Construction of Clear water 3 million gallon water tank. Work on 3 million gallon clear water tank in progress. Non-uniform spacing between the reinforcement bars observed ii. Concrete spacer were not provided to maintain uniform concrete cover Concrete strength was checked by concrete test hammer found as per specification Construction of Staff Quarter and Rest House: Construction of staff quarter Page 5 ofand 33 rest house in progress Concrete strength was checked by concrete test hamer found as per specification Efflorescence observed in brick masonry Quality of brick was not found satisfactory Non-uniform concrete cover observed CONSTRUCTION OF INTERMEDIATE PUMPING STATION, BALK STORAGE TANK AND ALLIED WORKS AT KHALIQABAD. SEWERAGE i. Waste Water Treatment Plants Page 14 of 33 Treatment plant No 1 at industrial area under construction for which only brick lining completed The concrete strength of drain covers was checked found as per specifications ii. Barbed wire has not been provided resulting in damage caused by infiltrators. Septic Tank Quality of work done of Septic tank at Kharik road was found not satisfactory. SEWERAGE PIPE 9 inch dia sewerage pipe was broken for checking reinforcement. Nnumber and spacing of longitudinal bars and rings was checked and found as per specification. Cross section of 9 inch dia sewerage line was checked found as per specification. As per approved specification, PMU is bound to carryout following tests to ensure the quality of spiral welded pipes. i. 100% X-ray testing of pipe joints ii. Holiday detection test of coating. iii. Pressure test But only pressure test results were produced during visit. Page 16 of 33 Findings/Recommendations S.# 1 Finding/Recommendations Action to be taken Mirpur City i) ii) iii) iv) v) vi) vii) viii) ix) x) xi) The quality of bricks used in the construction of staff quarters and rest house at Jarikas did not seem satisfactory. The project authority failed to produce test reports to verify the quality of bricks. Hence it is recommended that compressive strength test and absorption test be carried out to check the quality of bricks. Department should fix the responsibility for the poor workmanship observed at site i.e. non uniform/less concrete cover, poor concrete finishing observed, improper spacing between the reinforcement bars, poor vertical alignment in storage water tank etc. Strength of columns in the ramp of intermediate pumping station at Khaliqabad was checked by concrete test hammer. Concrete strength was found less than the mentioned in specifications. Therefore these columns should be dismantled and reconstructed again at the expense of contractor & responsibility be fixed by for poor quality work. The construction of septic tank near Kharik road be dismantled and reconstructed as per specification and responsibility for execution of defective work must be fixed. i) PMU should carryout radiography & holiday detection tests as per approved specification and agreement to ensure the quality of material and workmanship of work. The progress of the construction of sewerage treatment plants/septic tanks in Mirpur City/Chiterpri is show. The PMU is advised to gear up the progress to operationalize the sewerage system at the earliest. The quality and size of earthen ware installed in different category quarters was not adequate. PMU is advised do replace the sub standard sanitary fitting. v) ii) The compressive strength test and absorption test be carried out by PMU to check the quality of bricks (Action by Project Management). Responsibility be fixed. (Action by Secretary PP&H) iii) a: Column in the ramp be dismantled and reconstructed (Action by Project Management). b: Responsibility be fixed. The quality of plinth protection for tube wells and quarters was very poor which needs complete replacement. At different sites the water supply system has been complete/functional which should have been taken over by the concerned department so that people at large can benefit from it. B) SEWERAGE i) Sewerage Pipe 12 inch Dia Thickness of 12 inch dia sewerage pipe was checked and found as per specification. No of Rings and longitudinal reinforcement in 12 inch dia RCC sewerage Pipe was checked fond as per specification. Page 20 of 33 ii. Untreated waste water being dropped in to the canals which is a serious environmental hazard Untreated waste water being dropped in to the canals iii. Highway department has laid stone metal over the manhole covers. Which would create difficulties during clearance and maintenance of sewerage system. Findings/Recommendations S.# 2 Finding/Recommendations Mangla Hamlets Action to be Taken i. i. Responsibility be fixed (Action by Secretary PP&H) ii. iii. 3. Department should fix the responsibility for the poor workmanship i.e steel fixing observed at site. Level of manhole covers should be raised for proper maintenance of sewerage system. Land issue of treatment plant should be resolved at the earliest to avoid further environmental degradation. ii. Action by Project Management iii. Tube wells of Islamgarh Findings/Recommendations S.# 3 Finding/Recommendations Islamagarh Hamlet i) The pipe network of Islamgharh should be removed and re-laid after proper provision of sand cushion as per specification. ii) Supervisory staff should be held responsible for laying of pipes without observing the specifications. 4. Action to be taken i. Action by Project Management ii. Responsibility be fixed (Action by Secretary PP&H) HAMEDABAD HAMLET Instead of restoration of street to the extent of sewerage line (about 2-3 feet) 8 feet vide PCC was laid. Thus the constructed storm water drain becomes useless Start point of drain ii. The executing agency has constructed RCC Storm water drain which is not included in scope of revised approved PC-I moreover, its justification and design have not been vetted by CDO. Action by the Secretary PP&H without approval of competent forum is serious irregularity which warrants proper inquiry. ii) Poor quality of construction was observed in ii .a: Reconstruction of the pump house at Chappri. Rising Main of MS pipe should be removed and re-laid with proper sand cushion. ii. As 9 inch dia RCC pipes and man holes are not according to specification hence same should be dismantled and re-laid. Moreover responsibility must be fixed for execution of poor quality work. 14) Action to be taken i.
