrunent, physicians, and the media were not quick to highlight one
of the early question marks with these vaccines, this
is largely because patients have a tendency to panic
if they are told an adverse event is a
'possibility'-and thereby will forego very beneficial therapy out of fear. I have found in practice,
however, if patients are given well-worded explanations so that they 'themselves' can arrive at the conclusion that the benefit does outweigh the risk, they
will make rational decisions and comply with physicians' health care recommendations.

year-olds. Therefore, if you are in this age group and
the vaccine is available to you, the answer to
whether you should get the vaccine is 'yes.'

WHERE CAN I GET SEASONAL
AND HlNl VACCINATIONS
People are aware that vaccinations have been available through physicians, county Health Departments,
nursing groups, or in supermarkets and pharmacies.
Floridians may not be aware, however, that 2008
marked a legislative change which allows certain
pharmacists to provide adult influenza vaccinations.
Prior to this, vaccinations provided in pharmacies
were under the umbrella of companies who contracted seasonal help from nurses.
With the advent of pharmacists who are now able to
offer clinical services that parallel their dmg knowledge to include vaccinations, last year marked the
dawn for many employers, social and fraternal groups,
and residential communities to begin utilizing pharmacists to vaccinate to their employees and residents.
For all groups, the risk of complication or death is
increased by the presence of underlying health
conditions or concomitant infections.
WHAT ABOUT THE VACCINES
There was some initial controversy regarding the
HINI vaccines because it was unknown whether
the government would opt to use ' adjuvanted '
vaccines. The fact is that the manufacturing processes for HI N I vaccines are no different than
with seasonal flu vaccines. What 'is ' new is that
there was a possibility the use of adjuvants would
be warranted. Therefore, there is no debate as to
the safety of these vaccines as compared to
seasonal influenza vaccines other than the issue of
adjuvants. An adjuvant is an added substance that
will help the vaccine produce a better immune
response while requiring less of the vaccine. This
is good in situations where much vaccine needs to
be produced in a hurry (as with HlNI). The
concern, however, was that there may be an
increased risk of immune-mediated events. Since
recent U.S . clinical trials with HINI formulations
have found that adequate antibody responses are
achieved with traditional unadjuvanted formulations, the issue of adjuvants is now null as the government has opted to use traditional vaccines.
This
leaves
us then,
simply with the debate on
the use of thimerosal in
flu vaccines.
It is beyond the scope of this article to force a con-
clusion or dissuade a link between thimerosal and
health disorders. However, due to pressures from
various groups, thimerosal-free formulations will be
administered to pregnant women and children. For
the rest of us, however, consider that more than a
million people each season receive the preservative-containing formulation and there exists decades
of favorable safety data. This is not to mention that
millions of people each year use products which still
contain thimerosal without incident.
The rate of complications and deaths are higher with
HI N I than the seasonal strain. And, while there are
individuals who develop only minor symptoms, I
have also seen many otherwise perfectly healthy
adults who decompensate very quickly and end up in
emergency rooms within six hours. Given that the
question mark of using unadjuvanted vaccines is
removed, the decision to receive the vaccine is a
rational choice.
Many pharmacists, like myself, who independently
provide services to patients and employers under
protocol with local physicians are in a prime position
to provide specialized services (such as vaccinations,
pharmacotherapy and disease management services)
for both convenience and to manage costs. As the
Director of Clinics To Go, I have an extensive background in clinical programming, pharmacotherapy,
and disease state management. I completed clinical
rotations at institutions such as Johns Hopkins, Cedars
Sinai, Mayo Clinic Hospital, and BioWatch within
Homeland Security. I also served as the previous
Director of the Florida Pharmacy Association, Director
of AHCA's Medicaid Retrospective Drug Utilization
Review Program, and as Network Coordinator for two
employer groups in the American Pharmacists
Association's 10 City Challenge. Because I am versed
in health care policy and finances, I am very sensitive
and responsible to budgetary constraints. Therefore, all
services provided by Clinics To Go are at rates that are
desirable to employers and patients. Seasonal flu vaccinations, for example are just $20. Medicare Part B is
accepted as payment in full. HINI vaccine, when
available will be just $10.
If you are an employer or belong to any type of
social, fraternal, church, or hobbyist group/club of
10 or more and you would like to schedule an
on-site vaccination event, know that Clinics To Go is
here for you.

Aerobic or Anaerobic Exercise?
There are two types of exercise that perform different
functions. Aerobic exercise is sustained activity
involving the major muscle groups, such as
swimming, running, or brisk walking. Your heart and
respiratory rate increase, and more oxygen is circulated through the body. This kind of exercise
strengthens your cardiovascular system and increases
your overall strength and stamina.
You've probably heard of "low-impact," or anaerobic,
exercise. This means that you are not exercising vigor0usly or long enough to reach and maintain your
training heart rate. It does not mean, however, that lowimpact exercise is useless. It improves your muscle
strength and flexibility and can still be a good outlet for
negative feelings that you might have bottled up.
There are three kinds of anaerobic exercise:
• Isotonics require that your muscles contract
against a resistant object with movement, such as in
weight lifting.
• Isometrics requires that your muscles contract
against resistance without movement. Isometric exercises increase strength without building bulk.
• Calisthenics are stretching exercises, such as
sit-ups, toe-touches, and knee-bends, help increase
flexibility and joint mobility.
The kinds of exercises you choose to do depend on
your physical ability as well as your preferences. The
most important rule is to choose activities that you
enjoy and that are accessible and feasible for you to
do regularly. Most importantly, develop a routine that
is fun and safe! Enjoy yourselfl
Before you begin an exercise program, you should
have a physical examination. If you are over the age
of forty, your doctor will probably want to do a stress
&

electrocardiogram to determine how much activity
your heart can handle. If you have not exercised regularly for some time, begin slowly with low-impact
exercise and gradually increase your activity.

Revive Yourself with Rejuvenation
I
By Alina Stanciu, MD, FAAO - Board Certified Eye Physician and Surgeon
JuvedermTM &other Fillers
Our most popular wrinkle fillers are Restylane™,
Perlane™, and luvederm™ Over the years these
Hyaluronic Acid Based fillers have proven to be a
safe and effective procedure for the correction of
facial lines and scars. These fillers are used extensively to correct mild to moderate nasolabial folds
and fine lines around the mouth. They are also used
to enhance lip & cheek size and contour, and to
restore facial symmetry.
Improvement with Hyaluronic fillers is temporary
with benefits typically lasting from 6 to 12 months
depending on individual physiology, however new
collagen formation does occur over time, resulting
in gradual permanent reduction of lines, wrinkles,
and scars with regular treatments.
luvederm® Ultra and Restylane are used primarily
for fine lines and wrinkles as well as lip volume
enhancement, while the heavier molecular weight
luvederm® Ultra Plus and Perlane are more effective in treating deeper facial lines as well as volume
enhancement of cheeks.
30 Minute Mini Face Lift
Facial wrinkles and creases begin to appear as we
age. Now, by combining two of the most popular
wrinkle fighters, Botox Cosmetic® and fillers, we
can offer a simple non-surgical alternative to a
facelift with minimal pain, no recovery time, instant
results, and significant financial savings. Botox
Cosmetic® is used to prevent new wrinkles from
appearing while fillers are used to fill the ones
you already have.
BOTOX@ Cosmetic is a simple, non-surgical,
physician-administered treatment that can temporarily smooth moderate to severe frown lines
between the brows in people from 18 to 65 years
of age.
One lO-minute treatment- a
few tiny injections- and within
days there's a noticeable
improvement in moderate to
severe frown lines between the
brows, which can last up to 6
months. There is no anesthesia
required, although Dr. Stanciu may numb the area
being injected with a cold pack or a numbing
cream. There may be minimal and brief discomfort. Most patients compare the injection with the
sensation of a bug bite. There may be temporary
bruising at the injection site, but this can easily be
covered up with makeup.

Blepharoplasty/ Eye Lid Lifts
Eyelid Lifts (technically called Blepharoplasty) is a
procedure to remove excess skin and fat from the
eyelids. Eyelid surgery can correct drooping upper
lids that make you look tired, older or angry. In more
extreme cases it may interfere with your vision.
Eyelid surgery has various functional benefits that
result in the improvement of one's vision. While the
removal of excess skin can have aesthetic benefits,
it can also restore peripheral vision that has been
obscured by droopy, sagging eyelids.
Many purely cosmetic procedures are not covered
by insurance providers because they are elective in
nature. Since blepharoplasty can be performed to
restore vision, many, if not most, insurance providers will typically cover the surgery as a non-elective
procedure. Preoperative testing includes a visual
field and external photos to determine to what
degree the excess skin inhibits peripheral vision and
to establish the true range of vision.

What's
in
a
name?
I
By Joseph Magnant, M.D.
spe-cial-ist
Pronunciation: \_spe-shL -)Iist\
Function: noun
Date: 1855
1: one who specializes in a particular occupation,
practice, or branch of learning
n this day of advances in medical treatments
with improvement and refinement in technology, patients have unlimited access to
health related information in print media,
television networks and through the intemet.
Although the information that is there for the general
public appears to be virtually unlimited, with access
to educational and procedural videos, web lectures
and galleries of pre and post procedure photos,
patients often may find themselves lost in the maze
of choices of providers. The question of who the
best or most well trained physician to see for Botox,
facelifts, foot surgery, vein procedures, or a host of
other clinical entities which might be taken care of
by a variety of differently trained physicians has
become a more challenging one. Advancements in
medical technology have resulted in a natural migration of physicians to greater specialization within
their areas of training to keep abreast of the most
modem treatment technologies.
Online resources are available not only for patient
educational materials but also for the research of credentials and training background of potential
medical providers. A good starting place when
researching specialty providers is their practice
website, it's depth of content as well as the completeness of their training record are important aspects to
review. Try to determine what percentage of the
practice is dedicated to their area of specialty
training and whether this is what you are specifically
interested in. Is the physician completely dedicated
to the area you are interested in? Are they specifically trained in this area and what sort of documentation can they provide to verilY their dedication and
interest in quality of care and patient outcomes? Feel
free to ask for and expect open answers to your questions about the insurance companies with which the
practitioner is "in-network" and if your planned pro-
cedure will be considered medically necessary.
Established patient references may be supportive,
but HTPAA compliance regulations make obtaining
personal references, other than written or video
patient testimonials, difficult. Primary care physician references, even from physicians other than
your own, such as your friends' primary care physician may prove helpful in making the right decision,
as physicians have much broader exposure to
patients with problems similar to yours. One other
and perhaps the most important determining piece
of homework is the free in-office screening. So if
the opportunity presents itself, take full advantage
of it. At the very least you get the chance to kick the
tires of the practice, so to speak. Meet the doctor
and staff, gather information regarding the treatments they offer and any literature regarding the
physician 's training. Check out the cleanliness of
the facility and, at the minimum, leave with an
overall impression of the office, whether positive,
neutral or negative. Any other information you
glean whi le at the screening regarding your specific
condition and the treatment options available should
be considered a bonus.
In summary, do your homework when choosing a
medical specialist professional. Dedicate at least as
much time to making this decision as you do when
you make a major decision like an auto or home
purchase. You should expect a high level of specialization and dedication, professionalism and
personal care from your health care specialist and
their staff. Their training credentials and practice
focus should be readily available and transparent.
Full financial disclosure and honest answers to your
questions regarding network status with your
provider should be offered upfront, either on the
website or in the form of a printed financial disclosure policy of the practice. If you meet a roadblock
when making these types of inquiries, consider this
a red flag. If you have internet access the network
status can also be easily verified on your carrier's
provider site. The last and probably most important
piece of data which should be considered is peer,
physician and word of mouth referral. These are
powerful tools and will provide you with an added
measure of reassurance that you have made the
correct decision. Our community is more intercon-
&
Well ness
29
nected than we fully appreciate and a single inquiry
will often lead to a network of referral opinions and
experiences. The answer to the question "what
would you do if this was your mother or wife?"
should be an easy question for your specialist to
answer, whi le looking at you sincerely in the eyes.
"The same thing as T am suggesting to you, under
similar clinical circumstances".

What is a good definition of love?
treat your spouse when no one is around? This is
not an easy task, but God says He will give us the
strength to do it if we practice biblical love for
His glory, Christ is calling us to practice a sacrificial love that causes us to put our spouse above
ourselves because it glorifies God. "So whether
you eat or drink or whatever you do, do it all for
the glory of God!" (1 Corinthians IO:3!) Our
motivation to love our spouse must he to please
God, Within ourselves we do not have the
strength to love this way, so we must desperately
cling to God, and allow His Holy Spirit to work
through us, God's Word says, "I can do everything through Him who gives me strength,"
(Philippians 4: 13) When we learn how to love
hiblically God is glorified, and our marriage
begins to sing!

growing your marriage or learning how to
live everyday in real love, then please contact

ow that is a good question because
love has been butchered and
watered down to mean nothing
more than a warm and fuzzy feeling found in the pit
of your stomach! The shallow and superficial views
of love pervade the media, entertainment and surprisingly even in the pulpits of many evangelical
churches. Marital couples that are on the verge of
divorce complain that the love is gone. They want
to have that loving feeling back, but it seems to
have left the building. This type of love focuses on
what makes me happy because it is measured by my
feelings , This is not real love, because it is based on
what! desire or want, which is selfishness!
What is real love? Who can be looked to as an
example? Love was never meant to be a feeling, nor
was it supposed to be used selfishly because real
love is SELFLESS! The definition that we give our
counselees at Oasis Counseling Center is: A commitment or decision to do what is best for the other
person actively, sacrificially and unconditionally
without expectation of getting anything in return,
regardless of feelings , to the glory of God. This
definition of real love was revealed in it's entirety
when Christ submitted to God and gave His life
for us! "For God so LOVED the world that He
gave His one and only Son! " (John 3:16a) God's
love was expressed in a decision to sacrifice His
Son on our behalf, It was not something that made
Him feel good, but He did it because He loved us.
Similarly, Christ didn 't want to face the cross
either, but He did it anyway, because of His love
for the Father and for us!
Biblical love in its simplest form is to GIVE, That
means we are supposed to give everything we have
to our spouse. This type of love means we suffer as
our Savior suffered, Christ tells us that, "if anyone
would come after Me, he must deny himself and
pick up His cross daily and follow Me," (Luke
9:23) The cross was the Roman instrument of death
to kill and torture the common criminal. Christ said
to carry your cross daily or die everyday to the
self-oriented-life, When Jesus said this He was not
suggesting it, but expected His followers to have
this mindset of dying to self. Our marriages must
keep at the center of this perspective to foster real
love. You might evaluate your perspective on your
marriage by looking at who you are at home with
your spouse, It is easy to be the loving spouse in
front of others, but how do you
