Dear Colleagues:
On behalf of the organizing committee, I heartily welcome you to the
13th Asia-Oceania ORL-HNS Congress on March 19-22, 2015 in Taiwan.
The meeting will be hosted by Asia-Oceania Association of Otolarynology-Head
and Neck Surgery Societies, Asian Paediatric ORL Group (APOG), Taiwan
Otolarynological Society, and jointly sponsored by American Academy of Otolarynology- Head and
Neck Surgery.
Asia-Oceania Association of Otolarynology-Head and Neck Surgery Societies has been recognized by
IFOS as the official society to represent Asia-Oceania. Asia-Oceania ORL-HNS Congress is the major
ENT scientific event in Asia-Oceania region. The meeting was held in different city/country in every
four years since the initiation and establishment of the society in Korea, 1965 and the first scientific
meeting in Manila, Philippine in 1967. It has been more than 40 years since Taiwan host this meeting
in November, 1971. Truly, we are so eager to invite you to visit Taiwan and will be even more proud
to show you what we have achieved in this beautiful island of Taiwan along the years.
The main theme of the 13th Asia-Oceania ORL-HNS Congress will be “Asia Oceania First”. The first day
(March 19, 2015) of this meeting will be numerous instruction courses and workshops to cover some
of the cutting edge knowledge and techniques in Otolaryngology-Head and Neck Surgery. The other
two and half days (March 20 – 22, 2015) will be formal scientific meeting including keynote lectures,
great debate, round table, panel discussion and oral or poster presentations. The scientific program
will be covering nearly every aspect in Otolaryngology, Head & Neck Surgery including head & neck,
skull base, otology, audiology, rhinology, facial plastics, laryngology, sleep medicine, speech &
swallow medicine, pediatrics ENT、general ENT as well as basic research, et al.
In addition, we sincerely offer you some unique social program and post-congress tour. We strongly
encourage you to take advantage of this opportunity to explore the numerous attractions and enjoy
the all-hearted welcome from people in Taiwan. We believe that you will not only have updated

On behalf of the Asia-Oceania Council, and the team in Taiwan, I am pleased to
invite you to this prestigious scientific meeting - the 13th Asia Oceania ORL-NHS
Congress – to be held in in Taipei, March 2015. Professor Sheng-Po and his
Committee have worked very hard to ensure a high-quality and highly relevant
program
The Asia-Oceania Association of ORL-HNS Societies was founded in 1965 in Korea.
The first meeting was in Manila in 1967. From the beginning, the Asia-Oceania
ORL-HNS Congress has a reputation of being a friendly and fulfilling meeting, and
while I am sure this congress will stand out in the same way, the signs are that it
will be the best yet!! We all recognise that the hospitality of the people of Taiwan is legendary, so we
can be sure that there will be not only a first-class academic content, but also a wonderful social and
collegial experience.
I am especially drawn to the theme for this meeting: ”Asia-Oceania First”. We are all aware of the
emerging prominence and importance of Asia-Oceania in world affairs, and increasingly this is being
manifest in Medicine in general and Otolaryngology in particular. This meeting is a prime opportunity
for one to be involved in this “new” frontier for clinical and academic Otolaryngology.
I want to sincerely thank the Taiwan Otolaryngology Society and the Asian Paediatric Otolaryngology
Group for endorsing and supporting this meeting. Their involvement ensures the success of the
meeting, and should provide an added stimulus for people to attend, both from Asia and around the
world. I would encourage you all to not only consider registering for this special meeting but to also
submit an abstract of your work so that we all may have that added opportunity to learn.
My thanks also go out to everyone that has helped to bring us to this point. The strong positive
collegial vibes emanating from Taiwan signal to me of a happy and hard-working and effective group
that believe in this Congress. This is a recipe for great success. Please make the most of this chance

Dear Colleagues and Friends,
Welcome to Taipei, the Capital of Taiwan, for the 13 th Asia-Oceania ORL-HNS
Congress. This Congress will be held in Taipei International Convention
Center, Taipei, Taiwan, from 19th to the 22nd March 2015.
We are glad that after quite long years Taiwan Otolaryngological Society
again has the privilege to host this big event. It has been 44 years since many Otolaryngologists from
all over the world joined together in the 2 nd Asia-Oceania Otohinolaryngology-Head and Neck Surgery
Congress in Taipai in 1971. And this will be the second time we are to host this Congress. We feel
both honored and grateful. We will go all out to make this Congress a bountiful and memorable one,
like the previous ones!
Taipei is a multi-cultural city, famous for its various fusion good foods, relaxing subtropical scenery
and friendly people. Traveling is easy and enjoyable here, because it is a safe city and the public
transportation facilities are convenient.
Though all the metropolitans share similarities, still, when making a turn around the street corner,
we can always find surprises and unique traits belonging only to this city.
The Asia-Oceania Otohinolaryngology-Head and Neck Surgery Congress is one of the most important
platforms for all the Otolaryngologists to share and discuss study results as well as individual career
experiences. Professor Hao and all the members in the Organizing Committee are doing their best to
make this Congress a successful one.

Dear Colleagues and Friends,
On behalf of the Asian Paediatric Otolaryngology Group (APOG), I would
like to express my warmest welcome to the invited speakers and all
distinguished guests and participants to attend the 5 th Asian Paediatric
Otolaryngology Congress organized in collaboration with the
13th Asia-Oceania ORL-HNS Congress. The Asian Paediatric Otolaryngology
Group (APOG) is honored and privileged to be involved with Asia-Oceania
Association of Otolaryngology Societies. Our gratitude and sincerely thanks going to all the members
in the Organizing Committee and Taiwan Otolaryngology Society to the support in developing
Paediatric Otolaryngology in Asian-Oceania region. Besides, I would like to express my special
appreciations to all the founding members of the Asian Paediatric Otolaryngology Group who have
contributed to this group and Congress.
The APOG was formed to engage otolaryngologists who are interested in the care of ear, nose, throat
and head & neck diseases in children since 2007. After Dr. Gretchen Navarro Locsin convened the
first Asian Paediatric Otolaryngology meeting in Manila, Philippines, we have biannual conference
rotating among the board member countries. The 2 nd APOG conference was hosted by Dr. Retno S.
Wardani in Jakarta, Indonesia in 2009. The 3rd APOG conference was hosted by Dr. Henry Tan in
Singapore in 2011. The 4th APOG conference was hosted by Dr. Siti Sabzah Mohd Hashim in Kuala
Lumpur, Malaysia in 2013.
On behalf of the APOG with great honor, the 5 th Asian Pediatric Otolaryngology Conference will be
hosted by Taiwan in 2015. We do hope that the APOG can promote clinical and basic research in
pediatric otolaryngology by sharing and discussing clinical information and unique characteristics in
Asia-Oceania areas with different climates and cultures from other regions of the world. Therefore,
under the main theme “Asia Oceania First”, we sincerely invite all doctors interested in Pediatric
Otolaryngology to join us and visit Taipei, Taiwan in spring season in March, 2015.
Last but not least, I wish all delegates a successful meeting and memorable stay in Taipei under
special Taiwanese hospitality.

Summary of the course:
This is a two-day lectures and workshop, which covers common and important aspects of
paediatric airway disease that are relevant to surgical as well as medical disciplines such as
otolaryngology, pediatric medicine, emergency medicine, intensive care medicine and anesthesia.
For the surgical disciplines like otolaryngology and pediatric surgery, a special emphasis on various
aspects of pediatric bronchoscopic procedures and techniques will be demonstrated.
This will be further augmented with hands-on bronchoscopic techniques on dogs and live surgery
demonstration on paediatric patients.

Abstract: The risks inherent in paediatric airway surgery will be discussed in the context of
experience gained over more than 30 years.
Areas of risk management include outpatient assessment, preoperative precautions,
endoscopic procedures, open airway surgery, postoperative care and home care. Risks and
their management will be discussed for each area, with the emphasis upon avoiding risk rather
than dealing with complications when they arise.
Successful risk avoidance is born out of long experience with a large number of cases, and it is
essential to share the wisdom accumulated in this way for the benefit of all our patients.

Abstract: Obstructive sleep apnea (OSA) is presenting with recurrent upper airway
obstructions and desaturations during sleep terminated by arousals, leading to increased
daytime sleepiness and risk for accidents. OSA causes elevated sympathetic tone associated
with arterial hypertension and vascular inflammation. An increased risk for cardio- and
cerebrovascular incidents has been shown.
Nasal CPAP-therapy is able to normalise sleep and breathing, restore sleep, and reduce
cardiovascular risk. However, 20-50% do not use CPAP sufficiently.
In these patients surgery aims to improve CPAP compliance. If CPAP use is definitively
impossible or in mild OSA patients without CPAP trial surgery must eliminate or at least reduce
OSA, but also snoring. Even though surgery positively impacts sleep quality, daytime sleepiness,
and cardiovascular risk its extent of improvement becomes more and more clear as
well-designed studies have been published recently.

Complication of Cochlear Implant Surgery, Prevention and Management

Abstract: Background: Cochlear implantation (CI) has become a standard means of auditory
rehabilitation for profound sensori-neural hearing loss. As operation numbers increases, there
have been a number of reports of surgical complications after cochlear implantation. This
study was performed to report complications of cochlear implantation and its management.
Methods: Retrospective study in a tertiary referral cochlear implant centre. This study included
1,682 patients (1,242 children / 440 adults) who underwent cochlear implantation from Nov.
1988 to Dec. 2014.
Results: The rate of major complications, which required special treatment regimens or
revision surgery, was 5.5% (92/1,682). The most common cause of major complication was
device failure (47/1,682, 2.8%), then followed by major infection related complications
(20/1,682, 1.2%). Analyzing the rate of major infection related complications according to the
period of CI, the rates were 5.77% (3/52), 1.05% (6/569) and 1.04% (11/1061) in first, second
and last 9 years, respectively. The rates of major infection related complications in second and
last 9 years were significantly reduced compared to that in first 9 years (p = 0.032 and 0.024,
Fisher's exact test). A device was removed to control infection in 11 cases (11/20, 55.0%) and
methicillin-resistant Staphylococcus aureus (MRSA) was detected in 6 cases (6/20, 30.0%).
Devices were removed in all 6 cases in which MRSA was detected. The other major
complications included: magnet displacement in 6, misplaced electrode in 4, hematoma in 4
and others in 11. The rate of minor complications was 10.0% (168/1682) including vertigo in 54
patients (32.1%, 54/168), tinnitus in 22, non-surgical hematoma in 20, non-surgical seroma in
20, facial nerve stimulation in 16 and others in 36.
Conclusions: The major and minor complication rates were about 5% and 10%, respectively.
Cochlear implantation seemed to be the safe and effective surgical operation. Nevertheless,
advances in surgical techniques and the optimization of treatment modalities would further
reduce complications.

TransOral Robotic Surgery in Otolaryngology – HNS: Present and Future

Abstract: TransOral Robotic Surgery (TORS) is a novel series of minimally invasive surgical
procedures that offers technical advancements to existing endoscopic and transoral surgical
approaches for treating both malignant and benign tumors of the oropharynx, hypopharynx,
nasopharynx, parapharyngeal space and larynx. The development of TORS originating from Drs.
O’Malley and Weinstein at the University of Pennsylvania (now with over 2500 cases of
personal experience) has provided a platform of new surgical techniques to achieve less
morbid and potentially more effective overall management of head and neck tumors. The
development of TORS began as a prospective human clinical investigation at PENN and then
was subsequently evaluated at multiple U.S. institutions as part of a national clinical trial to
evaluate its efficacy and safety. Based on the results of the national trial, in December 2009
the U.S. Federal Drug Administration (FDA) approved the use of the da Vinci® surgical system
and TORS for selected malignancies and all benign tumors of the head and neck. The largest
experience and longest follow-up has been in the use of TORS for the management of
oropharyngeal carcinomas where comparable to improved oncologic outcomes, improved
quality of life, and reduced long-term feeding tube dependency and long term morbidity rates
have been reported. Experience to date has demonstrated that the excellent
three-dimensional visualization, bimanual dexterity, curved instrumentation, and the use of an
assistant surgeon that is afforded by the robotic system are key factors in the improved
oncologic resection, ease of resection, and the overall reduced morbidity. In addition, TORS is
now being investigated for its benefit in treating sleep apnea patients with a focus on tongue
base and lateral pharyngeal wall resection. Based on present studies and outcomes data in
conjunction with ongoing investigations and the enthusiastic response from both U.S. and
International Head and Neck Surgeons for training and developing TORS programs, it is
anticipated that TORS will make a major world-wide impact in the way we manage benign and
malignant tumors within the head and neck and skull base as well as for sleep apnea.

Endoscopic Anterior Fossa Tumor Resection (201DE)
Abstract: As ESS has progressed over the last 10 years, new techniques have been introduced
to aid with the resection of tumors in regions which traditionally have been difficult to access
or have required significant resection of normal tissue to access the tumors. In general the
approaches described in this talk are suitable for tumors involving the anterior skull base. To
assess the endoscopic resectability of a tumour, both CT and MRI scanning is necessary. By
being able to accurately define the extent of the tumor, resection can be carefully planned.
Planning of such a resection requires a detailed knowledge of the anatomy of the anterior skull
base. This will form a major part of this lecture. Illustrative cases are presented with video
clips showing the anatomy and tumor resection of these areas.

Challenging the Paradigms in the Management of Thyroid Cancer (103)

Abstract: The incidence of Thyroid Cancer is rising worldwide. However, majority of newly
diagnosed thyroid cancers are small, low risk differentiated cancers, with a uniformly excellent
outcome. Employing the established paradigms of thyroid cancer management, leads to a
tremendous burden on the health care systems, and results in gross over treatment of many
patients without any benefit to them. This presentation will shed light on challenging the
standard routines in thyroid cancer management, and provide data to employ a risk
stratification strategy to deliver cost effective care.
The following issues will be addressed in this presentation:








Age 45 is the appropriate cut off for worsening prognosis
T1b tumors do worse than T1a tumors
Size adversely impacts on prognosis even for intrathyroidal tumors
All patients with differentiated carcinoma need total thyroidectomy
Completion thyroidectomy is required after diagnostic lobectomy
Elective central node dissection is beneficial to the patient
All patients need RAI ablation after total thyroidectomy
Even microscopic extra thyroid extension has adverse prognosis

Multifocal papillary carcinomas have worse outcomes
Risk group stratification should not affect initial extent of surgery
All patients should have TGb and US every six months after surgery
All nodes seen on ultrasound should be biopsied
All papillary carcinomas need treatment, regardless of its size
Understanding the biology of differentiated thyroid cancer, and its prognostic factors, allows
risk group stratification even prior to treatment, and permits selective surgical treatment, and
post surgical management to deliver cost effective care. It also avoids over treatment in a
significant number of patients, and most importantly does not harm the patient.

Surgical Management of Eustachian Tube Dysfunction (201AF)

Abstract: There is increasing evidence that the middle ear and mastoid space is physiologically
analogous to an accessory sinus, which shares similar respiratory mucosa and pathology with
the rest of the nasal cavity and sinuses. The special sensory functions of the ear are optimized
and protected by the Eustachian tube, which serves as a unique, very long and
dynamic ”ostium.” The cartilaginous portion contains a functional valve that must open
periodically to allow for ventilation and clearance of secretions. It must close competently in
order to prevent reflux of material and sounds from the nasopharynx. Failure of these one or
more of these three functions, ventilation, clearance and prevention of reflux, constitutes
Eustachian tube dysfunction (ETD), which can be subdivided into dilatory or patulous
dysfunction. The most common etiology of dilatory ETD is inflammation within the
cartilaginous portion causing a functional obstruction. The initial management of dilatory
ETD should be to try to identify its source and treat the inflammation appropriately if possible.
In children, dilatory ETD is commonly associated with otitis media with effusion (OME) and the
risk factors for childhood OME include exposure to public day care, second hand smoke,
feeding in the supine position, second-hand smoke, reduced duration of breastfeeding,
exposure to known allergens and exposure to other children with OME.
In adults, dilatory ETD may be associated with allergic rhinitis, chronic rhinosinusitis,
laryngopharyngeal reflux or a family history of otitis media.
In children or adults with dilatory ETD despite optimal management of the underlying
etiologies or risk factors, tympanostomy tubes can be very effective in relieving the symptoms.
When dilatory ETD persists following extrusion of tubes, consideration may be given for repeat
placement of tubes, adenoidectomy if appropriate and the option of Eustachian tuboplasty is
now being employed for ages seven and above. Balloon dilation is the most commonly used
technique. Indications, techniques, pitfalls and results will be discussed.

The Changing Landscape of Parotid Surgery (201BC)
Abstract: Over the past 30 years we have witnessed a series of changes to the way in which
pathology of the Parotid gland has changed, driven in part by technological advances, and in
part by innovative approached to surgery.
The philosophy of minimally invasive surgery has been pursued, both for non-neoplastic
disorders such as ductal stenosis and sialolithiasis, and for ‘benign’ neoplastic disease. The
former has been transformed by the introduction of sialendoscopy; the latter by the
demonstration of the efficacy of directed, partial resection of parotid tissue.
Overall the above changes have resulted in preservation of parotid gland tissue while
successfully managing and controlling the pathologic process. Low morbidity form surgery has
generally been sustained.
This presentation describes the experience of the author and others in the pursuit of the
principle of organ preservation, and describes the author’s own preferred technique of
retrograde facial nerve dissection for partial superficial Parotidectomy (PSP), effectively
resulting in a hybrid of PSP and extra-capsular dissection (ECD).
Other changes relate to the sophistication of organ imaging and how pre-operative
information can be enhanced, and to the development of immune-histochemistry and

Abstract: As we plunge deeper and deeper into the sterile, algorithmic world of
evidence-based medicine (EBM), it helps to keep in mind these shrewd words from Sir William
Osler: “Common sense in medical matters is rare, and is usually in inverse ratio to the degree
of education.” Since the most vocal proponents of EBM are usually quite educated, and
often divorced from the day to day realities of clinical care, common sense may be an
uncommon component of their advice and recommendations. This presentation seeks to
balance the virtues of EBM with the perennial importance of plain, old-fashioned common
sense, as it applies to the care of children with ear, nose, and throat problems. We begin
with the importance of avoiding harm (and adverse events) and the need to define benefits,
and safety, clearly and honestly. Next, the virtues of humanistic care, which creates a
placebo response, are discussed alongside the need for accepting uncertainty. The goal is to
present a way of thinking about EBM that facilities best practices, humility, lifelong learning,
and – most importantly – common sense.

The Training of a Modern Head and Neck Surgeon (201BC)

Abstract: The training of a modern head and neck surgeon is a challenging task. However, as
in most subspecialties, the individuals are all well motivated having self-selected further
training despite the fact that they have sacrificed at least one year and often longer of their
professional career, lost income and give up a substantial amount of time with their families.
The best candidate for fellowship training in head and neck surgery is the one who has
substantial experience with head and neck surgery as a resident. The individual who is a
“remedial case” with little experience in head and neck as a resident requires a great deal of
time, effort and patience from the faculty. Most fellowship training programs depend upon
the Fellow to help teach the residents especially in the operating room. If the fellow has
insufficient training as a resident this is not a safe and workable system.
This Plenary Lecture will describe the many qualifications for training as a modern head and
neck surgeon. It will rely heavily upon current requirements specified by the American Board
of Otolaryngology, Residency Review Committee and Joint Council for Training in Head and

Neck Oncologic Surgery. The training program is a key element in maintaining a high standard
of excellence in head and neck surgery.

Evidence Based Recommendations in Management of Chronic Sinusitis (201DE)
Abstract: Treatment options for the management of chronic rhinosinusitis (CRS) have
traditionally relied on the clinical experience of physicians with little substantive evidence to
support their use. While some of these options are quite effective, others have been
ineffective at best and dangerous at worst, with distinguishing between the options difficult.
However, using the principles of Evidence Based Medicine over the last 10 years,
otolaryngologists have begun to better understand the treatment options in CRS and define a
set of best options for management. In this presentation, the author will present the current
state of the literature using these principles, providing the participant with understanding of
best evidence with regard to CRS management.

Abstract: The past 3-4 decades have seen the development of a unique practice of medicine,
to advance the evaluation and treatment of the elite vocalist and singer. The optimal care is
delivered in a multidisciplinary fashion that involves voice experts from many diverse
disciplines. Although this largely revolves around otolaryngologists (ENT specialists),
speech-language pathologists, and voice and music teachers, in many environments other
pedagogues and voice scientists play important roles in this care. Much of the efforts have
been dedicated to prevention of injury and optimization of environments and techniques that
facilitate the efficient and safe production of voice. Despite these efforts, injuries do occur.
That is when the voice team plays a more active treatment role.
This expansion of voice medicine and growing interests in diseases and disorders of the larynx
has led to a rapid escalation of the development of tools and technology to evaluate the voice
and larynx and innovative treatment regimens. Perhaps the most impactful was the
development of affordable, reproducible and technically simple means of evaluating vocal fold
vibration afforded by videostroboscopy. The past few years have seen multiple new
innovations in both assessment tools and treatment capabilities that not only facilitate better
treatment, but also provide for less expensive and more accessible care for the singer. This
presentation will introduce many of the innovations and new developments in technologies,
procedures and techniques that may be utilized in the evaluation and treatment of the injured
singer. Although the focus will be primarily on available technology that is in at least limited
clinical use throughout the voice treatment community, cutting-edge innovative technology
will be presented.

Diagnosing Otolith Dysfunction - Vestibular Evoked Myogenic Potentials

Abstract: Background: The usual stimuli for studying the function of the utricular and saccular
maculae have been maintained (static) linear accelerations from tilts or centrifuges or linear
sleds. New clinical tests use dynamic otolithic stimuli - bone-conducted vibration (BCV) and
air-conducted sound (ACS) - and measure ocular vestibular evoked myogenic potentials
(oVEMPs) from surface electrodes beneath the eyes or over the tensed sternocleidomastoid
muscles (cVEMPs). What is the justification for using ACS and BCV to diagnose otolithic
function?
Methods: Single primary otolithic neurons were recorded extracellularly in guinea pigs
anesthetized with Ketamine and Xylazine and their responses to BCV stimulation was delivered
by a Radioear B-71 bone oscillator cemented to the skull or a Bruel and Kjaer 4810 mini shaker
and ACS stimulation of up to 140dB SPL was delivered by a TDH-49 headphone.

Results : BCV and ACS both activate individual otolithic afferents with an irregular resting
discharge from both utricular and saccular maculae. Neurobiotin labelling showed that the
neurons formed calyx synapses on type I receptors at the striola of the utricular or saccular
macula.
Primary utricular and saccular afferents neurons have similar very low thresholds of around
0.02g for BCV from 100Hz up to about 900Hz. For ACS utricular neurons have a V-shaped
tuning curve with lowest thresholds (around 100dB) at 1000-2000Hz. Saccular afferents are
similar but with a lower threshold by about 15-20dB. Individual otolithic neurons show phase
locking for ACS up to 3000Hz (and possibly beyond), and for BCV up to 1500Hz. This tight
phase locking shows that each cycle of the sinusoid is the effective stimulus.
Discussion and Conclusion: We have proposed a dual mode of otolith operation. At low
stimulus frequencies (up to about 230 Hz) the standard otoconial gel-layer mechanical
operation is probably dominant. At higher frequencies this conventional mechanical
operation is bypassed and we suggest that fluid pressure waves generated by ACS or BCV
directly activate the short, stiff striola hair bundles up to high frequencies (even 2000Hz) with
the result that both ACS and BCV cause phase-locking of irregular primary afferents and so
both ACS and BCV are effective stimuli for tests of otolith function. Recent recordings of
intracellular potentials in Type I receptors show a very large response at the onset of a hair
bundle deflection and then very rapid adaptation. This is evidence that Type I receptors and
their irregular afferents are tuned to changes in linear acceleration. On this account, the
different frequency responses to BCV and ACS are most likely due to the different paths of the
fluid pressure waves from the stapes to the saccular and utricular maculae.
Implications for clinical testing:
1. The old assumption that ACS only activates saccular afferents in the inferior vestibular nerve
is not correct: saccular afferents are activated by BCV and utricular afferents are activated
by ACS.
2. Differential evaluation of utricular and saccular function is possible by measuring different
responses governed primarily by each macula.
3. Since these afferents are tuned to changes in linear acceleration, the onset of the stimulus is
vital – the ideal rise time is 0ms and the stimuli can be very short.
4. Dehiscence will enhance neural responses because the fluid pressure waves will be larger
These implications are confirmed by recent evidence.

Abstract: Management of early papillary thyroid cancer is still on debate in terms of screening,
timing of fine needle aspiration and operation, extent of thyroidectomy and lymph node
dissection.
Incidence of thyroid cancer has been increasing due to worldwide increment of carcinoma less
than 1cm. Recently, in Korea, screening by ultrasonography for thyroid gland is prevalent
resulting fastest increase of thyroid cancer incidence comparing to other countries. Although
screening is playing important role in this “epidemic”, true increase in incidence of thyroid

cancer has been proven by several studies and early detection may be beneficial for decreasing
the severity of disease, aggressiveness of treatment, and the recurrence rate. Therefore, we
need higher level of evidence elucidating the effect of thyroid cancer screening while
refraining from undiscerning screening.
Recently, American thyroid association released the revised guideline for differentiated thyroid
cancer. The indication of fine needle aspiration and extent of surgery was partly changed in the
direction to decrease the intervention. Active surveillance, the observational study for
papillary thyroid carcinoma, has been performed by several investigators. Growth rate of early
thyroid cancer without any unfavorable features was as low as 5% and novel lymph node
metastasis and disease specific mortality was not significantly different from surgical
treatment. It also seems that active surveillance is cost-effective comparing to thyroid surgery
and show no critical difficulty when observation was converted to operation. Now several
institutes are preparing for clinical trials. Although this protocol looks promising for early
thyroid cancer, further research is necessary for clear defining of indication and strategy of
active surveillance.
Fine needle aspiration for thyroid nodule is not recommended anymore for the nodules less
than 1cm without suspicious findings on ultrasonography. When performing thyroid surgery,
risk stratification for each patient is important in deciding the surgical extent. Lobectomy is
generally recommended when tumor size is less than 4cm without extrathyroidal extension or
lymph node metastasis, because the current studies had shown that the survivals after total
thyroidectomy and hemi-thyroidectomy were not significantly different if tumor size is as large
as 4cm. Quality of life after surgery is more emphasized, thus the complication is another key
factor that deciding the surgical extent. Compartmental lymph node dissection is essential for
clinically node positive patients. Indication of prophylactic central neck dissection and
completion thyroidectomy is still on debate.
In summary, more research is needed for elucidating the indication and extent of surgery,
values of molecular markers. Treatment strategy should be based on the personalized risk
stratification and postoperative quality of life.

Innate and Acquired Immunity in Nasopharyngeal Mucosa and its Clinical

Abstract: Various categories of sinonasal allergic or infective inflammation such as allergic
rhinitis and rhinosinusitis have been recently rearranged to have new nomenclatures based on
phenotype or endotype of these disease entities. These studies were very best performed by
a number of basic researchers and clinical investigators in line with the development and
paradigm shift of modern immunology. Nowadays, the immune reaction can be categorized
as an innate immunity and acquired immunity, according to the antigen specificity, various
receptors on immunocompetent cells and the necessity of clonal expansion. Toll-like
receptors (TLRs) expressed in various immunocompetent cells play an important role in a
defense mechanism against microbial infection and also posess various regulatory functions to
avoid excessive or persistent inflammation. For the last decade, our research team connecting
with a couple of other research institutes, have been working on the role of innate immunity
and acquired immunity of nasopharngeal mucosae upon the regulation of the upper

respiratory tract inflammation. Therefore, in my lecture, I would like to introduce three
issues, which are described in the followings and might give us a translational approach to
regulate the long-lasting inflammatory process in upper respiratory tract.
Role of TLRs and its modification with agents in chemokine/cytokine induction and intracellular
singaling pathway in the upper respiratory tract and possible role of mucin and mucin gene in
controlling excessive and prolonged inflammation in respitory tract mucosal linings.

Nerve Muscle Pedicle Flap with Arytenoid Adduction for Unilateral Vocal Fold

Abstract: Improvement in breathy dysphonia due to unilateral vocal fold paralysis (UVFP) is
usually achieved by implementing various kinds of phonosurgical procedures, including
intracordal injection, type I thyroplasty, arytenoid adduction (AA), and combinations thereof.
However, although postoperative voice improves compared with pre-operative voice, some
patients do not recover ‘normal’ voice of each patient before the occurrence of UVFP. Normal
voice can be attained by providing the immobile vocal fold with the median location and the
symmetrical bulk and tension of the unaffected vocal fold. Because the phonosurgical
procedures offer “static” adjustment of these features, the thyroarytenoid (TA) muscle does
not work as the body of the immobile vocal fold, resulting in little contribution to voice
production and tuning.
To restore tonus of the immobile vocal fold, direct anastomosis of the severed ends of the RLN,
ansa cervicalis nerve (ACN) to RLN anastomosis (nerve transfer), and nerve-muscle pedicle
(NMP) flap implantation to the TA muscle were reported to reconstruct innervation of the TA
muscle. The former two methods at the time of tumor extirpation have been popularized to
achieve nearly normal postoperative voice. However, no consensus has been reached
regarding the method to achieve effective reinnervation of the TA muscle in patients with
persistent UVFP. The authors proved that our refined NMP method is useful in establishing
reinnervation of the TA muscle and that the method can provide patients suffering from
breathy dysphonia due to UVFP with ‘pre-paralysis’ normal voice when combined with AA.
In this lecture, firstly, various laryngoplasty methods are reviewed. Then, denervation effects
on the TA muscle and effects of nerve-muscle pedicle (NMP) method on reinnervation of the
TA muscle (muscle fibers, nerve fibers within the muscle, and neuromuscular junctions) are
shown as the scientific basis for clinical application. Thirdly, our refined NMP method
combined with AA is displayed utilizing motion pictures. Finally, vocal function and
stroboscopically observed vocal fold vibration over two-year follow-up period are presented.
In conclusion, firstly, our goal to treat paralytic dysphonia is to recover ‘normal’ voice of each
patient. Secondly, NMP flap implantation combined with AA provides an excellent vocal
function. Thirdly, precise formation and implantation of a NMP flap under a microscopic
guidance are key techniques in the procedure.

Abstract: Morphological studies of the inner ear concern anatomy, pathology and treatment of
diseases. The membrana limitans separates the perilymphatic space into the pars superior and
inferior and functions as a barrier, preventing movement of substances across the membrane.

The perilymph and endolymph are separated by Reissner’s membrane and the organ of Corti.
Previous studies have shown that when the stria vascularis is destroyed by a laser beam,
Reissner’s membrane extends and repairs the area, and when the organ of Corti becomes
hydropic with loss of cells, its surface remains as a thin layer.
Capillary networks of the membranous labyrinth can be demonstrated by alkaline phosphatase
staining. The networks are dense underneath the vestibular sensory epithelium but poor
underneath the organ of Corti. In older patients, the distribution of capillaries in the osseous
spiral lamina of the lower basal turn is less dense than that in the upper turns.
Histopathologic examination of temporal bones from patients with sudden deafness indicates
that the changes in the tectorial membrane are similar to those induced by viral infection.
Experimental viral labyrinthitis using herpes simplex virus showed a rolled-up tectorial
membrane with the presence of viral antigen.
Perilymphatic fistula is not simply characterized by leakage of perilymph from the cochlea.
Experimentally produced perilymphatic fistula exhibits various changes in the membranous
labyrinth, such as the formation of endolymphatic hydrops in the cochlea and collapse of the
vestibular labyrinth. Loss of the trabecular mesh and drooping of the collapsed wall onto the
sensory epithelium are likely to cause dizziness.
Argon laser irradiation of the utricular and saccular maculae can selectively destroy the
sensory epithelium. Irradiation of the semicircular canal constricts the semicircular duct.
Intractable benign paroxysmal positional vertigo can be treated by laser irradiation.

Oropharynx Cancer: Evolution of Treatment over 30 Years (201BC)
Abstract: Treatment for cancer of the oropharynx has evolved greatly over thirty years.
Previously a disease linked to smoking and alcohol use a new disease has emerged. 30-years
ago the treatment was radical surgery followed by radiation therapy. Survival and functional
outcomes were poor even as we were better able to reconstruct surgical defects. Due to lack
of improved outcomes with surgery and radiation, organ preservation approaches emerged
with the use of combined chemotherapy and radiation. We are now entering a new era of
disease management due in part to many emerging and interrelated factors.
Oropharyngeal cancer is rapidly increasing in western countries. As the use of tobacco
products has declined, there has been the emergence of human papillomavirus associated
cancer of the oropharynx, primarily originating in the tongue base or tonsil. The demographics
of this disease have dramatically changed as well. In the prior era of heavy tobacco use these
patients tended to be male in their sixth or seventh decades of life. With the emergence of
HPV related oropharyngeal cancer, patients are presenting at a younger age, principally in their
forties and fifties and predominantly male. Risk factors for this disease include; oral genital
contact, number of lifetime sexual partners and the use of marijuana. Typically the patients
present with a neck mass which upon investigation proves to be metastatic squamous
carcinoma, generally considered of unknown origin. Further imaging and direct visualization of
the oropharynx will frequently demonstrate the primary site in the base of the tongue or the
tonsil. Nevertheless an occult disease can exist in the lymphoid tissue of the tonsil or the base
of tongue and escape detection without careful inspection and directed by biopsies and/or

tonsillectomy. Prognosis for this disease is favorable with current treatment strategies that
include; radiation alone, chemotherapy and radiation, or transoral surgical resection. The role
of transoral surgery for these tumors is evolving with the principal rationale being to decrease
the dose of radiation required as an adjuvant or to avoid combining chemotherapy and
radiation which can have long term side effects or function. This presentation will review the
evolution of treatment of oropharyngeal cancer and current therapeutic approaches based on
the results of clinical trials.

Revisiting the Inner Ear Disorders via an Inner Ear Test Battery (201AF)

Abstract: Stimulation via air-conducted sound or bone-conducted vibration enables recording
of vestibular-evoked myogenic potential (VEMP) from cervical muscles (called cervical VEMP,
cVEMP) and extraocular muscles (called ocular VEMP, oVEMP). These two emerging tests
expand the test battery available to clinicians for exploring dynamic otolithic function, and
create a potential use for the sacculo-collic reflex and vestibulo-ocular reflex, respectively.

Coupled with audiometry and caloric test, the inner ear test battery is designed for complete
assessment of the inner ear function including the cochlea, saccule, utricle and semicircular
canals.
Clinically, the inner ear test battery has been widely adopted in cases of peripheral vestibular
diseases such as 1) chronic otitis media with vertigo; 2) radiation-induced otitis media with
vertigo; 3) otosclerosis with vertigo; 4) acute acoustic trauma or noise-induced hearing loss; 5)
vestibular inflammatory diseases i.e. vestibular neuritis, herpes zoster oticus; 6) sudden
deafness; and 7) Meniere’s disease.
In central vestibular disorders, the inner ear test battery may help map the affected territory in
cases of 8) vestibular migraine; 9) posterior fossa stroke; and 10) posterior fossa tumor. The
use of oVEMP and cVEMP tests in vestibular schwannoma patients helps assess the tumor
origin from the superior or inferior vestibular nerve. For the brainstem lesion, the oVEMP and
cVEMP tests can discriminate it. Additionally, the oVEMP test can differentiate between
cerebellar and brainstem lesions. Abnormal oVEMPs in patients with cerebellar disorder may
indicate adjacent brainstem involvement.
In conclusion, it is believable that complete assessment of the inner ear function via an inner
ear test battery comprising audiometry, oVEMP, cVEMP and caloric tests may stimulate to
elucidate the mechanism of peripheral and central vestibular disorders.
